1Role of surgery for colorectal cancer in the elderly显示文摘The prevalence of subjects with colorectal cancer is expected to grow in the next future decades and surgery represents the most successful treatment modality for these patients. Anyway, currently elderly subjects undergo less elective surgical procedures than younger patients mainly due to the high rates of postoperative morbidity and mortality. Some authors suggest extensive surgery, including multistage procedures, as carried out in younger patients while others promote less aggressive surgery. In older patients, laparoscopic-assisted colectomy showed a number of advantages compared to conventional open surgery that include lower stress, higher rate of independency after surgery, quicker return to prior activities and a decrease in costs. The recent advances in chemotherapy and the introduction of new surgical procedures such as the endoluminal stenting, suggest the need for a revisitation of surgical practice patterns and the role of palliative surgery, mainly for patients with advanced disease. 2Markers of systemic inflammation and colorectal adenoma risk: Meta-analysis of observational studies显示文摘AIM To perform a meta-analysis of observational studies on inflammatory markers levels and occurrence of colorectal adenoma.METHODS Pub Med and EMBASE databases were searched until March 2016 for the articles reporting on the circulating levels of inflammatory markers, including: C-reactive protein(CRP), interleukin-6(IL-6), and tumor necrosis factor-alpha(TNF-a) and risk of colorectal adenoma. Random-effects models were used to calculate summary odds ratios(ORs) with 95%CIs for the highest vs lowest category of exposure. Heterogeneity was assessed by using the Q test and I2 statistic. Subgroup analyses were also performed to test for potential source of heterogeneity.RESULTS A total of 14 case-control studies were included. Ten studies on CRP including a total of 3350 cases and 4168 controls showed non-significant summary(OR = 1.23, 95%CI: 0.98-1.54; I2 = 54%, P heterogeneity = 0.01) in the general analysis, but significant increased odds when considering only advanced adenoma(OR = 1.59, 95%CI: 1.09-2.32; I2 = 44%, P heterogeneity = 0.15). Subgroup and stratified analyses revealed a potential influence of smoking status and aspirin use on the association between CRP levels and colorectal adenoma. Five studies examined the association between circulating levels of TNF-a and colorectal adenoma risk, including a total of 1,568 cases and 2,832 controls. The summary OR for the highest vs the lowest category of exposure was 1.00(95%CI: 0.77-1.29). The relationship between circulating IL-6 levels and colorectal adenoma risk was investigated in 7 studies including a total of 1936 cases and 3611 controls. The summary OR for the highest vs the lowest category of exposure was 1.19(95%CI: 0.92-1.55).CONCLUSION Summary of current evidence suggests a positive association of CRP levels and advanced colorectal adenoma risk. 3Molecular classifications of gastric cancers: Novel insights and possible future applications显示文摘Despite some notable advances in the systemic management of gastric cancer(GC), the prognosis of patients with advanced disease remains overall poor and their chance of cure is anecdotic. In a molecularly selected population, a median overall survival of 13.8 mo has been reached with the use of human epidermal growth factor 2(HER2) inhibitors in combination with chemotherapy, which has soon after become the standard of care for patients with HER2-overexpressing GC. Moreover, oncologists have recognized the clinical utility of conceiving cancers as a collection of different molecularlydriven entities rather than a single disease. Several molecular drivers have been identified as having crucial roles in other tumors and new molecular classifications have been recently proposed for gastric cancer as well. Not only these classifications allow the identification of different tumor subtypes with unique features, but also they serve as springboard for the development of different therapeutic strategies. Hopefully, the application of standard systemic chemotherapy, specifictargeted agents, immunotherapy or even surgery in specific cancer subgroups will help maximizing treatment outcomes and will avoid treating patients with minimal chance to respond, therefore diluting the average benefit. 9Role of contrast-enhanced ultrasound in the evaluation of vascularization of hepatocellular carcinoma显示文摘Aim:Early individualization of hepatocellular carcinoma is crucial to obtain good therapeutic results,thanks to several options such as percutaneous therapies,surgical resections and transplant.Aim of this study is to evaluate the vascularization of hepatocarcinoma using contrast enhanced ultrasound(CEUS)in comparison with multislice computed thomography(MSCT).Methods:Between January 2009 and May 2014,67 patients affected by hepatocarcinoma,who presented an overall of 92 nodules,were examined and enrolled in the study.Results:There was a significant difference in the percentage of comparison of the vascularization between the nodules situated at a depth not greater than 9 cm,compared to those studied at a greater depth.In reference to the size of the lesion,the percentage of vascularization to the CEUS in arterial phase,compared with the MSCT,was 84%in lesions with dimensions equal or less than 1 cm,91%in lesions with dimensions included between 1 and 2 cm,and 96%in the lesions greater than 2 cm.Conclusion:CEUS is a method capable of documenting with very reliable accuracy the intralesional vascularization of hepatic carcinoma,in a superimposable manner to the MSCT.However,CEUS also presents some limitations,mainly in relation to the site of lesions.Francesco Loria Giuseppe Loria Salvatore Basile Giuseppe Crea Luciano Frosina Francesca Frosina 2016Hepatoma Research2016,2,1:1
10Robot-assisted oncologic pelvic surgery with Hugo™robot-assisted surgery system:A single-center experience显示文摘Objective To report the outcomes of intra-and extra-peritoneal robot-assisted radical prostatectomy(RARP)and robot-assisted radical cystectomy(RARC)with Hugo™robot-assisted surgery(RAS)system(Medtronic,Minneapolis,MN,USA).Methods Data of twenty patients who underwent RARP and one RARC at our institution between February 2022 and January 2023 were reported.The primary endpoint of the study was to report the surgical setting of Hugo™RAS system to perform RARP and RARC.The secondary endpoint was to assess the feasibility of RARP and RARC with this novel robotic platform and report the outcomes.Results Seventeen patients underwent RARP with a transperitoneal approach,and three with an extraperitoneal approach;and one patient underwent RARC with intracorporeal ileal conduit.No intraoperative complications occurred.Median docking and console time were 12(interquartile range[IQR]7-16)min and 185(IQR 177-192)min for transperitoneal RARP,15(IQR 12-17)min and 170(IQR 162-185)min for extraperitoneal RARP.No intraoperative complications occurred.One patient submitted to extraperitoneal RARP had a urinary tract infection in the postoperative period that required an antibiotic treatment(Clavien-Dindo Grade 2).In case of transperitoneal RARP,two minor complications occurred(one pelvic hematoma and one urinary tract infection;both Clavien-Dindo Grade 2).Conclusion Hugo™RAS system is a novel promising robotic platform that allows to perform major oncological pelvic surgery.We showed the feasibility of RARP both intra-and extra-peritoneally and RARC with intracorporeal ileal conduit with this novel platform.Angelo Territo Alessandro Uleri Andrea Gallioli Josep Maria Gaya Paolo Verri Giuseppe Basile Alba Farré Alejandra Bravo Alessandro Tedde Óscar Rodríguez Faba Joan Palou Alberto Breda 2023Asian Journal of Urology2023,10,4:0
11Carbon concentrations and carbon storage capacity of three old-growth forests in the Sila National Park,Southern Italy显示文摘Old-growth forests play a key-role in reducing atmospheric carbon dioxide(CO_(2)) concentrations by storing large CO_(2)amounts in biomass and soil over time.This quantifies the carbon pool into different forest compartments in three Mediterranean old-growth forests of Southern Italy populated by Pinus laricio,Fagus sylvatica and Abies alba.Ecosystem carbon pools have been assessed per compartment,i.e.,living trees,dead wood,litterfall(foliar and woody),roots and 0-20 cm topsoil,combining the whole old-growth forest mass,(i.e.,using tree allometric relationships,deadwood factor conversions,root-to-shoot ratios,litterfall and soil samplings) by the respective organic carbon concentrations.The results show the considerable capacity of these forest ecosystems in storing CO_(2)in biomass and soil,with carbon pool values ranging from 532.2to 596.5 Mg C ha-1.Living trees and 0-20 cm topsoil had larger carbon pool,contributing 53.0 and 22.1%,respectively.In most cases,organic carbon concentration was higher(more than 60%) than the average carbon conversion rate of 50%,especially in living trees,deadwood,and woody litterfall.This study contributes further scientific evidence of the capacity of old-growth forests in storing CO_(2)in their different compartments,with special evidence on tree biomass,litterfall and mineral soil,thereby highlighting the key role of old-growth forests within the challenge of climate change mitigation.Giovanni Di Matteo Giuseppe Luzzi Antonio Basile Angelo Sposato Giada Bertini Ulderico Neri Bruno Pennelli Rosario Napoli Pierfrancesco Nardi 2023Journal of Forestry Research2023,34,1:0
