1One hundred and one over-the-scope-clip applications forsevere gastrointestinal bleeding,leaks and fistulas显示文摘AIM: To investigate the efficacy and clinical outcome of patients treated with an over-the-scope-clip(OTSC) system for severe gastrointestinal hemorrhage, perforations and fistulas.METHODS: From 02-2009 to 10-2012, 84 patients were treated with 101 OTSC clips. 41 patients(48.8%) presented with severe upper-gastrointestinal(GI) bleeding, 3(3.6%) patients with lower-GI bleeding, 7 patients(8.3%) underwent perforation closure, 18 patients(21.4%) had prevention of secondary perforation, 12 patients(14.3%) had control of secondary bleeding after endoscopic mucosal resection or endoscopic submucosal dissection(ESD) and 3 patients(3.6%) had an intervention on a chronic fistula. RESULTS: In 78/84 patients(92.8%), primary treatment with the OTSC was technically successful. Clinical primary success was achieved in 75/84 patients(89.28%). The overall mortality in the study patients was 11/84(13.1%) and was seen in patients with life threatning upper GI hemorrhage. There was no mortality in any other treatment group. In detail OTSC application lead to a clinical success in 35/41(85.36%) patients with upper GI bleeding and in 3/3 patients with lower GI bleeding. Technical success of perforation closure was 100% while clinical success was seen in 4/7 cases(57.14%) due to attendant circumstances unrelated to the OTSC. Technical and clinic success was achieved in 18/18(100%) patients for the prevention of bleeding or perforation after endoscopic mucosal resection and ESD and in 3/3 cases of fistula closure. Two application-related complications were seen(2%).CONCLUSION: This largest single center experience published so far confirms the value of the OTSC for GI emergencies and complications. 2Modular architecture for fully non-blocking silicon photonic switch fabric显示文摘Integrated photonics offers the possibility of compact,low energy,bandwidth-dense interconnects for large port count spatial optical switches,facilitating flexible and energy efficient data movement in future data communications systems.To achieve widespread adoption,intimate integration with electronics has to be possible,requiring switch design using standard microelectronic foundry processes and available devices.We report on the feasibility of a switch fabric comprised of ubiquitous silicon photonic building blocks,opening the possibility to combine technologies,and materials towards a new path for switch fabric design.Rather than focus on integrating all devices on a single silicon chip die to achieve large port count optical switching,this work shifts the focus towards innovative packaging and integration schemes.In this work,we demonstrate 1×8 and 8×1 microring-based silicon photonic switch building blocks with software control,providing the feasibility of a full 8×8 architecture composed of silicon photonic building blocks.The proposed switch is fully non-blocking,has path-independent insertion loss,low crosstalk,and is straightforward to control.We further analyze this architecture and compare it with other common switching architectures for varying underlying technologies and radices,showing that the proposed architecture favorably scales to very large port counts when considering both crosstalk and architectural footprint.Separating a switch fabric into functional building blocks via multiple photonic integrated circuits offers the advantage of piece-wise manufacturing,packaging,and assembly,potentially reducing the number of optical I/O and electrical contacts on a single die.Dessislava Nikolova David M.Calhoun Yang Liu Sébastien Rumley Ari Novack Tom Baehr-Jones Michael Hochberg Keren Bergman 2017Microsystems & Nanoengineering2017,3,1:3
3Bacterial microcosms obey Taylor’s law:effects of abiotic and biotic stress and genetics on mean and variance of population density显示文摘Introduction:According to the empirical regularity called Taylor’s law,the variance of population density in samples of populations is a power of the mean population density.The exponent is often between 1 and 2.Our experiments investigated how genetics,evolution,and environment shape Taylor’s law.Methods:Genetically different strains(wild type and hypermutator)of the bacterium Pseudomonas fluorescens evolved and were assayed under different environmental conditions(with and without antibiotic rifampicin and bacteriophage SBW252,separately and in combination).Results:Experimental treatments altered the exponent b,but not the power law form,of the relation between variance and mean population density.Bacterial populations treated only with rifampicin had a narrow range of mean population densities and exponent b=5.43.Populations exposed to rifampicin plus phage had b=1.51.In ancestral,control,and phage-exposed populations,mean abundance varied widely and b was not significantly different from 2.Evolutionary factors(mutation rate,selection)and ecological factors(abiotic,biotic)jointly influenced b.Conclusions:Taylor’s power law relationship accurately and robustly described variance as a function of mean population density,with overall exponent b=1.89.These and other experiments with different factors acting on bacterial population size support the relevance of models that predict‘universal’patterns of fluctuation scaling.Oliver Kaltz Patricia Escobar-Páramo Michael E Hochberg Joel E Cohen 2012Ecological Processes2012,1,1:3
