| 1 | 舒芬太尼复合罗哌卡因用于硬膜外无痛分娩的临床观察显示文摘目的探讨舒芬太尼复合罗哌卡因用于连续硬膜外无痛分娩的效果和安全程度。方法将2018年12月至2019年4月期间昆明医科大学附属延安医院100例在产房正常待产的初产妇随机分为2组(n=50例),实验组常规于L2~3间隙行连续硬膜外穿刺,试验量后头侧置硬膜外管4cm接PCEA泵,协助产妇平卧位后给予舒芬太尼复合罗哌卡因的首剂量和维持剂量,对照组产妇实施常规阴道分娩。观察2组产妇硬膜外自控镇痛效果、产程时间、出血量和新生儿Apgar评分情况,记录是否需要助产和紧急更改剖宫产,是否出现产后出血和胎儿窘迫等不良事件。结果 2组产妇的年龄、身高、体重、孕周值比较差异无统计学意义(P>0.05),实验组产妇分娩镇痛效果的优良率明显高于对照组(P<0.05),2组产妇出血量和新生儿Apgar评分比较差异无统计学意义(P>0.05),实验组产妇第一产程和第二产程时间明显短于对照组(P<0.05),实验组产妇出现需要阴道助产、紧急更改剖宫产和产后出血的比例明显低于对照组(P<0.05)。结论舒芬太尼复合罗哌卡因用于连续硬膜外无痛分娩,镇痛效果显著,能够显著缩短产程时间,提高产妇自然分娩率,降低分娩过程中不良事件的发生率,可以安全有效地应用于临床。 | Dinesh Kumar Shrestha 刘萍 张富荣 | 2019 | 昆明医科大学学报2019,40,10: | 20 |
| 2 | 不同剂量右美托咪定对腹腔镜胆囊切除术中各指标及术后镇静的影响显示文摘目的腹腔镜胆囊切除术中腹内压增加和二氧化碳吸收可引起明显的交感神经兴奋,致血压升高,心率增快。有效控制这些交感反应能减少并发症,有利于患者及时恢复。文中旨在研究不同剂量右美托咪定对腹腔镜胆囊切除术中血压、心率和术后镇静的影响。方法选择2018年1月2日至2018年2月9日荆州市中心医院肝胆外科拟行择期腹腔镜胆囊切除术的患者60例。根据术中是否输注右美托咪定随机分成3组:对照组(仅泵注等量等渗盐水)、低剂量组[0.5μg/(kg·h)右美托咪定]、高剂量组[1.0μg/(kg·h)右美托咪定],每组20例。在麻醉前、切皮前、气腹后0 min、10 min、20 min、放气后即刻、拔管前、苏醒前和苏醒后各时间点记录患者血压、心率、脉氧饱和度(SpO_2)、气道峰压、呼末二氧化碳分压(PetCO_2)、拔管后Ramsay镇静评分等。结果高剂量组拔管时间、恢复室留观时间较对照组、低剂量组明显延长(P<0.05)。放气后即刻,与对照组心率[(75.60±15.59)次/min]比较,高剂量组[(64.80±8.59)次/min]、低剂量组[(65.85±10.30)次/min]明显下降(P<0.05),苏醒前、苏醒后亦明显下降(P<0.05)。气腹后0 min至放气后即刻,高剂量组气道峰压明显高于对照组、低剂量组(P<0.05)。苏醒前,高剂量组Ramsay评分[(2.65±0.48)分]明显高于对照组[(2.30±0.57)分]、低剂量组[(2.25±0.44)分],差异有统计学意义(P=0.015)。结论右美托咪定持续泵注可稳定心率,但随着泵注速度或剂量的增加,可增加拔管后的镇静深度,明显延长拔管时间及恢复室留观时间。因此临床麻醉中使用右美托咪定时以不超过0.5μg/(kg·h)为宜。 | Kumar Shrestha 王力甚 | 2018 | 医学研究生学报2018,31,12: | 17 |
| 3 | 产前皮质激素及产后肺泡表面活性物质防治新生儿呼吸窘迫综合征的疗效(英文)显示文摘目的探讨产前应用皮质激素(ACS)联合产后应用外源性肺表面活性物质(PS)与单一应用防治新生儿呼吸窘迫综合征(RDS)的疗效。方法选取2003年1月-2007年1月本院NICU收治的143例RDS新生儿。随机分为4组:第1组为产前应用ACS联合产后应用PS (n=36);第2组为单一产前应用ACS (n=33);第3组为单一产后应用PS (n=39);第4组为2种治疗措施均未采用(n=35)。对4组患儿的一般资料如性别、胎龄、出生体质量、分娩方式、Apgar评分、产时复苏情况、围生期并发症进行分析,并对不同组间应用鼻导管吸氧、头罩吸氧、持续呼吸道正压(CPAP)、机械通气(MV)等不同氧疗模式的时间,治愈的平均治疗时间及疗效进行比较。结果 4组患儿的一般资料和临床特征相似(Pa>0.05)。第1、2、3、4组采用鼻导管吸氧时间分别为(75.81±15.63)、(130.09±27.32)、(150.67±28.59)、(174.32±25.92) h (P=0.041);头罩吸氧时间分别为(37.16±5.51)、(55.29±11.71)、(62.69±12.39)、(100.75±28.10) h (P=0.047);CPAP时间分别为(24.33±4.41)、(27.44±4.47)、(26.53±3.13)、(56.50±5.50) h (P=0.005);MV时间分别为(56.12±15.65)、(110.19±21.59)、(127.79±26.36)、(156.61±12.92) h (P=0.009);第1组采用MV的几率最低。4组患儿住院天数分别为(15.89±1.29)、(21.61±2.30)、(28.31±3.40)、(32.73±4.57) d(P=0);治愈率分别为63.89%、51.52%、35.90%、20.0%(P=0.005)。结论产前应用ACS联合产后外源性补充PS是治疗RDS的最佳措施,单一产前应用ACS对RDS的疗效优于单一产后应用PS。 | 徐发林 Shrestha Him Kumar 程秀永 | 2008 | 实用儿科临床杂志2008,23,16: | 14 |
| 4 | Predicting the impact of climate change on the distribution of two threatened Himalayan medicinal plants of Liliaceae in Nepal显示文摘Predicting the potential distribution of medicinal plants in response to climate change is essential for their conservation and management. Contributing to the management program, this study aimed to predict the distribution of two threatened medicinal plants, Fritillaria cirrhosa and Lilium nepalense. The location of focal species gathered from herbarium specimen housed in different herbaria and online databases were geo-referenced and checked for spatial autocorrelation. The predictive environmental variables were selected, and MaxE nt software was used to model the current and future distributions of focal species. Four Representative Concentration Pathway(RCP) trajectories of the BCC-CSM1.1 model were used as the future(2050) projection layer. The MaxEnt modelling delineated the potential distribution of F. cirrhosa and L. nepalense. The current suitability is projected towards Central and Eastern Hilly/Mountainous regions. Both species gainmaximum suitability in RCP 4.5 which decline towards other trajectories for L. nepalense. Overall, both the focal species shift towards the north-west, losing their potential habitat in hilly and lower mountainous regions by 2050 across all trajectories. Our results highlight the impact of future climate change on two threatened and valuable species. | 8 | Habitat Range of two Alpine Medicinal Plants in a Trans-Himalayan Dry Valley,Central Nepal显示文摘Understanding of the habitat range of threatened Himalayan medicinal plants which are declining in their abundance due to high anthropogenic disturbances is essential for developing conservation strategies and agro-technologies for cultivation.In this communication, we have discussed the habitat range of two alpine medicinal plants, Aconitum naviculare(Brühl) Stapf and Neopicrorhiza scrophulariiflora(Pennel) Hong in a trans-Himalayan dry valley of central Nepal, Manang district.They are the most prioritized medicinal plants of the study area in terms of ethnomedicinal uses.A.naviculare occurs on warm and dry south facing slopes between 4090~4650 m asl along with sclerophyllous and thorny alpine scrubs, while N.scrophulariiflora is exclusively found on cool and moist north facing slope between 4000 and 4400 m asl where adequate water is available from snow melt to create a suitable habitat for this wetland dependent species.The soil in rooting zone of the two plants differs significantly in organic carbon(OC), organic matter(OM), total nitrogen(N) and carbon to nitrogen(C/N) ratio.Due to cool and moist condition of N.scrophulariiflora habitat, accumulation of soil OC is higher, but soil N content is lower probably due to slow release from litter, higher leaching loss and greater retention in perennial live biomass of the plant.The C/N ratio of soil is more suitable in A.navuculare habitat than that of N scrophulariiflora for N supply.Warm and sunny site with N rich soil can be suitable for cultivation of A.naviculare, while moist and cool site with organic soil for N.scrophulariiflora.The populations of both the plants are fragmented and small.Due to collection by human and tramplingdamage by livestock, the population of A.naviculare was found absent in open areas in five of the six sampling sites and it was confined only within the bushes of alpine scrubs.For N.scrophulariiflora, high probability of complete receding of small glaciers may be a new threat in future to its habitat.The information about habitat conditions, together with the information from other areas, can be useful to identify potential habitats and plan for cultivation or domestication of the two medicinal plants. | Bharat Babu SHRESTHA Pramod Kumar JHA | 2009 | Journal of Mountain Science2009,6,1: | 2 |
| 9 | Bamboo resources of Sikkim Himalaya: diversity, distribution and utilization显示文摘We recorded 8 genera and over 30 species of bamboo in Sikkim of the reported 23 genera and 125 species in India. Thirty percent of bamboo species were recorded in tropical forests(0–900 m), 40% of species in sub-tropical forests(900–1,800 m), 17% of species in temperate forests(1,800–2,700 m), 9% of species in sub-alpine forests(2,700–3,500 m) and 4% of species in alpine vegetation(3,500–4,500 m). Bamboo is used for various purposes and its versatility qualifies it to be a multiple-use alternative to timber, food for villagers and for tribal in particular. Bamboo is also equally important to Red Panda(Ailurus fulgens), the state animal of Sikkim. We develop a list of bamboos occurring in Sikkim with their vernacular names, distributions and uses in the Himalayan state. | 13 | Predicting suitable habitat of an invasive weed Parthenium hysterophorus under future climate scenarios in Chitwan Annapurna Landscape,Nepal显示文摘Chitwan-Annapuma Landscape(CHAL)in central Nepal is known for its rich biodiversity and the landscape is expected to provide corridors for species range shift in response to climate change.Environmental assessments have identified biological invasions and other anthropogenic activities as major threats to the biodiversity in the CHAL.One of the rapidly spreading Invasive Alien Plant species(IAPs)in the CHAL is Parthenium hysterophorus L.,a neotropical invasive weed of global significance.This study aimed to investigate the current and future projected suitable habitat of P.hysterophorus in the CHAL using MaxEnt modelling in three'Representative Concentration Pathways'(RCPs 2.6,4.5 and 8.5)corresponding to different greenhouse gases emissiontrajectories for the year 2050 and 2070.A total of 288species occurrence points,six bioclimatic variablesmean diurnal range,isothermality,annual precipitation,precipitation of driest month,precipitation seasonality,precipitation of driest quarter and two topographic variables(aspect and slope)were selected for MaxEnt modelling.Potential range shift in terms of increase or decline in the suitable habitat areas under the projected scenarios were calculated.Slope and annual precipitation were the most important variables that explained the current distribution of P.hysterophorus.Twenty percent of the total area of CHAL was predicted to be suitable habitat for the growth of P.hysterophorus in the current climatic condition.Highest gain in the suitable habitat of this noxious weed was found under RCP 4.5 scenario in 2050 and 2070.whereas there will be a loss in thesuitable habitat under RCP 8.5 scenario in 2050 and2070.Out of four physiographic regions present in CHAL,three regions-Siwalik,Middle Mountain and High Mountain have suitable habitat for P.hysterophorus under current climatic condition.The mountainous region is likely to be affected more than the Siwalik region by further spread of P.fhysteropfhorus in the future under low(RCP 2.6)to medium(RCP 4.5)emission scenarios.The suitable habitat for this weed is likely to increase in the protected areas of mountain regions(Langtang National Park,Annapurna Conservation Area and Manaslu Conservation Area)in the future.The results have revealed a risk of spreading P.hysterophorus from present localities to non-invaded areas in the current and future climatic condition.Such risk needs to be considered by decision makers and resource managers while planning for effective management of this weed to reduce its ecological and economic impacts in the CHAL. | MAHARJAN Seerjana SHRESTHA Bharat Babu JOSHI Mohan Dev DEVKOTA Anjana MUNIAPPAN Rangaswamy ADIGA Abhijin JHA Pramod Kumar | 2019 | Journal of Mountain Science2019,16,10: | 1 |

| 17 | Effect of Pest Exclusion Net(PEN)on the Sustainable Production of Late Season Broad Leaf Mustard(BLM)in Chitwan,Nepal显示文摘Broad leaf mustard(BLM)(Brassica juncea L.var.rugosa,Brassicaceae)is one of the most widely consumed vegetable crops in Nepal.The production of crop in open field is constrained by major pests like mustard aphid and flea beetle.This study was aimed to determine the effects of pest exclusion net(PEN)on pest-infestation and yield of BLM cultivar“Khumal Broad Leaf”.The experiment was carried out in randomized complete block design(RCBD)with five treatments and four replications.The treatments included control(no mulch+no net+no pesticide spray),black plastic mulch only,reflective plastic mulch only,black plastic mulch+no net+pesticide spray,and PEN+black plastic mulch+no pesticide spray.Data were analyzed with RSTAT software package and means were separated by Duncan’s multiple range test(DMRT)at 5%level of significance.The result revealed that crop under PEN had highest plant height(38.99 cm),lowest number of aphid(0-0.86 per plant),flea beetle(0-1.07 per plant)and highest total marketable yield(77.50 ton/ha)while control had lowest plant height(20.06 cm),highest number of aphid(maximum 2.49 per plant),flea beetle(maximum 2.00 per plant)and lowest total marketable yield(13.75 ton/ha).Economic analysis revealed highest benefit-cost ratio in PEN(9.90).This study indicates that the use of PEN protect BLM against aphid and flea beetle,increases yield and can be considered as a viable technology for BLM production by smallholder growers of Nepal. | Ram Chandra Neupane Arjun Kumar Shrestha Rajendra Regmi Kim Fooyontphanich Ananta Raj Devkota | 2018 | Journal of Agricultural Science and Technology(A)2018,8,5: | 1 |
