1Parenteral nanosuspensions:a brief review from solubility enhancement to more novel and specific applications显示文摘Advancements in in silico techniques of lead molecule selection have resulted in the failure of around 70% of new chemical entities(NCEs). Some of these molecules are getting rejected at final developmental stage resulting in wastage of money and resources. Unfavourable physicochemical properties affect ADME profile of any efficacious and potent molecule, which may ultimately lead to killing of NCE at final stage. Numerous techniques are being explored including nanocrystals for solubility enhancement purposes. Nanocrystals are the most successful and the ones which had a shorter gap between invention and subsequent commercialization of the first marketed product. Several nanocrystal-based products are commercially available and there is a paradigm shift in using approach from simply being solubility enhancement technique to more novel and specific applications. Some other aspects in relation to parenteral nanosuspensions are concentrations of surfactant to be used, scalability and in vivo fate. At present, there exists a wide gap due to poor understanding of these critical factors,which we have tried to address in this review. 2Role of mucoadhesive polymers in enhancing delivery of nimodipine microemulsion to brain via intranasal route显示文摘Intranasal drug administration is receiving increased attention as a delivery method for by-passing the blood–brain barrier and rapidly targeting therapeutics to the CNS.However,rapid mucociliary clearance in the nasal cavity is a major hurdle.The purpose of this study was to evaluate the effect of mucoadhesive polymers in enhancing the delivery of nimodipine microemulsion to the brain via the intranasal route.The optimized mucoadhesive microemulsion was characterized,and the in vitro drug release and in vivo nasal absorption of drug from the new formulation were evaluated in rats.The optimized formulation consisted of Capmul MCM as oil,Labrasol as surfactant,and Transcutol P as co-surfactant,with a particle size of 250 nm and zeta potential value of?15 mV.In vitro and ex vivo permeation studies showed an initial burst of drug release at 30 min and sustained release up to 6 h,attributable to the presence of free drug entrapped in the mucoadhesive layer.In vivo pharmacokinetic studies in rats showed that the use of the mucoadhesive microemulsion enhanced brain and plasma concentrations of nimodipine.These results suggest that incorporation of a mucoadhesive agent in a microemulsion intranasal delivery system can increase the retention time of the formulation and enhance brain delivery of drugs.Rudree Pathak Ranjeet Prasad Dash Manju Misr Manish Nivsarkar 2014Acta Pharmaceutica Sinica B2014,4,2:5
3The role of serratiopeptidase in the resolution of inflammation显示文摘Inflammation remains a key event during most of the diseases and physiological imbalance. Acute inflammation is an essential physiological event by immune system for a protective measure to remove cause of inflammation and failure of resolution lead to chronic inflammation. Over a period of time, a number of drugs mostly chemical have been deployed to combat acute and chronic inflammation. Recently, enzyme based anti-inflammatory drugs became popular over conventional chemical based drugs. Serratiopeptidase, a proteolytic enzyme from trypsin family, possesses tremendous scope in combating inflammation. Serine protease possesses a higher affinity for cyclooxygenase(COX-Ⅰ and COX-Ⅱ), a key enzyme associated with production of different inflammatory mediators including interleukins(IL),prostaglandins(PGs) and thromboxane(TXs) etc. Currently, arthritis, sinusitis, bronchitis,fibrocystic breast disease, and carpal tunnel syndrome, etc. are the leading inflammatory disorders that affected the entire the globe. In order to conquer inflammation, both acute and chronic world, physician mostly relies on conventional drugs. The most common drugs to combat acute inflammation are Nonsteroidal anti-inflammatory drugs(NSAIDs) alone and or in combination with other drugs. However, during chronic inflammation, NSAIDs are often used with steroidal drugs such as autoimmune disorders. These drugs possess several limitations such as side effects, ADR, etc. In order to overcome these limitations and complications, enzyme based drugs(anti-inflammatory) emerged, and aim for a new high since the last decade. Serine protease, the largest proteolytic family has been reported for several therapeutic applications, including anti-inflammatory. Serratiopeptidase is a leading enzyme which has a very long history in medical as an effective anti-inflammatory drug. Current study emphasizes present scenario and future prospect of serratiopeptidase as an antiinflammatory drug. 4Ampullary cancer of intestinal origin and duodenal cancer-A logical clinical and therapeutic subgroup in periampullary cancer显示文摘Periampullary cancers include pancreatic, ampullary, biliary and duodenal cancers. At presentation, the majority of periampullary tumours have grown to involve the pancreas, bile duct, ampulla and duodenum. This can result in difficulty in defining the primary site of origin in all but the smallest tumors due to anatomical proximity and architectural distortion. This has led to variation in the reported proportions of resected periampullary cancers. Pancreatic cancer is the most common cancer resected with a pancreaticoduodenectomy followed by ampullary(16%-50%), bile duct(5%-39%), and duodenal cancer(3%-17%). Patients with resected duodenal and ampullary cancers have a better reported median survival(29-47 mo and 22-54 mo) compared to pancreatic cancer(13-19 mo). The poorer survival with pancreatic cancer relates to differences in tumour characteristics such as a higher incidence of nodal, neural and vascular invasion. While small ampullary cancers can present early with biliary obstruction, pancreatic cancers need to reach a certain size before biliary obstruction ensues. This larger size at presentation contributes to a higher incidence of resection margin involvement in pancreatic cancer. Ampullary cancers can be subdivided into intestinal or pancreatobiliary subtype cancers with histomolecular staining. This avoids relying on histomorphology alone, as even some poorly differentiated cancers preserve the histomolecular profile of their mucosa of origin. Histomolecular profiling is superior to anatomic location in prognosticating survival. Ampullary cancers of intestinal subtype and duodenal cancers are similar in their intestinal origin and form a logical clinical and therapeutic subgroup of periampullary cancers. They respond to 5-FU based chemotherapeutic regimens such as capecitabine-oxaliplatin. Unlike pancreatic cancers, KRAS mutation occurs in only approximately a third of ampullary and duodenal cancers. Future clinical trials should group ampullary cancers of intestinal origin and duodenal cancers together given their similarities and their response to fluoropyrimidine therapy in combination with oxaliplatin. 5Glucose 6 phosphatase dehydrogenase (G6PD) and neurodegenerative disorders:Mapping diagnostic and therapeutic opportunities显示文摘Glucose 6 phosphate dehydrogenase(G6PD)is a key and rate limiting enzyme in the pentose phosphate pathway(PPP).The physiological significance of enzyme is providing reduced energy to specific cells like erythrocyte by maintaining co-enzyme nicotinamide adenine dinucleotide phosphate(NADPH).There are preponderance research findings that demonstrate the enzyme(G6PD)role in the energy balance,and it is associated with bloodrelated diseases and disorders,primarily the anemia resulted from G6PD deficiency.The Xlinked genetic deficiency of G6PD and associated non-immune hemolytic anemia have been studied widely across the globe.Recent advancement in biology,more precisely neuroscience has revealed that G6PD is centrally involved in many neurological and neurodegenerative disorders.The neuroprotective role of the enzyme(G6PD)has also been established,as well as the potential of G6PD in oxidative damage and the Reactive Oxygen Species(ROS)produced in cerebral ischemia.Though G6PD deficiency remains a global health issue,however,a paradigm shift in research focusing the potential of the enzyme in neurological and neurodegenerative disorders will surely open a new avenue in diagnostics and enzyme therapeutics.Here,in this study,more emphasis was made on exploring the role of G6PD in neurological and inflammatory disorders as well as non-immune hemolytic anemia,thus providing diagnostic and therapeutic opportunities.Manju Tiwari 2017Genes & Diseases2017,4,4:4
6Insulin receptors in the kidneys in health and disease显示文摘Insulin is an important hormone that affects various metabolic processes,including kidney function.Impairment in insulin's action leads to insulin resistance in the target tissue.Besides defects in post-receptor insulin signaling,impairment at the receptor level could significantly affect insulin sensitivity of the target tissue.The kidney is a known target of insulin;however,whether the kidney develops 'insulin resistance' is debatable.Regulation of the insulin receptor(IR) expression and its function is very well studied in major metabolic tissues like liver,skeletal muscles,and adipose tissue.The physiological relevance of IRs in the kidney has recently begun to be clarified.The credit goes to studies that showed a wide distribution of IR throughout the nephron segments and their reduced expression in the insulin resistance state.Moreover,altered renal and systemic metabolism observed in mice with targeted deletion of the IR from various epithelial cells of the kidney has strengthened this proposition.In this review,we recapitulate the crucial findings from literature that have expanded our knowledge regarding the significance of the renal IR in normal-and insulin-resistance states.Sarojini Singh Rajni Sharma Manju Kumari Swasti Tiwari 2019World Journal of Nephrology2019,8,1:3
7Management of infected pancreatic necrosis in the setting of concomitant rectal cancer:A case report and review of literature显示文摘BACKGROUND Pancreatitis with infected necrosis is a severe complication of acute pancreatitis and carries with it high rates of morbidity and mortality. The management of infected pancreatic necrosis alongside concomitant colorectal cancer has never been described in literature.CASE SUMMARY A 77 years old gentleman presented to the Emergency Department of our hospital complaining of ongoing abdominal pain for 8 h. The patient had clinical features of pancreatitis with a raised lipase of 3810 U/L, A computed tomography(CT) abdomen confirmed pancreatitis with extensive peri-pancreatic edema. During the course of his admission, the patient had persistent high fevers and delirium thought secondary to infected necrosis, prompting the commencement of broad-spectrum antibiotic therapy with Piperacillin/Tazobactam. Subsequent CT abdomen confirmed extensive pancreatic necrosis(over 70%). Patient was managed with supportive therapy,nutritional support and gut rest initially and improved over the course of his admission and was discharged 42 d post admission. He represented 24 d following his discharge with fever and chills and a repeat CT abdomen scan noted gas bubbles within the necrotic pancreatic tissue thereby confirming infected necrotic pancreatitis. This CT scan also revealed asymmetric thickening of the rectal wall suspicious for malignancy. A rectal cancer was confirmed on flexible sigmoidoscopy. The patient underwent two endoscopic necrosectomies and was treated with intravenous antibiotics and was discharged after 28 d.Within 1 wk post discharge, the patient commenced a course of neoadjuvant radiotherapy and subsequently underwent concomitant chemotherapy prior to undergoing a successful Hartmann's procedure for treatment of his colorectal cancer.CONCLUSION This case highlights the efficacy of endoscopic necrosectomy, early enteral feeding and targeted antibiotic therapy for timely management of infected necrotic pancreatitis.
