| 4 | Carvedilol vs endoscopic variceal ligation for primary and secondary prevention of variceal bleeding: Systematic review and metaanalysis显示文摘BACKGROUND Variceal hemorrhage is associated with high mortality and is the cause of death for 20–30%of patients with cirrhosis.Nonselectiveβblockers(NSBBs)or endoscopic variceal ligation(EVL)are recommended for primary prevention of variceal bleeding in patients with medium to large esophageal varices.Meanwhile,combination of EVL and NSBBs is the recommended approach for the secondary prevention.Carvedilol has greater efficacy than other NSBBs as it decreases intrahepatic resistance.We hypothesized that there was no difference between carvedilol and EVL intervention for primary and secondary prevention of variceal bleeding in cirrhosis patients.AIM To evaluate the efficacy of carvedilol compared to EVL for primary and secondary prevention of variceal bleeding in cirrhotic patients METHODS We searched relevant literatures in major journal databases(CENTRAL,MEDLINE,and EMBASE)from March to August 2018.Patients with cirrhosis and portal hypertension,regardless of aetiology and severity,with or without a history of variceal bleeding,and aged≥18 years old were included in this review.Only randomized controlled trials(RCTs)that compared the efficacy of carvedilol and that of EVL for primary and secondary prevention of variceal bleeding and mortality in patients with cirrhosis and portal hypertension were considered,irrespective of publication status,year of publication,and language.RESULTS Seven RCTs were included.In four trials assessing the primary prevention,no significant difference was found on the events of variceal bleeding(RR:0.74,95%CI:0.37-1.49),all-cause mortality(RR:1.10,95%CI:0.76-1.58),and bleedingrelated mortality(RR:1.02,95%CI:0.34-3.10)in patients who were treated with carvedilol compared to EVL.In three trials assessing secondary prevention,there was no difference between two interventions for the incidence of rebleeding(RR:1.10,95%CI:0.75-1.61).The fixed-effect model showed that,compared to EVL,carvedilol decreased all-cause mortality by 49%(RR:0.51,95%CI:0.33-0.79),with little or no evidence of heterogeneity.CONCLUSION Carvedilol had similar efficacy to EVL in preventing the first variceal bleeding in cirrhosis patients with esophageal varices.It was superior to EVL alone for secondary prevention of variceal bleeding in regard to all-cause mortality reduction. | Michael Dwinata David Dwi Putera Muhamad Fajri Adda’i Putra Nur Hidayat Irsan Hasan | 2019 | World Journal of Hepatology2019，11，5： | 6 |

| 6 | Pro-apoptotic and anti-proliferative effects of Physalis angulata leaf extract on retinoblastoma cells显示文摘AIM: To investigate the effect of Physalis angulata leaf extract on apoptotic and proliferation of retinoblastoma cells. Despite several previous studies evidencing the anticancer potential of Physalis angulata;however, certain study that proves its benefits in retinoblastoma cancer cells has been limited.METHODS: This study utilizes an in-vitro experimental study by applying Y79 human retinoblastoma cell line culture obtained from the American Type Culture Collection(ATCC;10801 University Boulevard Manassas, VA 20110, USA). The cell was divided into 4 groups. Group I was the control group without the administration of Physalis angulata leaf extract. Whereas, group II, II and IV are engaged with 25, 50, and 100 μg/mL of Physalis angulata leaf extract respectively. After a 24 h incubation, an examination with microtetrazolium(MTT) cell proliferation assay and Annexin V apoptosis detection was conducted. Statistical analysis was performed with the Tukey test.RESULTS: Physalis angulata leaf extract improved apoptosis and significantly reduced the number of living cells in retinoblastoma cells, along with the increase in the given dose. Based on the Tukey test, a significant difference was found in the treatment group at 50 μg/mL(P=0.025) and 100 μg/mL(P=0.001) in the measurement of apoptosis. Proliferation measurements also indicated a significant decrease in the number of living cells in the 50μg/m L treatment group(P=0.004), and in the 100 μg/mL treatment group(P=0.000). Meanwhile, a dose of 25 μg/mL indicated insignificant difference in the two measurements. Improved apoptosis and decreased number of living cells occured at a dose of 100 μg/mL. | 8 | Survey on gastrointestinal parasites and detection of Cryptosporidium spp. on cattle in West Java,Indonesia显示文摘Objective:To evaluate the presence of gastrointestinal parasites on cattle in Indonesia because the prevalence of parasites varies between counlries depending on the terrain surrounding livestock farms and investigations in Indonesia have never been performed.Methods:Fecal samples from cattle at 35 farms in 7 districts in West Java,Indonesia,has been examined using the floatation or sedimentation methods,and a immunofluorescence assay and experimentally inoculation to mice for Cryptosporidium or Giardia spp.Results:153 of 394 examined cattle(38.8%)were infected with gastrointestinal parasites.The prevalence of Eimeria spp.,Nematoda spp.(including Oesophagustomum and Bunostomum-like),Fasciola gigantica and Paramphistomum spp.was 22.4%,11.2%,12.5%and 3.8%,respectively.Cryptosporidium andersoni(C.andersoni)was also found in two samples.One isolate of this parasite was confirmed to be transmitted to mice,in contrast to the isolates from other countries.Conclusions:although this survey is preliminary,the results shows that the infection of gastrointestinal parasites in Indonesia was not high,but these infected cattle could be as a potential source leading to economic losses in livestock production. | Sylvia Maharani Ananta Suharno Adi Hidayat Makoto Matsubayashi | 2014 | Asian Pacific Journal of Tropical Medicine2014，7，3： | 3 |
| 9 | Scanometry as microplate reader for high throughput method based on DPPH dry reagent for antioxidant assay显示文摘The stable chromogenic radical 1,1'-diphenyl-2-picrylhydrazyl(DPPH) solution was immobilized on the microwell plate as dry reagent to construct a simple antioxidant sensor. Then, a regular flatbed scanner was used as microplate reader to obtain analytical parameters for antioxidant assay using one-shot optical sensors as scanometry technique. Variables affecting the acquisition of the images were optimized and the analytical parameters are obtained from an area of the sensing zone inside microwell using the average luminosity of the sensing zone captured as the mean of red, green, and blue(RGB)value using Image J~program. By using this RGB value as sensor response, it is possible to determine antioxidant capacity in the range 1–25 ppm as gallic acid equivalent(GAE) with the response time of 9 min. The reproducibility of sensor was good(RSD<1%) with recovery at 93%–96%. The antioxidant sensor was applied to the plant extracts, such as sappan wood and Turmeric Rhizome. | 10 | Optimizing the Double Inverted Pendulum′s Performance via the Uniform Neuro Multiobjective Genetic Algorithm显示文摘An inverted pendulum is a sensitive system of highly coupled parameters, in laboratories, it is popular for modelling nonlinear systems such as mechanisms and control systems, and also for optimizing programmes before those programmes are applied in real situations. This study aims to find the optimum input setting for a double inverted pendulum(DIP), which requires an appropriate input to be able to stand and to achieve robust stability even when the system model is unknown. Such a DIP input could be widely applied in engineering fields for optimizing unknown systems with a limited budget. Previous studies have used various mathematical approaches to optimize settings for DIP, then have designed control algorithms or physical mathematical models.This study did not adopt a mathematical approach for the DIP controller because our DIP has five input parameters within its nondeterministic system model. This paper proposes a novel algorithm, named Uni Neuro, that integrates neural networks(NNs) and a uniform design(UD) in a model formed by input and response to the experimental data(metamodel). We employed a hybrid UD multiobjective genetic algorithm(HUDMOGA) for obtaining the optimized setting input parameters. The UD was also embedded in the HUDMOGA for enriching the solution set, whereas each chromosome used for crossover, mutation, and generation of the UD was determined through a selection procedure and derived individually. Subsequently, we combined the Euclidean distance and Pareto front to improve the performance of the algorithm. Finally, DIP equipment was used to confirm the settings. The proposed algorithm can produce 9 alternative configured input parameter values to swing-up then standing in robust stability of the DIP from only 25 training data items and 20 optimized simulation results. In comparison to the full factorial design, this design can save considerable experiment time because the metamodel can be formed by only 25 experiments using the UD. | 19 | Schwannoma of the upper cervical spine-a case report显示文摘Upper cervical schwannoma is rare,and belongs to benign tumors that is usually asymptomatic.It accounted for only ten percent of schwannomas cases and mostly occurs in 40-50 years old patients.Aggressive and total resection is the treatment of choice for this tumor.Preoperative diagnosis is difficult,relying on clinical suspicion,and confirmed by surgical pathology.We report a 54-year-old male patient with chief complain of progressive weakness and numbness of his right arm for four months.He had a history of lymph node tumor in 2007 and excised in 2011.Neurological decrease was found on the right arm.The radiographic examination showed lytic lesion on the second,third,and fourth cervical spine.Computed tomography scan showed destruction extending to the first cervical vertebra.Capsulated extradural and extramedullary mass and compression to the spinal cord was found from the magnetic resonance imaging.Two-stage operation was performed.The first stage was posterior decompression with occipitocervical fusion and instrumentation,while the second was anterior decompression and iliac strut graft.Both operations performed using the surgical ultrasonic dissector.During three months'follow-up,it showed neurological improving.Neurological deficit appears when there is compression on the spinal cord.Total resection is the treatment of choice for these tumors.Preoperative imaging should be performed to exclude malignant tumor and found tumor extension. | Primadenny Ariesa Airlangga Bambang Prijambodo Aries Rakhmat Hidayat Steesy Benedicta | 2019 | Chinese Journal of Traumatology2019，22，6： | 1 |
