1Neutrophile-to-lymphocyte,lymphocyte-to-monocyte,and platelet-tolymphocyte ratios as prognostic and response biomarkers for resectable locally advanced gastric cancer显示文摘BACKGROUND Perioperative fluorouracil plus leucovorin,oxaliplatin,and docetaxel(FLOT)improves prognosis in locally advanced gastric cancer(LAGC).Neutrophil-to-lymphocyte(NLR),lymphocyte-tomonocyte(LMR),and platelet-to-lymphocyte(PLR)ratios are prognostic biomarkers but not predictive factors.AIM To assess blood ratios’(NLR,LMR and PLR)potential predictive response to FLOT and survival outcomes in resectable LAGC patients.METHODS This was a multicentric retrospective study investigating the clinical potential of NLR,LMR,and PLR in resectable LAGC patients,treated with at least one preoperative FLOT cycle,from 12 Portuguese hospitals.Means were compared through non-parametric Mann-Whitney tests.Receiver operating characteristic curve analysis defined the cut-off values as:High PLR>141 for progression and>144 for mortality;high LMR>3.56 for T stage regression(TSR).Poisson and Cox regression models the calculated relative risks/hazard ratios,using NLR,pathologic complete response,TSR,and tumor regression grade(TRG)as independent variables,and overall survival(OS)as the dependent variable.RESULTS This study included 295 patients(mean age,63.7 years;59.7% males).NLR was correlated with survival time(r=0.143,P=0.014).PLR was associated with systemic progression during FLOT(P=0.022)and mortality(P=0.013),with high PLR patients having a 2.2-times higher risk of progression[95% confidence interval(CI):0.89-5.26]and 1.5-times higher risk of mortality(95%CI:0.92-2.55).LMR was associated with TSR,and high LMR patients had a 1.4-times higher risk of achieving TSR(95%CI:1.01-1.99).OS benefit was found with TSR(P=0.015)and partial/complete TRG(P<0.001).Patients without TSR and with no evidence of pathological response had 2.1-times(95%CI:1.14-3.96)and 2.8-times(95%CI:1.6-5)higher risk of death.CONCLUSION Higher NLR is correlated with longer survival time.High LMR patients have a higher risk of decreasing T stage,whereas high PLR patients have higher odds of progressing under FLOT and dying.Patients with TSR and a pathological response have better OS and lower risk of dying.Tiago Cruz Tomás Ines Eiriz Marina Vitorino Rodrigo Vicente Joao Gramaca Alicia Guadalupe Oliveira Paulo Luz Mafalda Baleiras Ana Sofia Spencer Luísa Leal Costa Patrícia Liu Joana Mendonca Magno Dinis Teresa Padrao Marisol Correia Goncalo Atalaia Michelle Silva Teresa Fiúza 2022World Journal of Gastrointestinal Oncology2022,14,7:2
2Antithrombotic treatment in chronic heart failure and sinus rhythm: Systematic review显示文摘AIM: To assess the efficacy and safety of antithrombotic drugs(antiplatelet or anticoagulant drugs) compared to no antithrombotic treatment or placebo in patients with heart failure(HF) and sinus rhythm. METHODS: We searched Medline and Cochrane Library for randomized controlled trials evaluating antithrombotic treatment and no antithrombotic treatment in patients with HF and sinus rhythm. Risk ratio(RR) and 95%CIs were estimated performing meta-analysis with random effects method. RESULTS: Two studies met the inclusion criteria: Heart failure Long-term Antithrombotic Study and Warfarin/Aspirin Study in Heart failure, with 336 patients and mean follow-up 1.8-2.25 years. Stroke risk was not reduced by acetylsalicylic acid(RR = 1.18, 95%CI: 0.17-8.15), oral anticoagulation(RR = 0.30, 95%CI: 0.03-2.65) or overall antithrombotic drugs(RR = 0.52, 95%CI: 0.10-2.74). Acetylsalicylic acid showed a significant increased risk of worsening HF(RR = 1.78, 95%CI: 1.08-2.92), while oral anticoagulation had no impact in this outcome(RR = 1.03, 95%CI: 0.61-1.75). Overall antithrombotic drugs showed a significant risk increase of major bleeding(RR = 6.99, 95%CI: 0.89-54.64). CONCLUSION: Best available evidence does not support the routine use of antithrombotic drugs in patients with HF and sinus rhythm. 11Colorectal cancer screening:A review of current knowledge and progress in research显示文摘Colorectal cancer(CRC)is one of the most prevalent malignancies worldwide,being the third most commonly diagnosed malignancy and the second leading cause of cancer-related deaths globally.Despite the progress in screening,early diagnosis,and treatment,approximately 20%-25%of CRC patients still present with metastatic disease at the time of their initial diagnosis.Furthermore,the burden of disease is still expected to increase,especially in individuals younger than 50 years old,among whom early-onset CRC incidence has been increasing.Screening and early detection are pivotal to improve CRC-related outcomes.It is well established that CRC screening not only reduces incidence,but also decreases deaths from CRC.Diverse screening strategies have proven effective in decreasing both CRC incidence and mortality,though variations in efficacy have been reported across the literature.However,uncertainties persist regarding the optimal screening method,age intervals and periodicity.Moreover,adherence to CRC screening remains globally low.In recent years,emerging technologies,notably artificial intelligence,and non-invasive biomarkers,have been developed to overcome these barriers.However,controversy exists over the actual impact of some of the new discoveries on CRC-related outcomes and how to effectively integrate them into daily practice.In this review,we aim to cover the current evidence surrounding CRC screening.We will further critically assess novel approaches under investigation,in an effort to differentiate promising inno-vations from mere novelties.Sara Ramos Lopes Claudio Martins Inês Costa Santos Madalena Teixeira Élia Gamito Ana Luisa Alves 2024World Journal of Gastrointestinal Oncology2024,16,4:0
12Long-term response to avelumab and management of oligoprogression in Merkel cell carcinoma:A case report显示文摘BACKGROUND Merkel cell carcinoma(MCC)is a rare and aggressive cutaneous neuroendocrine neoplasia,with high risk of recurrence and metastasis and poor survival.Immune checkpoint inhibitors,like the anti-programmed death-ligand 1 agent avelumab,were recently approved for the treatment of advanced MCC.We,herein,report the first case of advanced MCC with oligoprogression managed with avelumab and local radical treatment.CASE SUMMARY A 61-year-old man was presented to the hospital with sporadic fever and an exudative malodorous mass(10 cm of diameter),located on the right gluteal region.The final diagnosis was MCC,cT4N3M1c(AJCC,TNM staging 8th edition,2017),with invasion of adjacent muscle,in-transit metastasis,and bone lesions.Patient started chemotherapy(cisplatin and etoposide),and after six cycles,the main tumor increased,evidencing disease progression.Two months later,the patient started second line treatment with avelumab(under an early access program).After two cycles of treatment,the lesion started to decrease,achieving a major response.Local progression was documented after 16 cycles.However,as the tumor became resectable,salvage surgery was performed,while keeping the systemic treatment with avelumab.Since the patient developed bilateral pneumonia,immunotherapy was suspended.More than 2.5 years after surgery(last 19 mo without systemic therapy),the patient maintains complete local response and stable bone lesions.CONCLUSION This report highlights the efficacy and long-term response of avelumab on the management of a chemotherapy resistant advanced MCC,with evidence of oligoprogression,in combination with local radical treatment.Inês Leão Joana Marinho Telma Costa 2021World Journal of Clinical Cases2021,9,18:0

14Vitamin D in Food Supplements:Labeling Survey显示文摘An adequate vitamin D(vitD)intake(Recommended Daily Allowance,RDA=5μg)is crucial for health maintenance and its deficiency is associated with several health problems.The increase in hypovitaminosis D cases and the proliferation of food supplements(FS)that are easily accessible by the population,have led to an unrestrained chronic consumption of FS.VitD may accumulate in the body and originate toxicity(Tolerable Upper Limit,UL=100μg).The aim of this study was to evaluate if daily vitD doses mentioned in FS labels are in conformity with RDA.210 solid and liquid FS(for pediatrics and adults)sold in Portuguese pharmacies,supermarkets,health shops and on the internet were examined for indicated daily intake of vitD and compared to RDA and UL values.51.43%of FS have values higher than RDA,8.57%higher than UL.The average vitD daily dose in FS is 24.48μg,with a high variability between samples(0.25-250μg).Majority of FS labels recommend vitD daily doses above RDA and some even above UL,regardless of being for adults or children.Therefore,it is crucial that vitD dose in FS is reviewed to ensure the safety of these products.Alexandra Figueiredo Maria Deolinda Auxtero Maria Guilhermina Moutinho Inês Neves Raquel Inez Isabel Margarida Costa 2021Journal of Integrative Medicine(双语)2021,10,2:0
15Fisher's perceptions about a marine protected area over time显示文摘The perceptions of fishers towards the Arrábida Marine Park,a marine protected area(MPA)in the west coast of Portugal,were studied through face-to-face interviews in two different moments of the MPA life cycle.Fishers'perceptions about the MPA and the impact it had on the fishing activity over time were identified just before the full implementation of the zoning and regulations of the management plan and 10 years later.This study aimed to investigate fishers'knowledge,acceptance and perceptions about the MPA changed with time,if support for the MPA was linked to the impact of the MPA on the fishing activity,and if fishers'perceptions about the impact of the MPA on the fishing activity match with local landings trends.Results show that although knowledge about the marine park significantly improved over time,fishers'acceptance did not.A decrease on fishers'support was not substantial but occurred.Issues such as the disagreement with regulations reinforced concerns raised during the implementation of the marine park,particularly in relation to the top-down decision-making,which commonly confers minor participation,recognition and legitimacy to fishers.Apparently,fisheries benefits were still not perceived by local fishers,though they are central for fishers'support.Further,the perceived negative impacts of the park seemed to be more related to social aspects and individual interests than to impacts on catches.Addressing adequate management,enforcement and participation of local fishers is still possible and are advocated here as to contribute to the expected socioecological outcomes and respective support,leading to the future successful performance of the Arrábida Marine Park.Assessing fishers’perceptions towards an MPA over time is central and should be included on periodical socioecological monitoring and inform an effective adaptive management.Cristina Pita Barbara Horta e Costa Gustavo Franco Rui Coelho Inês Sousa Emanuel J.Gonçalves Jorge M.S.Gonçalves Karim Erzini 2020Aquaculture and Fisheries2020,5,5:0
