| 3 | 累积性液体正平衡与急性呼吸窘迫综合征合并急性肾损伤患儿病死率相关显示文摘目的:急性肾损伤(acute kidney injury,AKI)和累积液体平衡(cumulative fluidbalance,CFB)升高常同时发生在ARDS患儿中,本文旨在识别其发生的危险因素并评估其与病死率的独立关系。本文假设CFB升高与炎症指标相关,CFB升高并发AKI患儿预后较其他患儿更差。设计前瞻性观察性研究,采用pRIFLE AKI分级。场所5家学术型PICUs。对象2008至2014年间符合ARDS柏林标准的260例1月至18岁患儿。干预措施无。结果PICU病死率为13%(34/260),病死患儿与存活患儿相比在ARDS第3天CFB较高(+90.1 ml/kg,IQR 26.6~161.7比+44.9 ml/ kg,IQR 10.0~111.3,P=0.008),且AKI发生率更高(50%比23%,P=0.001)。分层分析显示并发AKI患儿中,ARDS第3天CFB较高与病死率有关(病死者+111.5 ml/kg,IQR 82.6~236.8比存活者+58.5 ml/kg,IQR 0.9~176.2,P=0.041),而在不伴有AKI的ARDS患儿中无此相关性(P=0.308)。ARDS第3天并发AKI与正CFB患儿病死率相关(29.1%比10.4%,P=0.001),而在液体平衡或负平衡患儿中无此相关性(P=0.430)。ARDS第1天的血浆IL-6水平与第3天正CFB,AKI及PICU病死率相关,第1天IL-6与PICU病死率升高之间的关系由第3天正CFB和AKI部分介导(P<0.001)。结论儿童ARDS第3天正CFB与病死率相关,尤其在并发AKI患儿。血浆IL-6水平与正CFB及AKI发生相关,提示炎症可能是导致病死率增加的机制。 | Zinter MS Spicer AC Liu KD 方伯梁(译者) 钱素云(审校者) | 2019 | 中国小儿急救医学2019，26，4： | 3 |
