| 1 | Ultrasonically activated scalpel versus monopolar electrocautery shovel in laparoscopic total mesorectal excision for rectal cancer显示文摘AIM: To investigate the feasibility and safety of monopolar electrocautery shovel (ES) in laparoscopic total mesorectal excision (TME) with anal sphincter preservation for rectal cancer in order to reduce the cost of the laparoscopic operation, and to compare ES with the ultrasonically activated scalpel (US). METHODS: Forty patients with rectal cancer, who underwent laparoscopic TME with anal sphincter preservation from June 2005 to June 2007, were randomly divided into ultrasonic scalpel group and monopolar ES group, prospectively. White blood cells (WBC) were measured before and after operation, operative time, blood loss, pelvic volume of drainage, time of anal exhaust, visual analogue scales (VAS) and surgery-related complications were recorded. RESULTS: All the operations were successful; no one was converted to open procedure. No significant differences were observed in terms of preoperative and postoperative d 1 and d 3 WBC counts (P = 0.493, P = 0.375, P = 0.559), operation time (P = 0.235), blood loss (P = 0.296), anal exhaust time (P = 0.431), pelvicdrainage volume and VAS in postoperative d 1 (P = 0.431, P = 0.426) and d 3 (P = 0.844, P = 0.617) between ES group and US group. The occurrence of surgery-related complications such as anastomotic leakage and wound infection was the same in the two groups. CONCLUSION: ES is a safe and feasible tool as same as US used in laparoscopic TME with anal sphincter preservation for rectal cancer on the basis of the skillful laparoscopic technique and the complete understanding of laparoscopic pelvic anatomy. | 2 | Atmospheric particulate pollution of Lanzhou using magnetic measurements显示文摘To differentiate between natural and anthropogenic particulate sources in the atmosphere in Lanzhou City, samples were collected in different sites. The dust flux was calculated and magnetic measurements were conducted. Results show a distinct pattern of variation of dust flux within a year and it agrees with the shifts of atmospheric circulation regime. The magnetic parameters indicate that natural sources are the major components of atmospheric particulate during late spring and early summer, while anthropogenic sources contribute much more during winter months. | 4 | How Work Organization Affects the Prevalence of WMSDs: A Case-control Study显示文摘Objective In this study, we aimed at exploring the association between work-related musculoskeletal disorders(WMSDs) and work organization based on a case-control study. Methods A total of 1938 workers who claimed to suffer from WMSDs were selected from Beijing, Henan, Hubei, and the Guangdong province. The control group consisted of 2009 workers employed in similar industries without severe disease or musculoskeletal discomforts. We used a modified version of the questionnaire developed by the NMQ and the DMQ to investigate individual and work-related factors. Results A total of 13 variables(P<0.1) were selected by the chi-square test and finally, 7 variables entered into the equation, with 6 variables reaching statistical significance(P<0.05). The odds ratios(OR) of ‘work changing with season' and ‘sufficient rest time' did not reach 1(0.749 and 0.441, respectively). In addition, ‘sufficient rest time' seemed to be the stronger protective factor according to its higher standardized coefficient. And ‘repetitive work every minute', ‘constantly repetitive work'(every day), ‘shortage of site personnel', and ‘often switching shifts with others' seemed to be the risk factors. Conclusion Work organization may have comprehensive effects on the occurrence of WMSDs. | 7 | Co-effect of Demand-control-support Model and Effort-reward Imbalance Model on Depression Risk Estimation in Humans: Findings from Henan Province of China显示文摘Objective To investigate the co-effect of Demand-control-support(DCS) model and Effort-reward Imbalance(ERI) model on the risk estimation of depression in humans in comparison with the effects when they are used respectively. Methods A total of 3 632 males and 1 706 females from 13 factories and companies in Henan province were recruited in this cross-sectional study. Perceived job stress was evaluated with the Job Content Questionnaire and Effort-Reward Imbalance Questionnaire(Chinese version). Depressive symptoms were assessed by using the Center for Epidemiological Studies Depression Scale(CES-D). Results DC(demands/job control ratio) and ERI were shown to be independently associated with depressive symptoms. The outcome of low social support and overcommitment were similar. High DC and low social support(SS), high ERI and high overcommitment, and high DC and high ERI posed greater risks of depressive symptoms than each of them did alone.ERI model and SS model seem to be effective in estimating the risk of depressive symptoms if they are used respectively. Conclusion The DC had better performance when it was used in combination with low SS. The effect on physical demands was better than on psychological demands. | 14 | Clinical practice guideline for transurethral plasmakinetic resection of prostate for benign prostatic hyperplasia(2021 Edition)显示文摘Benign prostatic hyperplasia (BPH) is highly prevalent among older men, impacting on their quality of life, sexual function, and genitourinary health, and has become an important global burden of disease. Transurethral plasmakinetic resection of prostate (TUPKP) is one of the foremost surgical procedures for the treatment of BPH. It has become well established in clinical practice with good efficacy and safety. In 2018, we issued the guideline “2018 Standard Edition”. However much new direct evidence has now emerged and this may change some of previous recommendations. The time is ripe to develop new evidence-based guidelines, so we formed a working group of clinical experts and methodologists. The steering group members posed 31 questions relevant to the management of TUPKP for BPH covering the following areas: questions relevant to the perioperative period (preoperative, intraoperative, and postoperative) of TUPKP in the treatment of BPH, postoperative complications and the level of surgeons’ surgical skill. We searched the literature for direct evidence on the management of TUPKP for BPH, and assessed its certainty generated recommendations using the grade criteria by the European Association of Urology. Recommendations were either strong or weak, or in the form of an ungraded consensus-based statement. Finally, we issued 36 statements. Among them, 23 carried strong recommendations, and 13 carried weak recommendations for the stated procedure. They covered questions relevant to the aforementioned three areas. The preoperative period for TUPKP in the treatment of BPH included indications and contraindications for TUPKP, precautions for preoperative preparation in patients with renal impairment and urinary tract infection due to urinary retention, and preoperative prophylactic use of antibiotics. Questions relevant to the intraoperative period incorporated surgical operation techniques and prevention and management of bladder explosion. The application to different populations incorporating the efficacy and safety of TUPKP in the treatment of normal volume (< 80 ml) and large-volume (≥ 80 ml) BPH compared with transurethral urethral resection prostate, transurethral plasmakinetic enucleation of prostate and open prostatectomy;the efficacy and safety of TUPKP in high-risk populations and among people taking anticoagulant (antithrombotic) drugs. Questions relevant to the postoperative period incorporated the time and speed of flushing, the time indwelling catheters are needed, principles of postoperative therapeutic use of antibiotics, follow-up time and follow-up content. Questions related to complications incorporated types of complications and their incidence, postoperative leukocyturia, the treatment measures for the perforation and extravasation of the capsule, transurethral resection syndrome, postoperative bleeding, urinary catheter blockage, bladder spasm, overactive bladder, urinary incontinence, urethral stricture, rectal injury during surgery, postoperative erectile dysfunction and retrograde ejaculation. Final questions were related to surgeons’ skills when performing TUPKP for the treatment of BPH. | 17 | Transmission of the Chromosome 1R in Winter Wheat Germplasm Aimengniu and Its Derivatives Revealed by Molecular Markers显示文摘In order to clarify the transmission of the rye chromosome 1R in winter wheat germplasm Aimengniu and its derivatives, 17 derivatives and 7 types of Aimengniu were examined through molecular-marker technology. The results showed that the chromosome arm 1RS of Neuzucht was transmitted to 5 of the 7 types of Aimengniu, i.e., Aimengniu II and Aimengniu IV-Aimengniu VII, no segment of 1RS was identified in Aimengniu I or Aimengniu III. As for the 17 derivatives, the 1RS chromosome arm of Aimengniu was transmitted to 11 derivatives, part segments of 1RS were found in 1 derivative, while no segment was found in the remaining 5 ones. | 19 | Microstructure and homogenization process of as-cast GH4169D alloy for novel turbine disk显示文摘To investigate the microstructure, segregation, and suitable homogenization process of as-cast GH4169D alloy, the microstructure, elements segregation, and precipitates of cast GH4169D ingots prepared by vacuum induction melting (VIM) and vacuum arc remelting (VAR) were observed by optical microscopy (OM), scanning electron microscopy, and energy-dispersive X-ray spectroscopy (EDS). According to the residual segregation model and simulation results of DICTRA thermodynamic software, the homogenization temperature and time range were set as 1120–1170°C and 5–20 h, respectively. The experimental results showed that microscopic dendrite and element segregation occurred in the interior of ingots and the main segregation elements were Nb and Ti. In addition, the precipitates were mainly distributed in interdendritic regions and were composed of NbC, Laves,γ′, and δ phases. The homogenization process suggested that the interdendritic detrimental precipitated Laves phase can be eliminated or redissolved after homogenization at 1150°C for 20 h, suggesting it was the most suitable homogenization treatment. | 20 | What makes the Tc of monolayer FeSe on SrTiO3 so high: a sign-problem-free quantum Monte Carlo study显示文摘Monolayer FeSe films grown on SrTiO_3(STO)substrate show superconducting gap-opening temperatures(T_c) which are almost an order of magnitude higher than those of the bulk FeSe and are highest among all known Fe-based superconductors. Angle-resolved photoemission spectroscopy observed ‘‘replica bands' ' suggesting the importance of the interaction between FeSe electrons and STO phonons. These facts rejuvenated the quest for T_c enhancement mechanisms in iron-based, especially ironchalcogenide, superconductors. Here, we perform the first numerically-exact sign-problem-free quantum Monte Carlo simulations to iron-based superconductors. We(1) study the electronic pairing mechanism intrinsic to heavily electron doped FeSe films, and(2) examine the effects of electron–phonon interaction between FeSe and STO as well as nematic fluctuations on T_c.
