1Inhibition of SARS-CoV-2 (previously 2019-nCoV) infection by a highly potent pan-coronavirus fusion inhibitor targeting its spike protein that harbors a high capacity to mediate membrane fusion显示文摘The recent outbreak of coronavirus disease(COVID-19)caused by SARS-CoV-2 infection in Wuhan,China has posed a serious threat to global public health.To develop specific anti-coronavirus therapeutics and prophylactics,the molecular mechanism that underlies viral infection must first be defined.Therefore,we herein established a SARS-CoV-2 spike(S)protein-mediated cell-cell fusion assay and found that SARS-CoV-2 showed a superior plasma membrane fusion capacity compared to that of SARS-CoV.We solved the X-ray crystal structure of six-helical bundle(6-HB)core of the HR1 and HR2 domains in the SARS-CoV-2 S protein S2 subunit revealing that several mutated amino acid residues in the HR1 domain may be associated with enhanced interactions with the HR2 domain.We previously developed a pan-coronavirus fusion inhibitor,EK1,which targeted the HR!domain and could inhibit infection by divergent human coronaviruses tested,including SARS-CoV and MERS-CoV.Here we generated a series of lipopeptides derived from EK1 and found that EK1C4 was the most potent fusion inhibitor against SARS-CoV-2 S protein-mediated membrane fusion and pseudovirus infection with IC50s of 1.3 and 15.8 nM,about 241-and 149-fold more potent than the original EK1 peptide,respectively.EK1C4 was also highly effective against membrane fusion and infection of other human coronavirus pseudoviruses tested,including SARS-CoV and MERS-CoV,as well as SARSr-CoVs,and potently inhibited the replication of 5 live human coronaviruses examined,including SARS-CoV-2.Intranasal application of EK1C4 before or after challenge with HCoV-OC43 protected mice from infection,suggesting that EK1C4 could be used for prevention and treatment of infection by the currently circulating SARS-CoV-2 and other emerging SARSr-CoVs.Shuai Xia Meiqin Liu Chao Wang Wei Xu Qiaoshuai Lan Siliang Feng Feifei Qi Linlin Bao Lanying Du Shuwen Liu Chuan Qin Fei Sun Zhengli Shi Yun Zhu Shibo Jiang Lu Lu 2020Cell Research2020,30,4:81
2Perinatal complications and live-birth outcomes following assisted reproductive technology: a retrospective cohort study显示文摘Background:With the development of assisted reproductive technology(ART)and its increasing success rate in the mainland of China,more attention has been paid to the safety of ART.In this study,we explored the associations between conception by ART and pregnancy/perinatal complications,and neonatal outcomes compared with similar outcomes following spontaneous conception.Methods:This retrospective cohort study of pregnancies over a 3-year period(2013-2015)was performed at Beijing Obstetrics and Gynecology Hospital,Beijing,China.Subjects were divided into two groups:conception by ART(n=2256)or spontaneous conception(n=6768).According to different fertilization modes,the ART group was divided into in vitro fertilization(IVF,n=1873)and intracytoplasmic sperm injection(ICSI,n=383)subgroups.The ART group was also divided into two different embryo transfer methods;fresh embryo transfer(ET,n=1583)and frozen embryo transfer(FET,n=673)subgroups.Pregnancy complications,perinatal complications,and neonatal outcomes of the enrolled subjects were investigated and analyzed by univariate analysis and multivariate logistic regression.Results:After adjustment for maternal age,gravidity,parity,maternal education,smoking,alcohol consumption,and body mass index(BMI),pregnancies conceived by ART were associated with a significantly increased incidence of gestational diabetes mellitus(GDM;OR 1.88,95%CI 1.56-2.27),gestational hypertension(OR 2.18,95%CI 1.83-2.60),and intrahepatic cholestasis of pregnancy(ICP)(OR 2.79,95%CI 2.15-3.64),compared with spontaneous conception.These associations were similar for the singleton group.In the twin group,only the incidence of ICP was significantly higher than in controls.We found that pregnancies conceived by ART were associated with perinatal complications,including placental abruption(OR 2.14,95%CI 1.33-3.45),premature rupture of membranes(PROM;OR 1.24,95%CI 1.06-1.45),postpartum hemorrhage(OR 2.89,95%CI 2.33-3.59)and polyhydramnios(OR 2.01,95%CI 1.29-3.16).The singleton group had a similar result with placental abruption,but not with fetal membranes ruptures before labor and polyhydramnios.There were no significant differences in the incidence of these perinatal complications in the twin group.Some neonatal outcomes,including preterm labor(OR 4.29,95%CI 3.84-4.80)and low birth weight(OR 1.72,95%CI 1.42-2.08),were more likely to occur with singleton births after ART.However,there were no significant differences for these outcomes from twin pregnancies.Perinatal complications and neonatal outcomes were consistent between the IVF and ICSI subgroups.The FET and ET subgroups showed a similar increase in complications,except for the incidence of placental abruption.After taking into account the effects of parity,birth plurality and maternal age,the ART group still exhibited increased maternal and neonatal complications,although some differences narrowed or disappeared.Conclusions:This retrospective cohort study demonstrated that patients who underwent ART were at increased risk of several adverse pregnancy outcomes compared with women who conceived spontaneously.These complications may be attributed in part to the relatively high multiple pregnancy rate after ART.Elective single embryo transfer should be promoted in China to reduce the obstetrical risks of ART pregnancy.Singletons of ART pregnancy exhibited increased maternal and neonatal complications as well,suggesting that underlying infertility or other maternal or parental factors may contribute to the adverse outcomes.Ling-Ling Lei Yong-Lian Lan Shu-Yu Wang Wei Feng Zhi-Jin Zhai 2019Chinese Medical Journal2019,,20:24

A constitutive model with fractional derivative is developed for the generalized second grade fluid. Unsteady Couette flow of the generalized second grade fluid is studied by using the method of the discrete inverse Laplace transform and generalized Mittag-Leffler function. And then an exact solution is obtained for this problem with arbitrary fractional derivative. 4Value of superb micro-vascular imaging in predicting ischemic stroke in patients with carotid atherosclerotic plaques显示文摘BACKGROUND Unstable carotid atherosclerotic plaques are prone to cause ischemic stroke.Contrast-enhanced ultrasound(CEUS) is the primary method of assessing plaque stability, but CEUS cannot be a method for screening for unstable plaque. The emergence of superb micro-vascular imaging(SMI) offers the possibility of clinically screening for unstable plaque AIM To investigate the value of SMI in predicting ischemic stroke in patients with carotid atherosclerotic plaques.METHODS Patients with carotid atherosclerotic plaques(luminal stenosis of 50%-70%) were enrolled into the present study. All patients received conservative medication.The patient's clinical baseline data, serological data, CEUS and SMI data were analyzed. All patients underwent a 3-year follow-up. The follow-up endpoint was the occurrence of ischemic stroke and patients were divided into stroke group and non-stroke group according to whether the prognosis occurred or not.Subsequently, the difference in clinical data was compared, the correlation of SMI and CEUS was analyzed, and multiple Cox regression and receiver operating characteristic curve were applied to investigate the value of SMI and CEUS in predicting cerebral arterial thrombosis in three years.RESULTS In this study, 43 patients were enrolled in the stroke group and 82 patients were enrolled in the non-stroke group. Cox regression revealed that SMI level(P =0.013) and enhancement intensity(P = 0.032) were the independent factors influencing ischemic stroke. There was a positive correlation between SMI level and enhancement intensity(r = 0.737, P = 0.000). The area under curve of SMI level predicting ischemic stroke was 0.878. The best diagnostic point was ≥ level Ⅱ, and its sensitivity and specificity was 86.05% and 79.27%. The area under curve of enhancement intensity predicting ischemic stroke was 0.890. The best diagnostic point was 9.92 db, and its sensitivity and specificity was 88.37% and89.02%. 7Sequential use of transarterial chemoembolization and percutaneous cryosurgery for hepatocellular carcinoma显示文摘AIM: To evaluate the efficacy of sequential use of transarterial chemoembolization (TACE) and percutaneous cryosurgery for unresectable hepatocellular carcinoma (HCC). METHODS: Four hundred and twenty patients were enrolled in this study. The patients, who were considered to have unresectable tumors due to their location or size or comorbidity, were divided into sequential TACE-cryosurgery (sequential) group (n = 290) and cryosurgery alone (cryoalone) group (n = 130). Patients in the sequential group tended to have larger tumors and a greater number of tumors than those in the cryo-alone group. Tumors larger than 10 cm in diameter were only seen in the sequential group. TACE was performed with the routine technique and percutaneous cryosurgery was conducted under the guidance of ultrasound 2-4 wk after TACE. RESULTS: During a mean follow-up period of 42 ± 17 mo (range, 24-70 mo), the local recurrence rateat the ablated area was 17% for all patients, 11% and 23% for patients in sequential group and cryoalone groups, respectively (P = 0.001). The overall 1-, 2-, 3-, 4and 5-year survival rate was 72%, 57%, 47%, 39% and 31%, respectively. The 1and 2-year survival rates (71% and 61%) in sequential group were similar to those (73% and 54%) in cryo-alone group (P = 0.69 and 0.147), while the 4and 5-year survival rates were 49% and 39% in sequential group, higher than those (29% and 23%) in cryo-alone group (P = 0.001). Eighteen patients with large HCC (> 5 cm in diameter) survived for more than 5 years after sequential TACE while no patient with large HCC (> 5 cm in diameter) survived more than 5 years after cryosurgery. The overall complication rate was 24%, and the complication rates were 21% and 26% for the sequential and cryo-alone groups, respectively (P = 0.06). The incidence of hepatic bleeding was higher in cryo-alone group than in sequential group (P = 0.02). Liver crack only occurred in two patients of the cryoalone group. CONCLUSION: Pre-cryosurgical TACE can increase the cryoablation efficacy and decrease its adverse effects, especially bleeding. 10Management of granulomatous lobular mastitis: an international multidisciplinary consensus(2021 edition)显示文摘Granulomatous lobular mastitis(GLM) is a rare and chronic benign inflammatory disease of the breast. Difficulties exist in the management of GLM for many front-line surgeons and medical specialists who care for patients with inflammatory disorders of the breast. This consensus is summarized to establish evidence-based recommendations for the management of GLM. Literature was reviewed using PubMed from January 1, 1971 to July 31, 2020. Sixty-six international experienced multidisciplinary experts from 11 countries or regions were invited to review the evidence.Levels of evidence were determined using the American College of Physicians grading system, and recommendations were discussed until consensus. Experts discussed and concluded 30 recommendations on historical definitions,etiology and predisposing factors, diagnosis criteria, treatment, clinical stages, relapse and recurrence of GLM. GLM was recommended as a widely accepted definition. In addition, this consensus introduced a new clinical stages and management algorithm for GLM to provide individual treatment strategies. In conclusion, diagnosis of GLM depends on a combination of history, clinical manifestations, imaging examinations, laboratory examinations and pathology.The approach to treatment of GLM should be applied according to the different clinical stage of GLM. 11Methyl Jasmonate Reduces Chilling Injury and Maintains Postharvest Quality in Peaches显示文摘Zhaohui peaches (Prunus persica Batsch) were treated with 0 (CK), 1, 10 or 100 μmol L-1methyl jasmonate (MeJA) vapor at 20℃ for 24 h before stored at 0℃ for 35 d. The untreated fruits showed chilling injury (CI) symptoms after 4 wk of storage, as indicated by increased fruit firmness and reduced extractable juice, which is referred as leatheriness. Treatment with 1 and 10 μmol L-1 MeJA promoted normal ripening and softening, maintained higher levels of extractable juice, titratable acidity, pectinesterase (PE)and polygalacturonase (PG) activities, inhibited increases in fruit fresh weight loss, decay incidence, electrolyte leakage and MDA content, and improved color development, thereby preventing chilling injury symptoms development and maintaining edible quality. MeJA treatment also delayed the climacteric rise in respiratory rate, but promoted ethylene production during the later period of cold storage, suggesting that ethylene may involve in CI development of peaches. 12Target classification with low-resolution radar based on dispersion situations of eigenvalue spectra显示文摘Most low-resolution radar systems, especially ground surveillance radar systems, work at relatively low pulse repeat frequency (PRF) and with short time-on-target (TOT) (duration in scanning). Low PRF leads to Doppler ambiguity and short TOT results in low Doppler resolution, which poses a problem to target classification with low-resolution radar based on the jet engine modulation (JEM) characteristic of radar echo. From the pattern classification viewpoint, we propose a method of using dispersion situations of JEM eigenvalue spectra to categorize aeroplanes into three kinds, namely turbojet aircraft, prop aircraft and helicopter. We analyze the mathematical model of JEM echoes consisting of a series of line spectra and regard them as a sum of several series of harmonious waves. Classification features can be extracted based on the harmonious wave sum model. Some schemes for extracting features from echoes within or between pulses are proposed. Low-dimensional features are extracted to reduce computation burden. Our methods do not compensate for the fuselage echoes and are insensitive to the variation of fuselage Doppler. 13Mechanisms and pharmacokinetic/pharmacodynamic profiles underlying the low nephrotoxicity and ototoxicity of etimicin显示文摘Etimicin(ETM),a fourth-generation aminoglycosides(AGs),is now widely clinically used in China due to its high efficacy and low toxicity.However,the mechanisms underlying its low nephrotoxicity and ototoxicity remain unclear.In the present study we compared the antibacterial and toxicity profiles of etimicin,gentamicin(GM,a second-generation AG),and amikacin(AMK,a third-generation AG),and investigated their pharmacokinetic properties in the toxicity target organs(kidney and inner ear)and subcellular compartments.We first demonstrated that ETM exhibited superior antibacterial activities against clinical isolates to GM and AMK,and it exerted minimal nephrotoxicity and ototoxicity in rats following multi-dose administration.Then,we conducted pharmacokinetic studies in rats,showed that the three AGs accumulated in the kidney and inner ear with ETM being distributed to a lesser degree in the two toxicity target organs as compared with GM and AMK high-dose groups.Furthermore,we conducted in vitro experiments in NRK-52E rat renal tubular epithelial cells and HEI-OC1 cochlear hair cells,and revealed that all the three AGs were distributed predominantly in the mitochondria with ETM showing minimal accumulation;they not only directly inhibited the activity of mitochondrial complexesⅣandⅤbut also inhibited mitochondrial function and its related PGC-1α-NRF1-TFAM pathway;ETM caused minimal damage to the mitochondrial complex and mitochondrial biogenesis.Our results demonstrate that the minimal otonephrotoxicity of ETM results from its lesser accumulation in mitochondria of target cells and subsequently lesser inhibition of mitochondrial function.These results provide a new strategy for discovering novel AGs with high efficacy and low toxicity.Lan Yao Jing-wei Zhang Bin Chen Ming-min Cai Dong Feng Qi-zhi Wang Xin-yu Wang Jian-guo Sun Yi-wen Zheng Guang-ji Wang Fang Zhou 2020Acta Pharmacologica Sinica2020,41,6:10
14Microstructure,texture evolution and mechanical properties of cold rolled Ti-32.5Nb-6.8Zr-2.7Sn biomedical beta titanium alloy显示文摘Ti-32.5 Nb-6.8 Zr-2.7 Sn(TNZS,wt%) alloy was produced by using vacuum arc melting method,followed by solution treatment and cold rolling with the area reductions of 50% and 90%.The effects of cold rolling on the microstructure,texture evolution and mechanical properties of the experimental alloy were investigated by optical microscopy,X-ray diffraction,transmission electron microscopy and universal material testing machine.The results showed that the grains of the alloy were elongated along rolling direction and stress-induced α'' martensite was not detected in the deformed samples.The plastic deformation mechanisms of the alloy were related to {112} 111 type deformation twinning and dislocation slipping.Meanwhile,the transition from γ-fiber texture to α-fiber texture took place during cold rolling and a dominant {001} 110_(α-fiber) texture was obtained after 90% cold deformation.With the increase of cold deformation degree,the strength increased owing to the increase of microstrain,dislocation density and grain refinement,and the elastic modulus decreased owing to the increase of dislocation density as well as an enhanced intensity of {001} 110_(α-fiber)texture and a weakened intensity of {111} 112_(γ-fiber)texture.The 90% cold rolled alloy exhibited a great potential to become a new candidate for biomedical applications,since it possesses low elastic modulus(47.1 GPa),moderate strength(883 MPa) and high elastic admissible strain(1.87%),which are superior than those of Ti-6 Al-4 V alloy.Chunbo Lan Yu Wu Lili Guo Huijuan Chen Feng Chen 2018Journal of Materials Science & Technology2018,34,5:10

16Epidemiology,clinical and laboratory characteristics of currently alive HIV-1 infected former blood donors naive to antiretroviral therapy in Anhui Province,China显示文摘Background Unregulated commercial blood/plasma collection among farmers occurred between 1992 and 1995 in central China and caused the second major epidemic of human immunodeficiency virus type 1 (HIV-1) infection in China. It is important to characterize HIV-1-infected former blood donors and to study characteristics associated with disease progression for future clinical intervention and vaccine development. Methods A cross-sectional study was performed on HIV-1-infected former blood donors (FBDs) and age-matched HIV-seronegative local residents. Demographic, epidemiologic, clinical and key laboratory data were collected from all study participants. Both unadjusted and adjusted multivariate linear regressions were employed to analyze the association of the decrease of CD4+ T-cell counts with other characteristics. Results Two hundred and ninety-four HIV-1-infected FBDs and 59 age-matched HIV-seronegative local residents were enrolled in this study. The unregulated blood/plasma collection occurred more than a decade (10.8 -12.8 years) ago, which caused the rapid spread of HIV-1 infection and the high prevalence of co-infection with hepatitis C virus (HCV, 89.5%); hepatitis B virus (HBV) co-infection was observed in only 11 HIV+participants (3.7%). Deterioration in both clinical manifestation and laboratory parameters and increase of viral loads were observed in parallel with the decrease of CD4+ T-cell counts. The decrease of total lymphocyte counts (P<0.001) and hemoglobin levels (P<0.001) and the appearance of dermatosis (P=0.03) were observed in parallel with the decrease of CD4+ T-cell counts whereas viral loads (P<0.001) and CD8+ T-cell counts (P=0.01) were inverselyassociated with CD4+ T-cell counts. Conclusions Co-infection with HCV but not HBV is highly prevalent among HIV-1-infected FBDs. CD4+ T-cell counts is a reliable indicator for disease progression among FBDs. 18Treatment of Donor-derived Carbapenem-resistant Klebsiella pneumoniae Infection after Renal Transplantation with Tigecycline and Extended-infusion Meropenem显示文摘Objective Donor-derived carbapenem-resistant Klebsiella pneumoniae(CRKP)infection has recently emerged as a critical early complication after renal transplantation.Although CRKP is usually sensitive to tigecycline,monotherapy with this drug is often less than effective.We investigated the efficacy of a combined regimen of tigecycline with high-dose,extended-infusion meropenem in the treatment of donor-derived CRKP infection after kidney transplantation.Methods From Jan.2016 to Dec.2017,a total of 12 CRKP isolates were detected from cultures of the organ preservation solution used for soaking the donor kidneys at our institute.Probable or possible donor-derived infection(DDI)was identified in 8 transplant recipients.Clinical data were retrospectively analyzed.Results Klebsiella pneumoniae carbapenemase-2(KPC-2)-producing CRKP was reported to be positive in organ preservation solution cultures at 3.5±0.9 days after transplantation,leading to surgical site(n=3),urinary tract(n=4),and/or bloodstream(n=2)infections in 8 recipients.The drug susceptibility tests showed that CRKP was sensitive to tigecycline,but resistant to meropenem.In 7 patients who received tigecycline combined with high-dose extended-infusion meropenem,DDIs were successfully cured.The length of hospital stay was 31(18–129)days,and the serum creatinine at discharge was 105.8±16.7µmol/L.The one remaining patient who received tigecycline combined with intravenous-drip meropenem died of septic shock.A median follow-up of 43 months(33–55)showed no recurrence of new CRKP infection in the 7 surviving recipients.Conclusion It was suggested that a prompt and appropriate combination therapy using tigecycline with high-dose extended-infusion meropenem is effective in treating donor-derived KPC-2-producing CRKP infection after renal transplantation.Zhi-qiang WANG Zhi-liang GUO Hao FENG Cheng FU Guang-yuan ZHAO Ke MA Lan ZHU Gang CHEN 2021Current Medical Science2021,41,4:8
