1帕博利珠单抗单用或与放疗联用治疗转移性非小细胞肺癌:两个随机试验的汇总分析显示文摘背景放疗可以提高整个机体对免疫治疗的应答。在Ⅱ期PEMBRO-RT研究和Ⅰ/Ⅱ期MDACC研究中,患有转移性非小细胞肺癌(NSCLC)的患者被随机分配入组,接受免疫治疗(帕博利珠单抗)+放疗联合疗法,或免疫治疗单一疗法。当上述2个研究单独分析时,联合疗法组显示出潜在获益。由于每个研究的样本量较小,缓解率和结局并未显示出统计学意义,然而却有显著的临床获益。因此,本研究进行汇总分析,来判断放疗是否会改善转移性NSCLC患者的免疫治疗应答。方法PEMBRO-RT和MDACC研究纳入标准:患者年龄≥18岁,患有转移性NSCLC,且有≥1处未经放疗照射的病灶,以便进行射野外应答监测。PEMBRO-RT研究纳入曾接受过化疗患者,MDACC研究纳入曾接受过治疗或新诊断患者。2个研究中的患者均未接受过免疫治疗。在PEMBRO-RT研究中患者被等比例随机分配入组,并根据吸烟状态进行分层(分为<10年组和≥10年组)。MDACC研究的患者根据放疗计划可行性被等比例随机分配入2个受试组。由于联合治疗组的干预本质,每个研究中的放疗均不适用盲法。在2个研究中,不论是否进行放疗,均静脉滴入帕博利珠单抗(每3周200 mg)。在PEMBRO-RT研究中,在放疗(24 Gy 3次分割照射)结束后1周给予第1剂帕博利珠单抗。在MDACC研究中,在第1次放疗(50 Gy 4次分割照射或45 Gy 15次分割照射)同时给予帕博利珠单抗。仅检测未经照射病灶的应答。本研究的终点为最佳射野外(远隔)应答率(ARR)、最佳射野外疾病控制率(ACR)、12周时ARR、12周时ACR、无进展生存期(PFS)和总生存期(OS)。2个研究的意向治疗(ITT)人群均纳入分析。PEMBRO-RT研究(NCT02492568)和MDACC研究(NCT02444741)均在ClinicalTrials.gov上注册。发现纳入148例患者,76例接受帕博利珠单抗治疗,72例接受帕博利珠单抗+放疗治疗。所有患者随访时间中位数为33个月[四分位距(IQR):32.4~33.6]。148例患者中124例(84%)组织学特征为非鳞癌,111例(75%)患者曾经接受过化疗。组间没有基线特征差异,包括PD-L1表达状态和转移灶体积。最常见的照射部位为肺转移灶(39%,28/72)、胸腔内淋巴结(21%,15/72)和非原发灶(17%,12/72)。帕博利珠单抗组和联合治疗组的最佳ARR分别为19.7%(15/76)和41.7%(30/72),OR=2.96,95%CI:1.42~6.20,P=0.0039;最佳ACR分别为43.4%(33/76)和65.3%(47/72),OR=2.51,95%CI:1.28~4.91,P=0.0071;PFS中位数分别为4.4(IQR:2.9~5.9)和9.0个月(IQR:6.8~11.2),HR=0.67,95%CI:0.45~0.99,P=0.045;OS中位数分别为8.7(IQR:6.4~11.0)和19.2个月(IQR:14.6~23.8),OR=0.67,95%CI:0.54~0.84,P=0.0004。在汇总分析中没有发现新的安全问题。解读帕博利珠单抗免疫疗法+放疗显著提高转移性NSCLC患者的应答和改善治疗结局。这些结果需要在三期临床试验中进行验证。陈大卫(翻译) 于金明(校对) Willemijn S M E Theelen Vivek Verma Brian P Hobbs Heike M U Peulen Joachim G J V Aerts Idris Bahce Anna Larissa N Niemeijer Joe Y Chang Patricia M de Groot Quynh-Nhu Nguyen Nathan I Comeaux George R Simon Ferdinandos Skoulidis Steven H Lin Kewen He Roshal Patel John Heymach Paul Baas James W Welsh 2021中华肿瘤防治杂志2021,28,24:49

4Comparative effectiveness of i-SCAN^(TM) and high-definition white light characterizing small colonic polyps显示文摘AIM:To evaluate accuracy of in vivo diagnosis of adenomatous vs non-adenomatous polyps using i-SCAN digital chromoendoscopy compared with high-definition white light.METHODS:This is a single-center comparative effectiveness pilot study.Polyps(n = 103) from 75 averagerisk adult outpatients undergoing screening or surveillance colonoscopy between December 1,2010 and April 1,2011 were evaluated by two participating endoscopists in an academic outpatient endoscopy center.Polyps were evaluated both with high-definition white light and with i-SCAN to make an in vivo prediction of adenomatous vs non-adenomatous pathology.We determined diagnostic characteristics of i-SCAN and highdefinition white light,including sensitivity,specificity,and accuracy,with regards to identifying adenomatous vs non-adenomatous polyps.Histopathologic diagnosis was the gold standard comparison.RESULTS:One hundred and three small polyps,detected from forty-three patients,were included in the analysis.The average size of the polyps evaluated in the analysis was 3.7 mm(SD 1.3 mm,range 2 mm to 8 mm).Formal histopathology revealed that 54/103(52.4%) were adenomas,26/103(25.2%) were hyperplastic,and 23/103(22.3%) were other diagnoses include 'lymphoid aggregates','non-specific colitis,' and 'no pathologic diagnosis.' Overall,the combined accuracy of endoscopists for predicting adenomas was identical between i-SCAN(71.8%,95%CI:62.1%-80.3%) and high-definition white light(71.8%,95%CI:62.1%-80.3%).However,the accuracy of each endoscopist differed substantially,where endoscopist A demonstrated 63.0% overall accuracy(95%CI:50.9%-74.0%) as compared with endoscopist B demonstrating 93.3% overall accuracy(95%CI:77.9%-99.2%),irrespective of imaging modality.Neither endoscopist demonstrated a significant learning effect with i-SCAN during the study.Though endoscopist A increased accuracy using i-SCAN from 59%(95%CI:42.1%-74.4%) in the first half to 67.6%(95%CI:49.5%-82.6%) in the second half,and endoscopist B decreased accuracy using i-SCAN from 100%(95%CI:80.5%-100.0%) in the first half to 84.6%(95%CI:54.6%-98.1%) in the second half,neither of these differences were statistically significant.CONCLUSION:i-SCAN and high-definition white light had similar efficacy predicting polyp histology.Endoscopist training likely plays a critical role in diagnostic test characteristics and deserves further study.Johanna L Chan Li Lin Michael Feiler Andrew I Wolf Diana M Cardona Ziad F Gellad 2012World Journal of Gastroenterology2012,18,41:3
5Numerical prediction experiment on Typhoon Maggie (9903)显示文摘The movement of Typhoon Maggie (9903) in June 1999 is one of the rare cases ever seen in the history. At 00U on June 6 Maggie was located at about 70 km to the southwest of Taiwan. When it arrived at the coastal region of Shanwei City (22.8N, 116.5E), it turned suddenly to move southwestward along the southern China coastal line. De June 7 Maggie finally turned to move northward, making landfall to the north of Shangchuan Island. The experimental numerical prediction system on typhoon movement that was designed based on MM5 is proved quite successful for the 48h prediction of Maggie’s movement and rainfall. The mean prediction error of typhoon track is 81 km for 0-24 h and 74 km for 24-48 h. The location of typhoon center in the initial field of the model is approximately 100 km away from the actual observations. In order to modify the location of typhoon center, a bogus typhoon was intro- duced into the model and the prediction of typhoon track was improved in 0-24 h time interval. But the prediction error was enlarged in 24-36 h. We also performed a sensitivity experiment of changing the land of southern China into the ocean. 11Effect of Nitric Oxide on Alleviating Cadmium Toxicity in Rice (Oryza sativa L.)显示文摘Nitric oxide (NO) is a gaseous signaling molecule in plants that plays a key role in mediating a wide range of physiological processes and responses to biotic and abiotic stresses. The present study was conducted to investigate the effects of the exogenous application of sodium nitroprusside (SNP), an NO donor, on cadmium (Cd)-induced oxidative stress and Cd uptake in rice plants. Rice plants were exposed to Cd stress (0.2 mmol L^(-1) CdCl_2) and different concentrations of SNP (0.05, 0.1, 0.2, and 0.4 mmol L^(-1)). A SNP concentration of 0.1 mmol L^(-1) (SNP10) significantly reduced the Cd-induced decrease in shoot and root dry weights and leaf chlorophyll concentrations. The addition of NO also reduced the malondialdehyde (MDA), hydrogen peroxide (H_2O_2) and ascorbic acid (ASA) concentrations. However, the reduction in glutathione (GSH) concentration was inhibited by NO treatment. Moreover, NO prevented the Cd-induced increase in antioxidative enzyme activity. The amount of Cd accumulation in rice plants was also influenced by the addition of NO. The NO supplied by the SNP enhanced the Cd tolerance of the rice by increasing the Cd uptake by the roots and decreasing the Cd accumulation by the shoots. However, the application of potassium ferrocyanide (Cd+Fe) or sodium nitrate and nitrite (Cd+N) (without NO release), did not exhibit the effects of the SNP. Furthermore, the effects of the SNP were reversed by the addition of hemoglobin (an NO scavenger). 14Perihepatic nodes detected by point-of-care ultrasound in acute hepatitis and acute-on-chronic liver disease显示文摘AIM: To study the manifestations of perihepatic lymph nodes during the episode of acute hepatitis flare by point-of-care ultrasonography.METHODS: One hundred and seventy-six patients with an episode of acute hepatitis flare(ALT value > 5 × upper normal limit) were enrolled retrospectively. Diagnosis of etiology of the acute hepatitis flare was based on chart records and serological and virological assays. The patients were categorized into two groups(viral origin and non-viral origin) and further defined into ten subgroups according to the etiologies. An ultrasonograpy was performed within 2 h to 72 h(median, 8 h). The maximum size of each noticeable lymph node was measured. Correlation between clinical parameters and nodal manifestations was analyzed RESULTS: Enlarged lymph nodes(width ≥ 5mm)were noticeable in 110(62.5%) patients, mostly in acute on chronic hepatitis B(54.5%). The viral group had a higher prevalence rate(89/110 = 80.9%) and larger nodal size(median, 7 mm) than those of the non-viral group(21/66 = 31.8%; median, 0 mm)(P < 0.001 for both). Meanwhile, there were significant differences in the nodal size between acute and chronic viral groups(P < 0.01), and between acute hepatitis A and non-hepatitis A viral groups(P < 0.001). In logistical regression analysis, the nodal width still showed strong significance in multivariate analysis(P < 0.0001) to stratify the two groups. The area under the curve of ROC was 0.805, with a sensitivity of 80.9%, a specificity of 68.2%, positive predictive value of 80.92%, negative predictive value of 68.18%, and an accuracy of 76.14%.
