| 1 | 成人隐匿阴茎治疗的综述显示文摘背景与目的导致隐匿阴茎的原因很多,最常见的是肥胖、包皮环切术后或阴囊水肿。随着肥胖病日益增多,这种病症的发病率也必然增多。儿童先天性隐匿阴茎极为罕见,其结果也是转变成为成人隐匿阴茎。本文的目的旨在综述目前对于此类身心疾患的治疗策略。方法对有关隐匿阴茎治疗的文献进行综述,文献主要来源是1977年至2007年PubMed中刊登的整形外科和泌尿外科文献。结果病态肥胖症和糖尿病是成人隐匿阴茎的危险因素。文献中介绍了多种复位和重建的手术方法,如原位缝合、Z成形、植皮修复等,修复过程中均可能进行脂肪切除。最近的文献更多关注采用断层皮片和负压技术进行治疗,该技术有利于皮片成活,远期美观效果良好。结论隐匿阴茎临床少见,治疗困难,对于整形科和泌尿外科医师具有挑战性。潜在因素如病态肥胖和糖尿病的增加,提示这种疾病的发生率也将不断上升。没有某一特定的技术适用于所有患者,治疗时可以采用几种技术的联合。对于疑难重症患者,可以进行阴茎体断层皮片移植术、阴囊缩小术,并可辅助负压吸脂术或脂肪切除术。个体化治疗方法可以获得更高的修复成功率,并且达到满意的美观效果。 | Ivo A.
