1Pathogenesis and clinical management of Helicobacter pylori gastric infection显示文摘Helicobacter pylori(H.pylori)is a gram-negative bacterium that infects approximately 4.4 billion individuals worldwide.However,its prevalence varies among different geographic areas,and is influenced by several factors.The infection can be acquired by means of oral-oral or fecal-oral transmission,and the pathogen possesses various mechanisms that improve its capacity of mobility,adherence and manipulation of the gastric microenvironment,making possible the colonization of an organ with a highly acidic lumen.In addition,H.pylori presents a large variety of virulence factors that improve its pathogenicity,of which we highlight cytotoxin associated antigen A,vacuolating cytotoxin,duodenal ulcer promoting gene A protein,outer inflammatory protein and gamma-glutamyl transpeptidase.The host immune system,mainly by means of a Th1-polarized response,also plays a crucial role in the infection course.Although most H.pylori-positive individuals remain asymptomatic,the infection predisposes the development of various clinical conditions as peptic ulcers,gastric adenocarcinomas and mucosa-associated lymphoid tissue lymphomas.Invasive and non-invasive diagnostic methods,each of them with their related advantages and limitations,have been applied in H.pylori detection.Moreover,bacterial resistance to antimicrobial therapy is a major challenge in the treatment of this infection,and new therapy alternatives are being tested to improve H.pylori eradication.Last but not least,the development of effective vaccines against H.pylori infection have been the aim of several research studies.Breno Bittencourt de Brito Filipe Ant?nio Fran?a da Silva Aline Silva Soares Vinícius Afonso Pereira Maria Luísa Cordeiro Santos Mariana Miranda Sampaio Pedro Henrique Moreira Neves Fabrício Freire de Melo 2019World Journal of Gastroenterology2019,25,37:84

3Helicobacter pylori infection: Beyond gastric manifestations显示文摘Helicobacter pylori(H.pylori)is a bacterium that infects more than a half of world’s population.Although it is mainly related to the development of gastroduodenal diseases,several studies have shown that such infection may also influence the development and severity of various extragastric diseases.According to the current evidence,whereas this bacterium is a risk factor for some of these manifestations,it might play a protective role in other pathological conditions.In that context,when considered the gastrointestinal tract,H.pylori positivity have been related to Inflammatory Bowel Disease,Gastroesophageal Reflux Disease,Non-Alcoholic Fatty Liver Disease,Hepatic Carcinoma,Cholelithiasis,and Cholecystitis.Moreover,lower serum levels of iron and vitamin B12 have been found in patients with H.pylori infection,leading to the emergence of anemias in a portion of them.With regards to neurological manifestations,a growing number of studies have associated that bacterium with multiple sclerosis,Alzheimer’s disease,Parkinson’s disease,and Guillain-Barrésyndrome.Interestingly,the risk of developing cardiovascular disorders,such as atherosclerosis,is also influenced by the infection.Besides that,the H.pylori-associated inflammation may also lead to increased insulin resistance,leading to a higher risk of diabetes mellitus among infected individuals.Finally,the occurrence of dermatological and ophthalmic disorders have also been related to that microorganism.In this sense,this minireview aims to gather the main studies associating H.pylori infection with extragastric conditions,and also to explore the main mechanisms that may explain the role of H.pylori in those diseases.Maria Luisa Cordeiro Santos Breno Bittencourt de Brito Filipe Antonio França da Silva Mariana Miranda Sampaio Hanna Santos Marques Natalia Oliveira e Silva Dulciene Maria de Magalhaes Queiroz Fabricio Freire de Melo 2020World Journal of Gastroenterology2020,26,28:34
4Role of polymorphisms in genes that encode cytokines and Helicobacter pylori virulence factors in gastric carcinogenesis显示文摘The Helicobacter pylori(H. pylori) infection is a determinant factor in gastric cancer(GC) development. However, the infection outcomes are variable and depend on both host and bacterial characteristics. Some host cytokines such as interleukin(IL)-1β, IL-1 Ra, IL-8, IL-10 and tumor necrosis factor-α play important roles in the host immune system response to the pathogen, in the development of gastric mucosal lesions and in cell malignant transformation. Therefore, these host factors are crucial in neoplastic processes. Certain polymorphisms in genes that encode these cytokines have been associated with an increased risk of GC. On the other hand, various virulence factors found in distinct H. pylori bacterial strains, including cytotoxinassociated antigen A, vacuolating cytotoxin, duodenal ulcer promoting gene A protein, outer inflammatory protein and blood group antigen binding adhesin, have been associated with the pathogenesis of different gastric diseases. The virulent factors mentioned above allow the successful infection by the bacterium and play crucial roles in gastric mucosa lesions, including malignant transformation. Moreover, the role of host polymorphisms and bacterial virulence factors in gastric carcinogenesis seems to vary among different countries and populations. 6Nephrotoxicity in cancer treatment:An overview显示文摘Anticancer drug nephrotoxicity is an important and increasing adverse drug event that limits the efficacy of cancer treatment.The kidney is an important elimination pathway for many antineoplastic drugs and their metabolites,which occurs by glomerular filtration and tubular secretion.Chemotherapeutic agents,both conventional cytotoxic agents and molecularly targeted agents,can affect any segment of the nephron including its microvasculature,leading to many clinical manifestations such as proteinuria,hypertension,electrolyte disturbances,glomerulopathy,acute and chronic interstitial nephritis,acute kidney injury and at times chronic kidney disease.The clinician should be alert to recognize several factors that may maximize renal dysfunction and contribute to the increased incidence of nephrotoxicity associated with these drugs,such as intravascular volume depletion,the associated use of nonchemotherapeutic nephrotoxic drugs(analgesics,antibiotics,proton pump inhibitors,and bonetargeted therapies),radiographic ionic contrast media or radiation therapy,urinary tract obstruction,and intrinsic renal disease.Identification of patients at higher risk for nephrotoxicity may allow the prevention or at least reduction in the development and severity of this adverse effect.Therefore,the aim of this brief review is to provide currently available evidences on oncologic drug-related nephrotoxicity.Maria Luísa Cordeiro Santos Breno Bittencourt de Brito Filipe Antonio FranÇa da Silva Anelise Costa dos Santos Botelho Fabrício Freire de Melo 2020World Journal of Clinical Oncology2020,11,4:4

10Non-pharmacological management of pediatric functional abdominal pain disorders:Current evidence and future perspectives显示文摘Functional abdominal pain disorders(FAPDs) are an important and prevalent cause of functional gastrointestinal disorders among children, encompassing the diagnoses of functional dyspepsia, irritable bowel syndrome, abdominal migraine, and the one not previously present in Rome Ⅲ, functional abdominal pain not otherwise specified. In the absence of sufficiently effective and safe pharmacological treatments for this public problem, non-pharmacological therapies emerge as a viable means of treating these patients, avoiding not only possible side effects, but also unnecessary prescription, since many of the pharmacological treatments prescribed do not have good efficacy when compared to placebo. Thus, the present study provides a review of current and relevant evidence on non-pharmacological management of FAPDs, covering the most commonly indicated treatments, from cognitive behavioral therapy to meditation, acupuncture, yoga, massage, spinal manipulation, moxibustion, and physical activities. In addition, this article also analyzes the quality of publications in the area, assessing whether it is possible to state if non-pharmacological therapies are viable, safe, and sufficiently well-based for an appropriate and effective prescription of these treatments. Finally, it is possible to observe an increase not only in the number of publications on the non-pharmacological treatments for FAPDs in recent years, but also an increase in the quality of these publications. 11Analysis of alternative parameters of dynamic resistance measurement in high voltage circuit breakers显示文摘Throughout its lifetime,high-voltage circuit breaker(HVCB)contacts suffer from inevitable degradation processes,especially during the formation of electric arcs.Highly degraded contacts can reduce the current interruption capacity and increase the chances of operational faults,which might lead to considerable financial losses.Therefore,the use of efficient evaluating techniques is a key factor to achieve systems reliability.The dynamic resistance measurement(DRM)is a technique that can be used to evaluate the degradation level of HVCB'contacts.It stands out from the usual technique,static resistance measurement,by allowing the assessment of both the main contacts and the arcing contacts(ACs).Usually,the mean resistance or the area under the DRM curve is used to estimate the AC degradation.However,DRM is a non-standardised technique and there are no specified parameters to evaluate the real wear conditions.Therefore,an analysis of alternative parameters extractible from the DRM technique is presented in this study,such as amplitude and variance of the DRM curve.The results attest the possible use of more than one parameter for the continuous evaluation of HVCB ACs,characterising the DRM as a more complete and precise diagnosis tool.Herbet Filipe dos Santos Sousa Adriano Costa de Oliveira George Victor Rocha Xavier Henrique Nunes de Santana Edson Guedes da Costa Tarso Vilela Ferreira 2019High Voltage2019,4,3:1

13Antiglycation and antioxidant activities of the crude extract and saponin fraction of Tribulus terrestris before and after microcapsule release显示文摘Objective:The present study investigated antiglycation and antioxidant activities of crude dry extract and saponin fraction of Tribulus terrestris.It also developed a method of microencapsulation and evaluated antiglycation and antioxidant activities of crude dry extract and saponin fraction before and after microcapsule release.Methods:Antiglycation activity was determined by relative electrophoretic mobility(REM),free amino groups and inhibition of advanced glycation end-product(AGE)formation.Antioxidant activity was determined by 2,2-diphenyl-1-picrylhydrazyl(DPPH),ferric ion-reducing antioxidant power(FRAP),nitric oxide(NO)and thiobarbituric acid reactive species(TBARS)tests.Microcapsules were prepared using maltodextrin as wall material and freeze-drying as encapsulation technique.Morphological characterization of microcapsules was evaluated by scanning electron microscopy,and encapsulation efficiency and microcapsule release were determined by total saponins released.Antiglycation and antioxidant assays were performed using crude dry extract and saponin fraction of T.terrestris before and after release.Results:Saponin fraction showed an increase of 32.8%total saponins.High-performance liquid chromatography-mass spectrometry analysis showed the presence of saponins in the obtained fraction.Antiglycation evaluation by REM demonstrated that samples before and after release presented antiglycation activity similar to bovine serum albumin treated with aminoguanidine.Additionally,samples inhibited AGE formation,highlighting treatment with saponin fraction after release(89.89%).Antioxidant tests demonstrated antioxidant activity of the samples.Crude dry extract before encapsulation presented the highest activities in DPPH(92.00%)and TBARS(32.49%)assays.Saponin fraction before encapsulation in FRAP test(499μmol Trolox equivalent per gram of dry sample)and NO test(15.13μmol nitrite formed per gram of extract)presented the highest activities.Conclusion:This study presented antiglycation activity of crude dry extract and saponin fraction of T.terrestris,besides it demonstrated promising antioxidant properties.It also showed that the encapsulation method was efficient and maintained biological activity of bioactive compounds after microcapsule release.These results provide information for further studies on antidiabetic and antiaging potential,and data for new herbal medicine and food supplement formulations containing microcapsules with crude extract and/or saponin fraction of T.terrestris.Célia Cristina Malaguti Figueiredo Amanda da Costa Gomes Filipe Oliveira Granero Joao Luiz Bronzel Junior Luciana Pereira Silva Valdecir Farias Ximenes Regildo Márcio Goncalves da Silva 2022Journal of Integrative Medicine2022,20,2:1

16Cross-sectional study to determine viral hepatitis knowledge in different urban populations in Brazil显示文摘AIM To evaluate viral hepatitis knowledge among individuals from different resource areas and health conditions to identify possible gaps.METHODS A cross-sectional, descriptive study was carried out among 447 individuals from five distinct populations in Brazil: Southeast Viral Hepatitis Ambulatory(n = 100), South(n = 89) and Northeast(n = 114) Health Center, Southeast(n = 77) and Northeast(n = 67) low resource areas. All individuals answered a questionnaire assessing sociodemographic characteristics and viral hepatitis awareness. The perception was scored based on the average number of correct answers of all participants and categorized as 'low'(0-28 correct answers) or 'desirable'(29-46 correct answers). Associations between sociodemographic characteristics and perception were also evaluated.RESULTS A low level of knowledge was observed in individuals from Northeast Health Center, Northeast and Southeast low resource areas while desirable knowledge was observed in individuals from Viral Hepatitis Ambulatory and South Health Center. According to sociodemographic characteristics, desirable scores were more common among those with secondary education(47.1%), those who declared themselves as white(46.3%), and those who lived in houses with three individuals(25.5%). Multivariate analysis showed an association between viral hepatitis perception and type of population. 17COVID-19 liver and gastroenterology findings:An in silico analysis of SARS-CoV-2 interactions with liver molecules显示文摘BACKGROUND Coronavirus disease 19(COVID-19)has not only been shown to affect the respiratory system,but has also demonstrated variable clinical presentations including gastrointestinal tract disorders.In addition,abnormalities in liver enzymes have been reported indicating hepatic injury.It is known that severe acute respiratory syndrome coronavirus-2(SARS-CoV-2)might infect cells via the viral receptor angiotensin-converting enzyme 2(ACE2)which is expressed in several organs including the liver.The viral Spike glycoprotein binds to ACE2 and must be cleaved by Furin and Type 2 Serine Protease to enter the cells.After that,the Akt/mTOR signaling pathway is activated and several COVID-19 changes are triggered.AIM To analyze liver and gastrointestinal symptoms and cell signaling pathways triggered by SARS-CoV-2 infection due to virus-liver interactions in silico.METHODS In this in silico study,the three-dimensional structures of the Akt,mTORC1 and Furin(receptors)were selected from the Protein Data Bank(PDB)and the structures of inhibitors(ligands)MK-2206,CC-223 and Naphthofluorescein were selected from PubChem and ZINC databases.Ligand files were downloaded as 2D structures and converted to optimized 3D structures using ViewerLite 4.2 software.Marvin Sketch®software was used to calculate prediction of the protonated form of inhibitors in a physiological environment(pH 7.4).AutoDock Tools(ADT)software was used to calculate and delimit the Grid box used in the molecular docking of each structure selected in the PDB.In addition,protonated ligands were prepared for molecular docking using ADT software.Molecular docking was performed using ADT software tools connected to Vina software.Analysis of the amino acid residues involved in ligand interactions,as well as ligand twists,the atoms involved in interactions,bond type and strength of interactions were performed using PyMol^(■)and Discovery Studio^(■)(BIOVIA)software.RESULTS Molecular docking analysis showed that the mTORC1/CC-223 complex had affinity energy between the receptor and ligand of-7.7 kcal/moL with interactions ranging from 2.7 to 4.99Å.There were four significant chemical bonds which involved two of five polypeptide chains that formed the FKBP12–Rapamycin-Binding(FRB)domain.The strongest was a hydrogen bond,the only polar interaction,and Van der Waals interactions shown to be present in 12 residues of mTORC1’s FRB domain.With regard to the Akt/MK-2206 complex there were three Van der Waals interactions and 12 chemical bonds in which seven residues of Akt were involved with all five rings of the MK-2206 structure.In this way,both ASP 388 and GLN 391 bind to the same MK-2206 ring,the smaller one.However,LYS 386 had four chemical bonds with the inhibitor,one with each structure ring,while LYS 387 binds two distinct rings.One of the MK-2206 inhibitor's rings which binds to LYS 387 also binds simultaneously to ILE 367 and LEU 385 residues,and the fifth ring of the structure was involved in a bond with the ALA 382 residue.The hydrogen bonds were the shortest bonds in the complex(2.61 and 3.08Å)and all interactions had an affinity energy of-8.8 kcal/moL.The affinity energy in the Furin/Naphhofluorescein complex was-9.8 kcal/moL and involved six interactions ranging from 2.57 to 4.98Å.Among them,two were polar and the others were non-polar,in addition to twelve more Van der Waals interactions.Two distinct hydrogen bonds were formed between Furin and its inhibitor involving GLN 388 and ALA 532 residues.ALA 532 also binds to two distinct rings of Naphthofluorescein,while TRP 531 residue has two simultaneous bonds with the inhibitor.CONCLUSION Liver infection and signaling pathways altered by SARS-CoV-2 can be modulated by inhibitors that demonstrate significant interaction affinity with human proteins,which could prevent the development of infection and symptoms.Gabrielle Caroline Peiter Cristiano de Bem Torquato de Souza Lucca Miketen de Oliveira Luis Gustavo Pagliarin Valentina Nunes Fontoura dos Anjos Filipe Antônio França da Silva Fabrício Freire de Melo Kádima Nayara Teixeira 2022World Journal of Hepatology2022,14,6:1
