1Effect of IOP based infusion system with and without balanced phaco tip on cumulative dissipated energy and estimated fluid usage in comparison to gravity fed infusion in torsional phacoemulsification显示文摘Background:To evaluate the effect of three different combinations of tip designs and infusion systems in torsional phacoemulsification(INFINITI and CENTURION)in patients with cataract.According to the manufacturer,two unique improvements in the Centurion are:active fluid dynamic management system and use of an intrepid balanced tip.The study specifically aimed to evaluate the beneficial effects,if any,of change in tip design and infusion system individually and in combination on both per-operative parameters as well as endothelial health over 6 months.Methods:One hundred and twenty six consenting patients of grade 4.0-6.9 senile cataract were randomized into three groups for phacoemulsification:Group A(n=42):Gravity fed infusion system and 45° Kelman miniflared ABS phaco tip;Group B(n=42):intraocular pressure(IOP)based infusion system and 45° Kelman miniflared ABS phaco tip;Group C(n=42):IOP based infusion system and 45° Intrepid balanced phaco tip.The cumulative dissipated energy(CDE),estimated fluid usage(EFU)and total aspiration time(TAT)were compared peroperatively.The endothelial parameters were followed up postoperatively for six months.Results:The three arms were matched for age(p=0.525),gender(p=0.96)and grade of cataract(p=0.177).Group C was associated with significant reductions in CDE(p=0.001),EFU(p<0.0005)as well as TAT(p=0.001)in comparison to the other groups.All three groups had comparable baseline endothelial cell density(p=0.876)and central corneal thickness(p=0.561).On post-operative evaluation,although all groups were comparable till 3 months,by 6 months,the percentage losses in endothelial cell density were significantly lower in group C as compared to the other groups.Conclusions:Use of an IOP based phacoemulsification system in association with use of the Intrepid balanced tip reduces the CDE,EFU and TAT in comparison to a gravity fed system with a mini flared tip or IOP based system with a mini flared tip while also providing better endothelial preservation thus favouring the use of an IOP fed system with a balanced tip.Praveen K.Malik Taru Dewan Arun Kr Patidar Ekta Sain 2017Eye and Vision2017,4,1:11
2老年髋部骨折围术期风险及术后1年效果评估显示文摘目的:了解老年髋部骨折患者围术期情况及术后1年结局并探讨进一步降低手术风险、提高手术疗效的可能措施。方法回顾性地分析复旦大学附属华东医院2010年1月~12月因髋部骨折住院并施行手术治疗的老年患者234例(年龄≥60岁),通过病史查阅及电话随访,记录患者年龄、性别、髋部骨折类型、手术前后营养状况(BMI、血浆白蛋白水平、血红蛋白水平)、日常生活能力、认知功能、主要伴发疾病、手术方式、麻醉方式、术后并发症等。结果(1)老年人占髋部骨折总数92.36%(278/301),女性多于男性。(2)术前身体状况多处于ASA Ⅱ~Ⅲ级之间,认知功能多在正常或轻度减退,心功能多在NYHA Ⅰ~Ⅱ级;术前主要营养指标均处于正常值低限;高血压病、心血管病、糖尿病、神经系统疾病、慢性支气管炎、肺部感染等是常见伴发疾病。(3)术后30d及1年的死亡率分别为2.99%和13.68%;术后生活自理能力完全恢复、部分恢复及无改善的患者各占64.32%,25.55%和10.13%;骨折类型、骨折前患者BMI、手术类型、术后并发症及术前身体状况对手术效果具有显著性意义(P<0.05)。结论老年髋部骨折应及时进行手术治疗,充分进行多学科评估和术前准备,选择合适的手术及麻醉方式,加强围术期管理,降低术后潜在并发症。陆文良 Dewan Sheilesh KUMAR 林伟龙 沈海敏 程群 郑松柏 2014中华老年多器官疾病杂志2014,13,6:9
3胃窦幽门腺组织学和超微结构的增龄变化研究显示文摘目的 探讨人体胃窦幽门腺组织学和超微结构的增龄变化特点. 方法 选择拥有相对正常胃组织的60例患者为研究对象,包括中青年组(20~59岁)20例,老年组(≥60岁)40例.老年组包括60~69岁组15例,70~79岁组13例,80~89岁组12例.在光学显微镜下测定各组胃窦固有膜厚度、幽门腺实质比,对比观察各组胃窦固有膜、幽门腺及腺细胞形态;在电子显微镜下对比观察各组幽门腺细胞的超微结构. 结果 胃窦固有膜厚度20~59岁组为(649.8±127.9)μm,60~69岁组为(634.6±121.4)μm,70~79岁组为(615.7±74.6)μm,随增龄呈降低趋势;但80~89岁组为(645.5±99.9)μm,与20~59岁组相当(t=0.080,P=0.937).幽门腺实质比从20~59岁组、60~69岁组、70~79岁组到80~89岁组呈明显随增龄降低趋势,依次为(37.6±6.5)%、(34.7±5.0)%、(31.5±6.4)%和(28.3±4.7)%(F=5.010,P=0.005);老年组幽门腺实质比例明显低于中青年组(t=3.181,P=0.003).光学显微镜下显示,老年各亚组(尤其是80~89岁)与中青年组比较,胃窦固有膜内幽门腺腺体排列较稀疏,间质结缔组织增生明显,幽门腺细胞核增大、深染、细胞边界模糊.电子显微镜下显示,各组幽门腺细胞及与分泌黏液相关的细胞器形态分布无明显差异;定量测定黏原颗粒面积分数显示呈明显随增龄降低趋势(F=19.831,P=0.000),老年组(25.68±4.62)%明显低于中青年组(34.66±4.34)%(t=-6.456,P=0.000). 结论 随增龄,人体胃窦固有膜厚度有降低趋势,幽门腺腺体密度逐渐降低,腺体间结缔组织增生逐渐增加,幽门腺细胞内与分泌黏液有关的黏原颗粒密度逐渐降低,提示老年人胃窦黏膜及其功能腺体在形态学上存在退化现象.陈敏敏 郑松柏 肖立 俞彰 Dewan Sheilesh Kumar 2014中华老年医学杂志2014,33,8:7
4Curcumin reverses T cell-mediated adaptive immune dysfunctions in tumor-bearing hosts显示文摘Immune dysfunction is well documented during tumor progression and likely contributes to tumor immune evasion.CD81 cytotoxic T lymphocytes(CTLs)are involved in antigen-specific tumor destruction and CD41 T cells are essential for helping this CD81 T cell-dependent tumor eradication.Tumors often target and inhibit T-cell function to escape from immune surveillance.This dysfunction includes loss of effector and memory T cells,bias towards type 2 cytokines and expansion of T regulatory(Treg)cells.Curcumin has previously been shown to have antitumor activity and some research has addressed the immunoprotective potential of this plant-derived polyphenol in tumor-bearing hosts.Here we examined the role of curcumin in the prevention of tumor-induced dysfunction of T cell-based immune responses.We observed severe loss of both effector and memory T-cell populations,downregulation of type 1 and upregulation of type 2 immune responses and decreased proliferation of effector T cells in the presence of tumors.Curcumin,in turn,prevented this loss of T cells,expanded central memory T cell(TCM)/effector memory T cell(TEM)populations,reversed the type 2 immune bias and attenuated the tumor-induced inhibition of T-cell proliferation in tumor-bearing hosts.Further investigation revealed that tumor burden upregulated Treg cell populations and stimulated the production of the immunosuppressive cytokines transforming growth factor(TGF)-b and IL-10 in these cells.Curcumin,however,inhibited the suppressive activity of Treg cells by downregulating the production of TGF-b and IL-10 in these cells.More importantly,curcumin treatment enhanced the ability of effector T cells to kill cancer cells.Overall,our observations suggest that the unique properties of curcumin may be exploited for successful attenuation of tumor-induced suppression of cell-mediated immune responses.Sankar Bhattacharyya Dewan Md Sakib Hossain Suchismita Mohanty Gouri Sankar Sen Sreya Chattopadhyay Shuvomoy Banerjee Juni Chakraborty Kaushik Das Diptendra Sarkar Tanya Das Gaurisankar Sa 2010Cellular & Molecular Immunology2010,7,4:7
5A Review of Heat Transfer Enhancement through Flow Disruption in a Microchannel显示文摘In this paper a comprehensive review of heat transfer enhancement through microchannels has been presented. Over the past few years due to multifunction, shrinking package size and high power dissipation, the heat flux per unit area has increased significantly. Microchannels, with their large heat transfer surface to volume ratio and their small volumes, have shown a good thermal performance. Microchannels have been proven to be a high performance cooling technique which is able to dissipate heat flux effectively from localized hot spots over small surface area. A good amount of heat transfer augmentation techniques have been reported on flow disruption through microchannel. These techniques promote free stream separation at the leading edge which results in boundary layer development and enhanced mixing leading to increased heat transfer. Flow disruption can be achieved through passive surface modifications, such as, shape of channel, dimple surfaces, ribs, cavities, groove structures, porous medium, etc. Combined effects of these geometrical configurations in heat transfer augmentation are also reported in the literature. In this paper recent developments in experimental and numerical simulations of single-phase liquid cooled microchannel have been discussed to analyze the pressure drop, friction and heat transfer characteristics due to different flow conditions, roughness structure and passive surface modifications. It has been observed that the flow disruption techniques are effective for heat transfer enhancement with lower penalties of increased pressure drop. 6新复合杂合突变致婴儿型低磷酸酯酶症1例及其家系分析显示文摘该文对1例婴儿型低磷酸酯酶症(HPP)患儿及其家系进行临床特点分析及碱性磷酸酯酶基因(ALPL)检测。先证者,男,5个月,多发骨骼畸形:胸骨凹陷、双侧桡骨弯曲畸形、双膝外翻畸形,伴喂养困难、体重下降、发育迟滞、反复肺炎并呼衰,血碱性磷酸酶显著降低。患儿父母、姐姐、叔父、姨母(其他家系成员未能配合)中除父母及姨母的碱性磷酸酶略低,姨母可见脊柱侧弯畸形,余均无临床表型及实验室异常。患者ALPL基因检测到来源于母亲的c.228delG突变及来源于父亲的c.407G>A复合杂合突变,其姨母携带c.228delG突变。c.407G>A突变为已报道的HPP致病突变,c.228delG为新的致病性突变。低磷酸酯酶症是由ALPL基因突变所致,ALPL基因检测是有效的诊断方法。该研究拓展了ALPL基因突变谱,为HPP的基因诊断提供了理论依据。李登峰 蓝丹 钟京梓 Roma Kajal Dewan 谢彦舒 杨莹 2017中国当代儿科杂志2017,19,5:6
7AN INVESTIGATION OF TURBULENT RECTANGULAR JET DISCHARGED INTO A NARROW CHANNEL WEAK CROSSFLOW显示文摘A computational investigation of the mean flow field of turbulent rectangular jets issuing into a narrow channel crossflow is presented. The length of the jet slot spans more than 55% of the crossflow channel bed, leaving a small clearance between the jet edge and sidewalls. A finite volume code employing the standard k-ε model is used to predict the mean, three-dimensional flow field. The mean flow field is investigated for two velocity ratios (6 and 9). Important flow features, such as the formation of different vortical structures and their characteristics owing to different values of the velocity ratio, are discussed. Some predicted results are compared with the experimental data reported in the literature. 9Location-allocation modeling for emergency evacuation planning with GIS and remote sensing:A case study of Northeast Bangladesh显示文摘This work developed models to identify optimal spatial distribution of emergency evacuation centers(EECs)such as schools,colleges,hospitals,and fire stations to improve flood emergency planning in the Sylhet region of northeastern Bangladesh.The use of location-allocation models(LAMs)for evacuation in regard to flood victims is essential to minimize disaster risk.In the first step,flood susceptibility maps were developed using machine learning models(MLMs),including:Levenberg-Marquardt back propagation(LM-BP)neural network and decision trees(DT)and multi-criteria decision making(MCDM)method.Performance of the MLMs and MCDM techniques were assessed considering the area under the receiver operating characteristic(AUROC)curve.Mathematical approaches in a geographic information system(GIS)for four well-known LAM problems affecting emergency rescue time are proposed:maximal covering location problem(MCLP),the maximize attendance(MA),p-median problem(PMP),and the location set covering problem(LSCP).The results showed that existing EECs were not optimally distributed,and that some areas were not adequately served by EECs(i.e.,not all demand points could be reached within a 60-min travel time).We concluded that the proposed models can be used to improve planning of the distribution of EECs,and that application of the models could contribute to reducing human casualties,property losses,and improve emergency operation.Mahfuzur Rahman Ningsheng Chen MdMonirul Islam Ashraf Dewan Hamid Reza Pourghasemi Rana Muhammad AliWashakh Nirdesh Nepal Shufeng Tian Hamid Faiz Mehtab Alam Naveed Ahmed 2021Geoscience Frontiers2021,12,3:3

12Flooding and its relationship with land cover change, population growth, and road density显示文摘Bangladesh experiences frequent hydro-climatic disasters such as flooding.These disasters are believed to be associated with land use changes and climate variability.However,identifying the factors that lead to flooding is challenging.This study mapped flood susceptibility in the northeast region of Bangladesh using Bayesian regularization back propagation(BRBP)neural network,classification and regression trees(CART),a statistical model(STM)using the evidence belief function(EBF),and their ensemble models(EMs)for three time periods(2000,2014,and 2017).The accuracy of machine learning algorithms(MLAs),STM,and EMs were assessed by considering the area under the curve—receiver operating characteristic(AUC-ROC).Evaluation of the accuracy levels of the aforementioned algorithms revealed that EM4(BRBP-CART-EBF)outperformed(AUC>90%)standalone and other ensemble models for the three time periods analyzed.Furthermore,this study investigated the relationships among land cover change(LCC),population growth(PG),road density(RD),and relative change of flooding(RCF)areas for the period between 2000 and 2017.The results showed that areas with very high susceptibility to flooding increased by 19.72%between 2000 and 2017,while the PG rate increased by 51.68%over the same period.The Pearson correlation coefficient for RCF and RD was calculated to be 0.496.These findings highlight the significant association between floods and causative factors.The study findings could be valuable to policymakers and resource managers as they can lead to improvements in flood management and reduction in flood damage and risks.Mahfuzur Rahman Chen Ningsheng Golam Iftekhar Mahmud Md Monirul Islam Hamid Reza Pourghasemi Hilal Ahmad Jules Maurice Habumugisha Rana Muhammad Ali Washakh Mehtab Alam Enlong Liu Zheng Han Huayong Ni Tian Shufeng Ashraf Dewan 2021Geoscience Frontiers2021,12,6:3

14Wear and mechanical characterization of Mg-Gr self-lubricating composite fabricated by mechanical alloying显示文摘In this research,the wear and mechanical responses of pure magnesium-graphite(Mg-Gr)composite have been investigated aiming to get the optimum composition of reinforcement.The composite materials were fabricated by mechanical alloying.The percentage of graphite reinforcement was chosen as 3,5,7 and 10 wt.%to identify its potential for self-lubricating property under dry sliding conditions.The mechanical properties including hardness,tensile strength and flexural strength of the composites and the base material were tested.The wear tests were conducted by using a pin-on-disc tribometer.The results show that the mechanical properties decrease with increasing graphite content as compared to that of the base material.The wear rate and average coefficient of friction decrease with the addition of graphite and was found to be minimum at 5 wt.%graphite reinforcement.The addition of 5 wt.%graphite in the composite exhibits superior wear properties as compared to that of the matrix material and other compositions of the Mg-Gr composites.Azzat Esam Abdulqader Al-maamari AKMAsif Iqbal Dewan Muhammad Nuruzzaman 2019Journal of Magnesium and Alloys2019,7,2:2
