1Non-invasive diagnostic imaging of colorectal liver metastases显示文摘Colorectal cancer is one of the few malignant tumors in which synchronous or metachronous liver metastases [colorectal liver metastases(CRLMs)] may be treated with surgery. It has been demonstrated that resection of CRLMs improves the long-term prognosis. On the other hand, patients with un-resectable CRLMs may benefit from chemotherapy alone or in addition to liverdirected therapies. The choice of the most appropriate therapeutic management of CRLMs depends mostly on the diagnostic imaging. Nowadays, multiple non-invasive imaging modalities are available and those have a pivotal role in the workup of patients with CRLMs. Although extensive research has been performed with regards to the diagnostic performance of ultrasonography, computed tomography, positron emission tomography and magnetic resonance for the detection of CRLMs, the optimal imaging strategies for staging and follow up are still to be established. This largely due to the progressive technological and pharmacological advances which are constantly improving the accuracy of each imaging modality. 2A case of multiple intra-abdominal splenosis with computed tomography and magnetic resonance imaging correlative findings显示文摘Hepatic splenosis refers to heterotopic auto- transplantation and implantation of splenic tissue resulting from the spillage of cells from the spleen after splenic trauma or splenectomy. The true incidence of splenosis is unknown, because this entity is usually an incidental finding at surgery. Splenic implants are usually multiple, and can be localized anywhere in the peritoneal cavity. Splenic implants in the peritoneal cavity may be confused with renal tumors, abdominal lymphomas and endometriosis. 5Contrast enhanced multi-detector CT and MR findings of a well-differentiated pancreatic vipoma显示文摘Pancreatic vipoma is an extremely rare tumor accounting for less than 2% of endocrine pancreatic neoplasms with a reported incidence of 0.1-0.6 per million. While cross-sectional imaging findings are usually not specific, exact localization of the tumor by means of either computed tomography(CT) or magnetic resonance(MR) is pivotal for surgical planning. However, cross-sectional imaging findings are usually not specific and further characterization of the tumor may only be achieved bysomatostatin-receptor scintigraphy(SRS). We report the case of a 70 years old female with a two years history of watery diarrhoea who was found to have a solid, inhomogeneously enhancing lesion at the level of the pancreatic tail at Gadolinium-enhanced MR(Somatom Trio 3T, Siemens, Germany). The tumor had been prospectively overlooked at a contrast-enhanced multi-detector CT(Aquilion 64, Toshiba, Japan) performed after i.v. bolus injection of only 100 cc of iodinated non ionic contrast media because of a chronic renal failure(3.4 mg/mL) but it was subsequently confirmed by SRS. 6A spontaneous strangulated transomental hernia: Prospective and retrospective multi-detector computed tomography findings显示文摘Transomental hernias are among the rarest type of all internal hernias which overall account for less than 6% of small bowel obstructions. Most transomental hernias occurring in adults are either iatrogenic or post-traumatic. More rarely, a spontaneous herniation of small bowel loops may result from senile atrophy of the omentum. We report a case of an 86-year-old male who presented with signs and symptoms of small bowel obstruction but had no past surgical or traumatic abdominal history. At contrast-enhanced multi-detector row computed tomography(CT), a cluster of fluid-filled dilated small bowel loops could be appreciated in the left flank, with associated signs of bowel wall ischemia. Swirling of the mesenteric vessels could also be appreciated and CT findings were prospectively considered consistent with a strangulated small bowel volvulus. At laparotomy, no derotation had to be performed but up to 100 cm of gangrenous small bowel loops had to be resected because of a transomental hernia through a small defect in the left part of the greater omentum. 7Pseudopneumoperitoneum in chronic intestinal pseudo-obstruction:A case report显示文摘Chronic intestinal pseudo-obstruction(CIPO) is a rare disease due to a severe gastrointestinal motility disorder which may mimic,on both clinical and radiological grounds,mechanical obstruction.We report a case of a 26-year-old woman who presented to our institution for plain abdominal radiography for referred long-lasting constipation with recurrent episodes of abdominal pain and distension.At X-ray,performed both in the upright and supine position,an isolated air-fluid level was depicted in the left flank,together with a number of radiological signs suggestive of pneumoperitoneum.First,subphrenic radiolucency could be observed in the upright film.Second,the intestinal wall of some jejunal loops appeared to be outlined in the right flank.Third,the inferior cardiac border was clearly depicted in the upright film.The patient however had no evidence ofperitoneal signs but only hypoactive bowel movements.Unenhanced multi-detector computed tomography(MDCT) of the abdomen and pelvis was therefore performed.MDCT revealed abnormal air-driven distension of the small and large bowel,without evidence of extraluminal air.All radiological signs of pneumoperitoneum turned out to be false-positive results.The patient was submitted to pan-colonoscopy and to anorectal manometry to rule out Hirshprung's disease,and was finally discharged with a diagnosis of CIPO.Luigi Camera Milena Calabrese Giovanni Sarnelli Margaret Longobardi Alba Rocco Rosario Cuomo Marco Salvatore 2011World Journal of Gastroenterology2011,17,24:0
8Colonic perforation by a transmural and transvalvular migrated retained sponge:Multi-detector computed tomography findings显示文摘Transmural migrated retained sponges usually impact at the level of the ileo-cecal valve leading to a small bowel obstruction.Once passed through the ileo-cecal valve,a retained sponge can be propelled forward by peristaltic activity and eliminated with feces.We report the case of a 52-year-old female with a past surgical history and recurrent episodes of abdominal pain and constipation.On physical examination,a generalized resistance was observed with tenderness in the right flank.Contrast-enhanced multi-detector computed tomography findings were consistent with a perforated right colonic diverticulitis with several out-pouchings at the level of the ascending colon and evidence of free air in the right parieto-colic gutter along with an air-fluid collection within the mesentery.In addition,a ring-shaped hyperdense intraluminal material was also noted.At surgery,the ascending colon appeared irregularly thickened and folded with a focal wall interruption and a peri-visceral abscess at the level of the hepatic flexure,but no diverticula were found.A right hemi-colectomy was performed and on dissection of the surgical specimen a retained laparotomy sponge was found in the bowel lumen.Luigi Camera Marco Sagnelli Paolo Guadagno Pier Paolo Mainenti Teresa Marra Maria Scotto di Santolo Landino Fei Marco Salvatore 2014World Journal of Gastroenterology2014,20,15:0
9Volvulus of the ascending colon in a non-rotated midgut:Plain film and MDCT findings显示文摘Colonic volvulus is a relatively uncommon cause of large bowel obstruction usually involving mobile,intraperitoneal,colonic segments.Congenital or acquired anatomic variation may be associated with an increased risk of colonic volvulus which can occasionally involve retro-peritoneal segments.We report a case of 54-year-old female who presented to our Institution to perform a plain abdominal film series for acute onset of cramping abdominal pain.Both the upright and supine films showed signs of acute colonic obstruction which was thought to be due to an internal hernia of the transverse colon into the lesser sac.The patient was therefore submitted to a multi-detector contrast-enhanced computed tomography(CT).CT findings were initially thought to be consistent with the presumed diagnosis of internal hernia but further evaluation and coronal reformatting clearly depicted the presence of a colonic volvulus possibly resulting from a retro-gastric colon.At surgery,a volvulus of the ascending colon was found and a right hemi-colectomy had to be performed.However,a non rotated midgut with a right-sided duodeno-jejunal flexure and a left sided colon was also found at laparotomy and over-looked in the pre-operative CT.Retrospective evaluation of CT images was therefore performed and a number of CT signs of intestinal malrotation could be identified.Luigi Camera Milena Calabrese Pier Paolo Mainenti Stefania Masone Walter Del Vecchio Giovanni Persico Marco Salvatore 2012World Journal of Radiology2012,4,10:0
10Prognostic role of sarcopenia in metastatic colorectal cancer patients during first-line chemotherapy:A retrospective study显示文摘BACKGROUND Sarcopenia is a condition characterized by decreased skeletal muscle mass due to physiological ageing or to a concomitant disease such as neoplasia.In cancer patients,a low lean body mass is suggested to be a negative prognostic factor for survival and for the development of dose-limiting chemotherapy toxicities irrespective of disease stage.AIM To evaluate the prognostic role of sarcopenia in patients with metastatic colorectal cancer(mCRC)undergoing first-line chemotherapy.METHODS Our retrospective analysis included 56 mCRC patients who received first-line chemotherapy from 2014 to 2017 at the Medical Oncology Unit of our hospital.Computerized scans were performed before starting chemotherapy and at the first disease reassessment.Sarcopenia was assessed using the skeletal mass index=muscle area in cm^(2)/(height in m^(2))calculated at the L3 vertebra.Overall survival and objective response rate were evaluated.Toxicities were analyzed during the first four cycles of therapy and graded according to Common Terminology Criteria for Adverse Events version 4.0.A loss of skeletal muscle mass≥5%was considered indicative of deterioration in muscle condition.RESULTS Median age was 67 years and 35.7%of patients were≥70 years old.Fourteen patients(25%)were sarcopenic at baseline computed tomography(CT)scan(7/33 men;7/23 women);5/14 sarcopenic patients were≥70 years old.Median followup was 26.8 mo(3.8-66.8 mo)and median overall survival was 27.2 mo(95%CI:23.3-37.3).Sarcopenia was not correlated to overall survival(P=0.362),to higher toxicities reported during the first 4 cycles of chemotherapy(P=1.0)or to response to treatment(P=0.221).At the first disease reassessment,a skeletal muscle loss(SML)≥5%was found in 17 patients(30.3%)3 of whom were already sarcopenic at baseline CT scan,while 7 patients became sarcopenic.SML was not correlated to overall survival(P=0.961).No statistically significant correlation was found between baseline sarcopenia and age(P=1.0),body mass index(P=0.728),stage at diagnosis(P=0.355)or neutrophil/lymphocyte ratio(P=0.751).CONCLUSION Neither baseline sarcopenia nor SML affected survival.In addition,baseline sarcopenia was not related to worse treatment toxicity.However,these results must be interpreted with caution due to the limited sample size.Chiara Maddalena Andrea Ponsiglione Luigi Camera Lidia Santarpia Fabrizio Pasanisi Dario Bruzzese Camilla Panico Giovanni Fiore Simona Camardella Tolomeo Caramia Alessia Farinaro Sabino De Placido Chiara Carlomagno 2021World Journal of Clinical Oncology2021,12,5:0
