| 1 | Full Outline of Unresponsiveness score and the Glasgow Coma Scale in prediction of pediatric coma显示文摘BACKGROUND: This study was done to compare the admission Full Outline of Unresponsiveness(FOUR) score and Glasgow Coma Scale(GCS) as predictors of outcome in children with impaired consciousness.METHODS: In this observational study, children(5–12 years) with impaired consciousness of <7 days were included. Children with traumatic brain injury, on sedatives or neuromuscular blockade; with pre-existing cerebral palsy, mental retardation, degenerative brain disease, vision/hearing impairment; and seizure within last 1 hour were excluded. Primary outcomes: comparison of area under curve(AUC) of receiver operating characteristic(ROC) curve for in-hospital mortality. Secondary outcomes: comparison of AUC of ROC curve for mortality and poor outcome on Pediatric Overall Performance Category Scale at 3 months.RESULTS: Of the 63 children, 20 died during hospital stay. AUC for in-hospital mortality for GCS was 0.83(CI 0.7 to 0.9) and FOUR score was 0.8(CI 0.7 to 0.9) [difference between areas –0.0250(95%CI 0.0192 to 0.0692), Z statistic 1.109, P=0.2674]. AUC for mortality at 3 months for GCS was 0.78(CI 0.67 to 0.90) and FOUR score was 0.74(CI 0.62 to 0.87)(P=0.1102) and AUC for poor functional outcome for GCS was 0.82(CI 0.72 to 0.93) and FOUR score was 0.79(CI 0.68 to 0.9)(P=0.2377), which were also comparable. Inter-rater reliability for GCS was 0.96 and for FOUR score 0.98.CONCLUSION: FOUR score was as good as GCS in prediction of in-hospital and 3-month mortality and functional outcome at 3 months. | 2 | Advancing frontiers in anaesthesiology with laparoscopy显示文摘The introduction of laparoscopy in the surgeon's armamentarium was in fact a 'revolution in the history of surgery'. Since this technique involves insufflation of carbon dioxide it produces several pathophysiological changes which have to be understood by the anaesthesiologist who can modify the anaesthesia technique accordingly. Advantages of laparoscopy include reduced pain, small scars and early return to work. Certain complications specific to laparoscopic surgery are due to carboperitoneum and increased intra-abdominal pressure. Venous air embolism, although very rare, can be lethal if not managed promptly. Other complications include subcutaneous emphysema, haemodynamic compromise and arrhythmias. Although associated with minimal postoperative morbidity, postoperative pain, nausea and vomiting can be quite problematic. The limitations of laparoscopy have been overcome by the introduction of robotic surgery. There are important implications for the anaesthesiologist during robotic surgeries which have to be practiced accordingly. Robotic surgery has a learning curve for both the surgeon and the anaesthesiologist. The robot is bulky, and cannot be disengaged after docking. Therefore it is important that the anaesthetized patient remains immobile throughout surgery and anaesthesia is reversed only after the robot has been disengaged at the end of surgery. | 3 | Identification and characterization of genotype A and D recombinant hepatitis B virus from Indian chronic HBV isolates显示文摘AIM: To confirm the presence of recombination,full-length hepatitis B virus (HBV) from chronic patients was sequenced and analyzed.METHODS: Full-length HBV genomes from 12 patients were amplified and sequenced in an automated sequencer.Phylogenetic analysis was carried out on full-length,Core and preS2/Surface regions using MEGA software.SimPlot Boot Scanning and amino acid sequence analysis were performed for confirmation of recombination.RESULTS: Eight patients were infected with genotype D strain; one patient with genotype A and three patients had genotype A and D recombination; two of them had cirrhosis and one had hepatocellular carcinoma.Phylogenetic analysis of core and preS2/surface regions separately showed evidence of genotype A and D recombination.The breakpoints of recombination were found to be at the start of preS2 and at the end of surface coding regions.CONCLUSION: We identified and characterized recombinant A and D genotype HBV in hepatitis B surface antigen (HBsAg)-positive patients. | Ranjit Chauhan Syed Naqui Kazim Manoj Kumar Jayashree Bhattacharjee Narayanasamy Krishnamoorthy Shiv Kumar Sarin | 2008 | World Journal of Gastroenterology2008，14，40： | 4 |
| 4 | Myanmar-Andaman-Sumatra Subduction Margin Revisited:Insights of Arc-Specific Deformations显示文摘We address the role of the concave and convex arcs(as observed from the subducting plate)on the deformation occurring along the Myanmar-Andaman-Sumatra margin.We categorize the pre-and post-seismic deformations of the lithosphere using earthquake database occurring either prior to 26th December 2004 M_w 9.3 off-Sumatra mega-event or after the incidence.Analysis under pre-seismic domain shows that area near Sumatra records highest seismicity,which largely drops in the area past the North Andaman,and further increases towards north.Shallowest depth and minimum dip of the subducting lithosphere is recorded at the central segment where the arc transformed into concave shape.The annual moment energy release during earthquake decreases to more than two orders of magnitude past the North Andaman towards north under post-seismic deformation phase.Higher depths of continuity of events are presumably associated with more dipping Benioff zones in both the Indo-Myanmar and Andaman-Nicobar convex arcs.These observations obviously account for tectonic subdivision of the margin near concave shape arc around the central part.Absence of volcanism,presence of splay faults in the back-arc,sharp reduction in seismicity near central segment are interpreted to be caused by major tectonic impact of the NNE-ward converging buoyant Ninety-east Ridge against the Asian Plate.Shallowest dip,small elastic thickness,weak converging Indian lithosphere,and evidences of series of en-echelon blocks off the eastern side of the broken northern Ninetyeast Ridge might be incapable of generating great earthquake in this area. | Prosanta Kumar Khan Sk Shamim Manoranjan Mohanty Prabhat Kumar Jayashree Banerjee | 2017 | Journal of Earth Science2017，28，4： | 4 |
