| 1 | Intraoperative cell salvage with autologous transfusion in liver transplantation显示文摘Liver transplant(LT) is the primary treatment for patients with end-stage liver disease. About 25000 LTs are performed annually in the world. The potential for intraoperative bleeding is quite variable. However, massive bleeding is common and requires blood transfusion. Allogeneic blood transfusion has an immunosuppressive effect and an impact on recipient survival, in addition to the risk of transmission of viral infections and transfusion errors, among others.Techniques to prevent excessive bleeding or to use autologous blood have been proposed to minimize the negative effects of allogeneic blood transfusion.Intraoperative reinfusion of autologous blood is possible through previous selfdonation or blood collected during the operation. However, LT does not normally allow autologous transfusion by prior self-donation. Hence, using autologous blood collected intraoperatively is the most feasible option. The use of intraoperative blood salvage autotransfusion(IBSA) minimizes the perioperative use of allogeneic blood, preventing negative transfusion effects without negatively impacting other clinical outcomes. The use of IBSA in patients with cancer is still a matter of debate due to the theoretical risk of reinfusion of tumor cells. However, studies have demonstrated the safety of IBSA in several surgical procedures, including LT for hepatocellular carcinoma. | 2 | Cyclooxygenase-2 polymorphisms and the risk of esophageal adeno-or squamous cell carcinoma显示文摘AIM:To determine whether-1195 A→G and/or-765 G→C polymorphisms in Cyclooxygenase-2(COX-2 ) may have a risk modifying effect on the development of esophageal carcinoma in a Dutch Caucasian population.METHODS:Two study groups were recruited, 252 patients with esophageal carcinoma and 240 healthy controls, matched for race, age, gender and recruiting area.DNA was isolated from whole blood and used for genotyping.PCR products were digested with restriction enzymes and products were analyzed by agarose gel electrophoresis.Odds ratios(OR) and 95% confldence intervals(CI) were estimated.RESULTS:The distribution of the-1195 A→G polymorphism was signif icantly different in esophageal cancer patients compared to controls.The-1195 GG genotype resulted in a higher risk of developing esophageal adenocarcinoma(OR = 3.85, 95% CI:1.45-10.3) compared with the-1195 AA genotype as a reference.The-765 G→C genotype distribution was not different between the two groups.The GG/ GG haplotype was present more often in esophageal adenocarcinoma patients than in controls(OR = 3.45, 95% CI:1.24-9.58;with AG/AG as a reference).The same trends were observed in patients with squamous cell carcinomas, however, the results did not reach statistical signif icance.CONCLUSION:Presence of the COX-2-1195 GG genotype and of the GG/GG haplotype may result in a higher risk of developing esophageal carcinoma. | Jón O Kristinsson Paul van Westerveld Rene HM te Morsche Hennie MJ Roelofs T Wobbes Ben JM Witteman Adriaan CITL Tan Martijn GH van Oijen Jan BMJ Jansen Wilbert HM Peters | 2009 | World Journal of Gastroenterology2009，15，28： | 11 |
| 3 | Near-infrared fluorescence sentinel lymph node detection in gastric cancer: A pilot study显示文摘AIM: To investigate feasibility and accuracy of nearinfrared fluorescence imaging using indocyanine green: nanocolloid for sentinel lymph node(SLN) detection in gastric cancer.METHODS: A prospective, single-institution, phaseI feasibility trial was conducted. Patients suffering from gastric cancer and planned for gastrectomy were included. During surgery, a subserosal injection of 1.6 m L ICG:Nanocoll was administered around the tumor. NIR fluorescence imaging of the abdominal cavity was performed using the Mini-FLARE? NIR fluorescence imaging system. Lymphatic pathways and SLNs were visualized. Of every detected SLN, the corresponding lymph node station, signal-to-background ratio and histopathological diagnosis was determined. Patients underwent standard-of-care gastrectomy. Detected SLNs outside the standard dissection planes were also resected and evaluated.RESULTS: Twenty-six patients were enrolled. Four patients were excluded because distant metastases were found during surgery or due to technical failure of the injection. In 21 of the remaining 22 patients, at least 1 SLN was detected by NIR Fluorescence imaging(mean 3.1 SLNs; range 1-6). In 8 of the 21 patients, tumor-positive LNs were found. Overall accuracy of the technique was 90%(70%-99%; 95%CI), which decreased by higher p T-stage(100%, 100%, 100%, 90%, 0% for respectively Tx, T1, T2, T3, T4 tumors). All NIR-negative SLNs were completely effaced by tumor. Mean fluorescence signal-to-background ratio of SLNs was 4.4(range 1.4-19.8). In 8 of the 21 patients, SLNs outside the standard resection plane were identified, that contained malignant cells in 2 patients.CONCLUSION: This study shows successful use of ICG:Nanocoll as lymphatic tracer for SLN detection in gastric cancer.
