| 1 | Effect of phosphorus fluctuation caused by river water dilution in eutrophic lake on competition between blue-green alga Microcystis aeruginosa and diatom Cyclotella sp.显示文摘Tega-numa (Lake Tega) is one of the eutrophic lakes in Japan. For the improvement of water quality in Lake Tega, the North-chiba Water Conveyance Channel was constructed in 2000, which transfer water from Tone River into the lake. After 2000, the dominant species of diatoms, mainly Cyclotella sp., have been replacing blue-green algae, mainly Microcystis aeruginosa in Lake Tega. This transition of dominant species would be due to the dilution, but the detail mechanism has not been understood yet. This study examined the relationship between phosphorus fluctuation caused by river water dilution to Lake Tega and dominance of algal species, M. aeruginosa or Cyclotella sp. based on the single-species and the mixed-species culture experiments. The single-species culture experiment showed that the half-saturation constant and uptake rate of phosphorus were one order lower and seven times higher for M. aeruginosa than those for Cyclotella sp. These findings implied that M. aeruginosa would possess a potential for the growth and survival over Cyclotella sp. in the phosphorus limited condition. The superiority of M. aeruginosa was reflected in the outcome of the mixed-species culture experiment, i.e., dominance of M. aeruginosa, even phosphorus concentration was lowered to 0.01 mg-P/L. | 2 | Biomarkers for individualized dosage adjustments in immunosuppressive therapy using calcineurin inhibitors after organ transplantation显示文摘Calcineurin inhibitors (CNIs), such as cyclosporine A and tacrolimus, are widely used immunosuppressive agents for the prevention of post-transplantation rejection and have improved 1-year graft survival rates by up to 90%. However, CNIs can induce severe reactions, such as acute or chronic allograft nephropathy, hypertension, and neurotoxicity. Because CNIs have varied bioavailabilities, narrow therapeutic ranges, and individual propensities for toxic effects, therapeutic drug monitoring is necessary for all CNIs. Identifying the genetic polymorphisms in drug-metabolizing enzymes will help to determine personalized dosage regimens for CNIs, as CNIs are substrates for CYP3A5 and P-glycoprotein (P-gp, MDR1). CNIs are often concomitantly administered with voriconazole or proton pump inhibitors (PPIs), giving rise to drug interaction problems. Voriconazole and PPIs can increase the blood concentrations of CNIs, and both are primarily metabolized by CYP2C19. Thus, it is expected that interactions between CNIs and voriconazole or PPI would be affected by CYP2C19 and CYP3A5 polymorphisms. CNI-induced acute kidney injury (AKI) is a serious complication of transplantations. Neutrophil gelatinase-associated lipocalin (NGAL) and kidney injury molecule 1 (KIM-1) are noninvasive urinary biomarkers that are believed to be highly sensitive to CNI-induced AKI. In this article, we review the adverse events and pharmacokinetics of CNIs and the biomarkers related to CNIs, including CYP3A5, CYP2C19, MDR1, NGAL, and KIM-1. | 3 | Brown carbon in the cryosphere: Current knowledge and perspective显示文摘Recently, the light-absorbing organic carbon, i.e., brown carbon(Br C), has received an increasing attention, because they could significantly absorb the solar radiation in the range of short wavelengths rather than the purely scattering effect. Br C is ubiquitous in the troposphere. It could undergo long range transport within the atmospheric circulation. After the deposition on the surface of snow or ice in the cryospheric region, as the major light absorbing impurities with black carbon and dust, Br C could reduce the snow albedo and accelerate the glacier melting. In this context, this paper summarized the current knowledge of Br C(in aerosols and snow) in the cryospheric regions including the Arctic, Antarctic,and Alpines. Although some works have been conducted in those region, the current dataset on the optical properties of Br C like Absorption?ngstr€om Exponent(AAE) and Mass Absorption Efficiency(MAE) is still limited, which hampers stimulating an accurate evaluation of its climate effects. Especially in the Himalayas and Tibetan Plateau, where very limited information concerning Br C is available. | 5 | A study on vestibular-evoked myogenic potentials via galvanic vestibular stimulation in normal people显示文摘Objectives: The aim of our study is to examine vestibular-evoked myogenic potentials(VEMPs) elicited by the galvanic vestibular stimulation in the sternocleidomastoid muscle(SCM) in healthy subjects for clinical applications of auditory neuropathy or vestibular neuropathy in the future.Methods: We enrolled sixteen healthy subjects to record the average responses of SCM to galvanic vestibular stimulation(GVS) [current 3 mA;duration 1 ms] by electromyography(EMG). SPSS 18.0 software was used to analyze the obtained data for mean and standard deviation.Results: In all healthy subjects mastoid-forehead galvanic vestibular stimulation produced a positive-negative biphasic EMG responses on SCM ipsilateral to the cathodal electrode. The latency of p13 was 11.7 ± 3.0 ms. The latency of n23 was 17.8 ± 3.4 ms. The amplitude of p13-n23 was147.0 ± 69.0 μV. The interaural asymmetry ratio(AR) of p13, n23 latency and the amplitude was respectively 0.12 ± 0.09, 0.08 ± 0.08 and0.16 ± 0.10.Discussions: Galvanic vestibular stimulation could elicit biphasic EMG responses from SCM via the vestibular nerve but not from the otolith organs. | 6 | 前庭器官的胚胎发育及前庭中枢的髓鞘形成(英文)显示文摘近年对眩晕疾病的关注程度越来越高,但多集中于临床诊疗技术而对基础研究的关注稍显不足,为此我们特邀原日本东京大学加我君孝教授撰写了关于'前庭系统胚胎发育和前庭中枢髓鞘形成'的文章。作者根据多年的临床经验和研究并在参考相关文献基础上完成此稿,本文虽是关于基础研究的文章但却具有较强的实用性,对指导临床思维具有一定帮助。为使广大读者能更好的理解作者思想,现将全文予以刊登同时做导读一篇供大家参考。伴有半规管功能下降的听力障碍儿童在出生后的前2~3年会出现粗大运动与平衡功能的发育延缓,此后患儿的多数粗大动作如头部控制、独自直立、行走和奔跑等都可以随着年龄的增长得到进一步发育。然而,在进入学龄期6岁左右其平衡功能仍存在不同程度的问题,情况比较好的患儿可以游泳,而较差的则只能在睁眼情况下保持静态平衡。尽管所有儿童都可以具有良好的运动能力,但这是由于前庭中枢代偿的结果,是基于视觉、躯体感觉和本体感觉等其他感觉传入信号的整合以及出生后小脑运动控制系统和运动皮层的成熟共同协调完成的。前庭中枢代偿在帮助先天性前庭功能下降的婴儿或儿童发展运动能力上起着重要作用,因此为了更好的理解前庭中枢代偿过程,有必要了解前庭外周器官的胚胎发育以及前庭中枢系统的髓鞘形成过程。作者分章节详细介绍了前庭外周器官,如椭圆囊、球囊、半规管的胚胎发育过程以及前庭神经节、前庭中枢系统的发育特点,重点介绍了神经发育过程中的髓鞘形成及相关研究。相对于成人前庭神经节细胞的组织病理学研究而言,胎儿和新生儿/婴儿的前庭神经节组织学细胞病理学研究很少。在对人胚胎颞骨连续切片的研究中,追踪了从第13周到第39周胎龄的前庭神经节细胞的形态改变。发现在妊娠第24周时前庭神经节细胞已成熟,但直到第39周细胞质体积仍在持续增加。同时,本文详细地阐述了髓鞘在神经冲动传导和轴突发育中的重要作用,髓鞘的形成是神经发育的最后一步,亦是决定功能发育的主要形态学参数。作者在文章中还引用了许多经典的研究成果。Flechsig(1901)首次提出中枢神经系统的髓鞘形成程度可能与其功能相关。他的贡献(1920)在于完成了一套新生儿、早产儿、足月产儿以及出生早期婴儿的脑切片(参见原文图7)。其髓鞘形成理论遵循由后向前,从低到高,由内向外的顺序发育完善。首先是传入神经髓鞘发育(感觉),其次为传出神经(运动),最后是联合神经纤维的髓鞘发育,第八对颅神经是首批形成髓鞘的神经之一。Yakovlev and Lecours(1967)提出了髓鞘周期理论(参见原文图8)。他们认为胚胎期的髓鞘形成开始于周围神经系统,此后中枢神经系统开始形成髓鞘,感觉系统的髓鞘形成先于中枢运动系统。而大脑半球内的髓鞘形成出生后才开始并可持续数十年,尤其是涉及更高级联合功能和感觉辨别区域的纤维髓鞘发育较晚。作者通过对脑核磁前庭中枢系统的研究发现,虽出生前的脑干部分已具备髓鞘形成,但与颞骨组织切片的研究结果相比,中枢前庭系统的发育是滞后于前庭终末器官发育的,即前庭终末器官在胚胎发育早期即发育成熟,而中枢前庭系统在出生后还在发育成熟中,直到1~2岁时其中枢髓鞘发育才逐渐达到成人水平。因此至学龄前随着生长发育和中枢代偿的作用,所有儿童大部分的粗大运动都可以得到良好的发育。通过对前庭神经系统发育过程的研究,使我们更好的了解了前庭系统的中枢代偿作用至关重要,而前庭中枢代偿对加速婴儿及儿童发育的运动功能具有重要的临床价值。 | Kimitaka Kaga Chenchieh Kuan 刘博 张娜 | 2008 | 中国医学文摘(耳鼻咽喉科学)2008,23,5: | 2 |
| 7 | Binaural integration abilities in bilateral cochlear implant user显示文摘Bilateral Cochlear implants(CIs) improved speech intelligibility, speech perception in background noise, and sound localization in quiet and noisy situations. However, it is unclear whether these advantages essentially result in binaural integration of acoustic stimuli from each ear. In this study, we investigated the effectiveness of binaural integration by bilateral CIs placement using binaural hearing tests and subjective auditory perceptual assessment. A 61-year-old bilateral CIs subject underwent the following four tests: the Japanese Hearing in Noise Test(HINT-J), the dichotic listening test(DLT), the Rapidly Alternating Speech Perception(RASP) test, and subjective auditory perceptual assessment. The HINTJ score was significantly higher for bilateral CIs than for a unilateral CI. However, DLT and the RASP test revealed contradictory results.Subjective auditory perceptual assessment revealed active and bright impressions for bilateral hearing, which were also noisy and strong compared with those for unilateral hearing. The results of this study revealed that bilateral CIs improved speech perception in background noise and an improved auditory impression, although the bilateral integration abilities were not improved. This was probably because the patient was required to combine information from the two ears into a single perception in DLT and the RASP test. | 9 | Validation study of postoperative liver failure and mortality risk scores after liver resection for perihilar cholangiocarcinoma显示文摘Background:Surgery for perihilar cholangiocarcinoma(PHCC)remains a challenging procedure with high morbidity and mortality.The Academic Medical Center(Amsterdam UMC)and Memorial Sloan Kettering Cancer Center proposed a postoperative mortality risk score(POMRS)and post-hepatectomy liver failure score(PHLFS)to predict patient outcomes.This study aimed to validate the POMRS and PHLFS for PHCC patients at Hokkaido University.Methods:Medical records of 260 consecutive PHCC patients who had undergone major hepatectomy with extrahepatic bile duct resection without pancreaticoduodenectomy at Hokkaido University between March 2001 and November 2018 were evaluated to validate the PHLFS and POMRS.Results:The observed risks for PHLF were 13.7%,24.5%,and 39.8%for the low-risk,intermediate-risk,and high-risk groups,respectively,in the study cohort.A receiver-operator characteristic(ROC)analysis revealed that the PHLFS had moderate predictive value,with an analysis under the curve(AUC)value of 0.62.Mortality rates based on the POMRS were 1.7%,5%,and 5.1%for the low-risk,intermediate-risk,and high-risk groups,respectively.The ROC analysis demonstrated an AUC value of 0.58.Conclusions:This external validation study showed that for PHLFS the threshold for discrimination in an Eastern cohort was reached(AUC>0.6),but it would require optimization of the model before use in clinical practice is acceptable.The POMRS were not applicable in the eastern cohort.Further external validation is recommended. | Takehiro Noji Satoko Uemura Jimme KWiggers Kimitaka Tanaka Yoshitsugu Nakanishi Toshimichi Asano Toru Nakamura Takahiro Tsuchikawa Keisuke Okamura Pim BOlthof William RJarnagin Thomas Mvan Gulik Satoshi Hirano | 2022 | Hepatobiliary Surgery and Nutrition2022,11,3: | 2 |

| 11 | A child with severe inner ear malformations with favorable hearing utilization and balance functions after wearing hearing aids显示文摘Infants with congenital deafness caused by severe bilateral inner ear malformations frequently suffer from severe hearing loss and poor balance. Unfortunately, the use of hearing aids is usually ineffective in recovering hearing, necessitating cochlear implants. We report a case of a 6-year-old boy with congenital deafness and bilateral inner ear malformations(right side, incomplete partition type I [IP-I]; left side, common cavity deformity). Hearing aids had a remarkable effect in this patient, enabling sufficient and favorable hearing recovery such as to allow the patient to engage in daily conversations. Per-rotatory nystagmus was recorded on an electronystagmogram for both right and left rotations in a damped rotational chair test. It is rare for deaf children with severe bilateral inner ear malformation to demonstrate favorable development in hearing and good equilibrium function.
