1Drainage vs. non-drainage after cholecystectomy for acute cholecystitis:a retrospective study显示文摘Many surgeons practice prophylactic drainage after cholecystectomy without reliable evidence,this study was conducted to answer the question whether to drain or not to drain after cholecystectomy for acute calculous cholecystitis.A retrospective review of all patients who had cholecystectomy for acute cholecystitis in Aseer Central Hospital,Abha,Saudi Arabia,was conducted from April 2010 to April 2012.Data were extracted from hospital case files.Preoperative data included clinical presentation,routine investigations and liver function tests.Operative data included excessive adhesions,bleeding,bile leak,and drain insertion.Complicated cases such as pericholecystic collections,mucocele and empyema were also reported.Patients who needed therapeutic drainage were excluded.Postoperative data included hospital stay,volume of drained fluid,time of drain removal,and drain site problems.The study included 103 patients allocated into two groups;group A(n = 38) for patients with operative drain insertion and group B(n = 65) for patients without drain insertion.The number of patients with preoperative diagnosis of acute non-complicated cholecystitis was significantly greater in group B(80%) than group A(36.8%)(P < 0.001).Operative time was significantly longer in group A.All patients who were converted from laparoscopic to open cholecystectomy were in group A.Multivariate analysis revealed that hospital stay was significantly(P < 0.001) longer in patients with preoperative complications.There was no added benefit for prophylactic drain insertion after cholecystectomy for acute calculous cholecystitis in non-complicated or in complicated cases.Mohammed A Bawahab Walid M Abd El Maksoud Saeed A Alsareii Fahad S Al Amri Hala F Ali Abdul Rahman Nimeri Abdul Rahman M Al Amri Adel A Assiri Mohammed I Abdul Aziz 2014The Journal of Biomedical Research2014,28,3:3
2Histopathological characteristics and causes of kidney graft failure in the current era of immunosuppression显示文摘BACKGROUND The histopathological findings on the failing kidney allograft in the modern era is not well studied. In this study, we present our experience working with kidney transplant recipients with graft failure within one year of the biopsy.AIM To report the histopathological characteristics of failed kidney allografts in the current era of immunosuppression based on the time after transplant, cause of the end-stage renal disease and induction immunosuppressive medications.METHODS In a single-center observational study, we characterized the histopathological findings of allograft biopsies in kidney transplant recipients with graft failure within one year after the biopsy.RESULTS We identified 329 patients with graft failure that met the selection criteria between January 1, 2006 and December 31, 2016. The three most common biopsy findings were interstitial fibrosis and tubular atrophy(IFTA, 53%), acute rejection (AR, 43%) and transplant glomerulopathy(TG, 33%). Similarly, the three most common causes of graft failure based on the primary diagnosis were AR(40%),TG(17%), and IFTA(13%). Most grafts failed within two years of post-transplant(36%). Subsequently, approximately 10%-15% of grafts failed every two years: >2-4 years(16%), > 4-6 years(13%), > 6-8 years(11%), > 8-10 years(9%) and > 10 years(16%). 4Role of novel biomarkers in kidney transplantation显示文摘Clinical application of biomarkers is an integral component of transplant care.Clinicians and scientists alike are in search of better biomarkers than the current serologic(serum creatinine,donor-specific antibodies),urine-derived(urinalysis,urine protein),and histologic ones we now use.The science behind recent biomarker discovery spans across multiple molecular biologic disciplines,including transcriptomics,proteomics,and metabolomics.Innovative methodology and integration of basic and clinical approaches have allowed researchers to unearth molecular phenomena preceding clinical disease.Biomarkers can be classified in several ways.In this review,we have classified them via their origin and outcome:Primarily immunologic,i.e.,representative of immune regulation and dysfunction and non-immunologic,pertaining to delayed graft function,cardiovascular events/mortality,infection,malignancy,posttransplant diabetes,graft,and patient survival.Novel biomarker uses to guide the diagnosis and management of transplant-related outcomes is a promising area of research.However,the use of biomarkers to predict outcomes after kidney transplantation is not well studied.In this review,we summarize the recent studies illustrating biomarker use and transplant outcomes.Kurtis J Swanson Fahad Aziz Neetika Garg Maha Mohamed Didier Mandelbrot Arjang Djamali Sandesh Parajuli 2020World Journal of Transplantation2020,10,9:1

6Post-transplant erythrocytosis after kidney transplantation:A review显示文摘Post-transplant erythrocytosis(PTE)is defined as persistently elevated hemoglobin>17 g/dL or hematocrit levels>51%following kidney transplantation,independent of duration.It is a relatively common complication within 8 months to 24 months post-transplantation,occurring in 8%-15%of kidney transplant recipients.Established PTE risk factors include male gender,normal hemoglobin/hematocrit pre-transplant(suggestive of robust native kidney erythropoietin production),renal artery stenosis,patients with a well-functioning graft,and dialysis before transplantation.Many factors play a role in the development of PTE,however,underlying endogenous erythropoietin secretion pre-and post-transplant is significant.Other contributory factors include the renin-angiotensin-aldosterone system,insulin-like growth factors,endogenous androgens,and local renal hypoxia.Most patients with PTE experience mild symptoms like malaise,headache,fatigue,and dizziness.While prior investigations showed an increased risk of thromboembolic events,more recent evidence tells a different story-that PTE perhaps has lessened risk of thromboembolic events or negative graft outcomes than previously thought.In the evaluation of PTE,it is important to exclude other causes of erythrocytosis including malignancy before treatment.Angiotensin converting enzyme inhibitors(ACE-I)and angiotensin receptor blockers(ARBs)are the mainstays of treatment.Increased ACE-I/ARB use has likely contributed to the falling incidence of erythrocytosis.In this review article,we summarize the current literature in the field of post-transplant erythrocytosis after kidney transplantation.Beyann Alzoubi Abish Kharel Rushad Machhi Fahad Aziz Kurtis J Swanson Sandesh Parajuli 2021World Journal of Transplantation2021,11,6:1

8Assessment of Phytochemical Analysis,Nutritional Composition and Antimicrobial Activity of Moringa oleifera显示文摘Moringa oleifera is a miracle plant rich in nutrients,antioxidants,and antibiotic properties.Present study was designed to evaluate various biochemical attributes of leaves and flowers of M.oleifera.Plant parts(leaves,flowers)of M.oleifera,collected from different roadsides of Multan district,Punjab,Pakistan,were used as experimental material.Result indicates that alkaloids,saponin,carbohydrates,fats,and protein had a high value in the aqueous extract of both leaves and flowers of M.oleifera.Whereas phenol content was high in methanolic leaves extract and the phenol contents were high in aqueous extract of flowers.The extract yield of M.oleifera leaves and flowers both showed a higher percentage in aqueous extract(57.5%),followed by methanol extract and lowest in ethyl acetate extract.Flavonoids contents were higher in ethyl acetate extract of leaves(33.67%)and aqueous extract of flowers(53.71%).While crude fiber was high in methanolic extract of leaves(12.40%)and in flowers crude fiber was high in ethyl acetate extract(15.86%).The moisture contents were higher in leaves(8.87%)than flowers(7.3%)and similarly,ash percentage in flowers(52.60%)than leaves(41.84%).Ethyl acetate extracts of M.oleifera leaves show antibacterial activity against Pseudomonas aeruginosa while methanolic extract of M.oleifera flowers shows antibacterial activity against Xanthomonas sp.Maximum growth inhibits show in all extracts of leaves against Aspergillus flavus,F.oxysporum,and P.glabrum except for the concentrated aqueous extract of leaves.While in flowers maximum growth inhibits all extracts against P.glabrum,A.niger,and A.flavus except the diluted ethyl acetate extract.Phytochemicals present in different parts of moringa have significant edible and commercial potential.Moringa extracts exhibited significant antimicrobial activity,therefore have applications in pharmaceuticals.Naima Asghar Abida Aziz Muhammad Farooq Azhar Mohamed El-Sharnouby Uzma Irfan Iqra Rafiq Husnain Farooq Muhammad Ishaq Asif Rehmani Mohamed I.Sakran A.Altalhi Fahad M.Alzuaibr Ayman El Sabagh 2022Phyton-International Journal of Experimental Botany2022,91,8:1
9Ambiguity Resolution in Direction of Arrival Estimation with Linear Antenna Arrays Using Differential Geometry显示文摘Linear antenna arrays(LAs)can be used to accurately predict the direction of arrival(DOAs)of various targets of interest in a given area.However,under certain conditions,LA suffers from the problem of ambiguities among the angles of targets,which may result inmisinterpretation of such targets.In order to cope up with such ambiguities,various techniques have been proposed.Unfortunately,none of them fully resolved such a problem because of rank deficiency and high computational cost.We aimed to resolve such a problem by proposing an algorithm using differential geometry.The proposed algorithm uses a specially designed doublet antenna array,which is made up of two individual linear arrays.Two angle observation models,ambiguous observation model(AOM)and estimated observation model(EOM),are derived for each individual array.The ambiguous set of angles is contained in the AOM,which is obtained from the corresponding array elements using differential geometry.The EOM for each array,on the other hand,contains estimated angles of all sources impinging signals on each array,as calculated by a direction-finding algorithm such as the genetic algorithm.The algorithm then contrasts the EOM of each array with its AOM,selecting the output of that array whose EOM has the minimum correlation with its corresponding AOM.In comparison to existing techniques,the proposed algorithm improves estimation accuracy and has greater precision in antenna aperture selection,resulting in improved resolution capabilities and the potential to be used more widely in practical scenarios.The simulation results using MATLAB authenticates the effectiveness of the proposed algorithm.Alamgir Safi Muhammad Asghar Khan Fahad Algarni Muhammad Adnan Aziz M.Irfan Uddin Insaf Ullah Tanweer Ahmad Cheema 2022Computers, Materials & Continua2022,,1:0
