| 19 | 世界家庭医生组织(WONCA)研究论文摘要汇编——儿童发热手册在急诊初级保健中的应用:一项群体随机对照试验显示文摘目的发热是儿童常见的就诊原因之一,目前抗生素不合理使用比例仍很高。本研究通过给急诊初级保健临床医生发放一本关于儿童发热的以疾病为重点的互动手册,观察该手册对医生开具抗生素处方的影响。方法本研究为一项在荷兰20个急诊全科医疗中心进行的群体随机对照试验,患者为年龄<12岁的发热儿童。于2015年11月—2016年6月给10个干预组的全科医疗中心的家庭医生发放以疾病为重点的互动手册。主要观察指标为首次问诊期间抗生素处方的开具情况。采用二分类随机截距Logistic回归模型进行数据分析。结果 3 518名家庭医生和25 355名儿童参与了本次试验。在干预组的11 945次问诊中,28.5%使用了该手册。查阅手册治疗组与常规治疗组首次问诊抗生素处方开具率比较无明显差异[23.5%比25.2%;OR=0.90,95%CI(0.79,1.02);群内相关系数=0.005];而使用手册治疗组抗生素处方开具率低于常规治疗组[21.9%比25.2%;OR=0.83,95%CI(0.74,0.94);群内相关系数=0.002]。获得手册组的家庭医生开具药物处方的比例和对类似疾病进行再次问询家长者所占比例低于未获得手册组。结论在急诊初级保健中,以疾病为重点的互动手册能否改善儿童发热治疗状况主要取决于家庭医生实际使用手册的情况。深入了解家庭医生使用和不使用手册的原因,可以明确以后如何使用该手册来进行干预。 | 本刊编辑部 BONT E G P M DINANT G J ELSHOUT G | 2018 | 中国全科医学2018，21，25： | 0 |
