2Endoscopic management of difficult common bile duct stones:Where are we now?A comprehensive review显示文摘Endoscopic management for difficult common bile duct(CBD)stones still presents a challenge for several reasons,including anatomic anomalies,patients’individual conditions and stone features.In recent years,variable methods have emerged that have attributed to higher stone removal success rates,reduced cost and lower adverse events.In this review,we outline a stepwise approach in CBD stone management.As first line therapy,endoscopic sphincterotomy and large balloon dilation are recommended,due to a 30%-50%reduction of the use of mechanical lithotripsy.On the other hand,cholangioscopy-assisted lithotripsy has been increasingly reported as an effective and safe alternative technique to mechanical lithotripsy but remains to be reserved in special settings due to limited large-scale evidence.As discussed,findings suggest that management needs to be tailored to the patient’s characteristics and anatomical conditions.Furthermore,we evaluate the management of CBD stones in various surgical altered anatomy(Billroth II,Roux-en-Y and Roux-en-Y gastric bypass).Moreover,we could conclude that cholangioscopy-assisted lithotripsy needs to be evaluated for primary use,rather than following a failed management option.In addition,we discuss the importance of dissecting other techniques,such as the primary use of interventional endoscopic ultrasound for the management of CBD stones when other techniques have failed.In conclusion,we recognize that endoscopic sphincterotomy and large balloon dilation,mechanical lithotripsy and intraductal lithotripsy substantiate an indication to the management of difficult CBD stones,but emerging techniques are in rapid evolution with encouraging results.Alberto Tringali Deborah Costa Alessandro Fugazza Matteo Colombo Kareem Khalaf Alessandro Repici Andrea Anderloni 2021World Journal of Gastroenterology2021,27,44:8
3Spatial assessment of forest cover and land-use changes in the Hindu-Kush mountain ranges of northern Pakistan显示文摘Anthropogenic activities and natural processes are continuously altering the mountainous environment through deforestation, forest degradation and other land-use changes. It is highly important to assess, monitor and forecast forest cover and other land-use changes for the protection and conservation of mountainous environment. The present study deals with the assessment of forest cover and other land-use changes in the mountain ranges of Dir Kohistan in northern Pakistan, using high resolution multi-temporal SPOT-5 satellite images. The SPOT-5 satellite images of years 2004, 2007, 2010 and 2013 were acquired and classified into land-cover units. In addition, forest cover and land-use change detection map was developed using the classified maps of 2004 and 2013. The classified maps were verified through random field samples and Google Earth imagery(Quick birds and SPOT-5). The results showed that during the period 2004 to 2013 the area of forest land decreased by 6.4%, however, area of range land and agriculture land have increased by 22.1% and 2.9%, respectively. Similarly, barren land increased by 1.1%, whereas, area of snow cover/glacier is significantly decreased by 21.3%. 4Tall Buildings with Dynamic Facade Under Winds显示文摘Burgeoning growth of tall buildings in urban areas around the world is placing new demands on their performance under winds.This involves selection of the building form that minimizes wind loads and structural topologies that efficiently transfer loads.Current practice is to search for optimal shapes,but this limits buildings with static or fixed form.Aerodynamic shape tailoring that consists of modifying the external form of the building has shown great promise in reducing wind loads and associated structural motions as reflected in the design of Taipei 101 and Burj Khalifa.In these buildings,corner modifications of the cross-section and tapering along the height are introduced.An appealing alternative is to design a building that can adapt its form to the changing complex wind environment in urban areas with clusters of tall buildings,i.e.,by implementing a dynamic facade.To leap beyond the static shape optimization,autonomous dynamic morphing of the building shape is advanced in this study,which is implemented through a cyber–physical system that fuses together sensing,computing,actuating,and engineering informatics.This approach will permit a building to intelligently morph its profile to minimize the source of dynamic wind load excitation,and holds the promise of revolutionizing tall buildings from conventional static to dynamic facades by taking advantage of the burgeoning advances in computational design.Fei Ding Ahsan Kareem 2020Engineering2020,6,12:3

8Rubber plunger surface texturing for friction reduction in medical syringes显示文摘Friction is a genuine issue in the use of many medical devices involving rubbery materials such as plungers in medical syringes. This paper presents a new direction for the reduction of friction in medical syringes based on surface texturing of the rubber plunger. The specimens were prepared by casting poly(vinylsiloxane)(PVS) rubber into a pre-fabricated negative template obtained by 3D printing. Friction tests were performed on a home-made test-rig. 9Robust superamphiphobic coatings with gradient and hierarchical architecture and excellent anti-flashover performances显示文摘Biomimetic superhydrophobic(SH)coatings have emerged as a promising alternative to traditional room temperature vulcanizing(RTV)silicone rubber coatings for improving the flashover strength of insulators.However,organic contamination occurs in outdoor applications and thus a superamphiphobic(SAP)surface is more desirable but not yet reported for improving flashover performance.Herein,we developed a novel anti-flashover technique by fabricating robust SAP coating with unique gradient and micro-nanoscale hierarchical architecture.The SAP coating was fabricated by sequentially spray-depositing a resin-based primer and a silica-based topcoat on substrates(i.e.,glass slides and insulators).The primer not only functions as an adhesive offering strong adhesion to the substrate but also offers a micromastoid-like structure facilitating the subsequent formation of hierarchical micro-nanostructure.The appropriate spraying pressure leads to a diffusion of the fluorocarbon-modified silica nanoparticles into the primer to form a unique gradient structure,by analogy to inserting bullets into a wood.These features render the SAP coating excellent robustness with strong abrasive resistance,excellent ultraviolet(UV)resistance,and excellent chemical and thermal stability.Pollution flashover property of the SAP coating was explored and compared with that of SH and RTV specimens,from which a novel organic-contamination model to evaluate the flashover performance was proposed.The coated SAP glass insulator demonstrated 42.9%pollution flashover voltage improvement than RTV-coated insulator.These stated unique features reveal the convincing potential of the present SAP coatings to be applied for not only outdoor transmission line insulators for antiflashover but also other fields for self-cleaning,anti-fouling,and anti-icing.Yi Xie Wei Xiong Shefiu Kareem Chuxiong Qiu Yongfei Hu Ivan PParkin Shengwu Wang Huayun Wang Junwu Chen Lee Li Zhi Chen Huajun Sun Xiujian Zhao 2022Nano Research2022,15,8:2

13Isolated small bowel transplantation outcomes and the impact of immunosuppressants: Experience of a single transplant center显示文摘AIM: To investigate patient and graft outcomes in isolated small bowel transplant(SBTx) recipients and immunosuppressant induction agent impact on outcomes.METHODS: A retrospective review of the perioperative data of patients who underwent SBTx transplant during an 8-year period was conducted. The intraoperative data were: patient demographics, etiology of short gut syndrome, hemodynamic parameters, coagulation profiles, intraoperative fluid and blood products transfused, and development of post-reperfusion. The postoperative data were: hospital/intensive care unit stays, duration of mechanical ventilation, postoperative incidence of acute kidney injury, and 1-year patient and graft outcomes. The effects of the three immunosuppressant induction agents(Zenapax, Thymoglobulin, Campath) on patient and graft outcomes were reviewed.RESULTS: During the 8-year period there were 77 patients; 1-year patient and graft survival were 95% and 86% respectively. Sixteen patients received Zenapax, 22 received Thymoglobulin, and 39 received Campath without effects on patient or graft survival(P = 0.90, P = 0.14, respectively). The use of different immune induction agents did not affect the incidence of rejection and infection during the first 90 postoperative days(P = 0.072, P = 0.29, respectively). The Zenapax group received more intraoperative fluid and blood products and were coagulopathic at the end of surgery. Zenapax and Thymoglobulin significantly increased serum creatinine at 48 h(P = 0.023) and 1 wk(P = 0.001) post-transplant, but none developed renal failure or required dialysis at the end of the first year.CONCLUSION: One-year patient and graft survival were 95% and 86%, respectively.
