| 2 | 针刺太冲穴得气及疼痛激发相对抗的脑功能网络效应fMRI研究显示文摘目的:采用磁共振脑功能成像技术,比较针刺太冲穴得气及针刺疼痛时所激发的脑功能网络效应异同。方法:47例初次接受针刺的志愿者参加手针右侧太冲穴fMRI实验。针刺时脑fMRI扫描10min,共2次捻针,各2min,间隔3min。其中17例接受体表触觉刺激作为针刺对照,分为3组:①针刺得气组;②针刺得气合并尖锐性疼痛组(简称针刺疼痛组);③触觉对照组。采用Fisher’s精确检验和t检验分析感觉频率及强度;用AFNI处理fMRI数据,得到激活、负激活脑区;用种子相关分析法,以前扣带回膝部为种子点,分析其与全脑的功能网络联系图谱,并比较得气与疼痛组之间的差异。结果:针刺2组的酸痛、酸感、压感、麻感、钝痛等得气感觉的频率和强度均明显强于触觉刺激组;针刺2组间除尖锐性疼痛及酸痛外,各种感觉差异无统计学意义。针刺得气及疼痛2组均激活了体感及丘脑岛叶皮层,重要的是,针刺得气时在大脑边缘叶-旁边缘叶-新皮层系统(LPNN)产生了较强而广泛的负激活区,加强了负激活区之间的脑功能网络联系。此网络包括杏仁核、海马、旁海马、颞极、前额叶腹内侧回、前扣带回膝及下部、前楔叶内侧回和后扣带回(BA31,23_腹侧核,29,30),另外,小脑蚓部、导水管周围灰质及脑干网状结构也见负激活现象。然而,在针刺疼痛时,这些脑区负激活程度减低且区域缩小,大部分脑区fMRI信号逆转为激活信号,被疼痛激活的脑区之间功能网络明显增强。这些脑区参与了疼痛、镇痛(如感觉、情感、认知、自主性、内源性镇痛)功能调制环路。结合既往研究表明,负激活与脑血氧消耗及血流量下降呈正相关。本结果支持针刺通过调制LPNN和疼痛中枢网络的活动,产生其镇痛、抗焦虑和其他调节效应的假说。结论:针刺得气及针刺得气伴随疼痛产生了相对抗的脑功能网络效应。针刺得气对LPNN及疼痛脑中枢网络(Pain matrix)的负激活效应,可能与针刺镇痛脑中枢机制相关。LPNN和静息态默认网络(Default mode)之间的相似性,提示针刺需通过这一内在脑功能网络系统发挥其广泛的调制作用。 | 方继良 Hui KS Kathleen Liu Jing Nixon Erika 周科华 王小玲 朱兵 | 2012 | 中国中西医结合影像学杂志2012,10,1: | 52 |
| 3 | A life support-based comprehensive treatment regimen dramatically lowers the in-hospital mortality of patients with fulminant myocarditis: a multiple center study显示文摘Fulminant myocarditis(FM) has unacceptable high mortality. This study aimed to evaluate the therapeutic efficacy of a life support-based comprehensive treatment regimen(LSBCTR), a completely novel treatment regimen, for FM. A total of 169 FM patients recruited from January 2008 to December 2018 were divided into two groups: patients receiving LSBCTR(81 cases),which includes(i) mechanical life support(positive pressure respiration, intra-aortic balloon pump with or without extracorporeal membrane oxygenation),(ii) immunomodulation therapy using sufficient doses of glucocorticoids and immunoglobulins, and(iii) application of neuraminidase inhibitors, and those receiving conventional treatment(88 cases). The endpoints were in-hospital death and heart-transplantation. Of all the population, 44 patients(26.0%) died in hospitals. Inhospital mortality was 3.7%(3/81) for LSBCTR group and 46.6%(41/88) for traditional treatment(P<0.001). Early application of LSBCTR, mechanical life support, neuraminidase inhibitors, and immunomodulation therapy significantly contributed to reduction in in-hospital mortality. This study describes a novel treatment regimen for FM patients that dramatically reduces inhospital mortality. | 5 | A General Description for Chinese Medicine in Treating Premature Ovarian Failure显示文摘Premature ovarian failure(POF) is a kind of gynecological disease that causes amenorrhea, infertility, menopause and urogenital symptoms. Currently hormone replacement therapy(HRT) is the most popular choice for women with POF to get rid of menopausal syndrome. However, as the popularization of Chinese herbs made Chinese medicine(CM) shine new lights, physicians are able to treat POF with both meno-herbs and integrated therapy. HRT has its own indications and contraindications. For example, unexplained vaginal bleeding, acute liver damage, liver dysfunction, vascular embolization, and breast cancer are all contraindications of HRT, and CM is taken by more physicians as an adjuvant therapy. This review, including a range of common Chinese herbs and formulations according to the existing literature, provides a general description of CM treating POF from the aspects of mechanisms and clinical application. It also highlights acupuncture as a unique physiotherapy for POF. Although the validity of CM has been supported by the evidence of many preclinical trials, clinical trials and meta-analysis, the adverse events with CM therapy still exist and no guarantee has been made for its safety. | 6 | Prevalence and evolution of drug resistance HIV-1 variants in Henan, China显示文摘To understand the prevalence and evolution of drug resistant HIV strains in Henan China after the implementation of free antiretroviral therapy for AIDS patients. 45 drug nave AIDS patients, 118 AIDS patients who received three months antiretroviral therapy and 124 AIDS patients who received six months antiretroviral treatment were recruited in the southern part of Henan province. Information on general condition, antiretroviral medicines, adherence and clinical syndromes were collected by face to face interview. Meanwhile, 14ml EDTA anticoagulant blood was drawn. CD4/CD8 T cell count, viral load and genotypic drug resistance were tested. The rates of clinical improvement were 55.1% and 50.8% respectively three months and six months after antiretroviral therapy. The mean CD4 cell count after antiretroviral therapy was significantly higher than in drug nave patients. The prevalence rate of drug resistant HIV strains were 13. 9%, 45.4% and 62.7% in drug nave patients, three month treatment patients and six month treatment patients, respectively. The number of resistance mutation codons and the frequency of mutations increased significantly with continued antiretroviral therapy. The mutation sites were primarily at the 103, 106 and 215 codons in the three-month treatment group and they increased to 15 codon mutations in the six-month treatment group. From this result, the evolution of drug resistant strains was inferred to begin with the high level NNRTI resistant strain, and then develop low level resistant strains to NRTIs. The HIV strains with high level resistance to NVP and low level resistance to AZT and DDI were highly prevalent because of the AZT+DDI+NVP combination therapy. These HIV strains were also cross resistant to DLV, EFV, DDC and D4T. Poor adherence to therapy was believed to be the main reason for the emergence and prevalence of drug resistant HIV strains. The prevalence of drug resistant HIV strains was increased with the continu- ation of antiretroviral therapy in the southern part of Henan province. | 10 | Shenfu Injection(参附注射液) Inhibits Inflammation in Patients with Acute Myocardial Infarction Complicated by Cardiac Shock显示文摘Objectives: To investigate the effect of Shenfu Injection(参附注射液, SFI) on inflammatory factors in patients with acute myocardial infarction complicated by cardiogenic shock(CS) treated with and intra-aortic balloon pump(IABP). Methods: This study enrolled 60 patients with ST-segment elevation myocardial infarction(STEMI) complicated by CS. Patients underwent IABP and emergency percutaneous coronary intervention(PCI) were randomly divided into two groups by random number table with 30 cases in each group, one given SFI treatment(100 m L/24 h), one not. The two groups were then compared in a clinical setting for left ventricular function, biochemical indicators and Inflammatory factors, including C-reactive proteins(CRP), interleukin-1(IL-1) and tumor necrosis factor alpha(TNF-α). Major adverse cardiac and cerebrovascular events(MACCE) events were compared between patients of the two groups both in-hospital and in follow-ups. Results: The IABP support treatment times of patients in the IABP+SFI group were significantly shorter than the IABP group(52.87±28.84 vs. 87.45±87.31, P=0.047). In the patients of the IABP+SFI group, the CRP peak appeared in 24 h after PCI operation. The CRP peak in the patients of the IABP+SFI group was significantly lower than that in the IABP group(31.27±3.93 vs. 34.62±3.47, P=0.001). The increases in range of TNF-α in the patients of the IABP+SFI group were significantly lower than those of the IABP group(182.29±22.79 vs. 195.54±12.02, P=0.007). The increases in range of IL-1 in the patients of the IABP+SFI group were significantly lower than those of the IABP group(214.98±29.22 vs. 228.60±7.03, P=0.019). The amplitude elevated TNF-α 72 h after admission was an independent risk factor of in-hospital MACCE events(OR 0.973, 95% CI 0.890–0.987, P=0.014) in patients with STEMI and CS. METHODS: The proportion of FOXP3+ Tregs was identified in peripheral blood and tumor tissues of 60 hepatocellular carcinoma (HCC) patients. TLR4 expression was examined in tumor tissues and cell lines. The correlation was examined between FOXP3+ Tregs in peripheral blood and TLR4 expression of HCC tissues. Following activation of TLR4 in H22 murine hepatoma cells pre-incubated with lipopolysaccharide (LPS) and co-cultured with macrophage cell line RAW246.7, the synthesis of cytokines tumor necrosis factor-α, CCL22, and interleukin (IL)-10 by the two cell lines was detected and analyzed. RESULTS: FOXP3+ Tregs were enriched in tumor sites, and circulating FOXP3+ Tregs were increased in HCC patients in correlation with multiple tumor foci and up-regulated TLR4 expression in HCC tissues. Semi-quantitative analysis indicated that TLR4 was over-expressed in HCC compared with the matched normal tissues. Cell cultivation experiments indicated that the mRNAs of IL-10 and CCL22 were significantly up-regulated in the RAW246.7 cell line when co-cultured with LPS preincubated H22 cells. | 18 | Surveillance-based evidence:elimination of schistosomiasis as a public health problem in the Peoples’Republic of China显示文摘Background:A steady progress on schistosomiasis control in the Peoples’Republic of China(P.R.China)was achieved and broadened into the twelve-year medium and long term national plan(MLNP)which marled the implementation of an integrated control strategy across all endemic areas in P.R.China in 2004.To understand the endemic trends of schistosomiasis to assess the effectiveness of an integrated strategy,we conducted an analysis of schistosomiasis surveillance data spanned from 2005 to 2015.Methods:The schistosomiasis sentinel surveillance data from sentinel sites were collected and analyzed from 2005 to 2015.In these sentinel sites,residents aged 6 years or above were screened annually by indirect hemagglutination assay(IHA),while only antibody positives were followed by stool examination either Kato-katz method(KK)and/or hatching technique(HT).Domestic animals raised in sentinel sites were examined by HT for confirming the infection of schistosomes.Snail investigation was conducted each year through systematic sampling method combined with environmental sampling method.The snails collected from field were tested by microscopic dissection method.The infection rates of schistosomes in residents,domestic animals and snails,as well as the indicators reflecting the snails’distribution were calculated and analyzed.ANOVA analysis was used to examine the changes of the number of eggs per gram feces in population and Chi-square test was used to examine any change in proportions among groups.Results:A total of 148902 residents from sentinel sites attended this study and 631676 blood samples were examined by IHA test during the 11 covered years.The annual average antibody positive rates presented a significant decrease trends,from 17.48%(95%CI:17.20–17.75%)in 2005 to 5.93%(95%CI:5.71–6.15%)(χ2=8890.47,P<0.001)in 2015.During 2005–2015,the average infection rate of schistosomes in residents declined from 2.07%(95%CI:1.96–2.17%)to 0.13%(95%CI:0.09–0.16%),accompanied by significant decrease of infection intensity in population.In 2015,the stool positives were only found in farmers,fishermen and boatmen with infection rate of 0.16%(95%CI:0.11–0.20%),0.17%(95%CI:0–0.50%)respectively.The infection rate of schistosomes in domestic animals dropped from 9.42%(538/5711,95%CI:8.66–10.18%)to 0.08%(2/2360,95%CI:0–0.20%)from 2005 to 2015.Infections were found in eight species of domestic animals at the beginning of surveillance while only two cattle were infected in 2015.Totally 98 ha of new snail habitats were found,while 94.90%(93/98)distributed in lake and marshland regions.The percentage of frames with snails decreased from 16.96%(56884/335391,95%CI:16.83–17.09%)in 2005 to 4.28%(18121/423755,95%CI:4.22–4.34%)in 2014,with a slightly increase in 2015.Meanwhile,the infection rate of schistosomes in snails was decreased from 0.26%(663/256531,95%CI:0.24–0.28%)to zero during 2005–2015.Conclusions:The infection rate of schistosomes declined significantly,providing evidence that the goal of the MLNP was achieved.Elimination of schistosomiasis as a public health problem defined as WHO was also reached in P.R.China nationwide.Surveillance-response system should be improved and strengthened to realize the final goal of schistosomiasis elimination. | Jing Xu Shi-Zhu Li Li-Juan Zhang Robert Bergquist Hui Dang Qiang Wang Shan Lv Tian-Ping Wang Dan-Dan Lin Jian-Bing Liu Guang-Hui Ren Kun Yang Yang Liu Yi Dong Shi-Qing Zhang Xiao-Nong Zhou | 2020 | Infectious Diseases of Poverty2020,9,3: | 20 |

| 20 | Randomized controlled trial: Moxibustion and acupuncture for the treatment of Crohn's disease显示文摘AIM: To evaluate the clinical efficacy and safety of acupuncture and moxibustion for the treatment of active Crohn's disease(CD).METHODS: Ninety-two patients were equally and randomly divided into the treatment group and received herb-partitioned moxibustion combined with acupuncture, and the control group received wheat bran-partitioned moxibustion combined with superficial acupuncture. The patients received three treatment sessions per week for 12 wk and were followed up for 24 wk. The main outcome was evaluated using the CD Activity Index(CDAI) score, and the secondary outcomes were evaluated using laboratory indicators such as hemoglobin(HGB), C-reactive protein(CRP), erythrocyte sedimentation rate, quality-of-life, endoscopic ratings, and intestinal histology scores.RESULTS: The CDAI scores of both the treatment and control groups were significantly reduced after treatment compared with those measured before treatment. However, the degree of improvement in the treatment group was significantly greater than that of the control group. The improvement in symptoms in patients of the treatment group was sustained at follow-up, whereas that of the control group was not. The overall efficacy of the treatment was significantly greater than that of the control. Both groups demonstrated significant improvements in quality-of-life ratings after treatment, but the improvement was significantly greater in the treatment group than in the control group.
