2Genetic susceptibility to Candida infection: a new look at an old entity显示文摘The identification of the first molecular defect leading to Mendelian Susceptibility to Mycobacterial Disease (MSMD),1 a rare syndrome conferring predisposition to disease caused by weakly virulent mycobacteria (such as Mycobacterium bovis,Bacille Calmette Guérin vaccines and environmental mycobacteria),has led to a paradigm shift in the field of primary immunodeficiencies in the last two decades.The 'classic' patient with multiple immunologic abnormalities,conferring a broad susceptibility to multiple and recurrent infectious diseases caused by both weakly pathogenic and more virulent microorganisms,was the main Primary Immunodeficiency (PID) phenotype identified.Since 1996,mutations causing MSMD have been found in at least 6 genes related to interferon (IFN)-γ-mediated immunity.2 Intriguingly,this has opened the door to the identification of a molecular pathogenesis in patients affected by a narrow to extremely narrow susceptibility to bacterial,viral and fungal infections.3-7 These patients,more common than originally thought,are often otherwise healthy and are not prone to other unusually severe infections.Davide Firinu Maria Pisanu Bruno Piras Raffaella Meleddu Maria Maddalena Lorrai Paolo Emilio Manconi Stefano R. 12COVID-19: Considerations about immune suppression and biologicals at the time of SARS-CoV-2 pandemic显示文摘The extent of the profound immunological and nonimmunological responses linked to severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)infection is currently being investigated worldwide due to the large burden associated with death due to SARS-CoV-2 and the short-term consequences of coronavirus disease 2019(COVID-19).It has been hypothesized that patients on immunosuppressive treatments,including biologics,may have an augmented risk of being infected by SARS-CoV-2;however,there are currently no definitive data about biological drugs and COVID-19 in immune-mediated inflammatory diseases.Current epidemiological models developed to understand how long the COVID-19 epidemic may last are not conclusive and range from sustained epidemics to complete elimination.Nevertheless,even in the best-case scenario of apparent elimination,there is concordance about a possible contagion resurgence as late as 2024.Therefore,knowledge of the impact of SARS-CoV-2 on immunemediated diseases and among patients treated with biologicals,together with the results of novel and promising COVID-19 treatment strategies targeting the virus and the host immune response(or both),will help us to best manage our patients during this pandemic over the next few years.Giulia Costanzo William Cordeddu Luchino Chessa Stefano Del Giacco Davide Firinu 2021World Journal of Clinical Cases2021,9,20:0
13Exercise-induced anaphylaxis with an Ayurvedic drug as cofactor:A case report显示文摘BACKGROUND The practice of Indian Ayurvedic medicine is spreading in Western countries and Shilajit is one of the most used drugs, for its antioxidant activities and immunomodulatory effects. Albeit Shilajit has showed a high degree of safety, it can act as cofactor of anaphylaxis, especially in condition at high risk, such as mast cell activation syndrome(MCAS). We reported this case to sensitize practitioners to investigate to the use of complementary and alternative medicine,in case of exercise-induced anaphylaxis(EIAn).CASE SUMMARY A 43-year-old woman, working as a dance teacher, developed urticaria after ingestion of rice, tuna and Shilajit, which did not respond to intramuscular corticosteroids. Subsequently, she developed dyspnoea and hypotension with loss of consciousness that arose 1 h after sexual activity. The patient did not refer personal history of atopy. Specific IgE for main food allergens resulted negative,with total IgE levels of 14 IU/L. Oral provocation test with Shilajit was not perfomed because the patient refused, but we performed prick-by-prick and patch test that resulted negative. Serum tryptase at the time of anaphylaxis was 20.6 μg/L that fell down to of 10.6 μg/L after therapy, but has remained at the high value after two days and during the follow-up. We performed an analysis of the c-KIT gene in peripheral blood, which was negative. We felt the diagnosis consistent with EIAn in a patient with a possible MCAS.CONCLUSION In Western countries the use of drugs from Ayurvedic medicine is more common than in the past.
