1One hundred and one over-the-scope-clip applications forsevere gastrointestinal bleeding,leaks and fistulas显示文摘AIM: To investigate the efficacy and clinical outcome of patients treated with an over-the-scope-clip(OTSC) system for severe gastrointestinal hemorrhage, perforations and fistulas.METHODS: From 02-2009 to 10-2012, 84 patients were treated with 101 OTSC clips. 41 patients(48.8%) presented with severe upper-gastrointestinal(GI) bleeding, 3(3.6%) patients with lower-GI bleeding, 7 patients(8.3%) underwent perforation closure, 18 patients(21.4%) had prevention of secondary perforation, 12 patients(14.3%) had control of secondary bleeding after endoscopic mucosal resection or endoscopic submucosal dissection(ESD) and 3 patients(3.6%) had an intervention on a chronic fistula. RESULTS: In 78/84 patients(92.8%), primary treatment with the OTSC was technically successful. Clinical primary success was achieved in 75/84 patients(89.28%). The overall mortality in the study patients was 11/84(13.1%) and was seen in patients with life threatning upper GI hemorrhage. There was no mortality in any other treatment group. In detail OTSC application lead to a clinical success in 35/41(85.36%) patients with upper GI bleeding and in 3/3 patients with lower GI bleeding. Technical success of perforation closure was 100% while clinical success was seen in 4/7 cases(57.14%) due to attendant circumstances unrelated to the OTSC. Technical and clinic success was achieved in 18/18(100%) patients for the prevention of bleeding or perforation after endoscopic mucosal resection and ESD and in 3/3 cases of fistula closure. Two application-related complications were seen(2%).CONCLUSION: This largest single center experience published so far confirms the value of the OTSC for GI emergencies and complications. 2Plasma Sprayed NiAl Intermetallic Coating Produced with Mechanically Alloyed Powder显示文摘In the present research, mechanically alloyed Ni-Al powder was utilized to develop plasma sprayed coatings, and the effect of the spray distance and heat treatment on the phases, microstructure, and hardness of the coat- ings were examined. Coatings were analyzed by X-ray diffraction (XRD), scanning electron microscopy (SEM), energy-dispersive spectroscopy (EDS) and through microhardness measurements. Although mechanically al- loyed Ni-Al powder showed no intermetallic phases, the coatings did. Different spray distances from 5 to 19 cm were employed for plasma spray and the specimens were heat treated at different temperatures, then the amount of oxides, porosity and hardness of the coatings were changed according to the spray condition. The thermal energy of the plasma spray caused the formation of NiAl phases while particles flew to the substrate or after that. Extreme increase in heat treatment temperature and spray distance resulted in oxidation and reduction in the quality of the coating. 3Fluid Flow and Mixing in Non-Isothermal Water Model of Continuous Casting Tundish显示文摘Fluid flow and mixing of molten steel in a twin-slab-strand continuous casting tundish were investigated using a mixing model under non-isothermal conditions.This model led to a set of ordinary differential equations that were solved with a Runge-Kutta algorithm.Steady state water modeling was carried out under non-isothermal conditions.Experimental data obtained from the water model were used to calibrate the mixing model.Owing to the presence of a mixed convection in the non-isothermal conditions,a channelizing flow would be created in the fluid inside the tundish.A mixing model was designed that was capable of predicting RTD(residence time distribution)curves for different cases in non-isothermal conditions.The relationship between RTD parameters and the Tu(tundish Richardson number)was obtained for various cases under non-isothermal conditions.The results show that the RTD parameters were completely different under isothermal and non-isothermal conditions.The comparison of the RTD curves between the isothermal and non-isothermal conditions presents that the extent of mixing in the tundish in non-isothermal conditions is lower than the mixing extent in isothermal conditions.Mehdi ALIZADEH Hossein EDRIS Ali SHAFYEI 2008Journal of Iron and Steel Research(International)2008,15,2:3
4Upward Molten Flow for Optimization of Fluid Flow in Continuous Casting Tundish显示文摘Fluid flow pattern and buoyancy force support the motion of nonmetallic inclusions toward the tundish slag. Upward molten flow was investigated. To understand the fundamentals of the process, physical modelling was carried out with the utilization of a 1∶4 scale model. Numerical modelling was carried out in line with the physical modelling to examine details of the flow pattern and rotational effect caused by the upward flow with the commercial CFD (Computational Fluid Dynamics) package environment, FLUENT. The two-equation k-ε model was used to simulate the turbulence. Multiphase fluid flow was numerically simulated by using the Volume of Fluid (VoF) method. 5Groundwater quality modeling using self-organizing map(SOM) and geographic information system(GIS) on the Caspian southern coasts显示文摘Groundwater is the main source for water provision in the arid and semi-arid areas such as Iran.The groundwater quality was simulated by using a hybrid model integrating a Self-Organizing Map(SOM)and geographic information system(GIS).SOM and GIS were used as pre-processing and postprocessing tools in the Mazandaran Plain.Further,the Ground Water Quality Index(GWQI)and its effective factors were estimated by using digital maps and the secondary data.Neuro Solutions software was used for simulating the groundwater quality.To do this,a model was trained and optimized in the SOM and then the optimized model was tested.In the next step,the performance of SOM in groundwater quality simulation was confirmed(test stage,Rsqr=0.8,and MSE=0.008).Then,the digital maps of the SOM inputs were converted to raster format in GIS.In the last step,a raster layer was generated by combining the model input layers which comprised the model inputs values.The tested SOM was used to simulate GWQI in the sites without the secondary data of the groundwater quality.Finally,the groundwater quality map was generated by coupling the results of SOM estimations and GIS capabilities.The results revealed that the coupling of SOM and GIS has high performance in the simulation of the groundwater quality.According to the results,a limited area of the studied plain has groundwater resources with low quality(GWQI>0.04).Therefore,that will be a threat to the life of humans,animals,and vegetative species.Therefore,it is necessary to plan for managing the groundwater quality in the Mazandaran plain.Vahid GHOLAMI Mohammad Reza KHALEGHI Edris TAGHVAYE SALIMI 2020Journal of Mountain Science2020,17,7:2
6Survival and outcomes for co-infection of chronic hepatitis C with and without cirrhosis and COVID-19: A multicenter retrospective study显示文摘BACKGROUND Chronic liver disease,particularly cirrhosis,is associated with worse outcomes in patients infected with coronavirus disease 2019(COVID-19).AIM To assess outcomes of COVID-19 infection among patients with pre-existing hepatitis C with or without liver cirrhosis.METHODS This multicenter,retrospective cohort study included all cases of confirmed coinfection of severe acute respiratory syndrome coronavirus 2 and chronic hepatitis C with or without liver cirrhosis who were admitted to six hospitals(Al-Sahel Hospital,Al-Matareya Hospital,Al-Ahrar Hospital,Ahmed Maher Teaching Hospital,Al-Gomhoreya Hospital,and the National Hepatology and Tropical Medicine Research Institute)affiliated with the General Organization for Teaching Hospitals and Institutes in Egypt.Patients were recruited from May 1,2020,to July 31,2020.Demographic,laboratory,imaging features,and outcomes were collected.Multivariate regression analysis was performed to detect factors affecting mortality.RESULTS This retrospective cohort study included 125 patients with chronic hepatitis C and COVID-19 co-infection,of which 64(51.20%)had liver cirrhosis and 40(32.00%)died.Fever,cough,dyspnea,and fatigue were the most frequent symptoms in patients with liver cirrhosis.Cough,sore throat,fatigue,myalgia,and diarrhea were significantly more common in patients with liver cirrhosis than in noncirrhotic patients.There was no difference between patients with and without cirrhosis regarding comorbidities.Fifteen patients(23.40%)with liver cirrhosis presented with hepatic encephalopathy.Patients with liver cirrhosis were more likely than non-cirrhotic patients to have combined ground-glass opacities and consolidations in CT chest scans:28(43.75%)vs 4(6.55%),respectively(P value<0.001).These patients also were more likely to have severe COVID-19 infection,compared to patients without liver cirrhosis:29(45.31%)vs 11(18.04%),respectively(P value<0.003).Mortality was higher in patients with liver cirrhosis,compared to those with no cirrhosis:33(51.56%)vs 9(14.75%),respectively(P value<0.001).All patients in Child-Pugh class A recovered and were discharged.Cirrhotic mortality occurred among decompensated patients only.A multivariate regression analysis revealed the following independent factors affecting mortality:Male gender(OR 7.17,95%CI:2.19–23.51;P value=0.001),diabetes mellitus(OR 4.03,95%CI:1.49–10.91;P value=0.006),and liver cirrhosis(OR 1.103,95%CI:1.037–1.282;P value<0.0001).We found no differences in liver function,COVID-19 disease severity,or outcomes between patients who previously received direct-acting antiviral therapy(and achieved sustained virological response)and patients who did not receive this therapy.CONCLUSION Patients with liver cirrhosis are susceptible to higher severity and mortality if infected with COVID-19.Male gender,diabetes mellitus,and liver cirrhosis are independent factors associated with increased mortality risk.Shimaa Afify Basem Eysa Fatma Abdel Hamid Omnia M Abo-Elazm Mohamed A Edris Rabab Maher Ahmed Abdelhalim Muhammad Mostafa Abdel Ghaffar Dalia A Omran Hend Ibrahim Shousha 2021World Journal of Gastroenterology2021,27,42:2

8A new hybrid control technique for operation of DC microgrid under islanded operating mode显示文摘This study proposes a novel combined primary and secondary control approach for direct current microgrids,specifi-cally in islanded mode.In primary control,this approach establishes an appropriate load power sharing between the distributed energy resources based on their rated power.Simultaneously,it considers the load voltage deviation and provides satisfactory voltage regulation in the secondary control loop.The proposed primary control is based on an efficient droop mechanism that only deploys the local variable measurements,so as to overcome the side effects caused by communication delays.In the case of secondary control,two different methods are devised.In the first,low bandwidth communication links are used to establish the minimum required data transfer between the converters.The effect of communication delay is further explored.The second method excludes any communication link and only uses local variables.Accordingly,a self-sufficient control loop is devised without any communication require-ment.The proposed control notions are investigated in MATLAB/Simulink platform to highlight system performance.The results demonstrate that both proposed approaches can effectively compensate for the voltage deviation due to the primary control task.Detailed comparisons of the two methods are also provided.Vahid Mortezapour Sajjad Golshannavaz Edris Pouresmaeil Amin Yazdaninejadi 2022Protection and Control of Modern Power Systems2022,7,1:2
