| 1 | Comparison between endoscopic sphincterotomy vs endoscopic sphincterotomy associated with balloon dilation for removal of bile duct stones:A systematic review and meta-analysis based on randomized controlled trials显示文摘AIM To compare gallstones removal rate and incidence of bleeding, pancreatitis, use of mechanical lithotripsy, cholangitis and perforation between isolated sphincterotomy vs sphincterotomy associated with balloon dilation of papilla in choledocholithiasis through the meta-analysis of randomized clinical trials. METHODS We conducted a systematic review according to the PRISMA guidelines. Literature search was restricted to randomized controlled trials(RCTs) on Med Line, Cochrane Library, LILACS, and EMBASE database platforms in July 2017. The manual search included references of retrieved articles. We extracted data focusing on outcomes: The primary endpoint was the stones removal rate; Secondary endpoints were rates of pancreatitis, bleeding, use of mechanical lithotripsy(ML), perforation and cholangitis. RESULTS Eleven RCTs with 1824 patients were included. EST was associated with more post-endoscopic retrograde cholangiopancreatography(ERCP) bleeding [FE RD-0.02, CI(-0.03,-0.00), I2 = 33%, P = 0.05] and more need of mechanical lithotripsy in general [RE RD-0.16, CI(-0.25,-0.06), I2 = 90%, P = 0.002] and in subgroup analysis of stones greater than 15 mm [RE RD-0.20, CI(-0.38,-0.02), I2 = 82%, P = 0.003]. Incidence of pancreatitis [FE RD-0.01, CI(-0.03, 0.01), I2 = 0, P = 0.36], cholangitis [FE RD-0.00, CI(-0.01, 0.01), I2 =0, P = 0.97] and perforation [FE RD-0.01, CI(-0.01, 0.00), I2 = 0, P = 0.23] was similar between the groups as well as similar stone removal rates in general [FE RD-0.01, CI(-0.01, 0.04), I2 = 0, P = 0.23] and pooled analysis of stones greater than 15 mm [FE RD-0.02, CI(-0.02, 0.07), I2 = 11%, P = 0.31]. CONCLUSION Through meta-analysis of randomized clinical trials we found that isolated sphincterotomy was associated with more post-ERCP bleeding and more need for mechanical lithotripsy. | 2 | Direct laser writing of volumetric gradient index lenses and waveguides显示文摘Direct laser writing(DLW)has been shown to render 3D polymeric optical components,including lenses,beam expanders,and mirrors,with submicrometer precision.However,these printed structures are limited to the refractive index and dispersive properties of the photopolymer.Here,we present the subsurface controllable refractive index via beam exposure(SCRIBE)method,a lithographic approach that enables the tuning of the refractive index over a range of greater than 0.3 by performing DLW inside photoresist-filled nanoporous silicon and silica scaffolds.Adjusting the laser exposure during printing enables 3D submicron control of the polymer infilling and thus the refractive index and chromatic dispersion.Combining SCRIBE’s unprecedented index range and 3D writing accuracy has realized the world’s smallest(15μm diameter)spherical Luneburg lens operating at visible wavelengths.SCRIBE’s ability to tune the chromatic dispersion alongside the refractive index was leveraged to render achromatic doublets in a single printing step,eliminating the need for multiple photoresins and writing sequences.SCRIBE also has the potential to form multicomponent optics by cascading optical elements within a scaffold.As a demonstration,stacked focusing structures that generate photonic nanojets were fabricated inside porous silicon.Finally,an all-pass ring resonator was coupled to a subsurface 3D waveguide.The measured quality factor of 4600 at 1550 nm suggests the possibility of compact photonic systems with optical interconnects that traverse multiple planes.SCRIBE is uniquely suited for constructing such photonic integrated circuits due to its ability to integrate multiple optical components,including lenses and waveguides,without additional printed supports. | Christian R.Ocier Corey A.Richards Daniel A.Bacon-Brown Qing Ding Raman Kumar Tanner J.Garcia Jorik van de Groep Jung-Hwan Song Austin J.Cyphersmith Andrew Rhode Andrea N.Perry Alexander J.Littlefield Jinlong Zhu Dajie Xie Haibo Gao Jonah F.Messinger Mark L.Brongersma Kimani C.Toussaint Jr Lynford L.Goddard Paul V.Braun | 2020 | Light(Science & Applications)2020，9，1： | 6 |
| 3 | Myelin-based inhibitors of oligodendrocyte myelination: clues from axonal growth and regeneration显示文摘The differentiation of and myelination by oligodendrocytes (oLs) are exquisitely regulated by a series of intrinsic and extrinsic mechanisms. As each oL can make differing numbers of myelin segments with variable lengths along similar axon tracts, myelination can be viewed as a graded process shaped by inhibitory/ inductive cues during development. Myelination by oLs is a prime example of an adaptive process determined by the microenvironment and architecture of the central nervous system (CNS). in this review, we discuss how myelin formation by oLs may be controlled by the heterogeneous microenvironment of the CNS. Then we address recent findings demonstrating that neighboring OLs may compete for available axon space, and highlight our current understanding of myelin-based inhibitors of axonal regeneration that are potentially responsible for the reciprocal dialogue between oLs and determine the numbers and lengths of myelin internodes. | 4 | LSTM RNN:detecting exploit kits using redirection chain sequences显示文摘While consumers use the web to perform routine activities,they are under the constant threat of attack from malicious websites.Even when visiting‘trusted’sites,there is always a risk that site is compromised,and,hosting a malicious script.In this scenario,the injected script would typically force the victim’s browser to undergo a series of redirects before reaching an attacker-controlled domain,which,delivers the actual malware.Although these malicious redirection chains aim to frustrate detection and analysis efforts,they could be used to help identify web-based attacks.Building upon previous work,this paper presents the first known application of a Long Short-Term Memory(LSTM)network to detect Exploit Kit(EK)traffic,utilising the structure of HTTP redirects.Samples are processed as sequences,where each timestep represents a redirect and contains a unique combination of 48 features.The experiment is conducted using a ground-truth dataset of 1279 EK and 5910 benign redirection chains.Hyper-parameters are tuned via K-fold cross-validation(5f-CV),with the optimal configuration achieving an F1 score of 0.9878 against the unseen test set.Furthermore,we compare the results of isolated feature categories to assess their importance. | Jonah Burgess Philip O’Kane Sakir Sezer Domhnall Carlin | 2021 | Cybersecurity2021，4，1： | 4 |
