1Tenofovir vs lamivudine plus adefovir in chronic hepatitis B:TENOSIMP-B study显示文摘AIM To demonstrate the non-inferiority(15% non-inferiority limit) of monotherapy with tenofovir disoproxil fumarate(TDF) vs the combination of lamivudine(LAM) plus adefovir dipivoxil(ADV) in the maintenance of virologic response in patients with chronic hepatitis B(CHB) and prior failure with LAM.METHODS This study was a Phase IV prospective, randomized, open, controlled study with 2 parallel groups(TDF and LAM+ADV) of adult patients with hepatitis B e antigen(HBe Ag)-negative CHB, prior failure with LAM, on treatment with LAM+ADV for at least 6 mo, without prior resistance to ADV and with an undetectable viral load at the start of the study, in 14 Spanish hospitals. The follow-up time for each patient was 48 wk after randomization, with quarterly visits in which the viral load, biochemical and serological parameters, adverse effects, adherence to treatment and consumption of hospital resources were analysed.RESULTS Forty-six patients were evaluated [median age: 55.4 years(30.2-75.2); 84.8% male], including 22 patients with TDF and 24 with LAM+ADV. During study development, hepatitis B virus DNA(HBV-DNA) remained undetectable, all patients remained HBe Ag negative, and hepatitis B surface antigen(HBs Ag) positive. Alanine aminotransferase(ALT) values at the end of the study were similar in the 2 groups(25.1± 7.65, TDF vs 24.22 ± 8.38, LAM+ADV, P = 0.646). No significant changes were observed in creatinine or serum phosphorus values in either group. No significant differences between the 2 groups were noted in the identification of adverse effects(AEs)(53.8%, TDF vs 37.5%, LAM+ADV, P = 0.170), and none of the AEs which occurred were serious. Treatment adherence was 95.5% and 83.3% in the TDF and the LAM+ADV groups, respectively(P = 0.488). The costs associated with hospital resource consumption were significantly lower with the TDF treatment than the LAM+ADV treatment(€4943 ± 1059 vs €5811 ± 1538, respectively, P < 0.001).CONCLUSION TDF monotherapy proved to be safe and not inferior to the LAM+ADV combination therapy in maintaining virologic response in patients with CHB and previous LAM failure. 3Umbilical cord as a mesenchymal stem cell source for treating joint pathologies显示文摘Articular cartilage disorders and injuries often result in life-long chronic pain and compromised quality of life.Regrettably,the regeneration of articular cartilage is a continuing challenge for biomedical research.One of the most promising therapeutic approaches is cellbased tissue engineering,which provides a healthy population of cells to the injured site but requires differentiated chondrocytes from an uninjured site.The use of healthy chondrocytes has been found to have limitations.A promising alternative cell population is mesenchymal stem cells(MSCs),known to possess excellent proliferation potential and proven capability for differentiation into chondrocytes.The“immunosuppressive”property of human MSCs makes them an important candidate for allogeneic cell therapy.The use of allogeneic MSCs to repair large defects may prove to be an alternative to current autologous and allogeneic tissue-grafting procedures.An allogeneic cell-based approach would enable MSCs to be isolated from any donor,expanded and cryopreserved in allogeneic MSC banks,providing a readily available source of progenitors for cell replacement therapy.These possibilities have spawned the current exponential growth in stem cell research in pharmaceutical and biotechnology communities.Our objective in this review is to summarize the knowledge about MSCs from umbilical cord stroma and focus mainly on their applications for joint pathologies.María Carmen Arufe Alexandre De la Fuente Isaac Fuentes Francisco Javier De Toro Francisco Javier Blanco 2011World Journal of Orthopedics2011,2,6:7
4Gene expression changes in spleens of the wildlife reservoir species,Eurasian wild boar(Sus scrofa),naturally infected with Brucella suis biovar 2显示文摘Brucella suis is responsible for swine brucellosis worldwide.Of the five different B.suis biovars(bv.),bv.2 appears restricted to Europe where it is frequently isolated from wild boar and hares,can infect pigs and can cause human brucellosis.In this study,the differential gene expression profile was characterized in spleens of Eurasian wild boar naturally infected with B.suis bv.2.Of the 20,201 genes analyzed in the microarray,633 and 1,373 were significantly(fold change > 1.8;P < 0.01) upregulated and downregulated,respectively,in infected wild boar.The analysis was focused on genes that were over represented after conditional test for biological process gene ontology.Upregulated genes suggested that B.suis bv.2 infection induced cell maturation,migration and/or proliferation in infected animals.The genes downregulated in infected wild boar impaired the activity of several important cellular metabolic pathways such as metabolism,cytoskeleton organization and biogenesis,immune response and lysosomal function and vesicle-mediated transport.In addition,the response to stress,sperm fertility,muscle development and apoptosis seemed to be also impaired in infected animals.These results suggested that B.suis bv.2 may use strategies similar to other smooth brucellae to facilitate intracellular multiplication and the development of chronic infections.To our knowledge,this is the first report of the analysis of gene expression profile in hosts infected with B.suis bv.2,which is important to understand the molecular mechanisms at the host-pathogen interface in the main reservoir species with possible implications in the zoonotic cycle of the pathogen.Ruth C.Galindo Pilar M.Muoz María J.de Miguel Clara M.Marin Javier Labairu Miguel Revilla José M.Blasco Christian Gortazar José de la Fuente 2010Journal of Genetics and Genomics2010,37,11:6

7Duodenal biopsy may be avoided when high transglutaminase antibody titers are present显示文摘AIM:To evaluate the predictive value of tissue transglutaminase (tTG) antibodies for villous atrophy in adult and pediatric populations to determine if duodenal biopsy can be avoided.METHODS: A total of 324 patients with celiac disease (CD; 97 children and 227 adults) were recruited prospectively at two tertiary centers. Human IgA class anti-tTG antibody measurement and upper gastrointestinal endoscopy were performed at diagnosis. A second biopsy was performed in 40 asymptomatic adults on a gluten-free diet (GFD) and with normal tTG levels.RESULTS: Adults showed less severe histopathology (26% vs 63%, P<0.0001) and lower tTG antibody titers than children. Levels of tTG antibody correlated with Marsh type in both populations (r=0.661, P<0.0001). Multiple logistic regression revealed that only tTG antibody was an independent predictor for Marsh type 3 lesions, but clinical presentation type and age were not. A cut-off point of 30 U tTG antibody yielded the highest area under the receiver operating characteristic curve (0.854). Based on the predictive value of this cut-off point, up to 95% of children and 53% of adults would be correctly diagnosed without biopsy. Despite GFDs and decreased tTG antibody levels, 25% of the adults did not recover from villous atrophy during the second year after diagnosis.CONCLUSION: Strongly positive tTG antibody titers might be sufficient for CD diagnosis in children. 12Management of lung cancer in the COVID-19 pandemic:a review显示文摘Since its inception,the COVID-19 pandemic has affected health care as a whole.Cancer patients in general and those suffering from lung cancer in particular are a vulnerable group because of their many intrinsic characteristics and care needs.How SARS-CoV-2(COVID-19)infection affects these patients regarding their risk of infection and outcome in this patient cohort is still to be determined.In this review,we tried to summarize our main concerns regarding COVID-19 in the context of cancer patients from a clinical and multidisciplinary approach.Different types of lung cancer treatments(chemotherapy,radiation therapy and immunotherapy)may also influence the risk of infection and condition the patient´s risk of having a worse outcome.Lung cancer patients require frequent radiologic study follow-ups,which may be affected by COVID-19 pandemic.COVID-19 related incidental radiologic findings can appear in routinely scheduled radiology tests,which may be difficult to interpret.Also cancer treatment induced pneumonitis may have similar radiologic features similar to those in acute SARS-CoV-2 pneumonia and lead to a wrong diagnosis.The different health care needs,the requirement for continuous health care access and follow-ups,and the clinical traials in which this patient population might be enrrolled are all being affected by the current COVID-19 health crisis.The COVID-19 pandemic has put health care providers and institutions in difficult situations and obliged them to face challenging ethical scenarios.These issues,in turn,have also affected the psychological well-being of health care workers.Fernando Maroto Piñeiro Carmen Trinidad López Javier de la Fuente Aguado 2021Journal of Cancer Metastasis and Treatment2021,7,1:0
13Hepatitis C virus NS5A region mutation in chronic hepatitis C genotype 1 patients who are non-responders to two or more treatments and its relationship with response to a new treatment显示文摘AIM To determine the number of mutations in the NS5 A region of the hepatitis C virus(HCV) and its relationship to the response to antiviral therapy in patients with chronic hepatitis C genotype 1 who are non-responders to two or more treatments. METHODS Sequences within HCV NS5 A [PKR binding domain(PKRBD) and the interferon-sensitivity-determining region(ISDR)] were analysed via direct sequencing in a selected cohort of 72 patients, with a total of 201 treatments [interferon-alpha(IFN-α), n = 49; IFN-α + ribavirin(RBV), n = 75; pegylated(peg) IFN-α + RBV, n = 47; first-generation direct-acting antivirals(DAAs), n = 13; and second-generation DAAs, n = 17]. Of these, 48/201 achieved a sustained virological response(SVR) and 153/201 achieved no virological response(NVR).RESULTS For both regions, treatments resulting in SVR were associated with more baseline mutations than were treatments resulting in NVR(SVR vs NVR; PKRBD: 5.82 ± 3 vs 4.86 ± 2 mutations, P = 0.045; ISDR: 2.65 ± 2 vs 1.51 ± 1.7 mutations, P = 0.005). A decrease or no change in the number of mutations over time between treatments in the PKRBD or ISDR, as shown by sequencing, was associated with patients who usually failed to respond to treatment(PKRBD, P = 0.02; ISDR, P = 0.001). Moreover, patients showing a post-treatment baseline viral load > 600000 IU/m L and increased ISDR mutations with respect to the previous treatment were 9.21 times more likely to achieve SVR(P = 0.001). 14Binary Fruit Fly Swarm Algorithms for the Set Covering Problem显示文摘Currently,the industry is experiencing an exponential increase in dealing with binary-based combinatorial problems.In this sense,metaheuristics have been a common trend in the field in order to design approaches to solve them successfully.Thus,a well-known strategy consists in the use of algorithms based on discrete swarms transformed to perform in binary environments.Following the No Free Lunch theorem,we are interested in testing the performance of the Fruit Fly Algorithm,this is a bio-inspired metaheuristic for deducing global optimization in continuous spaces,based on the foraging behavior of the fruit fly,which usually has much better sensory perception of smell and vision than any other species.On the other hand,the Set Coverage Problem is a well-known NP-hard problem with many practical applications,including production line balancing,utility installation,and crew scheduling in railroad and mass transit companies.In this paper,we propose different binarization methods for the Fruit Fly Algorithm,using Sshaped and V-shaped transfer functions and various discretization methods to make the algorithm work in a binary search space.We are motivated with this approach,because in this way we can deliver to future researchers interested in this area,a way to be able to work with continuous metaheuristics in binary domains.This new approach was tested on benchmark instances of the Set Coverage Problem and the computational results show that the proposed algorithm is robust enough to produce good results with low computational cost.Broderick Crawford Ricardo Soto Hanns de la Fuente Mella Claudio Elortegui Wenceslao Palma Claudio Torres-Rojas Claudia Vasconcellos-Gaete Marcelo Becerra Javier Pena Sanjay Misra 2022Computers, Materials & Continua2022,,6:0
15Acceptance of living liver donation among medical students: A multicenter stratified study from Spain显示文摘AIM: To analyze the attitude of Spanish medical students toward living liver donation(LLD) and to establish which factors have an influence on this attitude.METHODS: Study type: A sociological, interdisciplinary, multicenter and observational study. Study population: Medical students enrolled in Spain(n = 34000) in the university academic year 2010-2011. Sample size: A sample of 9598 students stratified by geographical area and academic year. Instrument used to measure attitude: A validated questionnaire(PCID-DVH RIOS) was self-administered and completed anonymously. Data collection procedure: Randomly selected medical schools. The questionnaire was applied to each academic year at compulsory sessions. Statistical analysis: Student′s t test, χ2 test and logistic regression analysis.RESULTS: The completion rate was 95.7%(n = 9275). 89%(n = 8258) were in favor of related LLD, and 32%(n = 2937) supported unrelated LLD.
