2室内氡与肺癌关系研究显示文摘目的 直接调查居室水平氡浓度对居民肺癌危险度的影响 ,并与由矿工受照结果推导的结论相比较。方法 在甘肃省平凉、庆阳两地区进行病例 对照研究。该地区居民稳定而室内氡水平较高。肺癌病例必须是在 1994年 1月至 1998年 4月期间诊断 ,年龄在 30~ 75岁之间 ,居住在平凉和庆阳两地区的居民。根据人口调查资料随机选取对照 ,对照要在年龄、性别、地区方面与病例相匹配。在过去 5~ 30年期间 ,凡住过两年以上的房间都布放了氡探测器。氡测量涵盖了 77%的照射周期。结果 病例居室的平均氡浓度为 2 30 4Bq m3 (n =76 8) ,对照为 2 2 2 2Bq m3 (n =16 5 9)。居民肺癌危险度随室内氡水平的增高有所上升 (P <0 0 0 1)。根据线性模型 ,对全部研究个体而言 ,在 10 0Bq m3 情况下 ,附加比值比 (EOR)为 0 19(95 %CI:0 0 5 ,0 47)。对测量涵盖了 10 0 %照射周期的个体 ,为 0 31(95 %CI:0 10 ,0 81)。在对照射的不确定度进行调整后 ,估算值可增加70 %。结论 研究结果证明室内水平氡照射会增加肺癌危险度 。王作元 Lubin JH 王陇德 张守志 Boice JD Jr 崔宏星 张淑蓉 Conrath SC 夏英 尚兵 Brenner A 雷苏文 Metayer KC 曹吉生 Chen KW 雷淑杰 Kleinerman RA 2002中华放射医学与防护杂志2002,22,3:28

6Implication of altered proteasome function in alcoholic liverinjury显示文摘The proteasome is a major protein-degrading enzyme, which catalyzes degradation of oxidized and aged proteins, signal transduction factors and cleaves peptides for antigen presentation. Proteasome exists in the equilibrium of 26S and 20S particles. Proteasome function is altered by ethanol metabolism, depending on oxidative stress levels: low oxidative stress induces proteasome activity, while high oxidative stress reduces it. The proposed mechanisms for modulation of proteasome activity are related to oxidative modification of proteasomal proteins with primary and secondary products derived from ethanol oxidation. Decreased proteolysis by the proteasome results in the accumulation of insoluble protein aggregates, which cannot be degraded by proteasome and which further inhibit proteasome function. Mallory bodies, a common signature of alcoholic liver diseases, are formed by liver cells, when proteasome is unable to remove cytokeratins. Proteasome inhibition by ethanol also promotes the accumulation of pro-apoptotic factors in mitochondria of ethanol-metabolizing liver cells that are normally degraded by proteasome. In addition, decreased proteasome function also induces accumulation of the negative regulators of cytokine signaling (I-kB and SOCS), thereby blocking cytokine signal transduction. 7Mycophenolate mofetil for drug-induced vanishing bile duct syndrome显示文摘Amoxicillin/clavulanate is associated with liver injury, mostly of a cholestatic pattern. While outcomes are usually benign, progression to cirrhosis and death has been reported. The role of immunosuppressive therapy for patients with a protracted course is unclear. We report the case of an elderly patient who developed prolonged cholestasis secondary to amoxicillin/clavulanate. Vanishing bile duct syndrome was confirmed by sequential liver biopsies. The patient responded to prednisone treatment, but could not be weaned off corticosteroids, even when azathioprine was added. Complete withdrawal of both prednisone and azathioprine was possible by using mycophenolate mofetil, an inosine monophosphate dehydrogenase inhibitor. 8急性脊髓损伤患者治疗的临床操作指南:关于MRI基线在临床治疗决策与结果预测中作用的几点建议(英文)显示文摘The objective of this guideline is to outline the role of magnetic resonance imaging(MRI) in clinical decision making and outcome prediction in patients with traumatic spinal cord injury(SCI).Methods A systematic review of the literature was conducted to address key questions related to the use of MRI in patients with traumatic SCI.This review focused on longitudinal studies that controlled for baseline neurologic status.A multidisciplinary Guideline Development Group(GDG) used this information,their clinical expertise,and patient input to develop recommendations on the use of MRI for SCI patients.Based on GRADE(Grading of Recommendation,Assessment,Development and Evaluation),a strong recommendation is worded as ' we recommend,' whereas a weaker recommendation is indicated by 'we suggest.' 9Murine lung eosinophil activation and chemokine production in allergic airway inflammation显示文摘Eosinophils play important roles in asthma and lung infections.Murine models are widely used for assessing the functional significance and mechanistic basis for eosinophil involvements in these diseases.However,little is known about tissue eosinophils in homeostasis.In addition,little data on eosinophil chemokine production during allergic airway inflammation are available.In this study,the properties and functions of homeostatic and activated eosinophils were compared.Eosinophils from normal tissues expressed costimulation and adhesion molecules B7-1,B7-2 and ICAM-1 for Ag presentation but little major histocompatibility complex(MHC)class II,and were found to be poor stimulators of T-cell proliferation.However,these eosinophils expressed high levels of chemokine mRNA including C10,macrophage inflammatory protein(MIP)-1a,MIP-1c,MIP-2,eotaxin and monocyte chemoattractant protein-5(MCP-5),and produced chemokine proteins.Eosinophil intracellular chemokines decreased rapidly with concomitant surface marker downregulation upon in vitro culturing consistent with piecemeal degranulation.Lung eosinophils from mice with induced allergic airway inflammation exhibited increased chemokines mRNA expression and chemokines protein production and upregulated MHC class II and CD11c expression.They were also found to be the predominant producers of the CCR1 ligands CCL6/C10 and CCL9/MIP-1c in inflamed lungs.Eosinophil production of C10 and MIP-1c correlated with the marked influx of CD11bhigh lung dendritic cells during allergic airway inflammation and the high expression of CCR1 on these dendritic cells(DCs).The study provided baseline information on tissue eosinophils,documented the upregulation of activation markers and chemokine production in activated eosinophils,and indicated that eosinophils were a key chemokine-producing cell type in allergic lung inflammation.C Edward Rose Jr Joanne A Lannigan Paul Kim James J Lee Shu Man Fu Sun-sang J Sung 2010Cellular & Molecular Immunology2010,7,5:6
10Soil boron fertilization:The role of nutrient sources and rootstocks in citrus production显示文摘Boron(B) is a key element for citrus production, especially in tropical regions, where the nutrient availability is commonly low in the soil. In addition, information about doses, fertilizer sources, methods of application, and particularly, differential nutrient demand of scion/rootstock combinations are required for efficient fertilization of commercial groves. In a non-irrigated sweet orange orchard(cv. Natal), grafted onto Rangpur lime, Swingle citrumelo or Sunki mandarin, we studied the application of two sources of B: boric acid(17% B, soluble in water) and ulexite(12% B, partially soluble in water) at four levels of supply(control without B, and soil application of 2, 4 and 6 kg ha^(-1) yr^(-1) of B). The experiment was carried out for three years(2004-2006). Boron availability in the soil and concentration in the leaves, as well as the fruit yield and quality of trees were evaluated. Soil B extracted with hot water and total leaf B positively correlated with doses of the nutrient applied to the trees. Levels of B in the soil and in the leaves did not vary with fertilizer sources. Fruit yield of trees grafted onto Rangpur lime and Swingle citrumelo was more responsive to B doses than those grafted onto Sunki mandarin. The maximum fruit yield of trees grafted onto Swingle was obtained with 3.2 kg ha^(-1) yr^(-1) of B, and leaf B level of 280 mg kg^(-1) that point out to a highest demand for B when this combination was compared with other rootstocks. Furthermore, fertilization with B did not affect the quality of fruits, but correlated with B and potassium(K) concentrations in the leaves. 12视神经脊髓炎IgG刺激原代培养大鼠星形胶质细胞免疫反应显示文摘视神经脊髓炎(NMO)主要累及星形胶质细胞,导致中枢神经系统炎症、脱髓鞘和组织损伤。脑内病灶常高表达水通道蛋白4(AQP4),AQP4被认为是NMO血清IgG的抗原目标。根据NMO的可逆性及对NMO病灶演变特点的分析,笔者推测NMO IgG可能诱导了星形胶质细胞的动态免疫过程。培养原代大鼠星形胶质细胞,加入从NMO患者血清中提取或纯化的IgG,星形胶质细胞出现炎性增殖,同时基因表达出现与之匹配的明显变化。且星形胶质细胞反应因子脂质运载蛋白-2、多种趋化因子、细胞因子、应激反应因子的表达均增加。而在培养的原代星形胶质细胞中加入从正常对照者血清中提取的IgG则无上述变化。此星形胶质细胞的反应还与疾病种类相关。从复发缓解型多发硬化、Sj觟gren's或系统性红斑狼疮患者血清中提取的IgG加入培养的原代星形胶质细胞中也不能导致上述变化。笔者推测是NMO IgG与AQP4结合,导致星形胶质细胞出现炎症反应,并出现基因在转录和翻译水平的相应变化。抑制星形胶质细胞的炎症反应可能有助于减少星形胶质细胞增殖,缓解NMO病灶的发展。Howe CL Kaptzan T Magaa SM Ayers-Ringler JR LaFrance-Corey RG Lucchinetti CF 2014神经损伤与功能重建2014,9,3:4
13Effect of Helicobacter pylori cdrA on interleukin-8 secretions and nuclear factor kappa B activation显示文摘AIM:To investigate genetic diversity of Helicobacter pylori (H.pylori) cell division-related gene A (cdrA) and its effect on the host response.METHODS:Inactivation of H.pylori cdrA,which is involved in cell division and morphological elongation,has a role in chronic persistent infections.Genetic property of H.pylori cdrA was evaluated using polymerase chain reaction and sequencing in 128 (77 American and 51 Japanese) clinical isolates obtained from 48 and 51 patients,respectively.Enzyme-linked immunosorbent assay was performed to measure interleukin-8 (IL-8) secretion with gastric biopsy specimens obtained from American patients colonized with cdrA-positive or-negative strains and AGS cells cocultured with wild-type HPK5 (cdrA-positive) or its derivative HPKT510 (cdrA-disruptant).Furthermore,the cytotoxin-associated gene A (cagA) status (translocation and phosphorylation) and kinetics of transcription factors [nuclear factor-kappa B (NF-κB) and inhibition kappa B] were investigated in AGS cells co-cultured with HPK5,HPKT510 and its derivative HPK5CA (cagA disruptant) by western blotting analysis with immunoprecipitation.RESULTS:Genetic diversity of the H.pylori cdrA gene demonstrated that the cdrA status segregated into two categories including four allele types,cdrA-positive (allele types;Ⅰand Ⅱ) and cdrA-negative (allele types;Ⅲ and Ⅳ) categories,respectively.Almost all Japanese isolates were cdrA-positive (Ⅰ:7.8% and Ⅱ:90.2%),whereas 16.9% of American isolates were cdrA-positive (Ⅱ) and 83.1% were cdrA-negative (Ⅲ:37.7% and Ⅳ:45.5%),indicating extended diversity of cdrA in individual American isolates.Comparison of each isolate from different regions (antrum and corpus) in the stomach of 29 Americans revealed that cdrA status was identical in both isolates from different regions in 17 cases.However,12 cases had a different cdrA allele and 6 of them exhibited a different cdrA category between two regions in the stomach.Furthermore,in 5 of the 6 cases possessing a different cdrA category,cdrA-negative isolate existed in the corpus,suggesting that cdrA-negative strain is more adaptable to colonization in the corpus.IL-8 secretions from AGS revealed that IL-8 levels induced by a cdrA-disrupted HPKT510 was significantly lower (P < 0.01) compared to wildtype HPK5:corresponding to 50%-60% of those of wild-type HPK5.These data coincided with in vivo data that an average value of IL-8 in biopsy specimens from cdrA-positive and cdrA-negative groups was 215.6 and 135.9 pg/mL,respectively.Western blotting analysis documented that HPKT510 had no effect on CagA translocation and phosphorylation,however,nuclear accumulation of NF-κB was lower by HPKT510 compared to HPK5.CONCLUSION:Colonization by a cdrA-negative or cdrA-dysfunctional strain resulted in decreased IL-8 production and repression of NF-κB,and hence,attenuate the host immunity leading to persistent infection.Hiroaki Takeuchi Ya-Nan Zhang Dawn A Israel Richard M Peek Jr Mikio Kamioka Hideo Yanai Norihito Morimoto Tetsuro Sugiura 2012World Journal of Gastroenterology2012,18,5:3
14Animal models of ex vivo lung perfusion as a platform for transplantation research显示文摘Ex vivo lung perfusion(EVLP) is a powerful experimental model for isolated lung research. EVLP allows for the lungs to be manipulated and characterized in an external environment so that the effect of specific ventilation/perfusion variables can be studied independent of other confounding physiologic contributions. At the same time,EVLP allows for normal organ level function and real-time monitoring of pulmonary physiology and mechanics. As a result,this technique provides uniqueadvantages over in vivo and in vitro models. Small and large animal models of EVLP have been developed and each of these models has their strengths and weaknesses. 16Quality of life, work productivity impairment and healthcare resources in inflammatory bowel diseases in Brazil显示文摘BACKGROUND Inflammatory bowel diseases(IBD)have been associated with a low quality of life(QoL)and a negative impact on work productivity compared to the general population.Information about disease control,patient-reported outcomes(PROs),treatment patterns and use of healthcare resources is relevant to optimizing IBD management.AIM To describe QoL and work productivity and activity impairment(WPAI),treatment patterns and use of healthcare resources among IBD patients in Brazil.METHODS A multicenter cross-sectional study included adult outpatients who were previously diagnosed with moderate to severe Crohn’s disease(CD)or ulcerative colitis(UC).At enrolment,active CD and UC were defined as having a Harvey Bradshaw Index≥8 or a CD Activity Index≥220 or calprotectin>200μg/g or previous colonoscopy results suggestive of inadequate control(per investigator criteria)and a 9-point partial Mayo score≥5,respectively.The PRO assessment included the QoL questionnaires SF-36 and EQ-5D-5L,the Inflammatory Bowel Disease Questionnaire(IBDQ),and the WPAI questionnaire.Information about healthcare resources and treatment during the previous 3 years was collected from medical records.Chi-square,Fisher’s exact and Student’s t-/Mann-Whitney U tests were used to compare PROs,treatment patterns and the use of healthcare resources by disease activity(α=0.05).RESULTS Of the 407 patients in this study(CD/UC:64.9%/35.1%,mean age 42.9/45.9 years,54.2%/56.6%female,38.3%/37.1%employed),44.7%/25.2%presented moderate-to-severe CD/UC activity,respectively,at baseline.Expressed in median values for CD/UC,respectively,the SF-36 physical component was 46.6/44.7 and the mental component was 45.2/44.2,the EQ-visual analog scale score was 80.0/70.0,and the IBDQ overall score was 164.0/165.0.Moderate to severe activity,female gender,being unemployed,a lower educational level and lower income were associated with lower QoL(P<0.05).Median work productivity impairment was 20%and 5%for CD and UC patients,respectively,and activity impairment was 30%,the latter being higher among patients with moderate to severe disease activity compared to patients with mild or no disease activity(75.0%vs 10.0%,P<0.001).For CD/UC patients,respectively,25.4%/2.8%had at least one surgery,38.3%/19.6%were hospitalized,and 70.7%/77.6%changed IBD treatment at least once during the last 3 years.The most common treatments at baseline were biologics(75.3%)and immunosuppressants(70.9%)for CD patients and 5-ASA compounds(77.5%)for UC patients.CONCLUSION Moderate to severe IBD activity,especially among CD patients,is associated with a substantial impact on QoL,work productivity impairment and an increased number of IBD surgeries and hospitalizations in Brazil.Rogerio Serafim Parra Julio MF Chebli Heda MBS Amarante Cristina Flores Jose ML Parente Odery Ramos Milene Fernandes Jose JR Rocha Marley R Feitosa Omar Feres Antonio S Scotton Rodrigo B Nones Murilo M Lima Cyrla Zaltman Carolina D Goncalves Isabella M Guimaraes Genoile O Santana Ligia Y Sassaki Rogerio S Hossne Mauro Bafutto Roberto LK Junior Mikaell AG Faria Sender J Miszputen Tarcia NF Gomes Wilson R Catapani Anderson A Faria Stella CS Souza Rosana F Caratin Juliana T Senra Maria LA Ferrari 2019World Journal of Gastroenterology2019,25,38:2
17L-4F抑制低密度脂蛋白诱导的内皮细胞氧自由基产生显示文摘目的 探讨L - 4F ,一个载脂蛋白A -Ⅰ (apolipoproteinA -Ⅰ ,ApoA -Ⅰ )类似物 ,能否抑制低密度脂蛋白(LowDensityLipoprotein ,LDL)诱导的内皮细胞 (Endothelialcell,EC)的氧自由基 (Superoxideanion ,O2 )产生。方法 ①采用牛主动脉EC(BovineAorticEndothelialcell,BAEC)作细胞培养 ,分成 4组。第一组不作预处理 ,第二组L -4F 10 μg/ml,第三组LDL 6 .2mmol/L ,第四组LDL +L - 4F ,预处理BAEC 2 4h后 ,用超氧化物歧化酶可抑制的铁化细胞色素C还原法 ,测量A2 3187刺激状态下BAEC的O2 的产生情况。②采用成年犬的颈内动脉作组织的培养 ,分成 3组。第一组不作预处理 ,第二组LDL 6 .2mmol/L ,第三组LDL +LF ,预处理 2 4h后用Hydroethidine 10 μmol/L染色后用共聚焦显微镜测量组织EC的O2 的产量。结果 ①LDL导致A2 3187刺激状态下的BAECO2 产生增加 ,L - 4F可以抑制LDL导致的BAECO2 产生增加 ,而L - 4F本身对BAEC的O2 产生无影响。②LDL导致的血管组织O2 的产生增加 ,L - 4F可抑制LDL导致的血管组织O2 的产生增加。结论 L - 4F可以明显抑制LDL诱导的ECO2 产生增加 ,为将来寻找预防动脉粥样硬化形成的方法提供新的线索和理论依据。区景松 欧志君 Kirkwood A Pritchard Jr 2004中国心血管病研究2004,2,10:2
18Impaired swallowing mechanics of post radiation therapy head and neck cancer patients: A retrospective videofluoroscopic study显示文摘AIM: To determine swallowing outcomes and hyolaryngeal mechanics associated with post radiation therapy head and neck cancer(rt HNC) patients using videofluoroscopic swallow studies. METHODS: In this retrospective cohort study, video-fluoroscopic images of rt HNC patients(n = 21) were compared with age and gender matched controls(n = 21). Penetration-aspiration of the bolus and bolus residue were measured as swallowing outcome variables. Timing and displacement measurements of the anterior and posterior muscular slings elevating the hyolaryngeal complex were acquired. Coordinate data of anatomical landmarks mapping the action of the anterior muscles(suprahyoid muscles) and posterior muscles(long pharyngeal muscles) were used to calculate the distance measurements, and slice numbers were used to calculate time intervals. Canonical variate analysis with post-hoc discriminant function analysis was performed on coordinate data to determine multivariate mechanics of swallowing associated with treatment. Pharyngeal constriction ratio(PCR) was also measured to determine if weak pharyngeal constriction is associated with post radiation therapy.RESULTS: The rt HNC group was characterized by poor swallowing outcomes compared to the control group in regards to: Penetration-aspiration scale(P < 0.0001), normalized residue ratio scale(NRRS) for the valleculae(P = 0.002) and NRRS for the piriform sinuses(P = 0.003). Timing and distance measurements of the anterior muscular sling were not significantly different in the two groups, whereas for the PMS time of displacement was abbreviated(P = 0.002) and distance of excursion was reduced(P = 0.02) in the rt HNC group. A canonical variate analysis shows a significant reduction in pharyngeal mechanics in the rt HNC group(P < 0.0001). The PCR was significantly higher in the test group than the control group(P = 0.0001) indicating reduced efficiency in pharyngeal clearance. CONCLUSION: Using videofluoroscopy, this study shows rt HNC patients have worse swallowing outcomes associated with reduced hyolaryngeal mechanics and pharyngeal constriction compared with controls.William G Pearson Jr Alisa A Davidoff Zachary M Smith Dorothy E Adams Susan E Langmore 2016World Journal of Radiology2016,8,2:2
