1Using old liver grafts for liver transplantation: Where are the limits?显示文摘The scarcity of ideal liver grafts for orthotopic liver transplantation(OLT) has led transplant teams to investigate other sources of grafts in order to augment the donor liver pool. One way to get more liver grafts is to use marginal donors, a not well-defined group which includes mainly donors > 60 years, donors with hypernatremia or macrosteatosis > 30%, donors with hepatitis C virus or hepatitis B virus positive serologies, cold ischemia time > 12 h, non-heart-beating donors, and grafts from split-livers or living-related donations. Perhaps the most practical and frequent measure to increase the liver pool, and thus to reduce waiting list mortality, is to use older livers. In the past years the results of OLT with old livers have improved, mainly due to better selection and maintenance of donors, improvements in surgical techniques in donors and recipients, and intra- and post-OLT management. At the present time, sexagenarian livers are generally accepted, but there still exists some controversy regarding the use of septuagenarian and octogenarian liver grafts. The aim of this paper is to briefly review the aging process of the liver and reported experiences using old livers for OLT. 2Incidence, risk factors and outcome of de novo tumors in liver transplant recipients focusing on alcoholic cirrhosis显示文摘Orthotopic liver transplantation(OLT) is an established life-saving procedure for alcoholic cirrhotic(AC) patients, but the incidence of de novo tumors ranges between 2.6% and 15.7% and is significantly increased in comparison with patients who undergo OLT for other etiologies. Tobacco, a known carcinogen, has been reported to be between 52% and 83.3% in AC patients before OLT. Other risk factors that contribute to the development of malignancies are dose-dependent immunosuppression, advanced age, viral infections, sun exposure, and premalignant lesions(inflammatory bowel disease, Barrett's esophagus). A significantly more frequent incidence of upper aerodigestive(UAD) tract, lung, skin, and kidney-bladder tumors has been found in OLT recipients for AC in comparison with other etiologies. Liver transplant recipients who develop de novo non-skin tumors have a decreased long-term survival rate compared with controls. This significantly lower survival rate is more evident in AC recipients who develop UAD tract or lung tumors after OLT mainly because the diagnosis is usually performed at an advanced stage. All transplant candidates, especially AC patients, should be encouraged to cease smoking and alcohol consumption in the pre- and postOLT periods, use skin protection, avoid sun exposure and over-immunosuppression, and have a yearly otopharyngolaryngeal exploration and chest computed tomography scan in order to prevent or reduce the incidence of de novo malignancies. 3Long-term follow-up study of gastroduodenal lesions after radioembolization of hepatic tumors显示文摘AIM:To evaluate the long-term natural history of the gastroduodenal lesions secondary to extrahepatic embolization with Ytrium 90(90 Y) spheres.METHODS:From September 2003 to January 2012,379 procedures of liver radioembolization(RE) using resin microspheres loaded with 90 Y were performed in our center.We have retrospectively compiled the data from 379 RE procedures performed in our center.We report a comprehensive clinical,analytical,endoscopic and histologic long-term follow-up of a series of patients who developed gastroduodenal lesions after the treatment.RESULTS:Six patients(1.5%) developed gastrointestinal symptoms and had gastrointestinal lesions as shown by upper endoscopy in the next 12 wk after RE.The mean time between RE and the appearance of symptoms was 5 wk.Only one patient required endoscopic and surgical treatment.The incidence of gastrointestinal ulcerations was 3.75%(3/80) when only planar images were used for the pre-treatment evaluation.It was reduced to 1%(3/299) when singlephoton emission computed tomography(SPECT) images were also performed.The symptoms that lasted for a longer time were nausea and vomiting,until 25 mo after the treatment.CONCLUSION:All patients were free from severe symptoms at the end of follow-up.The routine use of SPECT has decreased the incidence of gastrointestinal lesions due to unintended deployment of 90 Y particles.Iago Rodríguez-Lago Cristina Carretero Maite Herráiz José C Subtil Maite Betés Macarena Rodríguez-Fraile Jesús J Sola José I Bilbao Miguel Muoz-Navas Bruno Sangro 2013World Journal of Gastroenterology2013,19,19:2
4Analyzing predictors of graft survival in patients undergoing liver transplantation with donors aged 70 years and over显示文摘AIM To increase the number of available grafts.METHODS This is a single-center comparative analysis performed between April 1986 and May 2016. Two hundred and twelve liver transplantation(LT) were performed with donors ≥ 70 years old(study group). Then, we selected the first cases that were performed with donors < 70 years old immediately after the ones that were performed with donors ≥ 70 years old(control group).RESULTS Graft and patient survivals were similar between both groups without increasing the risk of complications, especially primary non-function, vascular complications and biliary complications. We identified 5 risk factors as independent predictors of graft survival: recipient hepatitis C virus(HCV)-positivity [hazard ratio(HR) = 2.35; 95% confidence interval(CI): 1.55-3.56; P = 0.00]; recipient age(HR = 1.04; 95%CI: 1.02-1.06; P = 0.00); donor age X model for end-stage liver disease(D-MELD)(HR = 1.00; 95%CI: 1.00-1.00; P = 0.00); donor value of serum glutamic-pyruvic transaminase(HR = 1.00; 95%CI: 1.00-1.00; P = 0.00); and donor value of serum sodium(HR = 0.96; 95%CI: 0.94-0.99; P = 0.00). After combining D-MELD and recipient age we obtained a new scoring system that we called DR-MELD(donor age X recipient age X MELD). Graft survival significantly decreased in patients with a DR-MELD score ≥ 75000, especially in HCV patients(77% vs 63% at 5 years in HCV-negative patients, P = 0.00; and 61% vs 25% at 5 years in HCV-positive patients; P = 0.00). 9Octogenarian liver grafts:Is their use for transplant currently justified?显示文摘AIM To analyse the impact of octogenarian donors in liver transplantation.METHODS We present a retrospective single-center study, performed between November 1996 and March 2015, that comprises a sample of 153 liver transplants. Recipients were divided into two groups according to liver donor age: recipients of donors ≤ 65 years(group A; n = 102), and recipients of donors ≥ 80 years(group B; n = 51). A comparative analysis between the groups was performed. Quantitative variables were expressed as mean values and SD, and qualitative variables as percentages. Differences in properties between qualitative variables were assessed by χ2 test. Comparison of quantitative variables was made by t-test. Graft and patient survivals were estimated using the Kaplan-Meier method.RESULTS One, 3 and 5-year overall patient survival was 87.3%, 84% and 75.2%, respectively, in recipients of younger grafts vs 88.2%, 84.1% and 66.4%, respectively, in recipients of octogenarian grafts(P = 0.748). One, 3 and 5-year overall graft survival was 84.3%, 83.1% and 74.2%, respectively, in recipients of younger grafts vs 84.3%, 79.4% and 64.2%, respectively, in recipients of octogenarian grafts(P = 0.524). After excluding the patients with hepatitis C virus cirrhosis(16 in group A and 10 in group B), the 1, 3 and 5-year patient(P = 0.657) and graft(P = 0.419) survivals were practically the same in both groups. Multivariate Cox regression analysis demonstrated that overall patient survival was adversely affected by cerebrovascular donor death, hepatocarcinoma, and recipient preoperative bilirubin, and overall graft survival was adversely influenced by cerebrovascular donor death, and recipient preoperative bilirubin.CONCLUSION The standard criteria for utilization of octogenarian liver grafts are: normal gross appearance and consistency, normal or almost normal liver tests, hemodynamic stability with use of < 10 μg/kg per minute of vasopressors before procurement, intensive care unit stay < 3 d, CIT < 9 h, absence of atherosclerosis in the hepatic and gastroduodenal arteries, and no relevant histological alterations in the pre-transplant biopsy, such as fibrosis, hepatitis, cholestasis or macrosteatosis > 30%.Carlos Jiménez-Romero Felix Cambra Oscar Caso Alejandro Manrique Jorge Calvo Alejandro Marcacuzco Paula Rioja David Lora Iago Justo 2017World Journal of Gastroenterology2017,23,17:1

15Social buffer or avoidance depends on the similarity of stress between queen ants显示文摘The association of unrelated ant queens(pleometrosis)is supposed to improve nest foundation and competitiveness under environmental stress,but its evolutionary maintenance is difficult to explain because only one of the queens survives after nest foundation.My aim was to test the potential effect of queen association as a social buffer,that is,as a mechanism reducing stress and improving fitness due to the benefits of social contact.I analyzed the survival,fecundity,and behavior of isolated and paired Lasius flavus queens exposed and not exposed to stressors(disturbing environmental conditions).I found no difference in survivorship between isolated and paired queens or between stressed and unstressed isolated queens.Groups in which 1 or 2 paired queens were stressed showed higher mortality.Unstressed queens died similarly to their stressed nestmates,suggesting stress transmission.A trend suggested that paired queens produced eggs more quickly,but eggs were produced similarly between isolated and paired queens.Social avoidance was observed in groups with 1 stressed and 1 unstressed queen.However,the groups with 2 stressed queens showed the expected behaviors according to social buffering:lower mobility and more interindividual inspection.My findings suggest the synergistic effect of pleometrosis and stress and the dependence of stress level similarity between nestmates on social buffering or rejection on ant queens.Iago Sanmartin-Villar 2023Current Zoology2023,69,2:0
16Direct and cross-generational effects of reproduction on fitness and behavioral variability in male-biased environments显示文摘Population structure determines individuals’interactions and trade-offs with evolutionary consequences.Male-biased populations increase intrasexual competition and intersexual harassment,reducing female resource acquisition,and thus,resources availability for the following generation.We analyzed direct and cross-generational effects of male harassment in two generations of damselflies(Odonata).We exposed adult females to treatments with different sex-ratio and density(balanced and male-biased)to modify the male harassment level.We analyzed female fecundity,fertility,and number of faecal deposits as an indirect measure of resources acquisition.We studied female flight performance after repeated exposures to males.We analyzed survivorship,development,exploration,thigmotaxis,and feeding latency of larvae produced by the experimental females.In both generations,we analyzed four metrics of behavior:mean value,interindividual differences in plasticity,intra-individual unpredictability,and repeatability.Mating duration increased in male-biased treatment,whereas female resources acquisition and fertility decreased.Females that mated longer showed higher fecundity when they were exposed to balanced treatment,but not if they were exposed to male-biased treatment.Females from the male-biased treatment showed interindividual differences in plasticity and no repeatability in flight performance.Offspring showed balanced sex-ratio and similar survivorship,development,and feeding latency independently of the parental treatment;however,females exposed to male-biased treatment produced offspring with higher differences in exploration plasticity and daughters less explorative and with higher unpredictable thigmotaxis.We propose prolonged copulation as courtship at balanced sex-ratio but a cost to females under male-biased sex-ratio.Cross-generational effects in behavioral variability may be a mechanism to cope with predicted future environments.Iago Sanmartin-Villar Xin Yu Adolfo Cordero-Rivera 2023Current Zoology2023,69,3:0

19Ergatoid reproductives in the Neotropical termite Nasutitermes aquilinus (Holmgren) (Blattaria: Isoptera: Termitidae): developmental origin, fecundity, and genetics显示文摘Termite colonies are usully headed by primary reproductives,which estab-listh nests during the swar ming scason.However,secondary reproductives may develop in some specics and become supplementary or replacement breeders,extending colony lifes-pan.Here we investigate the developmental origin,fecundity and genetic characterizaion of ergatoid reproductives in the Neotropical termite Nasuitermes aquilinus(Holmgren),using morphometrical and histological techniques,five micosiallitle loci and the COI mitochondrial DNA.Twelve measurements performed on 208 apterous individuals of Nuauidius revealed 10 groups,including ergatoid females,which developed from major workers through two successive molts.and were characterized by the presence of imaginal features such as eyes andl wing buds.The difentiation of these features was correlated to physogastric development in these ergatoids,Histology revealed ooeytes in all matu-ration sages in worker-derived reproductives of N aquidinus,presence of nonflagellate spermatozoa inside the spermatheea,and royal fa body.Thus,ergatoid reproduwetives were reproductively functional.According to the genotypes of 221 individuals from 11 nests,and mitochondrial haplotypes of 43 ergatoids,73%of the colomies were simple families,whereas 27%were extendbed families.Despite the oecurenee of related reproductives,low inbreeding rates were detected within and among colonies.Such values could be explained given that sib mating itself cannot result in a higher inbreeding rate but depend on several factors discussed in detail.This is the first study to investigate the genetic structure of termite colonies influenced by the development of ergatoids and further investigations are encouraged 10 understand the inflwence of these reproductives on colony lifespan.Iago Bueno da Silva Ives Haifig Edward LVargo Fabiana Elaine Casarin Marcelo Luiz da Mota Juliana Toledo Lima Ana Maria Costa-Leonardo 2020Insect Science2020,27,6:0
