4Molecular mechanism of noradrenaline during the stress-induced major depressive disorder显示文摘Chronic stress-induced depression is a common hallmark of many psychiatric disorders with high morbidity rate.Stress-induced dysregulation of noradrenergic system has been implicated in the pathogenesis of depression.Lack of monoamine in the brain has been believed to be the main causative factor behind pathophysiology of major depressive disorder(MDD) and several antidepressants functions by increasing the monoamine level at the synapses in the brain.However,it is undetermined whether the noradrenergic receptor stimulation is critical for the therapeutic effect of antidepressant.Contrary to noradrenergic receptor stimulation,it has been suggested that the desensitization of β-adrenoceptor is involved in the therapeutic effect of antidepressant.In addition,enhanced noradrenaline(NA) release is central response to stress and thought to be a risk factor for the development of MDD.Moreover,fast acting antidepressant suppresses the hyperactivation of noradrenergic neurons in locus coeruleus(LC).However,it is unclear how they alter the firing activity of LC neurons.These inconsistent reports about antidepressant effect of NA-reuptake inhibitors(NRIs) and enhanced release of NA as a stress response complicate our understanding about the pathophysiology of MDD.In this review,we will discuss the role of NA in pathophysiology of stress and the mechanism of therapeutic effect of NA in MDD.We will also discuss the possible contributions of each subtype of noradrenergic receptors on LC neurons,hypothalamic-pituitary-adrenal axis(HPA-axis) and brain derived neurotrophic factor-induced hippocampal neurogenesis during stress and therapeutic effect of NRIs in MDD.Kenjiro Seki Satomi Yoshida Manoj Kumar Jaiswal 2018Neural Regeneration Research2018,13,7:10
5MicroRNA-99a mimics inhibit M1 macrophage phenotype and adipose tissue inflammation by targeting TNFα显示文摘In human adipose tissue and obesity,miR-99a expression is negatively correlated with inflammation.Therefore,the present study investigated the role of miR-99a in macrophage phenotype activation and adipose tissue inflammation.M2 BMDMs showed a significant increase in miR-99a expression when compared to the M0 and M1 phenotypes.Phenotype-switching experiments established an association between upregulated miR-99a expression and the M2 phenotype.Overexpression of miR-99a prevented M1 phenotype activation and attenuated bactericidal activity.Likewise,knockdown of miR-99a abolished M2 phenotype activation.By means of in silico target prediction tools and a luciferase reporter assay,TNFαwas identified as a direct target of miR-99a.Knockdown of TNFαrecapitulated the effect of miR-99a overexpression in M1 BMDMs.In a db/db mice model,miR-99a expression was reduced in eWAT and F4/80+ATMs.Systemic overexpression of miR-99a in db/db mice attenuated adipocyte hypertrophy with increased CD301 and reduced CD86 immunostaining.Flow cytometry analysis also showed an increased M2 and a reduced M1 macrophage population.Mimics of miR-99a also improved the diabetic dyslipidemia and insulin signaling in eWAT and liver,with an attenuated expression of gluconeogenesis and cholesterol metabolism genes in the liver.Furthermore,adoptive transfer of miR-99a-overexpressing macrophages in the db/db mice recapitulated in vivo miR-99a mimic effects with increased M2 and reduced M1 macrophage populations and improved systemic glucose,insulin sensitivity,and insulin signaling in the eWAT and liver.The present study demonstrates that miR-99a mimics can regulate macrophage M1 phenotype activation by targeting TNFα.miR-99a therapeutics in diabetic mice reduces the adipose tissue inflammation and improves insulin sensitivity.Anant Jaiswal Sukka Santosh Reddy Mohita Maurya Preeti Maurya Manoj Kumar Barthwal 2019Cellular & Molecular Immunology2019,16,5:7
6Is prophylactic placement of drains necessary after subtotal gastrectomy?显示文摘AIM: To determine the evidence-based values of prophylactic drainage in gastric cancer surgery. METHODS: One hundred and eight patients, who underwent subtotal gastrectomy with D1 or D2 lymph node dissection for gastric cancer between January 2001 and December 2005, were divided into drain group or no-drain group. Surgical outcome and post-operative complications within four weeks were compared between the two groups. RESULTS: No significant differences were observed between the drain group and no-drain group in terms of operating time (171 ± 42 min vs 156 ± 39 min), number of post-operative days until passage of flatus (3.7 ± 0.5 d vs 3.5 ± 1.0 d), number of post-operative days until initiation of soft diet (4.9 ± 0.7 d vs 4.8 ± 0.8 d), length of post-operative hospital stay (9.3 ± 2.2 d vs 8.4 ± 2.4 d), mortality rate (5.4% vs 3.8%), and overall post- operative complication rate (21.4% vs 19.2%). 8Tissue-based metabolite profiling and qualitative comparison of two species of Achyranthes roots by use of UHPLC-QTOF MS and laser micro-dissection显示文摘Achyranthes bidentata and Achyranthes aspera are saponin and steroid rich medicinal plants, used extensively for therapeutic treatments in Traditional Chinese Medicine(TCM) and Ayurveda. A. bidentata is reported to be one of the rare and extensively exploited medicinal plant species that face the issue of being endangered.Finding qualitative substitute with identical phyto-constituents contributing to similar composition and pharmacological benefits will help in reducing the burden of exploitation of the natural habitats of such plants.In the present study, a comparative metabolite analysis of the whole drug and specific tissues isolated by laser micro-dissection(LMD) was carried out for both the selected species, by use of ultra-high performance liquid chromatography-quadrupole time-of-flight mass spectrometry(UHPLC-QTOF MS). The results of the study indicate that the cortex and the medullary ray tissues are rich in their content of steroidal and saponin constituents such as(25 S)-inokosterone-20,22-acetonide, ginsenoside Ro, bidentatoside II and achyranthoside B.Metabolite profiling of the whole tissues of both the species indicates presence of identical constituents. 9Unexpected alliance between syndecan-1 and innatelike T cells to protect host from autoimmune effects of interleukin-17显示文摘Innate-like T cells, namely natural killer T(NKT) and γδ T cells, play critical roles in linking innate and adaptive immune responses through rapid production of cytokines. Prominent among these cytokines is interleukin-17(IL-17), which is a potent proinflammatory cytokine that plays a critical role in host defense against fungi and extracellular bacteria. However, excessive IL-17-production promotes autoimmune diseases, including psoriasis, multiple sclerosis, rheumatoid arthritis, inflammatory bowel disease, and systemic lupus erythematosus. IL-17 has also been implicated in regulating body fat, which is highly relevant given rises in obesity and type 2 diabetes. NKT cells, γδ T cells and mucosal-associated invariant T cells(MAIT) are the major sources of IL-17 involved in protection of mucosal surfaces from opportunistic infections and causing autoimmunity when become dysregulated. Given the pathogenic effects of IL-17, efforts have been directed towards understanding mechanisms that guard against IL-17 overproduction. One novel potent mechanism is mediated by the heparan sulfate proteoglycan, syndecan-1(sdc1), which is selectively expressed by IL-17-producing subsets of NKT and γδ T cells. This unexpected role for sdc1 is uncovered by analysis of NKT and γδ T cells in sdc1-deficient mice. 10Novel role of phosphodiesterase inhibitors in the management of end-stage heart failure显示文摘In advanced heart failure(HF), chronic inotropic therapy with intravenous milrinone, a phosphodiesterase Ⅲ inhibitor, is used as a bridge to advanced management that includes transplantation, ventricular assist device implantation, or palliation. This is especially true when repeated attempts to wean off inotropic support result in symptomatic hypotension, worsened symptoms, and/or progressive organ dysfunction. Unfortunately, patients in this clinical predicament are considered hemodynamically labile and may escape the benefits of guidelinedirected HF therapy. In this scenario, chronic milrinone infusion may be beneficial as a bridge to introduction of evidence based HF therapy. However, this strategy is not well studied, and in general, chronic inotropic infusion is discouraged due to potential cardiotoxicity that accelerates disease progression and proarrhythmic effects that increase sudden death. Alternatively, chronic inotropic support with milrinone infusion is a unique opportunity in advanced HF. This review discusses evidence that long-term intravenous milrinone support may allow introduction of beta blocker(BB) therapy. When used together, milrinone does not attenuate the clinical benefits of BB therapy while BB mitigates cardiotoxic effects of milrinone. In addition, BB therapy decreases the risk of adverse arrhythmias associated with milrinone. We propose that advanced HF patients who are intolerant to BB therapy may benefit from a trial of intravenous milrinone as a bridge to BB initiation. 11A generalized three-dimensional failure criterion for rock masses显示文摘The smooth convex generalized failure function, which represents 1/6 part of envelope in the deviatoric plane, is proposed. The proposed function relies on four shape parameters (Ls, a, b and c), in which two parameters (a and b) are dependent on the others. The parameter L s is called extension ratio. The proposed failure function could be incorporated with any two-dimensional (2D) failure criteria to make it a three-dimensional (3D) version. In this paper, a mathematical formulation for incorporation of Hoek-Brown failure criterion with the proposed function is presented. The Hoek-Brown failure criterion is the most suited 2D failure criterion for geomaterials. Two types of analyses for best-fitting solution of published true tri-axial test data were made by considering (1) constant extension ratio and (2) variable extension ratio. The shape and strength parameters for different types of rocks have been determined by best-fitting the published true tri-axial test data for both the analyses. It is observed from the best-fitting solution by considering uniform extension ratio (Ls) that shape constants have a correlation with Hoek-Brown strength parameters. Thus, only two parameters (σc and m) are needed for representing the 3D failure criterion for intact rock. The statistical expression between shape and Hoek-Brown strength parameters is given. In the second analysis, when considering varying extension ratio, another parameter fis introduced. The modified extension ratio is related to f and extension ratio. The results at minimum mean misfit for all the nine rocks indicate that the range of f varies from 0.7 to 1.0. It is found that mean misfit by considering varying extension ratio is lower than that in the first analysis. But it requires three parameters. A statistical expression between f and Hoek-Brown strength parameters has been established. Though coefficient of correlation is not reasonable, we may eliminate it as an extra parameter. 17Bombings specific triage(Bost Tool) tool and its application by healthcare professionals显示文摘BACKGROUND: Bombing is a unique incident which produces unique patterns, multiple and occult injuries. Death often is a result of combined blast, ballistic and thermal effect injuries. Various natures of injury, self referrals and arrival by private transportation may lead to 'wrong triage' in the emergency department. In India there has been an increase in incidence of bombing in the last 15 years. There is no documented triage tool from the National Disaster Management Authority of India for Bombings. We have tried to develop an ideal bombing specific triage tool which will guide the right patients to the right place at the right time and save more lives.METHODS: There are three methods of studying the triage tool: 1) real disaster; 2) mock drill; 3) table top exercise. In this study, a table top exercise method was selected. There are two groups, each consisting of an emergency physician, a nurse and a paramedic.RESULTS: By using the proportion test, we found that correct triaging was significantly different(P=0.005) in proportion between the two groups: group B(80%) with triage tool performed better in triaging the bomb blast victims than group A(50%) without the bombing specific triage tool performed.CONCLUSION: Development of bombing specific triage tool can reduce under triaging.Jaiswal Sanjay Verma Ankur Kole Tamorish 2015World Journal of Emergency Medicine2015,6,4:2

19Therapeutic opportunities and challenges of induced pluripotent stem cells-derived motor neurons for treatment of amyotrophic lateral sclerosis and motor neuron disease显示文摘Amyotrophic lateral sclerosis(ALS) and motor neuron diseases(MNDs) are progressive neurodegenerative diseases that affect nerve cells in the brain affecting upper and lower motor neurons(UMNs/LMNs), brain stem and spinal cord. The clinical phenotype is characterized by loss of motor neurons(MNs), muscular weakness and atrophy eventually leading to paralysis and death due to respiratory failure within 3–5 years after disease onset. No effective treatment or cure is currently available that halts or reverses ALS and MND except FDA approved drug riluzole that only modestly slows the progression of ALS in some patients. Recent advances in human derived induced pluripotent stem cells have made it possible for the first time to obtain substantial amounts of human cells to recapitulate in vitro 'disease in dish' and test some of the underlying pathogenetic mechanisms involved in ALS and MNDs.
