3Tumor necrosis factor-α and interleukin-6 in cirrhotic patients with spontaneous bacterial peritonitis显示文摘AIM:To evaluate the role of tumor necrosis factor-α(TNF-α) and interleukin-6(IL-6) in cirrhotic patients who have hepatic and renal impairment with spontaneous bacterial peritonitis(SBP).METHODS:We prospectively studied 120 cirrhotic patients with SBP and 80 cirrhotic patients with sterile ascitic fluid.They included 144 males and 56 females with ages ranging between 34 and 62 years.The diagnosis of cirrhosis was established by clinical and laboratory criteria that did not require histological confirmation.The severity of underlying liver disease was evaluated using Pugh's modification of Child's criteria(Child-Pugh scores).Ascitic fluid was sent to the laboratory for cell count,culture,sensitivity testing,and measurement of chemical elements(i.e.,albumin,glucose).Specimens were inoculated into aerobic and anaerobic blood culture bottles.Serum and ascitic fluid were also collected in sterile tubes at study entry(before the initiation of antibiotic treatment) and 48 h later.Assays for TNF-α and IL-6 in the serum and ascitic fluid were performed with an immunoenzymometric assay using manufacture's instructions.RESULTS:Cytokine levels in serum and ascitic fluid were significantly higher in the patients with SBP.(plasma TNF-α:135.35 ng/mL ± 11.21 ng/mL vs 92.86 ng/mL ± 17.56 ng/mL,P < 0.001;plasma IL-6:32.30 pg/mL ± 7.07 pg/mL vs 12.11 pg/mL ± 6.53 pg/mL,P < 0.001;ascitic fluid TNF-α:647.54 ± 107.11 ng/mL vs 238.43 ng/mL ± 65.42 ng/mL,P < 0.001);ascitic fluid IL-6:132.84 ng/mL ± 34.13 vs 40.41 ± 12.85 pg/mL,P < 0.001).About 48(40%) cirrhotic patients with SBP developed renal and hepatic impairment and showed significantly higher plasma and ascitic fluid cytokine levels at diagnosis of infection.[(plasma TNF-α:176.58 ± 17.84 vs 135.35 ± 11.21 ng/mL)(P < 0.001) and(IL-6:57.83 ± 7.85 vs 32.30 ± 7.07 pg/mL)(P < 0.001);ascitic fluid TNF-α:958.39 ± 135.72 vs 647.54 ± 107.11 ng/mL,(P < 0.001),ascitic fluid IL-6:654.74 ± 97.43 vs 132.84 ± 34.13 pg/mL,(P < 0.001)].Twenty nine patients(60.4%) with SBP and renal impairment died whereas,only four patients(5.55%) with SBP but without renal impairment died from gastrointestinal hemorrhage(P < 0.0005).CONCLUSION:It appears that TNF-α production may enhance liver cell injury and lead to renal impairment.This correlated well with the poor prognosis and significantly increased mortality associated with SBP in cirrhotic patients.Muhammed AM Suliman Fawzy MH Khalil Salam SA Alkindi Anil V Pathare Ali AA Almadhani Neveen AAI Soliman 2012World Journal of Gastrointestinal Pathophysiology2012,3,5:39
4Androgen receptor: structure, role in prostate cancer and drug discovery显示文摘Androgens and androgen receptors (AR) play a pivotal role in expression of the male phenotype. Several diseases, such as androgen insensitivity syndrome (AIS) and prostate cancer, are associated with alterations in AR functions. Indeed, androgen blockade by drugs that prevent the production of androgens and/or block the action of the AR inhibits prostate cancer growth. However, resistance to these drugs often occurs after 2-3 years as the patients develop castration-resistant prostate cancer (CRPC). In CRPC, a functional AR remains a key regulator. Early studies focused on the functional domains of the AR and its crucial role in the pathology. The elucidation of the structures of the AR DNA binding domain (DBD) and ligand binding domain (LBD) provides a new framework for understanding the functions of this receptor and leads to the development of rational drug design for the treatment of prostate cancer. 6影响颅内动脉瘤破裂风险的患者个体特征(英文)显示文摘BACKGROUND AND PURPOSE Knowledge about risk factors contributes to understanding the pathophysiological mechanisms that cause intracranial aneurysm rupture and helps to develop possible treatment strategies.We aimed to study lifestyle and personal characteristics as risk factors for the rupture of intracranial aneurysms.METHODS We performed a case-control study with 250 patients with an aneurysmal subarachnoid hemorrhage and 206 patients with an unruptured intracranial aneurysm.All patients with an aneurysmal subarachnoid hemorrhage and patients with a unruptured intracranial aneurysm were asked to fill in a structured questionnaire about their lifestyle and medical history.For patients with an unruptured intracranial aneurysm,we also collected data on the indication for imaging.With logistic regression analysis,we identified independent risk factors for aneurysmal rupture.RESULTS Reasons for imaging in patients with an unruptured intracranial aneurysm were atherosclerotic disease(23%),positive family history(18%),headache(8%),preventive screening(3%),and other(46%).Factors that increased risk for aneurysmal rupture were smoking(odds ratio,1.9;95% confidence interval,1.2-3.0) and migraine(2.4;1.1-5.1);hypercholesterolemia decreased this risk(0.4;0.2-1.0),whereas a history of hypertension did not independently influence the risk.CONCLUSIONS Smoking,migraine and,inversely,hypercholesterolemia are independent risk factors for aneurysmal rupture.Data from the questionnaire are insufficient to conclude whether hypercholesterolemia or its treatment with statins exerts a risk-reducing effect.The pathophysiological mechanisms through which smoking and migraine increase the risk of aneurysmal rupture should be investigated in further studies.Although a history of hypertension does not increase risk of rupture,a sudden rise in blood pressure might still trigger aneurysmal rupture.Vlak MH Rinkel GJ Greebe P Algra A 2013中华神经外科疾病研究杂志2013,12,3:17
7Qingyi decoction protects against myocardial injuries induced by severe acute pancreatitis显示文摘BACKGROUND We studied the protective effects of Qingyi decoction(QYD)(a Traditional Chinese Medicine)against severe acute pancreatitis(SAP)-induced myocardial infarction(MI).AIM To study the function and mechanism of QYD in the treatment of myocardial injuries induced by SAP.METHODS Ultrasonic cardiography,hematoxylin and eosin staining,immunohistochemistry,qRT-PCR,western blot,enzyme-linked immunosorbent assays,and apoptosis staining techniques were used to determine the effects of QYD following SAP-induced MI in Sprague-Dawley rats.RESULTS Our SAP model showed severe myocardial histological abnormalities and marked differences in the symptoms,mortality rate,and ultrasonic cardiography outputs among the different groups compared to the control.The expression of serum cytokines[interleukin(IL)-1?,IL-6,IL-8,IL-12,amyloidβ,and tumor necrosis factor-α]were significantly higher in the SAP versus QYD treated group(P<0.05 for all).STIM1 and Orai1 expression in myocardial tissue extracts were significantly decreased post QYD gavage(P<0.001).There was no significant histological difference between the 2-aminoethyl diphenylborinate inhibitor and QYD groups.The SAP group had a significantly higher apoptosis index score compared to the QYD group(P<0.001).CONCLUSION QYD conferred cardio-protection against SAP-induced MI by regulating myocardial-associated protein expression(STIM1 and Orai1).Lei Li Yong-Qi Li Zhong-Wei Sun Cai-Ming Xu Jun Wu Ge-Liang Liu Ahmed MH Bakheet Hai-Long Chen 2020World Journal of Gastroenterology2020,26,12:17

13Antioxidative and anti-endotoxin effects of propofol on endothelial cells显示文摘Objective To investigate the antioxidant and anti-endotoxin effects of propofol on endothelial cells and the possible mechanisms.Methods Cultured endothelial cells were treated with hydrogen peroxide (H 2O 2), propofol + H 2O 2, lipopolysaccharide (LPS) and propofol + LPS, respectively. Endothelial cell damage was monitored for possible lactic dehydrogenase (LDH) release. The transcription and the protein expression levels of endothelial nitric oxide synthase (eNOS) were measured.Results LDH release was higher in groups treated with H 2O 2 or LPS than in the control group. After pretreatment with propofol, the effects induced by H 2O 2 were attenuated, but propofol did not decrease the LDH release induced by LPS. Both H 2O 2 and LPS significantly increased the eNOS transcript levels and the increases were significantly attenuated after pretreatment with propofol. Both H 2O 2 and LPS significantly increased the eNOS protein expression and the increase was attenuated after pretreatment with propofol. 20Impaired norepinephrine regulation of monocyte inflammatory cytokine balance in heart failure显示文摘AIM To evaluate the effect of norepinephrine on inflammatory cytokine expression in ex vivo human monocytes and monocytic THP-1 cells. METHODS For human monocyte studies, cells were isolated from 12 chronic heart failure(HF)(66 ± 12 years, New York Heart Association functional class Ⅲ-Ⅳ, left ventricular ejection fraction 22% ± 9%) and 14 healthy subjects(66 ± 12 years). Monocytes(1 × 106/mL) were incubated with lipopolysaccharide(LPS) 100 ng/m L, LPS + norepinephrine(NE) 10-6 mol/L or neither(control) for 4 h. Tumor necrosis factor-alpha(TNFα) and interleukin-10(IL-10) production were determined by ELISA. Relative contribution of α- and β-adrenergic receptor subtypes on immunomodulatory activity of NE was assessed in LPSstimulated THP-1 cells incubated with NE, the α-selective agonist phenylephrine(PE), and the β-selective agonist isoproterenol(IPN). NE-pretreated THP-1 cells were also co-incubated with the β-selective antagonist propranolol(PROP), α2-selective antagonist yohimbine(YOH) or the α1-selective antagonist prazosin(PRAZ). RESULTS Basal TNFα concentrations were higher in HF vs healthy subjects(6.3 ± 3.3 pg/mL vs 2.5 ± 2.6 pg/mL, P = 0.004). Norepinephrine's effect on TNFα production was reduced in HF(-41% ± 17% HF vs -57% ± 9% healthy, P = 0.01), and proportionately with NYHA FC. Increases in IL-10 production by NE was also attenuated in HF(16% ± 18% HF vs 38% ± 23% healthy, P = 0.012). In THP-1 cells, NE and IPN, but not PE, induced a dosedependent suppression of TNFα. Co-incubation with NE and antagonists revealed a dose-dependent inhibition of the NE suppression of TNFα by PROP, but not by YOH or PRAZ. Dose-dependent increases in IL-10 production were seen with NE and IPN, but not with PE. This effect was also antagonized by PROP but not by YOH or PRAZ. Pretreatment of cells with IPN attenuated the effects of NE and IPN, but did not induce a response to PE.CONCLUSION NE regulation of monocyte inflammatory cytokine production may be reduced in moderate-severe HF, and may be mediated through β-adrenergic receptors.Tien MH Ng Myron L Toews 2016World Journal of Cardiology2016,8,10:4
