1Striking elevation in incidence and prevalence of inflammatory bowel disease in a province of western Hungary between 1977-2001显示文摘AIM:An investigation into inflammatory bowel disease and colorectal cancer in Veszprem Province was conducted from 1977 to 2001.METHODS: Both hospital and outpatient records were collected and reviewed comprehensively. The majority of patients were followed up regularly.RESULTS:The population of the province was decreased from 386000 to 376000 during the period. Five hundred sixty new cases of ulcerative colitis (UC), 212 of Crohn's disease (CD), and 40 of indeterminate colitis (IC) were diagnosed. The incidence rates increased from 1.66 to 11.01 cases per 100 000 persons for UC, from 0.41 to 4.68 for CD and from 0.26 to 0.74 for IC. The prevalence rate at the end of 2001 was 142.6 for UC and 52.9 cases per 100 000 persons for CD. The peak onset age in UC patients was between 30 and 40 years, in CD between 20 and 30 years. A family history of IBD was present in 3.4% in UC and 9.9% in CD patients.Smoking increased the risk for CD (OR=1.94) while it decreased the risk for UC (OR=0.25). Twelve colorectal carcinomas were observed in this cohort, the cumulative colorectal cancer risk after 10 years in UC was 2%, after 20 years 8.8%, after 30 years 13.3%.CONCLUSION:The incidence and prevalence rates of IBD have increased steadily in Veszprem Province, now equivalent to that in Western European countries. Rapid increase in incidence rates supports a probable role for environmental factors. 2儿童肱骨髁上骨折模型克氏针固定构型的三维有限元分析显示文摘目的:建立带骺软骨儿童肱骨远端新力学模型,模拟肱骨髁上骨折并进行三维有限元分析,研究克氏针的数量、针道、出口高度及穿针构型对固定稳定性的影响。方法:于图片存档及通信系统(picture archiving and communications system,PACS)中挑选1名6岁男孩的正常肱骨远端三维CT,将图像数据导入Simpleware及SolidWorks 2016软件,构建1个含有肱骨小头骨化中心及远端骺软骨的儿童肱骨远端模型。制作常见伸直型儿童肱骨髁上骨折模型,并模拟交叉克氏针及单纯外侧克氏针的多种固定构型,在屈伸、内外翻方向施加30 N,内外旋方向施加5 N的力,通过远折端的位移情况分析其稳定性。结果:2枚克氏针固定构型中,出口在骨干-干骺端交界区(metaphyseal-diaphyseal junction,MDJ)上缘的2枚交叉针最稳定,可对抗超过2585 Nmm/°的旋转应力;而在对抗屈伸及内外翻应力测试中,低位针经过肱骨小头骨化中心的2枚外侧针最稳定,分别可对抗接近45 N/mm及190 N/mm的应力。在最稳定的外侧2枚克氏针构型中加入第3根针,所有方向上的稳定性显著增加,其中3枚交叉针最稳定,屈伸、内外翻及内外旋时分别可对抗198 N/mm、395 N/mm及6251 Nmm/°的应力。结论:2枚外侧分散针可为儿童肱骨髁上骨折提供足够的稳定性。在2枚交叉针构型中,出口高度在MDJ区域上缘的交叉针最稳定。3枚交叉针构型在所有研究构型中最稳定。刘祥飞 Allieu Kamara 刘闯 王恩波 季相禄 刘天婧 李麒麟 2020中国骨伤2020,33,10:8
3金堆城露天矿空气间隔装药减震控制爆破试验研究显示文摘为确保金堆城露天矿东川河隧洞的安全,采用空气间隔装药爆破方式来控制爆破震动。采用连续装药爆破和空气间隔装药爆破两种方式进行现场试验,分别选取离隧洞60 m、80 m、100 m、120 m、140 m的爆心距下爆破振动速度进行对比分析,得出空气间隔装药爆破试验具有明显的减震效果,减震率分别为22.76%、33.94%、27.59%、29.85%和31.91%。采用块度分析软件对爆破后的破碎效果进行分析比较,结果表明采用空气间隔装药比连续装药爆破产生的岩石块度分布均匀、平均块度和最大块度小、大块率低、破碎度高。徐文文 叶海旺 张迎吉 王永康 王建军 艾晓宗 兰振 张建华 Alieu Kamara 2013爆破2013,30,3:7
4儿童肱骨远端骨干干骺端交界区骨折不同固定方法生物力学的比较显示文摘目的比较儿童肱骨远端骨干干骺端交界区骨折不同固定方法的生物力学稳定性。方法采用复合人工骨36根,模拟制作高、中、低位骨干-干骺端交界区横行骨折线,对不同骨折模型分别行克氏针、单臂外固定架和弹性髓内钉固定,并将模型骨安装在计算机控制万能试验机上进行屈曲、伸直、外翻、内翻应力试验,将模型安装在机电扭力试验机上,进行内旋转、外旋转应力试验,比较不同固定方法力学稳定性。结果高位骨折模型采用弹性髓内针固定力学稳定性最佳,中位骨折采用3枚交叉克氏针(1枚内侧克氏针和2枚外侧克氏针)固定力学稳定性最佳,低位骨折采用枚交叉克氏针(1枚内侧克氏针和2枚外侧克氏针)固定力学稳定性最佳。结论弹性髓内钉固定对儿童肱骨远端骨干干骺端交界区上半区(高、中位骨折线区间)骨折的整体稳定性最好,而克氏针固定在稳定下半区(中、低位骨折线区间)骨折方面稳定性最好。2枚外侧针固定、1枚内侧针固定可以获得最佳的力学稳定性。李麒麟 Allieu Kamara 王恩波 刘天婧 李建军 2020中国骨与关节损伤杂志2020,35,9:7

7HPV16 E5蛋白免疫优势肽段分析及其多克隆抗体的制备显示文摘目的预测人乳头瘤病毒16型(human papillomavirus 16,HPV16)E5蛋白的免疫优势肽段,并制备其多克隆抗体。方法以人HPV16 E5蛋白全长氨基酸序列为基础,采用Hopp&Woods的亲水性方案、Zimmerman极性参数、Jameson-Wolf抗原指数方案和Emini表面可及性方案及抗原性指数(antigenic index,AI)等生物信息学方法,综合分析E5蛋白免疫优势肽段。通过免疫日本大耳白兔进行E5蛋白免疫优势肽段的免疫原性验证并制备多克隆抗体;进一步采用ELISA法和间接免疫荧光试验分别检测多克隆抗体的效价及特异性。结果HPV16 E5蛋白N-端1~17氨基酸序列(MTNLDTASTTLLACFLL)为免疫优势肽段,可诱导兔产生高水平的E5蛋白特异性IgG抗体,抗体滴度随免疫次数增加而升高;间接细胞免疫荧光试验结果显示,制备的多克隆抗体可特异性识别TC-1-E5细胞胞浆内表达的E5蛋白。结论HPV16 E5蛋白氨基酸肽段(MTNLDTASTTLLACFLL)具有良好的免疫原性;成功制备了HPV16 E5蛋白多克隆抗体,为进一步研究HPV16 E5蛋白的生物学特性奠定了基础。唐婉林 王路得 顾美萍 Kamara SAIDU 汪琪 李明洋 陈韶 张丽芳 2021中国生物制品学杂志2021,34,6:4
8发育性髋关节脱位闭合复位新判定方法的初步探讨显示文摘目的探讨术中髋关节造影结合再脱位角对2岁以内发育性髋关节脱位(developmental dislocation of the hip,DDH)患儿治疗的影响及新判定方法的可行性。方法回顾性分析2013年7月至2016年12月收治的符合条件的53例(56髋)发育性髋关节脱位患儿相关资料。髋关节脱位程度按国际髋关节发育不良协会(International Hip Dysplasia Institute,IHDI)提出的IHDI分型标准分型:Ⅱ型6髋,Ⅲ型27髋,Ⅳ型23髋。按Bowen造影标准判断手术方式,其中行闭合复位25例26髋(闭合复位组),闭合复位失败行单纯切开复位28例30髋(切开复位组)。尝试采用再脱位角≤35°、股骨头位于盂唇外缘下方及"安全带征"阳性的新DDH闭合复位成功判定方法与Bowen标准进行一致性检验,判断其临床可行性,并应用Salter标准判断有无股骨头缺血性坏死(avscular necrosis,AVN)。结果闭合复位组24髋(92.3%)"安全带征"阳性且股骨头位于盂唇外缘下方,再脱位角为18.4°±14.0°;1髋盂唇严重内翻,1髋"安全带征"阴性。切开复位组28髋(93.3%)符合切开复位标准,再脱位角为46.7°±13.4°,其中26髋再脱位角≥35°,2髋"安全带征"阴性;另2髋符合闭合复位新判定方法,复位成功。新方法与Bowen标准一致性检验结果Kappa值最小为0.858(P〈0.01),组内最小ICC值为0.867(P〈0.001)。随访超过一年的39髋中,有4髋(10.3%)股骨头坏死。随访超过2年的16例患儿中,有2例(12.5%)患儿在MRI上出现股骨头密度不均,但股骨头形态尚未出现改变;9例(56.3%)患儿出现盂唇软骨复合体密度不均,全部患儿盂唇呈现持续内翻状态。结论2岁以内患儿新闭合复位成功定性判定方法更有利于术中迅速判断手术方案,可行性佳。贾国强 王恩波 Allieu Kamara 史立伟 张立军 2018中华小儿外科杂志2018,39,3:4
9新型冠状病毒E蛋白结构与免疫优势表位的信息学预测分析显示文摘目的:应用生物信息学方法预测和分析新型冠状病毒(SARS-CoV-2)包膜蛋白(E蛋白)的结构及免疫优势表位。方法:从NCBI GenBank数据库中检索SARS-CoV-2 E蛋白的氨基酸序列,通过Expas y服务器上的软件分析预测其理化性质、二级结构、跨膜区域;并结合其亲水性、表面可及性、极性、抗原性和柔韧性等综合分析,预测其可能的B细胞表位;利用Net CTL1.2 server预测CTL表位;通过Vector NTI对冠状病毒属E蛋白进行同源性分析;同时应用Swiss Model对E蛋白进行三维结构同源建模及功能结构域预测分析。结果:SARS-CoV-2 E蛋白由75个氨基酸残基组成,为跨膜蛋白,跨膜区域以α螺旋为主;E蛋白可能含有2个B细胞表位和6个CTL表位,其免疫优势表位区域分别位于其N端16-34aa和C端50-70aa区段;SARS-CoV-2与SARS-CoV和Bat-SARS-like-CoV E蛋白的免疫优势表位存在极高的同源性;其疏水性跨膜结构和PDZ结合基序分别位于N端12-34aa和C端72-75aa区段。结论:E蛋白为跨膜蛋白,其氨基酸序列的16-34和50-70区段可能为免疫优势表位。李明洋 SAIDU Kamara 汪琪 郭艳茹 李青峰 朱珊丽 陈俊 蒋朋飞 张丽芳 2021温州医科大学学报2021,51,3:3
10Serological Investigation of Laboratory-Confirmed and Suspected Ebola Virus Disease Patients During the Late Phase of the Ebola Outbreak in Sierra Leone显示文摘This study aimed to investigate the serological characteristics of Ebola virus(EBOV) infection during the late phase of the Ebola outbreak in Sierra Leone. In total, 877 blood samples from 694 suspected Ebola virus disease(EVD) cases assessed from March to December 2015, were analyzed via real-time reverse transcription polymerase chain reaction(RT-PCR) for viral RNA and enzyme-linked immunosorbent assay(ELISA) and Luminex to detect antibodies against EBOV. Viral load and EBOV-specific IgM/IgG titers displayed a declining trend during March to December 2015. Viral RNA load decreased rapidly at earlier stages after disease onset, while EBOV-specific IgM and IgG still persisted in 58.1%(18/31) and 93.5%(29/31) of the confirmed EVD patients and in 3.8%(25/663) and 17.8%(118/663) of the RNA-negative suspected patients in the later phase, respectively. 12Next-generation Sequencing Study of Pathogens in Serum from Patients with Febrile Jaundice in Sierra Leone显示文摘Objective People in Western Africa suffer greatly from febrile jaundice, which is caused by a variety of pathogens. However, yellow fever virus(YFV) is the only pathogen under surveillance in Sierra Leone owing to the undeveloped medical and public health system there. Most of the results of YFV identification are negative. Elucidation of the pathogen spectrum is required to reduce the prevalence of febrile jaundice. Methods In the present study, we used Ion Torrent semiconductor sequencing to profile the pathogen spectrum in archived YFV‐negative sera from 96 patients in Sierra Leone who presented with unexplained febrile jaundice. Results The most frequently identified sequencing reads belonged to the following pathogens: cytomegalovirus(89.58%), Epstein‐Barr virus(55.21%), hepatitis C virus(34.38%), rhinovirus(28.13%), hepatitis A virus(20.83%), coxsackievirus(10.42%), Ebola virus(8.33%), hepatitis E virus(8.33%), lyssavirus(4.17%), leptospirosis(4.17%), chikungunya virus(2.08%), Crimean‐Congo hemorrhagic fever virus(1.04%), and hepatitis B virus(1.04%).
