| 2 | Specimen aspect ratio and progressive field strain development of sandstone under uniaxial compression by three-dimensional digital image correlation显示文摘The complete stress-strain characteristics of sandstone specimens were investigated in a series of quasistatic monotonic uniaxial compression tests.Strain patterns development during pre-and post-peak behaviours in specimens with different aspect ratios was also examined.Peak stress,post-peak portion of stress-strain,brittleness,characteristics of progressive localisation and field strain patterns development were affected at different extents by specimen aspect ratio.Strain patterns of the rocks were obtained by applying three-dimensional(3D) digital image correlation(DIC) technique.Unlike conventional strain measurement using strain gauges attached to specimen,3D DIC allowed not only measuring large strains,but more importantly,mapping the development of field strain throughout the compression test,i.e.in pre-and post-peak regimes.Field strain development in the surface of rock specimen suggests that strain starts localising progressively and develops at a lower rate in pre-peak regime.However,in post-peak regime,strains increase at different rates as local deformations take place at different extents in the vicinity and outside the localised zone.The extent of localised strains together with the rate of strain localisation is associated with the increase in rate of strength degradation.Strain localisation and local inelastic unloading outside the localised zone both feature post-peak regime. | H. | 9 | Polyethylene glycol microspheres loaded with irinotecan for arterially directed embolic therapy of metastatic liver cancer显示文摘AIM To study tumor response, and tolerability of arterially directed embolic therapy(ADET) with polyethylene glycol embolics loaded with irinotecan for the treatment of colorectal cancer liver metastases(CRC-LM). Secondary objectives were to monitor quality of life, time to progression and survival of patients.METHODS Patients were included in the study if they were affected by CRC-LM, refractory to systemic chemotherapy, treated with ADET using polyethylene glycol embolics, and had liver involvement < 50%. Tumor response, performance status(PS), tumor marker antigens, and quality of life(QoL) were monitored at 1, 3 and 6 mo after ADET. QoL was assessed with the Palliative Performance Scale(PPS).RESULTS We treated 50 consecutive CRC-LM patients with ADET using polyethylene glycol embolics. Their tumor response one month after ADET was: 28% of complete response(CR), 48% of partial response(PR), 8% stable disease(SD), and 16% of progression. Tumor response 3 mo after ADET was CR 24%, PR 38%, SD 19% and progression disease(PD) 19%. Tumor response 6 mo after ADET was CR 18%, PR 44%, SD 21% and PD 18%. QoL was 90% PPS at each time point. Median time to progression for patients who progressed was 2.5 mo(range 0.8-6). Median follow-up was 14 mo(0.8-25 range). ADETs were performed with no complications. Observed side effects(mild or moderate intensity) were: Pain in 32% of patients, increase of transaminase levels in 20% and fever in 14%, whereas 30% of patients did not complain any adverse event. | 10 | Short-term effectiveness of golimumab for ulcerative colitis: Observational multicenter study显示文摘AIM To evaluate the real-world effectiveness of golimumab in ulcerative colitis(UC) and to identify predictors of response.METHODS We conducted an observational, prospective and multi-center study in UC patients treated with golimumab, from September 2014 to September 2015. Clinical activity was assessed at wk 0 and 14 with the physician's global clinical assessment(PGA) and the partial Mayo score. Colonoscopies and blood tests were performed, following daily-practice clinical criteria, and the results were recorded in an SPSS database.RESULTS Thirty-three consecutive patients with moderately to severely active UC were included. Among them, 54.5% were female and 42 years was the average age. Thirty percent had left-sided UC(E2) and 70% had extensive UC(E3). All patients had an endoscopic Mayo score of 2 or 3 at baseline. Twenty-seven point three percent were anti-tumor necrosis factor(TNF) treatment na?ve, whereas 72.7% had previously received infliximab and/or adalimumab. Sixty-nine point seven percent showed clinical response and were steroid-free at wk 14(a decrease from baseline in the partial Mayo score of at least 3 points). Based on PGA, the clinical remission and clinical response rates were 24% and 55% respectively. Withdrawal of corticosteroids was observed in 70.8% of steroid-dependent patients at the end of the study. Three out of 10 clinical non-responders needed a colectomy. Mean fecal calprotectin value at baseline was 300 μg/g, and 170.5 μg/g at wk 14. Being anti-TNF treatment na?ve was a protection factor, which was related to better chances of reaching clinical remission. Twenty-seven point three percent of the patients required treatment intensification at 14 wk of followup. | 16 | Validity of automated refraction after segmented refractive multifocal intraocular lens implantation显示文摘AIM: To evaluate the clinical utility of automated refraction(AR) and keratometry(KR) compared with subjective or manifest refraction(MR) after cataract or refractive lens exchange surgery with implantation of Lentis Mplus X(Oculentis GmbH) refractive multifocal intraocular lens(IOL).METHODS: Eighty-six eyes implanted with the Lentis Mplus X multifocal IOL were included. MR was performed in all patients followed by three consecutive AR measurements using the Topcon KR-8000 autorefractor. Assessment of repeatability of consecutive AR before and after dilation with phenylephrine 10%, and comparison of the AR and KR with MR using vector analysis were performed at 3 mo follow-up.RESULTS: Analysis showed excellent repeatability of the AR measurements. Linear regression of AR versus MR showed good correlation for sphere and spherical equivalent, whereas the correlation for astigmatism was low. The mean difference AR-MR was-1.28±0.29 diopters(D) for sphere. Astigmatism showed better correlation between KR and MR. CONCLUSION: We suggest AR sphere plus 1.25 D and the KR cylinder as the starting point for MR in eyes with a Lentis Mplus X multifocal IOL. | 20 | Therapeutic alternatives for the treatment of type 1 hepatorenal syndrome: A Delphi technique-based consensus显示文摘AIM To propose several alternatives treatment of type 1 hepatorenal syndrome(HRS-1) what is the most severe expression of circulatory dysfunction on patients with portal hypertension.METHODS A group of eleven gastroenterologists and nephrologists performed a structured analysis of available literature.Each expert was designated to review and answer a question.They generated draft statements for evaluation by all the experts.Additional input was obtained from medical community.In order to reach consensus,a modified three-round Delphi technique method was used.According to United States Preventive Services Task Force criteria,the quality of the evidence and level of recommendation supporting each statement was graded.RESULTS Nine questions were formulated.The available evidence was evaluated considering its quality,number of patients included in the studies and the consistency of its results.The generated questions were answered by the expert panel with a high level of agreement.Thus,a therapeutic algorithm was generated.The role of terlipressin and norepinephrine was confirmed as the pharmacologic treatment of choice.On the other hand the use of the combination of octreotide,midodrine and albumin without vasoconstrictors was discouraged.The role of several other options was also evaluated and the available evidence was explored and discussed.Liver transplantation is considered the definitive treatment for HRS-1.The present consensus is an important effort that intends to organize the available strategies based on the available evidence in the literature,the quality of the evidence and the benefits,adverse effects and availability of the therapeutic tools described.CONCLUSION Based on the available evidence the expert panel was able to discriminate the most appropriate therapeutic alternatives for the treatment of HRS-1. | Juan P Arab Juan C Claro Juan P Arancibia Jorge Contreras Fernando Gomez Cristian Munoz Leyla Nazal Eric Roessler Rodrigo Wolff Marco Arrese Carlos Benítez | 2016 | World Journal of Hepatology2016，8，25： | 3 |
