3Robust H_∞ control for uncertain Markovian jump systems with mixed delays显示文摘We scrutinize the problem of robust H∞control for a class of Markovian jump uncertain systems with interval timevarying and distributed delays. The Markovian jumping parameters are modeled as a continuous-time finite-state Markov chain. The main aim is to design a delay-dependent robust H∞control synthesis which ensures the mean-square asymptotic stability of the equilibrium point. By constructing a suitable Lyapunov–Krasovskii functional(LKF), sufficient conditions for delay-dependent robust H∞control criteria are obtained in terms of linear matrix inequalities(LMIs). The advantage of the proposed method is illustrated by numerical examples. 9Hereditary hemochromatosis:Temporal trends,sociodemographic characteristics,and independent risk factor of hepatocellular cancer–nationwide population-based study显示文摘BACKGROUND Hereditary hemochromatosis(HH)has an increased risk of hepatocellular cancer(HCC)both due to genetic risks and iron overload as iron overload can be carcinogenic;HH impacts the increasing risk of HCC,not only through the development of cirrhosis but concerning hepatic iron deposition,which has been studied further recently.AIM To evaluate HH yearly trends,patient demographics,symptoms,comorbidities,and hospital outcomes.The secondary aim sheds light on the risk of iron overload for developing HCC in HH patients,independent of liver cirrhosis complications.The study investigated HH(without cirrhosis)as an independent risk factor for HCC.METHODS We analyzed data from National Inpatient Sample(NIS)Database,the largest national inpatient data collection in the United States,and selected HH and HCC cohorts.HH was first defined in 2011 International Classification of Disease-9th edition(ICD-9)as a separate diagnosis;the HH cohort is extracted from January 2011 to December 2019 using 275.01(ICD-9)and E83.110(ICD-10)diagnosis codes of HH.Patients were excluded from the HH cohort if they had a primary or secondary diagnostic code of cirrhosis(alcoholic,non-alcoholic,and biliary),viralhepatitis,alcoholic liver disease,non-alcoholic fatty liver disease(NAFLD),and non-alcoholic steatohepatitis(NASH).We removed these patients from the HH cohort to rule out bias or ICD-10 diagnostic errors.The HCC cohort is selected from January 2011 to December 2019 using the ICD-9 and ICD-10 codes of HCC.We selected a non-HCC cohort with the 1:1 fixed ratio nearest neighbor(greedy)propensity score method using the patients'age,gender,and race.We performed multivariate analysis for the risk factors of HCC in the HCC and non-HCC matched cohort.We further analyzed HH without cirrhosis(removing HH patients with a diagnosis of cirrhosis)as an independent risk factor of HCC after adjusting all known risk factors of HCC in the multivariate model.RESULTS During the 2011-2019 period,a total of 18031 hospitalizations with a primary or secondary diagnosis of HH(excluding liver diseases)were recorded in the NIS database.We analyzed different patients’characteristics,and we found increments in inpatient population trend with a Ptrend<0.001 and total hospital cost of care trend from$42957 in 2011 to$66152 in 2019 with a Ptrend<0.001 despite no change in Length of Stay over the last decade.The multivariate analyses showed that HH without cirrhosis(aOR,28.8;95%CI,10.4–80.1;P<0.0001),biliary cirrhosis(aOR,19.3;95%CI,13.4–27.6;P<0.0001),non-alcoholic cirrhosis(aOR,17.4;95%CI,16.5–18.4;P<0.0001),alcoholic cirrhosis(aOR,16.9;95%CI,15.9–17.9;P<0.0001),hepatitis B(aOR,12.1;95%CI,10.85–13.60;P<0.0001),hepatitis C(aOR,8.58;95%CI,8.20–8.98;P<0.0001),Wilson disease(aOR,4.27;95%CI,1.18–15.41;P<0.0001),NAFLD or NASH(aOR,2.96;95%CI,2.73–3.20;P<0.0001),alpha1-antitrypsin deficiency(aOR,2.10;95%CI,1.21–3.64;P<0.0001),diabetes mellitus without chronic complications(aOR,1.17;95%CI,1.13–1.21;P<0.0001),and blood transfusion(aOR,1.80;95%CI,1.69–1.92;P<0.0001)are independent risk factor for liver cancer.CONCLUSION Our study showed an increasing trend of in-hospital admissions of HH patients in the last decade.These trends were likely related to advances in diagnostic approach,which can lead to increased hospital utilization and cost increments.Still,the length of stay remained the same,likely due to a big part of management being done in outpatient settings.Another vital part of our study is the significant result that HH without cirrhosis is an independent risk factor for HCC with adjusting all known risk factors.More prospective and retrospective large studies are needed to re-evaluate the HH independent risk in developing HCC.Maryam Bilal Haider Ali Al Sbihi Ahmed Jamal Chaudhary Syed M Haider Ahmed Iqbal Edhi 2022World Journal of Hepatology2022,14,9:1

13Fabrication of biomimetic placental barrier structures within a microfluidic device utilizing two-photon polymerization显示文摘The placenta is a transient organ,essential for development and survival of the unborn fetus.It interfaces the body of the pregnant woman with the unborn child and secures transport of endogenous and exogenous substances.Maternal and fetal blood are thereby separated at any time,by the so-called placental barrier.Current in vitro approaches fail to model this multifaceted structure,therefore research in the field of placental biology is particularly challenging.The present study aimed at establishing a novel model,simulating placental transport and its implications on development,in a versatile but reproducible way.The basal membrane was replicated using a gelatin-based material,closely mimicking the composition and properties of the natural extracellular matrix.The microstructure was produced by using a high-resolution 3D printing method– the two-photon polymerization(2PP).In order to structure gelatin by 2PP,its primary amines and carboxylic acids are modified with methacrylamides and methacrylates(GelMOD-AEMA),respectively.High-resolution structures in the range of a few micrometers were produced within the intersection of a customized microfluidic device,separating the x-shaped chamber into two isolated cell culture compartments.Human umbilical-vein endothelial cells(HUVEC)seeded on one side of this membrane simulate the fetal compartment while human choriocarcinoma cells,isolated from placental tissue(BeWo B30)mimic the maternal syncytium.This barrier model in combination with native flow profiles can be used to mimic the microenvironment of the placenta,investigating different pharmaceutical,clinical and biological scenarios.As proof-of-principle,this bioengineered placental barrier was used for the investigation of transcellular transport processes.While high molecular weight substances did not permeate,smaller molecules in the size of glucose were able to diffuse through the barrier in a time-depended manner.We envision to apply this bioengineered placental barrier for pathophysiological research,where altered nutrient transport is associated with health risks for the fetus.Denise Mandt Peter Gruber Marica Markovic Maximillian Tromayer Mario Rothbauer Sebastian Rudi Adam Kratz Syed Faheem Ali Jasper Van Hoorick Wolfgang Holnthoner Severin Mühleder Peter Dubruel Sandra Van Vlierberghe Peter Ertl Robert Liska Aleksandr Ovsianikov 2018International Journal of Bioprinting2018,4,2:1

18Revolution in the diagnosis and management of hepatitis C virus infection in current era显示文摘Chronic hepatitis C virus(HCV)infection is a major global public health problem,particularly in developing part of the world.Significant advances have been made in the early diagnosis and treatment of the disease.Its management has been particularly revolutionized during the past two decades.In this review,we summarize the major advances in the diagnostic and management armamentarium for chronic HCV infection.The focus of the present review is on the newer directly acting anti-viral agents,which have revolutionized the management of chronic HCV infection.Management of uncomplicated chronic HCV infection and of specific complications and special at-risk populations of patients will be covered in detail.Despite the advent and approval of highly effective and well tolerable oral agents,still many challenges remain,particularly the affordability,the equitable distribution and access to later drugs.The World Health Organization aims to eliminate viral hepatitis including HCV by 2030 since its poses a major public health threat.There is an urgent need to ensure uniform and early access to diagnostic and therapeutic facilities throughout the world if the later goal has to be realized.Farina M Hanif Zain Majid Nasir Hassan Luck Abbas Ali Tasneem Syed Muddasir Laeeq Muhammed Mubarak 2022World Journal of Hepatology2022,14,4:1
19Bioactivity of seagrass against the dengue fever mosquito Aedes aegypti larvae显示文摘Objective:To identify the larvieidal activity of the seagrass extracts.Methods:Seagrass extracts,Syringodium isoelifolium(.S.isoetifolium),Cymodocea serrulata and Hulophila beccarii,were dissolved in DMSO to prepare a graded series of concentration.Batches of 25 early 4th instars larvae of Aedes aegypti(Ae.aegypti) were transferred to 250 ml.enamel bowl containing199 mL of distilled water and 1 mL of plant extracts(0.01 mg- 0.1 mg).After 24 h the mortality rate was identified with the formulae[(%of lest mortality-%of control mortality)/(100-%of control morlality)]× 100.Each experiment was conducted with three replicates and a concurrent control group.A control group consisted of 1 mL of DMSO and 199 mL of distilled water only.Results:The root extract of S.isoelifolium showed maximum larvicidal activity with minimum concentration of extract of LC_(50)= 0.0604 ± 0.0040)μg/mL with lower confidence limit(LCL)-upper confidence limit(UCL) =(0.051-0.071) and LC_(90)=0.0972 μg/mL followed by leaf extract of S.isoelifolium showed LC_(50)=(0.062 ± 0.005)μ'g/mL.The regression equation of pool and leaf extract of S.isoelifolium for4th instar larvae were Y =4.909- 1.32x(R^2= 0.909) and Y= 2.066+ 1.2x(R^2 =0.897) respectively.The results of the preliminary photochemical constituents shows the presence of saponin,steroids,terpenoid,phenols,protein and sugars.Conclusions:from the present study the ethanolic extracts of seagrass of S.isoelifolium possesses lead compound for development of larvieidal activity.M Syed Ali S Ravikumar J Margaret Beula 2012Asian Pacific Journal of Tropical Biomedicine2012,2,7:0
20Comparison of resistive index and shear-wave elastography in the evaluation of chronic kidney allograft dysfunction显示文摘BACKGROUND Detection of early chronic changes in the kidney allograft is important for timely intervention and long-term survival.Conventional and novel ultrasound-based investigations are being increasingly used for this purpose with variable results.AIM To compare the diagnostic performance of resistive index(RI)and shear wave elastography(SWE)in the diagnosis of chronic fibrosing changes of kidney allograft with histopathological results.METHODS This is a cross-sectional and comparative study.A total of 154 kidney transplant recipients were included in this study,which was conducted at the Departments of Transplantation and Radiology,Sindh Institute of Urology and Transplantation,Karachi,Pakistan,from August 2022 to February 2023.All consecutive patients with increased serum creatinine levels and reduced glomerular filtration rate(GFR)after three months of transplantation were enrolled in this study.SWE and RI were performed and the findings of these were evaluated against the kidney allograft biopsy results to determine their diagnostic utility.RESULTS The mean age of all patients was 35.32±11.08 years.Among these,126(81.8%)were males and 28(18.2%)were females.The mean serum creatinine in all patients was 2.86±1.68 mg/dL and the mean estimated GFR was 35.38±17.27 mL/min/1.73 m2.Kidney allograft biopsy results showed chronic changes in 55(37.66%)biopsies.The sensitivity,specificity,positive predictive value(PPV),and negative predictive value(NPV)of SWE for the detection of chronic allograft damage were 93.10%,96.87%%,94.73%,and 95.87%,respectively,and the diagnostic accuracy was 95.45%.For RI,the sensitivity,specificity,PPV,and NPV were 76.92%,83.33%,70.17%,and 87.62%,respectively,and the diagnostic accuracy was 81.16%.CONCLUSION The results from this study show that SWE is more sensitive and specific as compared to RI in the evaluation of chronic allograft damage.It can be of great help during the routine follow-up of kidney transplant recipients for screening and early detection of chronic changes and selecting patients for allograft biopsy.Ameet Kumar Jesrani Syed M Faiq Rahma Rashid Tariq Ali Kalwar Rehan Mohsin Tahir Aziz Nida Amin Khan Muhammed Mubarak 2024World Journal of Transplantation2024,14,1:0
