2Endoscopic third ventriculostomy in children:problems and surgical outcome:analysis of 34 cases显示文摘Background:Endoscopic third ventriculostomy(ETV)has been established as a viable treatment option for obstructive hydrocephalus of children over 6 weeks of age.ETV in pediatric groups may be unsuccessful due to the failure of absorption of cerebrospinal fluid(CSF)or reclosure of ventriculostomy stoma or due to infection.The exact cause is still debatable.Some issues like failure to eliminate the second membrane during the procedure or formation of the new arachnoid membrane at the stoma are still not clear.This study aims to assess the surgical failure of ETV and its predisposing factors.Methods:Thirty-four pediatric patients with hydrocephalus were analyzed retrospectively.The patients’age limit was between 2.5 months and 14 years.This is a retrospective study of 34 patients in a single private hospital between June 2012 and January 2018.Patients having hydrocephalus in pediatric groups more than 6 weeks of age were included in the study.Results:The mean age of all patients was 51.25±53.90 months and the mean follow-up period was 50.47±20.84 months.Of 34 surgeries,the success rate was 79%and the failure rate was 21%.Within 2 years,the success rate was 68.42%and above 2 years’success rate was 93.33%.In this series,7 cases of ETV were re-explored and found ventriculostomy stoma closure in 3 cases,the presence of the second membrane in re-exploration 2 cases,and presence of inflammatory arachnoid membrane in re-exploration 2 cases.The use of dexamethasone around the stoma in inflammatory stoma was useful,having no recurrence.In one patient of the second membrane probably due to absorption failure in communicating hydrocephalus re-exploration was failed and was managed successfully with VP shunt.Conclusions:Predisposing factors causing ETV failure are ventriculostomy stoma closure by new arachnoid granulation tissues,remnants of the second membrane inside the stoma,CSF absorption failure,infection/high protein in CSF and inappropriate patient selection.Md Moshiur Rahman S.I.M.Khairun Nabi Khan Robert Ahmed Khan Rokibul Islam Mainul Haque Sarker 2021Chinese Neurosurgical Journal2021,7,2:2

7Impact of COVID-19 pandemic on radiology education,training,and practice:A narrative review显示文摘Radiology education and training is of paramount clinical importance given the prominence of medical imaging utilization in effective clinical practice.The incorporation of basic radiology in the medical curriculum has continued to evolve,focusing on teaching image interpretation skills,the appropriate ordering of radiological investigations,judicious use of ionizing radiation,and providing exposure to interventional radiology.Advancements in radiology have been driven by the digital revolution,which has,in turn,had a positive impact on radiology education and training.Upon the advent of the corona virus disease 2019(COVID-19)pandemic,many training institutions and hospitals adhered to directives which advised rescheduling of non-urgent outpatient appointments.This inevitably impacted the workflow of the radiology department,which resulted in the reduction of clinical in-person case reviews and consultations,as well as in-person teaching sessions.Several medical schools and research centers completely suspended face-to-face academic activity.This led to challenges for medical teachers to complete the radiology syllabus while ensuring that teaching activities continued safely and effectively.As a result,online teaching platforms have virtually replaced didactic face-to-face lectures.Radiology educators also sought other strategies to incorporate interactive teaching sessions while adopting the e-learning approach,as they were cognizant of the limitations that this may have on students’clinical expertise.Migration to online methods to review live cases,journal clubs,simulation-based training,clinical interaction,and radiology examination protocolling are a few examples of successfully addressing the limitations in reduced clinical exposure.In this review paper,we discuss(1)The impact of the COVID-19 pandemic on radiology education,training,and practice;(2)Challenges and strategies involved in delivering online radiology education for undergraduates and postgraduates during the COVID-19 pandemic;and(3)Difference between the implementation of radiology education during the COVID-19 pandemic and pre-COVID-19 era.Md Anwarul Azim Majumder Uma Gaur Keerti Singh Latha Kandamaran Subir Gupta Mainul Haque Sayeeda Rahman Bidyadhar Sa Mizanur Rahman Fidel Rampersad 2021World Journal of Radiology2021,13,11:0

9Brain abscess:surgical experiences of 162 cases显示文摘Aim:Brain abscess still poses a public health challenge in spite of the advent of modern neurosurgical techniques and antibiotics.Here,we present our surgical experiences and ultimate outcome in the management of brain abscess.Methods:Totally,162 patients with proved brain abscess who underwent surgical treatment were included in this study.The prospectively recorded data of surgical management of brain abscess and the ultimate outcome(by Glasgow outcome scale)were studied retrospectively.Results:Total number of cases was 162,of which 113 were acute pyogenic abscess while 49 were chronic abscess.Among the chronic abscess,29 were chronic pyogenic abscess,14 were tubercular,3 aspergillus,and 3 abscesses were in malignant brain metastases.In acute cases,common clinical features were headache,fever,vomiting,focal deficit and seizure.In chronic abscesses,common clinical features were mild to moderate headache and progressive focal deficit.Seventy-three(45.06%)patients had adjacent localized sinus,middle ear or cranial infection.The common predisposing factors included postneurosurgery,postpenetrating injury to brain,chronic suppurative otitis media,and congenital heart disease,infective endocarditis,sinusitis and sub optimum immuno-status.Frontal lobe involved in 30.2%cases,temporal lobe is next to involved.Single time burr hole aspiration in 111(68.5%)cases,two or more times burr hole aspiration were done in 34(21%)cases.Pus culture was negative in 129(79.62%)cases.Total number of death was 22(13.58%)cases.Complete resolution of abscess with complete recovery of preoperative neuro-deficit was seen in 80.86%cases and recovery with major neuro-deficit was observed in 5.55%cases.There is a significant association between Glasgow coma scale(GCS)on admission and mortality in brain abscess.Conclusion:In most of the cases,pus culture did not yield growth of any causative organism.Mortality was not directly related to surgical intervention,but GCS on admission has a significant association with mortality.Early diagnosis,optimum follow-up and timely surgical interventions are the keys in the proper management of brain abscess.Forhad Hossain Chowdhury Md Raziul Haque Mainul Haque Sarkar S.M.Noman Khaled Chowdhury Zahed Hossain Shisir Ranjan 2015Neuroimmunology and Neuroinflammation2015,2,1:0
