1Management of granulomatous lobular mastitis: an international multidisciplinary consensus(2021 edition)显示文摘Granulomatous lobular mastitis(GLM) is a rare and chronic benign inflammatory disease of the breast. Difficulties exist in the management of GLM for many front-line surgeons and medical specialists who care for patients with inflammatory disorders of the breast. This consensus is summarized to establish evidence-based recommendations for the management of GLM. Literature was reviewed using PubMed from January 1, 1971 to July 31, 2020. Sixty-six international experienced multidisciplinary experts from 11 countries or regions were invited to review the evidence.Levels of evidence were determined using the American College of Physicians grading system, and recommendations were discussed until consensus. Experts discussed and concluded 30 recommendations on historical definitions,etiology and predisposing factors, diagnosis criteria, treatment, clinical stages, relapse and recurrence of GLM. GLM was recommended as a widely accepted definition. In addition, this consensus introduced a new clinical stages and management algorithm for GLM to provide individual treatment strategies. In conclusion, diagnosis of GLM depends on a combination of history, clinical manifestations, imaging examinations, laboratory examinations and pathology.The approach to treatment of GLM should be applied according to the different clinical stage of GLM. 2Numerical simulations of water flow and contaminants transport near mining wastes disposed in a fractured rock mass显示文摘A numerical tool, called Hydro-Geosphere, was used to simulate unsaturated water flow and contaminants migration around an open pit filled with mining wastes. Numerical simulations had been carried out to assess the influence of various factors on water flow and solute transport in and around the surface openings including recharge, properties of the waste material and presence of fractures in the surrounding rock mass. The effect of the regional hydraulic gradient was also investigated. The analyses were conducted by simulating various 2D cases using experimentally obtained material properties and controlled boundary conditions. The effects of the hydrogeological properties of the filling material(i.e., water retention curve and hydraulic conductivity function), fracture network characteristics and conductivity of the joints were assessed. The results illustrate that fractures control water flow and contaminants transport around the waste disposal area. 3Management of adults with coarctation of aorta显示文摘Coarctation of the aorta(CoA)is a relatively common congenital cardiac defect often causing few symptoms and therefore can be challenging to diagnose.The hallmark finding on physical examination is upper extremity hypertension,and for this reason,CoA should be considered in any young hypertensive patient,justifying measurement of lower extremity blood pressure at least once in these individuals.The presence of a significant pressure gradient between the arms and legs is highly suggestive of the diagnosis.Early diagnosis and treatment are important as long-term data consistently demonstrate that patients with CoA have a reduced life expectancy and increased risk of cardiovascular complications.Surgical repair has traditionally been the mainstay of therapy for correction,although advances in endovascular technology with covered stents or stent grafts permit nonsurgical approaches for the management of older children and adults with native CoA and complications.Persistent hypertension and vascular dysfunction can lead to an increased risk of coronary disease,which,remains the greatest cause of long-term mortality.Thus,blood pressure control and periodic reassessment with transthoracic echocardiography and threedimensional imaging(computed tomography or cardiac magnetic resonance)for should be performed regularly as cardiovascular complications may occur decades after the intervention.Pradyumna Agasthi Sai Harika Pujari Andrew Tseng Joseph N Graziano Francois Marcotte David Majdalany Farouk Mookadam Donald J Hagler Reza Arsanjani 2020World Journal of Cardiology2020,12,5:8
4Pathological,immunohistochemical and microbiologicalal analysis of lacrimal sac biopsies in patients with chronic dacrocystitis显示文摘AIM:To analyze cases of obstruction of the nasolacrimal duct which creates a fertile environment for secondary bacterial Infection and can result in dacryocystitis,which is a constant threat to cornea and orbital soft tissue and a potential source of endophthalmitis following intraocular surgery.The majority of obstructions of the lacrimal excretory outflow system are acquired ones occurring in adulthood and involving the distal parts of the system.Acquired obstruction may be primary/idiopathic or secondary to a wide variety of infectious,inflammatory,traumatic,mechanical,toxic or neoplastic causes mimicking idiopathic inflammation.These cases are treated by dacryocystorhinostomy(DCR).METHODS:The present study was conducted to determine the histopathologic,immunohistochemical and current microbiologic characteristics of lacrimal sac specimens in patients undergoing external dacryocystorhinostomy.RESULTS:Non-specific lacrimal sac pathology was present in all 33 cases and 81.8%of the cases showed moderate chronic inflammation with a chronic inflammatory score(CIS)ranging between 4 and 6,whereas 12.12%showed severe inflammatory changes with a CIS of 7.Mild degree of inflammation was seen in6.06%with a CIS of 3.The total prevalence of grampositive,gram-negative,and culture-negative samples were 59.4%,37.5%,and 3%respectively.CONCLUSION:Non-specific chronic inflammation withfibrosis is indeed the most commonly reported histopathological finding in lacrimal sac wall biopsy specimens.Rowayda Mahmoud Amin Faten Aly Hussein Hisham Farouk Idriss Nesrine Fathy Hanafy Dina Mohamed Abdallah 2013International Journal of Ophthalmology(English edition)2013,6,6:7
5Climatic variations and benign paroxysmal positional vertigo显示文摘Benign paroxysmal positional vertigo(BPPV) is probably the most common diagnosis at vertigo clinics.Seasonal cycles of several human illnesses could be attributed variously to changes in atmospheric or weather conditions.In this retrospective study,patients with BPPV from January 2010 to December 2012 were studied,and their charts were reviewed.Statistical analysis revealed a statistically significant difference in patients' numbers among different months of the year.Also there is a significant statistical correlation between the numbers of patients with climatic variations especially the temperature.The present paper discusses the possible explanations for these results which confirms the seasonal variations in BPPV,together with a review of literature to view the possible associations with other disorders that causes such seasonality.Basil M.N.Saeed Alyaa Farouk Omari 2016Journal of Otology2016,11,1:7
6Combining Committee-Based Semi-Supervised Learning and Active Learning显示文摘Many data mining applications have a large amount of data but labeling data is usually difficult,expensive,or time consuming,as it requires human experts for annotation.Semi-supervised learning addresses this problem by using unlabeled data together with labeled data in the training process.Co-Training is a popular semi-supervised learning algorithm that has the assumptions that each example is represented by multiple sets of features(views) and these views are sufficient for learning and independent given the class.However,these assumptions are strong and are not satisfied in many real-world domains.In this paper,a single-view variant of Co-Training,called Co-Training by Committee(CoBC) is proposed,in which an ensemble of diverse classifiers is used instead of redundant and independent views.We introduce a new labeling confidence measure for unlabeled examples based on estimating the local accuracy of the committee members on its neighborhood.Then we introduce two new learning algorithms,QBC-then-CoBC and QBC-with-CoBC,which combine the merits of committee-based semi-supervised learning and active learning.The random subspace method is applied on both C4.5 decision trees and 1-nearest neighbor classifiers to construct the diverse ensembles used for semi-supervised learning and active learning.Experiments show that these two combinations can outperform other non committee-based ones.Mohamed Farouk Abdel Hady Friedhelm Schwenker 2010Journal of Computer Science & Technology2010,25,4:5
7Identification of gas zones and chimneys using seismic attributes analysis at the Scarab field,offshore,Nile Delta,Egypt显示文摘Seismic attributes supported by composite logs are the best way that can enable the interpreter to understand seismic data very well and generate a new view of the output results.Detection of the reservoir zone can be enhanced by analyzing wells log data based on Gamma-ray,Resistivity,and Vp sonic logs respectively.Composite logs of Scarab-1,Scarab-De,Scarab-Da,Scarab-Dd,and Scarab-2 wells indicate the lateral and vertical variation of the gas reservoir in ElWastani Formation.However,there are several seismic attributes that can be used to support reservoirs identification.For enhancement the detection of the hydrocarbon reservoirs,it is important to carefully analyze the 2D seismic data,which in this study will be primarily prepared to enhance seismic attributes results for the identification of gas chimneys,gas zones as channels,enhance stratigraphic and structural interpretations.In this article,we have performed data conditioning,quality control and seismic well ties including the preliminary wavelet extractions to get accurate output.Then,we have extracted of several classes of physical,geometrical and complex attributes as a direct hydrocarbon indicator to identify the gas zones,channels and chimneys and to identify the faults and discontinuities.The main contribution of this work is to provide a more detailed seismic reflection image supported by several seismic attributes classes and well logs to show a visual and quantitative evidence to identify the gas channels and gas chimneys with improving the detection of the faults and discontinuities.Amir Ismail Hatem Farouk Ewida Mohammad Galal Al-Ibiary Sergio Gammaldi Aldo Zollo 2020Petroleum Research2020,5,1:4
8Clinical outcomes of transepithelial photorefractive keratectomy versus femtosecond laser assisted keratomileusis for correction of high myopia in South Egyptian population显示文摘AIM: To evaluate the safety and efficacy of transepithelial photorefractive keratectomy(t-PRK) with adjuvant mitomycin C(MMC) versus femtosecond laser assisted keratomileusis(Femto-LASIK) in correction of high myopia.METHODS: Prospective randomized comparative study including 156 eyes of 156 patients with high myopia and a spherical equivalent refraction(SER) <-6.00 D. They were divided randomly into two groups: Group A included 72 eyes treated with t-PRK with adjuvant MMC and Group B included 84 eyes treated with Femto-LASIK. Visual acuity, SER, corneal topography, pachymetry and keratometry were assessed for 12 mo postoperatively.RESULTS: The preoperative mean SER was-8.86±1.81 and-9.25±1.70 D in t-PRK MMC group and Femto-LASIK respectively(P=0.99) which improved to-0.65±0.43 D and-0.69±0.50 D at 12 mo follow up. Mean SER remained stable during the 12 mo of follow-up, with no statistically significant difference between the two groups(P=0.64). In t-PRK MMC group, only six eyes needed retreatment after six months of follow up. 9Experimental evaluation of eco-friendly flocculants prepared from date palm rachis显示文摘Sodium carboxymethylcellulose(CMCNa) is an anionic water soluble polyelectrolyte widely used in many industrial sectors including food,textiles,papers,adhesives,paints,pharmaceuticals,cosmetics and mineral processing.CMCNa was produced by chemical modification of cellulose,and represents many advantages:natural,renewable,non-toxic and biodegradable.In this study,different kinds of CMCNa,prepared from an agricultural waste date palm rachis,were tested as eco-friendly flocculants for drinking water treatment and their performances as flocculants in turbidity removal enhancement were assessed.The prepared materials were characterized by the degree of substitution(DS) and polymerisation(DP) .The study of the effect of some experimental parameters on the coagulation-flocculation performance,using the prepared materials combined with aluminium sulphate(as coagulant) ,showed that the best conditions for turbidity treatment were given for pH 8,coagulant dose 20 mg/L,flocculant concentration of 100 mg/L and stirring velocity(during the flocculation step) of 30 r/min.Under the optimum conditions,the turbidity removal using CMCNa,prepared from raw material,was about 95%.A comparison study between the flocculation performance of a commercial anionic flocculant(A100PWG:polyacrylamide) and that of the prepared CMCNa showed that the performance of the waste-based flocculant with a DS of 1.17 and a DP of 480 was 10%better than that achieved by the commercial one.Ramzi Khiari Sonia Dridi-Dhaouadi Chadlia Aguir Mohamed Farouk Mhenni 2010Journal of Environmental Sciences2010,22,10:3

12Tumor necrosis factor-α-G308A polymorphism is associated with liver pathological changes in hepatitis C virus patients显示文摘AIM To investigate the association of tumor necrosis factor alpha(TNFα)-G308 A polymorphism with different liver pathological changes in treatment-na?ve Egyptian patients infected with hepatitis C virus(HCV) genotype 4.METHODS This study included 180 subjects,composed of 120 treatment-na?ve chronic HCV patients with different fibrosis grades(F0-F4) and 60 healthy controls. The TNFα-G308 A region was amplified by PCR and the different genotypes were detected by restriction fragment length polymorphism analysis. The TNFα protein was detected by enzyme-linked immunosorbent assay. The influence of different TNFα-G308 A genotypes on TNFα expression and liver disease progression were statistically analyzed. The OR and 95%CI were calculated to assess the relative risk confidence.RESULTS Current data showed that the TNFα-G308 A SNP frequency was significantly different between controls and HCV infected patients(P = 0.001). Both the AA genotype and A allele were significantly higher in late fibrosis patients(F2-F4,n = 60) than in early fibrosis patients(F0-F1,n = 60)(P = 0.05,0.04 respectively). Moreover,the GA or AA genotypes increased the TNFα serum level greater than the GG genotype(P = 0.002). The results showed a clear association between severe liver pathological conditions(inflammation,steatosis and fibrosis) and(GA + AA) genotypes(P = 0.035,0.03,0.04 respectively). The stepwise logistic regression analysis showed that the TNFα genotypes(GA + AA) were significantly associated with liver inflammation(OR = 3.776,95%CI: 1.399-10.194,P = 0.009),severe steatosis(OR = 4.49,95%CI: 1.441-14.0,P = 0.010) and fibrosis progression(OR = 2.84,95%CI: 1.080-7.472,P = 0.034). Also,the A allele was an independent risk factor for liver inflammation(P = 0.003),steatosis(P = 0.003) and fibrosis(P = 0.014). 14The use of point load test for Dubai weak calcareous sandstones显示文摘Intact rock is typically described according to its uniaxial compressive strength(UCS). The UCS is needed in the design of geotechnical engineering problems including stability of rock slopes and design of shallow and deep foundations resting on and/or in rocks. Accordingly, a correct measurement/evaluation of the UCS is essential to a safe and economic design. Typically, the UCS is measured using the unconfined compression tests performed on cylindrical intact specimens with a minimum length to width ratio of 2. In several cases, especially for weak and very weak rocks, it is not possible to extract intact specimens with the needed minimum dimensions. Thus, alternative tests(e.g. point load test, Schmidt hammer) are used to measure rock strength. The UCS is computed based on the results of these tests through empirical correlations. The literature includes a plethora of these correlations that vary widely in estimating rock strength. Thus, it is paramount to validate these correlations to check their suitability for estimating rock strength for a specific location and geology. A review of the available correlations used to estimate the UCS from the point load test results is performed and summarized herein. Results of UCS, point load strength index and Young's modulus are gathered for calcareous sandstone specimens extracted from the Dubai area. A correlation for estimating the UCS from the point load strength index is proposed. 18A Challenging Approach to Improve the Low Affinity of Acrylic Fibres to Be Successfully Dyed with a Bio-Colorant Extracted from Grape Marc显示文摘Acrylic fibres are highly crystalline and non-polar polymers,which makes their dyeing a very difficult step that poses real technical challenges.In order to overcome this concern,it is intended in this paper to modify acrylic fibers by different methods namely cationisation using the Crosscolor DRT then amidoximation using hydroxylamine hydrochloride and ammonium acetate.The resulted samples were dyed then with the bio-colorant extracted from grape marc.The effect of the pretreatment on fibers fine structure using X-ray diffraction and the scanning electron microscope(SEM)images and its correlation with the colour strength of the dyed fabrics was investigated.The dyeing parameters,such as dye bath pH and temperature on the performances of this dyeing process were studied.Good dyeing qualities and new shades varying from brown to grey and dark green have been obtained following process optimization,mordanting and modification of acrylic fibers by the technique of cationisation.Noureddine Baaka Manel Ben Ticha Wafa Haddar Mohamed Farouk Mhenni 2019Journal of Renewable Materials2019,7,3:2
19基于天然XY雌鱼培育YY超雄尼罗罗非鱼的新方法显示文摘前期报道了养殖群体存在天然XY雌鱼,但其能否用于培育YY超雄罗非鱼尚不清楚。本研究首先引入遗传性别受LG23染色体严格控制的CQ(重庆)尼罗罗非鱼群体和具有天然XY雌鱼的WC(湛江吴川)群体,将CQ群体XY雄鱼与WC XY雌鱼杂交,检验杂交F1 YY超雄鱼是否可用于控制后代性别,并比较杂交F1 XY和YY罗非鱼体质量、性腺指数、血清激素水平和性腺基因表达情况。研究发现,CQ XY雄鱼和WC XY雌鱼交配,获得的F1中有25%为YY超雄鱼,经鉴定为全雄且可育。将F1 YY超雄鱼与WC XX雌鱼、WC XY雌鱼(母本)、杂交F1 XX雌鱼和CQ XX雌鱼交配,后代几乎全雄,仅在与F1 XX雌鱼交配的后代中有2尾雌鱼(雄性率98%)。在孵化后180 d,杂交F1中XY和YY个体的体质量、性腺指数、血清激素水平差异不显著。基因表达分析发现,YY鱼精巢中AmhX/AmhY mRNA表达显著高于XY鱼,而Dmrt1和Cyp11b2 mRNA表达水平差异不显著。杂交F1 YY和XY鱼生理指标无明显差异。因此,采用尼罗罗非鱼天然XY雌鱼能够培育YY超雄鱼,且该YY超雄鱼能够用于罗非鱼性别控制。江东能 彭友幸 黄远青 邝紫颖 杨空松 MUSTAPHA Umar Farouk 石红娟 李广丽 2020水产学报2020,44,11:2
