| 1 | Single-incision laparoscopic cholecystectomy:Single institution experience and literature review显示文摘Single-incision laparoscopic surgery is a rapidly evolving field as a bridge between traditional laparoscopic surgery and natural orifice transluminal endoscopic surgery.We report one of the initial clinical experiences in Japan with this new technique.Four cases of gallbladder diseases were selected for this new technique.A single curved intra-umbilical 25-mm incision was made by pulling out the umbilicus.A 12-mm trocar was placed through an open approach,and the abdominal cavity was explored with a 10-mm semiflexible laparoscope.Two 5-mm ports were inserted laterally from the laparoscope port.A 2-mm mini-loop retractor was inserted to retract the fundus of the gallbladder.Dissection was performed using an electric cautery hook and an Endograsper roticulator.There were two women and two men with a mean age of 50.5 years(range:40-61 years).All procedures were completed successfully without any perioperative complications.In all cases,there was no need to extend the skin incision.Average operative time was 88.8 min.Postoperative follow-up didnot reveal any umbili-cal wound complication.Single-incision laparoscopic cholecystectomy is feasible and a promising alternative method as scarless abdominal surgery for the treatment of some patients with gallbladder disease. | Yasumitsu Hirano Toru Watanabe Tsuneyuki Uchida Shuhei Yoshida Kanae Tawaraya Hideaki Kato Osamu Hosokawa | 2010 | World Journal of Gastroenterology2010,16,2: | 23 |
| 2 | Clinical usefulness of endoscopic ultrasonography for the evaluation of ulcerative colitis-associated tumors显示文摘AIM:To evaluate the clinical usefulness of endoscopic ultrasonography(EUS) for the diagnosis of the invasion depth of ulcerative colitis-associated tumors.METHODS:The study group comprised 13 patients with 16 ulcerative colitis(UC)-associated tumors for which the depth of invasion was preoperatively estimated by EUS.The lesions were then resected endoscopically or by surgical colectomy and were examined histopathologically.The mean age of the subjects was 48.2 ± 17.1 years,and the mean duration of UC was 15.8 ± 8.3 years.Two lesions were treated by endoscopic resection and the other 14 lesions by surgical colectomy.The depth of invasion of UCassociated tumors was estimated by EUS using an ultrasonic probe and was evaluated on the basis of the deepest layer with narrowing or rupture of the colonic wall.RESULTS:The diagnosis of UC-associated tumors by EUS was carcinoma for 13 lesions and dysplasia for 3 lesions.The invasion depth of the carcinomas was intramucosal for 8 lesions,submucosal for 2,the muscularis propria for 2,and subserosal for 1.Eleven(69%) of the 16 lesions arose in the rectum.The macroscopic appearance was the laterally spreading tumor-non-granular type for 4 lesions,sessile type for 4,laterally spreading tumor-granular type for 3,semipedunculated type(Isp) for 2,type 1 for 2,and type 3 for 1.The depth of invasion was correctly estimated by EUS for 15 lesions(94%) but was misdiagnosed as intramucosal for 1 carcinoma with high-grade submucosal invasion.The 2 lesions treated by endoscopic resection were intramucosal carcinoma and dysplasia,and both were diagnosed as intramucosal lesions by EUS.CONCLUSION:EUS provides a good estimation of the invasion depth of UC-associated tumors and may thus facilitate the selection of treatment. | Kiyonori Kobayashi Kana Kawagishi Shouhei Ooka Kaoru Yokoyama Miwa Sada Wasaburo Koizumi | 2015 | World Journal of Gastroenterology2015,21,9: | 5 |
| 3 | Mid-infrared dual-comb spectroscopy with electro-optic modulators显示文摘Absorption spectroscopy of fundamental ro-vibrational transitions in the mid-infrared region provides a powerful tool for studying the structure and dynamics of molecules in the gas phase and for sensitive and quantitative gas sensing.Laser frequency combs permit novel approaches to perform broadband molecular spectroscopy.Multiplex dual-comb spectroscopy without moving parts can achieve particularly high speed,sensitivity and resolution.However,achieving Doppler-limited resolution in the mid-infrared still requires overcoming instrumental challenges.Here we demonstrate a new approach based on difference-frequency generation of frequency-agile near-infrared frequency combs that are produced using electro-optic modulators.The combs have a remarkably flat intensity distribution,and their positions and line spacings can be freely selected by simply dialing a knob.Using the proposed technique,we record,in the 3-μm region,Doppler-limited absorption spectra with resolved comb lines within milliseconds,and precise molecular line parameters are retrieved.Our technique holds promise for fast and sensitive time-resolved studies of,for example,trace gases. | Ming Yan Pei-Ling Luo Kana Iwakuni Guy Millot Theodor W Hänsch Nathalie Picqué | 2017 | Light(Science & Applications)2017,6,1: | 5 |

| 9 | A model to estimate available timber and forest biomass and reforestation expenses in a mountainous region in Japan显示文摘We developed a model to estimate supply potentials and available amounts of timber and forest biomass resources from profitable sub-compartments of thinning and final felling operations. Economic balances were estimated while considering not only harvesting expenses but also reforestation expenses after final felling, which should be considered for sustainable forest management. Harvesting expenses were estimated based on two types of timber harvesting systems and three types of forest biomass harvesting systems in each sub-compartment. Then, the model was applied to Nasushiobara city of Tochigi prefecture, Japan. Reforestation expenses had large negative impacts on the financial balances of final felling operations. Few sub-compartments were profitable after considering reforestation expenses. Most profitable sub-compartments were those with mechanized operation systems and landing sales. These accounted for 17.19% of all sub-compartments, while only 5.75% of the sub-compartments were profitable based on their current operation systems and landing sales. Although the overall supply potentials of timber and forest biomass resources were 380,000 m3 and 210,000 Mg, respectively, and 15 times the planned harvest of coniferous tree volume of 25,000 m3year-1 and 50 times the annual demand for the woody gasification power generation of 4,000 Mg year-1 in Nasushiobara, available amounts of timber and forest biomass resources were only49,429 m3 and 33,333 Mg, which were 13.0% and 15.7% of supply potentials for landing sales with mechanized operation systems. | Kazuhiro Aruga Ayami Murakami Chikara Nakahata Reiko Yamaguchi Masashi Saito Kanae Kanetsuki | 2013 | Journal of Forestry Research2013,24,2: | 2 |
