| 1 | Single center experience of capsule endoscopy in patients with obscure gastrointestinal bleeding显示文摘AIM:To identify optimum timing to maximize diagnostic yield by capsule endoscopy (CE) in patients with obscure gastrointestinal bleeding (OGIB).METHODS:We identified patients who underwent CE at our institution from August 2003 to December 2009.Patient medical records were reviewed to determine type of OGIB (occult,overt),CE results and complications,and timing of CE with respect to onset of bleeding.RESULTS:Out of 385 patients investigated for OGIB,284 (74%) had some lesion detected by CE.In 222 patients (58%),definite lesions were detected that could unequivocally explain OGIB.Small bowel ulcer/erosions secondary to Crohn's disease,tuberculosis or non-steroidal anti-inflammatory agent use were the commonest lesions detected.Patients with overt GI bleeding for < 48 h before CE had the highest diagnostic yield (87%).This was significantly greater (P < 0.05) compared to that in patients with overt bleeding prior to 48 h (68%),as well as those with occult OGIB (59%).CONCLUSION:We established the importance of early CE in management of OGIB.CE within 48 h of overt bleeding has the greatest potential for lesion detection. | Mahesh Kumar Goenka Shounak Majumder Sanjeev Kumar Pradeepta Kumar Sethy Usha Goenka | 2011 | World Journal of Gastroenterology2011,17,6: | 39 |
| 2 | 不同入路冠状动脉介入治疗术围手术期周围血管并发症发生率的对比显示文摘目的:比较经股动脉和桡动脉入路冠状动脉介入治疗(percutaneous coronary intervention,PCI)术中周围血管并发症的发生率,为其临床诊治提供参考。方法:分析2014年7月至2016年12月在我院心血管介入治疗中心进行冠状动脉造影及介入治疗的780例患者的临床资料,比较周围血管并发症发生情况。结果:经股动脉途径471例,经桡动脉途径309例。经股动脉途径总体并发症发生率13.2%,高于经桡动脉途径的4.5%,差异有统计学意义(P<0.001)。经股动脉途径出血并发症发生率为7.9%,高于经桡动脉途径2.6%,差异有统计学意义(P=0.002)。经股动脉途径患者重大并发症(后腹膜血肿、假性动脉瘤、动静脉瘘、动脉夹层形成)发生率高于经桡动脉途径(2.1%vs 0%),但差异无统计学意义。结论:经股动脉途径冠状动脉介入治疗术围手术期周围血管并发症发生率高于经桡动脉途径,临床入路选择时需综合考虑。 | 张树清 崔磊 祁荣兴 SANJEEV KUMAR P S 黄胜 葛莉 钱澄 | 2017 | 中国临床医学2017,24,5: | 11 |
| 3 | Pediatric hepatocellular carcinoma显示文摘Pediatric hepatocellular carcinoma (HCC) is the second common malignant liver tumor in children after hepato-blastoma. It differs from the adult HCC in the etiological predisposition, biological behavior and lower frequency of cirrhosis. Perinatally acquired hepatitis-B virus, hepa-torenal tyrosinemia, progressive familial intrahepatic cho-lestasis, glycogen storage disease, Alagille's syndrome and congenital portosystemic shunts are important predis-posing factors. Majority of children (87%) are older than 5 years of age. Following mass immunization against hepatitis-B, there has been a drastic fall in the incidence of new cases of pediatric HCC in the Asia-Pacific region. Management is targeted on complete surgical removal either by resection or liver transplantation. There is a trend towards improving survival of children transplanted for HCC beyond Milan criteria. Chemotherapeutic regi-mens do not offer good results but may be helpful for down-staging of advanced HCC. | 5 | Anti-HBc screening in Indian blood donors:Still an unresolved issue显示文摘AIM:To study the seroprevalence of antibody to hepatitis B core antigen (anti-HBc) in healthy blood donors negative for HBsAg and to evaluate whether anti-HBc detection could be adopted in India as a screening assay for HBV in addition to HBsAg. METHODS: A total of 1700 serum samples collected from HBsAg-negative healthy blood donors were tested for the presence of anti-HBc antibody (IgM + IgG). All samples reactive for anti-HBc antibody were then investigated for presence of anti-HBs and for liver function tests (LFTs). One hundred serum samples reactive for anti-HBc were tested for HBV DNA by PCR method. RESULTS: Out of 1700 samples tested, 142 (8.4%) blood samples were found to be reactive for anti-HBc. It was signif icantly lower in voluntary (6.9%) as compared to replacement donors (10.4%, P = 0.011). Seventy- two (50.7%) anti-HBc reactive samples were also reactive for anti-HBs with levels > 10 mIU/mL and 70 (49.3%) samples were non-reactive for anti-HBs, these units were labeled as anti-HBc-only. These 142 anti-HBc reactive units were also tested for liver function test. HBV DNA was detected in only 1 of 100 samples tested. CONCLUSION: Keeping in view that 8%-18% of donor population in India is anti-HBc reactive, inclusion of anti- HBc testing will lead to high discard rate. Anti-HBs as proposed previously does not seem to predict clearance of the virus. | 13 | Phytopharmacological evaluation and anti-asthmatic activity of Ficus religiosa leaves显示文摘Objective:To discuss phytopharmacological potential and anti-asthmatic activity of Ficus religiosa(F.religiosa)(L.).Methods:Fresh leaves of F.religiosa were obtained from Vastrapur Lake,Ahmedabad,and dried to obtain powder.Histamine and acetylcholine were used to guinea pigs to establish bronchospasm model.In in vivo study,the aqueous extract of F.religiosa leaves (AEFR) at doses of 150 and 300 mg/ kg was administrated to guinea pigs,and the broncho-protective activity of AEFR was compared with aminophylline at 25 mg/kg.While in in vitro study,and 10 g/mL,20 g/mL,30 g/mL of AEFRL was administrated to guinea pigs,respectively, and mast cell stabilizing activity of AEFR was compared with ketotifen at 10 g/mL.Results: In the in-vivo model,pre-treatment with aminophylline(25 mg/kg,ip.) could significantly delay the onset of histamine induced pre-convulsive dyspnea,compared with vehicle control. Administration of AEFRL(150 and 300 mg/kg,ip.) also produced significant effect on latency to develop histamine & acetylcholine induced pre-convulsive dyspnea.In the mast cell stabilizing model,AEFRL at 10,20 and 30μg/mL could significantly increase the number of intact cells. | 16 | Effect of replenishment of vitamin D on survival in patients with decompensated liver cirrhosis:A prospective study显示文摘AIM To assess the vitamin D(VD) deficiency as a prognostic factor and effect of replenishment of VD on mortality in decompensated cirrhosis. METHODS Patients with decompensated liver cirrhosis were screened for serum VD levels. A total of 101 VD deficient patients(< 20 ng/mL) were randomly enrolled in two groups: Treatment group(n = 51) and control group(n = 50). Treatment group received VD treatment in the form of intramuscular cholecalciferol 300000 IU as loading dose and 800 IU/d oral as maintenance dose along with 1000 mg oral calcium supplementation. The VD level, clinical parameters and survival of both the groups were compared for 6-mo.RESULTS Prevalence of vitamin D deficiency(VDD) in decompensated CLD was 84.31%. The mean(SD) age of the patients in the treatment group(M:F: 40:11) and control group(M:F: 37:13) were 46.2(± 14.93) years and 43.28(± 12.53) years, respectively. Baseline mean(CI) VD(ng/mL) in control group and treatment group were 9.15(8.35-9.94) and 9.65(8.63-10.7), respectively. Mean(CI) serum VD level(ng/mL) at 6-mo in control group and treatment group were 9.02(6.88-11.17) and 29(23-35), respectively. Over the period of time the VD, calcium and phosphorus level was improved in treatment group compared to control group. There was nonsignificant trend seen in greater survival(69% vs 64%; P > 0.05) and longer survival(155 d vs 141 d; P > 0.05) in treatment group compared to control group. VD level had no significant association with mortality(P > 0.05). In multivariate analysis, treatment with VD supplement was found significantly(P < 0.05; adjusted hazard ratio: 0.48) associated with survival of the patients over 6-mo. CONCLUSION VD deficiency is very common in patients of decompensated CLD. Covert communication with steganography is a skill of concealing secret information within cover object and hence shields the data theft over rapidly growing network.Recently, diverse steganography techniques using edge identification have been proposed in literature.Numerous methods however utilize certain pixels in the cover image for inserting edge information,resulting in significant deformation. The conventional edge detection method limits the deployment of edge detection in steganography as concealing the information would introduce some variations to the cover image. Hence inserting data in pixel areas recognized by existing conventional edge detection techniques like canny cannot ensure the recognition of the exact edge locations for the cover and stego images. In this paper, an Adaptive steganography method based on novel fuzzy edge identification is proposed. The method proposed is proficient of estimating the precise edge areas of a cover image and also ensures the exact edge location after embedding the secret message.
