| 1 | Oral eplerenone for the management of chronic central serous chorioretinopathy显示文摘AIM: To examine eplerenone(Inspra, Pfizer), a mineralocorticoid receptor antagonist, as a treatment option for chronic central serous chorioretinopathy(CSCR).METHODS: A retrospective consecutive case series was conducted for patients receiving oral eplerenone for chronic CSCR. At baseline and each follow-up visit,spectral domain optical coherence tomography(SD-OCT)imaging was performed, including manual measurements of the height and diameter size of subretinal fluid. The primary outcome measure was the reduction in subretinal fluid following initiation of therapy.RESULTS: A total of 17 eyes of 13 patients treated with25 and 50 mg of oral eplerenone per day were identified.Subretinal fluid(SRF) decreased over time following eplerenone therapy(P = 0.007 and P =0.002, diameter and height respectively). Maximum SRF height decreased from a mean of 131.5 μm at baseline to 15.3 μm at day181+. SRF diameter decreased from an average of 2174.4μm at baseline to 46.9 μm at day 181 +. Log MAR visual acuity improved from 0.42(Snellen equivalent: 20/53) at baseline to 0.29(Snellen equivalent: 20/39) at day 181 +(P = 0.024). | 2 | Comparison of intravitreal ranibizumab and bevacizumab for the treatment of macular edema secondary to retinal vein occlusion显示文摘AIM: To compare the efficacy of ranibizumab and bevacizumab for macular edema due to retinal vein occlusion(RVO). ·METHODS: A retrospective study was conducted at a single academic institution. Eighty-one patients na觙ve to anti-VEGF therapy with RVO and macular edema were identified. Twenty-six eyes were treated with ranibizumab,33 eyes with bevacizumab,and 22 eyes with bevacizumab then switched to ranibizumab(crossover). The main outcome was change in visual acuity at 3 months,6 months,and final visit. ·RESULTS: The mean visual acuity improved from 20/80 to 20/40 in the ranibizumab(R) group and from 20/125 to 20/60 in the bevacizumab(B) group(P =0.66). The mean change in central subfield thickness(CST) was-186 and 212μm,respectively(P =0.69). Mean time between injections was 94±21.1d in the R group and 103.8 ±10.5d in the B group(P =0.78). In the crossover group,mean initial visual acuity was 20/125,reached 20/60 at crossover,and remained 20/60 at conclusion(P =0.91). ·CONCLUSION: Both ranibizumab and bevacizumab are effective for the treatment of RVO and appear to have similar visual and anatomic outcomes. | 18 | Corneal epithelial defects following vitreoretinal surgery:incidence and outcomes from the DISCOVER study显示文摘AIM:To investigate the incidence,risk factors,clinical course,and outcomes of corneal epithelial defects(CED)following vitreoretinal surgery in a prospective study setting.METHODS:This was a post-hoc analysis of all participants in DISCOVER intraoperative optical coherence tomography study.Subjects with CED 1 d after surgery without intraoperative corneal debridement was defined as the postoperative CED group.Subjects who underwent intraoperative debridement were defined as intraoperative debridement group.Eyes were matched 2:1 with controls(eyes without postoperative CED)for comparative assessment.The primary outcomes were the incidence of CED on postoperative day one and the incidence of required intraoperative debridement.Secondary outcomes included time to defect closure,delayed healing(>2 wk),visual acuity(VA)and presence of scarring at one year and cornea consult.RESULTS:This study included 856 eyes that underwent vitreoretinal surgery.Intraoperative corneal debridement was performed to 61(7.1%)subjects and postoperative CED developed spontaneously in 94(11.0%)subjects.Significant factors associated with postoperative CED included prolonged surgical duration(P=0.003),diabetes mellitus(P=0.04),postoperative ocular hypotension(P<0.001).Prolonged surgical duration was associated with intraoperative debridement.Delayed defect closure time(>2 wk)was associated with corneal scar formation at the end of the 1 y in all epithelial defect subjects(P<0.001).The overall rate of corneal scarring for all eyes undergoing vitrectomy was 1.8%.CONCLUSION:Prolonged duration of surgery is the strongest factor associated with both intraoperative debridement and spontaneous postoperative CED.Delayed defect closure is associated with a greater risk of corneal scarring at one year.The overall rate of corneal scarring following vitrectomy is low at<2%. | Kubra Sarici Daniel Petkovsek Alison Martin Alex Yuan Jeffrey M.Goshe Sunil K.Srivastava Jamie L.Reese Justis P.Ehlers | 2022 | International Journal of Ophthalmology(English edition)2022，15，1： | 1 |
