| 5 | Irritable bowel syndrome subtypes differ in body awareness, psychological symptoms and biochemical stress markers显示文摘AIM: To elucidate the differences in somatic, psycho-logical and biochemical pattern between the subtypes of irritable bowel syndrome (IBS). METHODS: Eighty IBS patients, 30 diarrhoea pre-dominant (D-IBS), 16 constipation predominant (C-IBS) and 34 alternating IBS (A-IBS) underwent physi-otherapeutic examinations for dysfunctions in body movements and awareness and were compared to an apparently healthy control group (AHC). All groups an-swered questionnaires for gastrointestinal and psycho-logical symptoms. Biochemical variables were analysed in blood. RESULTS: The D-IBS group showed less body aware-ness, less psychological symptoms, a more normal sense of coherence and psychosocial rating as well as higher C-peptide values. C-IBS had a higher degree of body dysfunction and psychological symptoms, as well as the lowest sense of coherence compared to controls and D-IBS. They also demonstrated the most elevated prolactin levels. A-IBS had the lowest degree of body disturbance, deteriorated quality of life and affected bi-ochemical pattern. All subtypes had higher pain scores compared to controls. In addition they all had signifi -cantly increased triglycerides and elevated morning cortisol levels, however, without statistical signifi cance compared with the controls.CONCLUSION: IBS subtypes showed different pro-files in body awareness, somatic and psychological symptoms and in biochemical variables. D-IBS differed compared to the other groups by lowered body aware-ness, less psychological symptoms and a higher sense of coherence and elevated C-peptide values. C-IBS and A-IBS subtypes suffered more from depression and anxiety, associated with a lower quality of life. | 6 | Aspects of the non-pharmacological treatment of irritable bowel syndrome显示文摘Irritable bowel syndrome(IBS) is one of the most commonly diagnosed gastrointestinal conditions. It represents a significant healthcare burden and remains a clinical challenge. Over the years IBS has been described from a variety of different perspectives; from a strict illness of the gastrointestinal tract(medical model) to a more complex multi-symptomatic disorder of the brain-gut axis(biopsychosocial/psychosomatic model). In this article we present aspects of the pathophysiology and the non-pharmacological treatment of IBS based on current knowledge. Effects of conditioned stress and/or traumatic influences on the emotional system(top-down) as well as effects on the intestine through stressors,infection,inflammation,food and dysbiosis(bottom-up) can affect braingut communication and result in dysregulation of the autonomic nervous system(ANS),playing an important role in the pathophysiology of IBS. Conditioned stress together with dysregulation of the autonomic nervous system and the emotional system may involve reactions in which the distress inside the body is not recognized due to low body awareness. This may explain why patients have difficulty identifying their symptoms despite dysfunction in muscle tension,movement patterns,and posture and biochemical functions in addition to gastrointestinal symptoms. IBS shares many features with other idiopathic conditions,such as fibromyalgia,chronic fatigue syndrome and somatoform disorders. The key to effective treatment is a thorough examination,including a gastroenterological examination to exclude other diseases along with an assessment of body awareness by a body-mind therapist. The literature suggests that early interdisciplinary diagnostic cooperation between gastroenterologists and body-mind therapists is necessary. Re-establishing balance in the ANS is an important component of IBS treatment. | 10 | Contribution of precursor compounds to the release of per-and polyfluoroalkyl substances(PFASs) from waste water treatment plants(WWTPs)显示文摘Per-and polyfluoroalkyl substances(PFASs) are ubiquitous in sludge and water from waste water treatment plants, as a result of their incorporation in everyday products and industrial processes. In this study, we measured several classes of persistent PFASs,precursors, transformation intermediates, and newly identified PFASs in influent and effluent sewage water and sludge from three municipal waste water treatment plants in Sweden, sampled in 2015. For sludge, samples from 2012 and 2014 were analyzed as well.Levels of precursors in sludge exceeded those of perfluoroalkyl acids and sulfonic acids(PFCAs and PFSAs), in 2015 the sum of polyfluoroalkyl phosphoric acid esters(PAPs) were 15–20 ng/g dry weight, the sum of fluorotelomer sulfonic acids(FTSAs) was 0.8–1.3 ng/g,and the sum of perfluorooctane sulfonamides and ethanols ranged from non-detected to 3.2 ng/g. Persistent PFSAs and PFCAs were detected at 1.9–3.9 ng/g and 2.4–7.3 ng/g dry weight, respectively. The influence of precursor compounds was further demonstrated by an observed substantial increase for a majority of the persistent PFCAs and PFSAs in water after waste water treatment. Perfluorohexanoic acid(PFHxA), perfluorooctanoic acid(PFOA), perfluorohexane sulfonic acid(PFHxS), and perfluorooctane sulfonic acid(PFOS)had a net mass increase in all WWTPs, with mean values of 83%, 28%, 37% and 58%,respectively. | 14 | Microbial mat-related structures shared by both siliciclastic and carbonate formations显示文摘Microbiota has always been the dominant life form,records of which are preserved in delicate forms within siliciclastic rocks.More pronounced record in the form of stromatolites possibly obscured the fact that many of the same delicate structures may be recognizable within carbonate rocks too.The Neoproterozoic Bhander Limestone in central India bears many such structures that are quintessentially similar to microbial mat-related structures reported from the Paleoproterozoic Chorhat Sandstone preserved within the same,Vindhyan Basin.Extensive microscopic,ultramicroscopic,and geochemical studies address the apprehension that such bedding plane structures in carbonate rocks could be merely weathering products.Trapping,binding and stabilitization of sediment by microbial mats are all evident.Preferred pyritization along the inferred,predefined microbial mats confirmed on the basis of EPMA(Electron Probe Microanalysis) results,and the enhanced carbon content along these mats layers and within suspected mat chips associated with them,are revealing.Raman spectroscopy,indeed,evinces enhanced kerogen content within both mats and mat chips.Interestingly,these microbial mat layers are recognized selectively within the lower of the two tiers of the Bhander Limestone.The lagoonal carbonate of the lower tier of the Bhander Limestone is muddy and contains a substantial proportion of silt-sized quartz grains that possibly impeded stromatolite growth.Stromatolites abound in the wave agitated upper tier of the Bhander Limestone which is dominated by oosparite.This paper provides evidence that the delicate microbial mat-related structures reported so far only from siliciclastic rocks can also be recognized within carbonate formations,and hopes to stimulate the search for additional such features,more preferably within carbonates originated in shallow and quiet water. | Subir Sarkar Adrita Choudhuri Sunipa Mandal Patrick G. | 16 | Recruitment in species-rich grasslands: the effects of functional traits and propagule pressure显示文摘Aims The assembly of plant communities is a complex process which combines impacts from the species pool,dispersal and propagule pressure,niche requirements of colonizing species and the niche structure of the community.Recent theory development has incorporated all these aspects,e.g.in‘stochastic niche theory’.We investigated recruitment into a species-rich grassland community,using an experimental approach where we manipulated the trait composition of the community and examined the success of colonizing species entering with various propagule pressure.Specifically,we examined two predictions:(i)colonization success increases with increasing difference between traits of the colonizing species and the trait profile of the community and(ii)colonization success increases with increasing propagule pressure.Methods The examined communities were species-rich semi-natural grasslands located in southern Sweden.After a careful documentation of the composition of the plant communities at the experimental sites,we manipulated the trait profile of species-rich grassland plots based on the plant functional trait specific leaf area(SLA),which is correlated with several key life history functions.In addition to SLA,seed mass was also used to describe the trait profile of grassland plots.Seeds of 12 plant species from the regional species pool,varying in SLA and seed mass,were sown into plots using four different levels of propagule pressure.Recruitment was examined after 1 year.We also planted juvenile‘plug plants’of the same species which allowed us to examine survivorship and growth beyond the seedling stage.Important Findings Overall we found very limited evidence for relationships between the traits of the colonizing species and the trait profile of the community and for recruitment after sowing these relationships were contrary to the prediction.Survival of plug plants after two seasons of growth was high irrespective of the trait profile of the community,but growth of plug plants was affected by the trait profile of the surrounding community.For four of the species there was a positive effect of increased propagule pressure on colonization.The results suggest that species assembly in species-rich grasslands is not strongly dependent on the niche structure of the community.However,the finding that colonization of only a third of the species responded positively to increased propagule pressure indicates that there might be niche-related effects that were not captured by our treatments.Overall,our results indicate that the factors determining colonization in this community are species specific.Some species are able to colonize irrespective of niche relationships,provided that the propagule pressure is sufficiently high to overcome stochastic mortality after seed arrival.For other species,however,we cannot exclude that niche assembly occurred,but we failed to identify the relevant niche factor. | MathiasÖster Ove Eriksson | 2012 | Journal of Plant Ecology2012,5,3: | 2 |

Its efficacy in severe dementia has not been assessed and is controversial. Our aim was to ascertain the effectiveness of donepezil in patients with severe Alzheimer’s disease, by focusing primarily on cognition and activities of daily living. Methods: We did a 6-month, double-blind, parallel-group, placebo-controlled study in 248 patients with severe Alzheimer‘s disease (mini mental state examination score 1-10) who were living in assisted care nursing homes ran by trained staff in Sweden. We assigned patients oral donepezil (5 mg per day for 30 days then up to 10 mg per day thereafter, n=128) or matched placebo (n=120). Our primary endpoints were change from baseline to month 6 in the severe impairment battery (SIB) and modified Alzheimer’s Disease Cooperative Study activities of daily living inventory for severe Alzheimer’s disease (ADCS-ADL-severe). We analysed outcomes for patients with data at baseline and at one or more other timepoints (modified intent-to-treat population) with last observation carried forward used to replace missing data. Findings: 95 patients assigned donepezil and 99 patients assigned placebo completed the study. Patients treated with donepezil improved more in SIB scores and declined less in ADCS-ADL-severe scores at 6 months after initiation of treatment compared with baseline than did controls (least squares [LS] mean difference, 5.7, 95%CI 1.5-9.8; p=0.008, and 1.7, 0.2-3.2; p=0.03, respectively). The incidence of adverse events was comparable between groups (donepezil 82%[n=105] vs placebo 76%[n=91]), with most being transient and mild or moderate in severity. More patients discontinued treatment because of adverse events in the donepezil group (n=20) than in the placebo group (n=8). Interpretation: Donepezil improves cognition and preserves function in individuals with severe Alzheimer’s disease who live in nursing homes. | Winblad B. Kilander L.
