| 2 | Noninvasive indocyanine green plasma disappearance rate predicts early complications,graft failure or death after liver transplantation显示文摘BACKGROUND:Early detection of graft malfunction or postoperative complications is essential to save patients and organs after orthotopic liver transplantation (OLT).Predictive tests for graft dysfunction are needed to enable earlier implementation of organ-saving interventions following transplantation.This study was undertaken to assess the value of indocyanine green plasma disappearance rates (ICG-PDRs) for predicting postoperative complications,graft dysfunction and patient survival following OLT.METHODS:Eighty-six patients undergoing OLT were included in this single-centre trial.ICG-PDR was assessed daily for the first 7 days following OLT.Endpoints were graft loss or death within 30 days and postoperative complications,graft loss,or death within 30 days.RESULTS:Postoperative complications of 31 patients included deaths (12 patients) or graft losses.ICG-PDR was significantly different in patients whose endpoints were graft loss or death beginning from day 3 and in those whose endpoints were graft loss,death,or postoperative complications beginning from day 4 after OLT.For day 7 measurements,receiver operating characteristic curve analysis revealed an ICG-PDR cut-off for predicting death or graft loss of 9.6% per min (a sensitivity of 75.0%,a specificity of 72.6%,positive predictive value 0.35 negative predictive value 0.94).For prediction of graft loss,death or postoperative complications,the ICG-PDR cut-off was 12.3%per min (a sensitivity of 68.9%,a specificity of 66.7%,positive predictive value 0.57,negative predictive value 0.77).CONCLUSIONS:ICG-PDR measurements on postoperative day 7 are predictive of early patient outcomes following OLT.The added value over that of routinely determined laboratory parameters is low. | Lutz Schneider Martin Spiegel Sebastian Latanowicz Markus A Weigand Jan Schmidt Jens Werner Wolfgang Stremmel Christoph Eisenbach | 2011 | Hepatobiliary & Pancreatic Diseases International2011，10，4： | 6 |
| 3 | Plexiform fibromyxoma:Review of rare mesenchymal gastric neoplasm and its differential diagnosis显示文摘Plexiform fibromyxoma(PF)is a very rare mesenchymal neoplasm of the stomach that was first described in 2007 and was officially recognized as a subtype of gastric mesenchymal neoplasm by World Health Organization(WHO)in 2010.Histologically,PF is characterized by a plexiform growth of bland spindle to ovoid cells embedded in a myxoid stroma that is rich in small vessels.The lesion is usually paucicellular.While mucosal and vascular invasion have been documented,no metastasis or malignant transformation has been reported.Its pathogenesis is largely unknown and defining molecular alterations are not currently available.There are other mesenchymal tumors arising in the gastrointestinal tract that need to be differentiated from PF given their differing biologic behaviors and malignant potential.Histologic mimics with spindle cells include gastrointestinal stromal tumor,smooth muscle tumor,and nerve sheath tumor.Histologic mimics with myxoid stroma include myxoma and aggressive angiomyxoma.Molecular alterations that have been described in a subset of PF may be seen in gastroblastoma and malignant epithelioid tumor with gliomaassociated oncogene homologue 1(GLI1)rearrangement.The recent increase in publications on PF reflects growing recognition of this entity with expansion of clinical and pathologic findings in these cases.Herein we provide a review of PF in comparison to other mesenchymal tumors with histologic and molecular resemblance to raise the awareness of this enigmatic neoplasm.Also,we highlight the challenges pathologists face when the sample is small,or such rare entity is encountered intraoperatively. | Mustafa Erdem Arslan Hua Li Zhiyan Fu Timothy A Jennings Hwajeong Lee | 2021 | World Journal of Gastrointestinal Oncology2021，13，5： | 2 |
| 4 | Resonance-enhanced multi-octave supercontinuum generation in antiresonant hollow-core fibers显示文摘Ultrafast supercontinuum generation in gas-filled waveguides is an enabling technology for many intriguing applications ranging from attosecond metrology towards biophotonics,with the amount of spectral broadening crucially depending on the pulse dispersion of the propagating mode.In this study,we show that structural resonances in a gas-filled antiresonant hollow core optical fiber provide an additional degree of freedom in dispersion engineering,which enables the generation of more than three octaves of broadband light that ranges from deep UV wavelengths to near infrared.Our observation relies on the introduction of a geometric-induced resonance in the spectral vicinity of the ultrafast pump laser,outperforming gas dispersion and yielding a unique dispersion profile independent of core size,which is highly relevant for scaling input powers.Using a krypton-filled fiber,we observe spectral broadening from 200 nm to 1.7μm at an output energy of B 23μJ within a single optical mode across the entire spectral bandwidth.Simulations show that the frequency generation results from an accelerated fission process of solitonlike waveforms in a non-adiabatic dispersion regime associated with the emission of multiple phase-matched Cherenkov radiations on both sides of the resonance.This effect,along with the dispersion tuning and scaling capabilities of the fiber geometry,enables coherent ultra-broadband and high-energy sources,which range from the UV to the mid‐infrared spectral range. | Rudrakant Sollapur Daniil Kartashov Michael Zürch Andreas Hoffmann Teodora Grigorova Gregor Sauer Alexander Hartung Anka Schwuchow Joerg Bierlich Jens Kobelke Mario Chemnitz Markus A Schmidt Christian Spielmann | 2017 | Light(Science & Applications)2017，6，1： | 2 |
| 5 | Antibiotic-loaded phosphatidylcholine inhibits staphylococcal bone infection显示文摘AIM: To test antibiotic-loaded coating for efficacy in reducing bacterial biofilm and development of osteomyelitis in an orthopaedic model of implant infection. METHODS: Phosphatidylcholine coatings loaded with 25% vancomycin were applied to washed and sterilized titanium wires 20 mm in length. A 10 mm segment was removed from rabbit radius(total = 9; 5 coated, 4 uncoated), and the segment was injected with 1 × 106 colony forming units(CFUs) of Staphylococcus aureus(UAMS-1 strain). Titanium wires were inserted throughthe intramedullary canal of the removed segment and into the proximal radial segment and the segment was placed back into the defect. After 7 d, limbs were removed, X-rayed, swabbed for tissue contamination. Wires were removed and processed to determine attached CFUs. Tissue was swabbed and streaked on agar plates to determine bacteriological score.RESULTS: Antibiotic-loaded coatings resulted in significantly reduced biofilm formation(4.7 fold reduction in CFUs; P < 0.001) on titanium wires and reduced bacteriological score in surrounding tissue(4.0 ± 0 for uncoated, 1.25 ± 0.5 for coated; P = 0.01). Swelling and pus formation was evident in uncoated controls at the 7 d time point both visually and radiographically, but not in antibiotic-loaded coatings.CONCLUSION: Active antibiotic was released from coated implants and significantly reduced signs of osteomyelitic symptoms. Implant coatings were well tolerated in bone. Further studies with additional control groups and longer time periods are warranted.
