1Radiation exposure and reduction in the operating room: Perspectives and future directions in spine surgery显示文摘Intraoperative imaging is vital for accurate placement of instrumentation in spine surgery. However, the use of biplanar fluoroscopy and other intraoperative imaging modalities is associated with the risk of significant radiation exposure in the patient, surgeon, and surgical staff. Radiation exposure in the form of ionizing radiation can lead to cellular damage via the induction of DNA lesions and the production of reactive oxygen species. These effects often result in cell death or genomic instability, leading to various radiation-associated pathologies including an increased risk of malignancy. In attempts to reduce radiation-associated health risks, radiation safety has become an important topic in the medical field. All practitioners, regardless of practice setting, can practice radiation safety techniques including shielding and distance to reduce radiation exposure. Additionally, optimization of fluoroscopic settings and techniques can be used as an effective method of radiation dose reduction. New imaging modalities and spinal navigation systems have also been developed in an effort to replace conventional fluoroscopy and reduce radiation doses. These modalities include Isocentric Three-Dimensional C-Arms, O-Arms, and intraoperative magnetic resonance imaging. 2Association between calcium sensing receptor gene polymorphisms and chronic pancreatitis in a US population:Role of serine protease inhibitor Kazal 1type and alcohol显示文摘AIM: To test the hypothesis that calcium sensing receptor (CASR) polymorphisms are associated with chronic pancreatitis (CP), and to determine whether serine protease inhibitor Kazal 1type (SPINK1) N34S oralcohol are necessary co-factors in its etiology. METHODS: Initially, 115 subjects with pancreatitis and 66 controls were evaluated, of whom 57 patients and 21 controls were predetermined to carry the high-risk SPINK1 N34S polymorphism. We sequenced CASR gene exons 2, 3, 4, 5 and 7, areas containing the majority of reported polymorphisms and novel mutations. Based on the initial results, we added 223 patients and 239 controls to analyze three common nonsynonymous single nucleotide polymorphisms (SNPs) in exon 7 (A986S, R990G, and Q1011E). RESULTS: The CASR exon 7 R990G polymorphism was signifi cantly associated with CP (OR, 2.01; 95% CI, 1.12-3.59; P = 0.015). The association between CASR R990G and CP was stronger in subjects who reported moderate or heavy alcohol consumption (OR, 3.12; 95% CI, 1.14-9.13; P = 0.018). There was no association between the various CASR genotypes and SPINK1 N34S in pancreatitis. None of the novel CASR polymorphisms reported from Germany and India was detected. CONCLUSION: Our United States-based study confirmed an association of CASR and CP and for the first time demonstrated that CASR R990G is a signifi cant risk factor for CP. 3The microsatellite polymorphism of heme oxygenase-1 is associated with baseline plasma IL-6 level but not with restenosis after coronary in-stenting显示文摘Background Vascular smooth muscle cells (VSMCs) can express heme-oxygenase (HO), a rate-limiting enzyme in the degradation of heme to bilirubin, ferritin and carbon monoxide (CO). VSMC-derived CO can suppress VSMC proliferation and may serve as an antiproliferation factor. The promoter region of HO-1 shows a polymorphism with different (GT)_n repeats that has been reported to differently induce gene expression. The objective of this study was to examine the effect of this variation on the occurrence of restenosis after in-stent treatment in patients with coronary artery disease. Methods Candidates who underwent coronary stent implantation were genotyped for the HO-1 promoter polymorphism using polymerase chain reaction (PCR) and automated DNA capillary sequencer. Serum levels of IL-6 and C-reactive protein (CRP) were obtained at baseline, 24 hours and 48 hours after stenting. The primary end point for the study was angiographic evidence of in-stent restenosis at 6 months. All parameters for evaluation of restenosis were analysed by quantitatve computer-assisted angiographic analysis (QCA). Results One hundred and eighty-seven patients who underwent coronary stent implantation were studied of whom 27.8% showed ≥50% restenosis after 6 months. The distribution of (GT)_n repeats of all patients in the promoter region of HO-1 genotype ranged from 22 to 42, with (GT)_ 25 and (GT)_ 32 being the two most common alleles. The allelic repeats were divided into the short class (S) with 29 (GT)_n, the middle class (M) with 30-37 (GT)_n and the long class (L) with 38 (GT)_n. There was no significant difference in the restenosis between the genotype groups or between post operation levels of inflammation markers, but carriers of the S allele (n=120) had 33.3% lower baseline IL-6 compared with non-S carriers (n=67, P=0.0008). Conclusions Although no association was observed between the HO-1 promoter polymorphism aod coronary in-stent restenosis following the stent procedure, the association with plasma IL-6 levels suggests that HO-1 S allele might protect from the atherosclerotic inflammatory process.LI Ping Mohamed A. 4Occurrence, spatial distribution and ecological risks of antibiotics in soil in urban agglomeration显示文摘Antibiotics in soil environment are regarded as emerging pollutants and have introduced increasing risks to soil ecosystem and human health in rapid urbanization areas. Identifying the occurrence and spatial variability of antibiotics in soils is an urgent issue in sustaining soil security. In this study, antibiotics in soils were investigated and analyzed in BeijingTianjin-Hebei urban agglomeration. The occurrence, spatial distribution, and related affecting factors of antibiotics in soils were identified and ecological risks of antibiotics in soil environment were assessed. Results showed that(1) The mean concentration of soil antibiotics in Beijing-Tianjin-Hebei urban agglomeration was 21.79 μg/kg. Land use substantially affected the occurrence and concentration of antibiotics in soils. Concentrations of antibiotics in cropland and orchard soils were 2-3 times higher than the other land use types.(2)The concentrations of antibiotics in soils in Beijing-Tianjin-Hebei urban agglomeration presented a spatial pattern of high values in southeast, and low values in northwest. Spatial variability of antibiotics in soils was closely related to the application of organic fertilizer and wastewater irrigation as well as topographical features. Furthermore, soil properties and land management policy had substantial influences on soil antibiotics, and soil heavy metals may aggravate the accumulation of antibiotics in soils.(3) Ecological risks assessment of antibiotics in soils demonstrated that erythromycin(ERY), sulfamethoxazole(SMX), and doxycycline(DOX) may introduce high risks to soil ecosystem health, and more attention should be paid to the areas with intensive human activities that had potential high risk to soil ecosystem health. It takes the form of an extremely dynamic and pleomorphic membrane-bounded network of tubules and cisternae which have numerous different cellular functions. 6Uncertainty analysis for integrated water system simulations using GLUE with different acceptability thresholds显示文摘Integrated water quantity and quality simulations have become a popular tool in investigations on global water crisis.For integrated and complex models,conventional uncertainty estimations focus on the uncertainties of individual modules,e.g.,module parameters and structures,and do not consider the uncertainties propagated from interconnected modules.Therefore,this study investigated all the uncertainties of integrated water system simulations using the GLUE(i.e.,generalized likelihood uncertainty estimation)method,including uncertainties associated with individual modules,propagated uncertainties associated with interconnected modules,and their combinations.The changes in both acceptability thresholds of GLUE and the uncertainty estimation results were also investigated for different fixed percentages of total number of iterations(100000).Water quantity and quality variables(i.e.,runoff and ammonium nitrogen)were selected for the case study.The results showed that module uncertainty did not affect the runoff simulation performance,but remarkably weakened the water quality responses as the fixed percentage increased during calibration and validation periods.The propagated uncertainty from hydrological modules could not be ignored for water quality simulations,particularly during validation.The combination of module and propagated uncertainties further weakened the water quality simulation performance.The uncertainty intervals became wider owing to an increase in the fixed percentages and introduction of more uncertainty sources.Moreover,the acceptability threshold had a negative nonlinear relationship with the fixed percentage.The fixed percentages(20.0%-30.0%)were proposed as the acceptability thresholds owing to the satisfactory simulation performance and noticeably reduced uncertainty intervals they produced.This study provided methodological foundations for estimating multiple uncertainty sources of integrated water system models.ZHANG YongYong XIA Jun SHAO QuanXi LI Lu YEN Haw ZHAI XiaoYan ZHAO TongTieGang LIN KaiRong 2021Science China(Technological Sciences)2021,64,8:3
7Numerical Simulation on a Tremendous Debris Flow Caused by Typhoon Morakot in the Jiaopu Stream,Taiwan显示文摘In August 2009,Typhoon Morakot brought a large amount of rainfall with both high intensity and long duration to a vast area of Taiwan.Unfortunately,this resulted in a catastrophic landslide in Hsiaolin Village,Taiwan.Meanwhile,large amounts of landslides were formed in the Jiaopu Stream watershed near the southeast part of the Hsiaolin Village.The Hsiaolin Village access road(Provincial Highway No.21 and Bridge No.8) was completely destroyed by the landslide and consequent debris flow.The major scope of this study is to apply a debris flow model to simulate the disaster caused by the debris flow that occurred in the Jiaopu Stream during Typhoon Morakot.According to the interviews with local residents,this study applied the destruction time of Bridge No.8 and Chen's house to verify the numerical debris flow model.By the spatial rainfall distributions information,the numerical simulations of the debris flow are conducted in two stages.In the first stage(before the landslide-dam failure),the elevation of the debris flow and the corresponding potential damages toward residential properties were investigated.In the second stage(after the landslidedam failure),comparisons of simulation results between the longitudinal and cross profiles of the Jiaopu Stream were performed using topographic maps and satellite imagery.In summary,applications of the adopted numerical debris flow model have shown positive impact on supporting better understanding of the occurrence and movement of debris flow processes.HSU Yu-chao YEN Haw TSENG Wen-hsiao JAN Chyan-deng 2014Journal of Mountain Science2014,11,1:3
81.3 μm InAs/GaAs quantum dot lasers on silicon with GaInP upper cladding layers显示文摘We report on the first electrically pumped continuous-wave(CW) In As/Ga As quantum dot(QD) laser grown on Si with a GaInP upper cladding layer. A QD laser structure with a Ga_(0.51)In_(0.49)P upper cladding layer and an Al_(0.53)Ga_(0.47)As lower cladding layer was directly grown on Si by metal–organic chemical vapor deposition. It demonstrates the postgrowth annealing effect on the QDs was relieved enough with the GaInP upper cladding layer grown at a low temperature of 550°C. Broad-stripe edge-emitting lasers with 2-mm cavity length and 15-μm stripe width were fabricated and characterized. 15Inflammatory response to coronary artery bypass surgery: Does the heme-oxygenase-1 gene microsatellite polymorphism play a role?显示文摘Background Heme-oxygenase 1 (HO-1) is a rate-limiting enzyme in the degradation of heme to bilirubin, ferritin and carbon monoxide (CO) and may have significant anti-inflammatory function. The HO-1 gene promoter region shows microsatellite polymorphism with different (GT)n repeats, reported to differently induce gene expression, with the short allele associated with higher gene expression. We measured the acute inflammatory response using coronary artery bypass surgery (CABG) as a well-characterized and uniform stimulus and examined the correlation between levels of IL-6, C-reactive protein (CRP) and fibrinogen and their relationship to HO-1 genotype.Methods Two hundred and seventy-five consecutive patients undergoing CABG were genotyped for the HO-1 promoter polymorphism using PCR and automated DNA capillary sequencer. IL-6, CRP and fibrinogen were measured at baseline and 6, 24, 48, 72, 96 and 120 hours after CABG.Results Complete IL-6,CRP and fibrinogen measures were available in 220 patients. Before surgery IL-6 levels showed a strong correlation with CRP and fibrinogen ( r =0.48, P <0.0001; r =0.41, P <0.0001 respectively), with a significant correlation between CRP and fibrinogen ( r =0.61, P <0.0001). All three acute phase reactants showed a significant increase after CABG. After surgery, peak IL-6 was strongly correlated with peak CRP ( r =0.34, P =0.0009) but not with peak fibrinogen ( r =0.15, P =0.13), while peak CRP and peak fibrinogen were significantly correlated ( r =0.415, P <0.0001). HO-1 allelic repeats ranged from 22-42, with (GT) 25 and (GT) 32 being the two most common alleles, and subsequently divided into three groups according to previous published work: <30(GT)n were designated as S (short), 30-37(GT)n as M (middle) and long repeats with >37(GT)n as L (long); allele frequency 0.35, 0.58 and 0.07 respectively. Baseline CRP differed by genotype: those carrying at least one long allele having higher CRP than those with no long allele (3.76±0.79 vs. 2.07±0.17, P =0.013). Conversely, those carrying at least one short allele had higher fibrinogen levels than those with no short allele (3.83±0.79 vs. 3.51±0.88, P =0.006). Conclusions There is a strong correlation between the measured acute phase reactants both at baseline and after the inflammatory response to CABG in patients with coronary disease. There was an association between the HO-1 microsatellite polymorphism and CRP and fibrinogen levels at baseline but there was no similar association following CABG.
