1Incidence of infectious complications is associated with a high mortality in patients with hepatitis B virus-related acute-on-chronic liver failure显示文摘BACKGROUND In China,hepatitis B virus-related acute-on-chronic liver failure (HBV-ACLF) is the most common liver failure characterized by serious clinical syndromes of liver decompensation with a very high mortality.Bacterial and/or fungal infections are the most common complications that are associated with high short-term mortality.Bacterial translocation from the intestine,impaired hepatic clearance,and immune paralysis of circulating immune cells are thought to contribute to infectious complications in liver failure.The control of bacterial and fungal infections is the key to improving HBV-ACLF outcomes.Active prevention,early diagnosis,and timely treatment of bacterial and fungal infections are essential for treating HBV-ACLF.AIM To investigate the frequency and role of bacterial and fungal infections in patients with HBV-ACLF.METHODS Patients with HBV-ACLF hospitalized at Taihe Hospital,Hubei University of Medicine from January 2014 to December 2017 were retrospectively enrolled.Patient-related information was retrieved from the hospital case database,including general information,blood biochemistry,complications,etc.According to the occurrence of secondary infection or not,the patients were divided into an infection group and a non-infection group.The sites,types,and incidences of bacterial and fungal infections and the influence of infections on the prognosis of HBV-ACLF were statistically analyzed.The risk factors for infections were assessed by unconditional logistic regression.RESULTS There were 174 cases of HBV-ACLF that met the enrollment criteria,of which 114 (65.52%) were diagnosed with infectious complications.Infections occurred in the abdominal cavity (87 cases),respiratory tract (51 cases),urinary tract (18 cases),and biliary tract (10 cases).Patients with infectious complications had a significantly higher 28-d mortality (70.18%,80/114) than those without (40.00%,24/60)(70.18% vs 40.00%,P < 0.05).And patients with infectious complications had a much higher incidence of non-infectious complications (54.39%,62/114)(54.39% vs 15.00%,P < 0.05),leading to an extremely high 28-d mortality of 88.71%(55/62)(P < 0.05).The grade of liver failure,period of hospital stay ≥ 30 d,age ≥ 45 years,and percentage of neutrophils > 70% were identified as risk factors for infectious complications.CONCLUSION The high incidence of infectious complications in patients with HBV-ACLF is associated with severity and deterioration of the disease and may contribute to the extremely high mortality of these patients.Chen Wang De-Qiang Ma Sen Luo Chuan-Min Wang De-Ping Ding You-You Tian Kang-Jian Ao Yin-Hua Zhang Yue Chen Zhong-Ji Meng 2019World Journal of Clinical Cases2019,7,16:18

5A novel approach for designing efficient broadband photodetectors expanding from deep ultraviolet to near infrared显示文摘Broadband photodetection(PD)covering the deep ultraviolet to near-infrared(200–1000 nm)range is significant and desirable for various optoelectronic designs.Herein,we employ ultraviolet(UV)luminescent concentrators(LC),iodinebased perovskite quantum dots(PQDs),and organic bulk heterojunction(BHJ)as the UV,visible,and near-infrared(NIR)photosensitive layers,respectively,to construct a broadband heterojunction PD.Firstly,experimental and theoretical results reveal that optoelectronic properties and stability of CsPbI3 PQDs are significantly improved through Er3+doping,owing to the reduced defect density,improved charge mobility,increased formation energy,tolerance factor,etc.The narrow bandgap of CsPbI3:Er3+PQDs serves as a visible photosensitive layer of PD.Secondly,considering the matchable energy bandgap,the BHJ(BTP-4Cl:PBDB-TF)is selected as to NIR absorption layer to fabricate the hybrid structure with CsPbI3:Er3+PQDs.Thirdly,UV LC converts the UV light(200–400 nm)to visible light(400–700 nm),which is further absorbed by CsPbI3:Er3+PQDs.In contrast with other perovskites PDs and commercial Si PDs,our PD presents a relatively wide response range and high detectivity especially in UV and NIR regions(two orders of magnitude increase that of commercial Si PDs).Furthermore,the PD also demonstrates significantly enhanced air-and UV-stability,and the photocurrent of the device maintains 81.5%of the original one after 5000 cycles.This work highlights a new attempt for designing broadband PDs,which has application potential in optoelectronic devices.Nan Ding Yanjie Wu Wen Xu Jiekai Lyu Yue Wang Lu Zi Long Shao Rui Sun Nan Wang Sen Liu Donglei Zhou Xue Bai Ji Zhou Hongwei Song 2022Light(Science & Applications)2022,11,5:5
6A Comparative Study on the Nonspecific Immunity of Juvenile Litopenaeus vannamei ever Inhabiting Freshwater and Seawater显示文摘A study on the nonspecific immunity of Litopenaeus vannamei ever inhabiting freshwater and seawater was carried out at different molt stages by comparing their total hemocyte count(THC) and respiratory burst(RB) and activity of phenol oxidase(PO), nitric oxide synthase(NOS) and lysozyme(LY). Two-way ANOVA showed that salinity and molt stage independently affected THC and RB and the activity of PO, NOS and LY of juvenile L. vannamei significantly(P < 0.05). The THC and RB and the activity of NOS gradually increased from the post-molt stages(A and B) to the pre-molt stages(D0–D3), which were common in shrimps inhabiting freshwater and seawater. The activity of PO peaked at the inter-molt stage(C), and touched the lowest at the post-molt stage in freshwater and pre-molt stage in seawater. The activity of LY was stable over the molt cycle. The RB and the activity of PO, NOS and LY of juvenile L. vannamei were significantly lower in freshwater than in seawater; whereas THC was significantly higher in freshwater than in seawater(P < 0.05). 7Real-time recognition of human lower-limb locomotion based on exponential coordinates of relative rotations显示文摘This paper proposes a data-driven method using a parametric representation of relative rotations to classify human lower-limb locomotion, which is designed for wearable robots. Three Inertial measurement units(IMUs) are mounted on the subject's waist,left knee, and right knee, respectively. Features for classification comprise relative rotations, angular velocities, and waist acceleration. Those relative rotations are represented by the exponential coordinates. The rotation matrices are normalized by Karcher mean and then the Support Vector Machine(SVM) method is used to train the data. Experiments are conducted with a time-window size of less than 40 ms. Three SVM classifiers telling 3, 5 and 6 rough lower-limb locomotion types respectively are trained, and the average accuracies are all over 98%. With the combination of those rough SVM classifiers, an easy SVMbased ensembled-system is proposed to classify 16 fine locomotion types, achieving the average accuracy of 98.22%, and its latency is 18 ms when deployed to an onboard computer.XU Sen DING Ye 2021Science China(Technological Sciences)2021,64,7:3

9A field-effect approach to directly profiling the localized states in monolayer MoS2显示文摘A fundamental understanding of the charge transport mechanism in two-dimensional semiconductors(e.g., MoS2) is crucial for fully exploring their potential in electronic and optoelectronic devices. By using monolayer graphene as the barrier-free contact to MoS2, we show that the field-modulated conductivity can be used to probe the electronic structure of the localized states. A series of regularly distributed plateaus were observed in the gate-dependent transfer curves. Calculations based on the variable-range hopping theory indicate that such plateaus can be attributed to the discrete localized states near mobility edge. 10Effects of Salinity Fluctuation Frequency on the Growth, Molting Rate and Hemolymph 20-Hydroxyecdysone Concentration in Juvenile Chinese shrimp, Fenneropenaeus chinensis显示文摘This experiment was conducted to examine the effects of salinity fluctuation frequency on the growth, molting rate and hemolymph 20-hydroxyecdysone (20-HE) concentration for juvenile Fenneropenaeus chinensis with an initial wet body weight of 1.115 g±0.012g. The salinity of the control treatment (represented by D0) was 28 throughout the experiment, whereas treatments D2, D4,D6 and D8 were subjected to different salinity fluctuation frequencies of 2, 4, 6 and 8 d, respectively. The range of salinity fluctuation was 4. The salinity in treatments D2, D4, D6 and D8 was kept at 28 for 2, 4, 6 and 8 d, respectively; each original salinity was decreased abruptly to salinity 24, which first lasted for another 2 d and was then raised to its initial value 28. This constituted a salinity fluctuation cycle and afterwards the cycle repeated. It was found that: 1) The weight gain of shrimp in treatment D4 was 202.9% and significantly higher than that in treatment D0 (P<0.05). The molting rate in treatment D2 was the lowest, while that in treatment D6 was the highest and significantly higher than those in treatments D0 and D2 (P<0.05). 2) The hemolymph 20-HE concentration of shrimps in all treatments was at low levels and increased first gradually during the inter-molt period and then increased sharply. It reached a peak value during the pre-molt stage, and then decreased abruptly during the post-molt stage when the lowest level occurred. 3) During the post-molt stage, the hemolymph 20-HE concentration in treatment D2 was 7.47 pg μL-1 and significantly higher than that in treatment D0 (P<0.05). There were, however, no significant differences in hemolymph 20-HE concentration among all five treatments during the inter-molt stage (P>0.05). During the pre-molt stage, the hemolymph 20-HE concentration in treatment D6 was significantly higher than that in treatment D0 (P<0.05), whereas the hemolymph 20-HE concentration in treatment D2 was significantly lower than that in other treatments (P<0.05). 13Comparison of the Respiratory Metabolism of Juvenile Litopenaeus vannamei Cultured in Seawater and Freshwater显示文摘Litopenaeus vannamei, a euryhaline species, can be cultured at a wide range of salinities. The emergence of freshwater pond-culture of L. vannamei is an important prelude to the continued development of shrimp culture in China. In this study, we compared the respiratory metabolism of juvenile L. vannamei cultured in freshwater and saltwater by measuring their oxygen consumption rate(OCR), ammonium-type nitrogen excretion rate(AER) and pyruvate kinase(PK) and lactate dehydrogenase(LDH) activities at different molting stages in order to physiecologically characterize juvenile L. vannamei under freshwater conditions. The results showed that OCR was significantly higher in saltwater than in freshwater at all stages of molting cycle. However, variation of OCR among molting stages in saltwater was similar with that in freshwater, and the highest OCR was observed at post-molting stage. At all stages of molting cycle, AER was significantly higher in freshwater than in saltwater, and the highest was observed at post-molting stage. The activity of PK was significantly higher in saltwater than in freshwater. Conversely, the activity of LDH was higher in freshwater than in saltwater in general. Significant variation of PK and LDH activities in molting cycle was observed in saltwater and freshwater. 14Relationships between the Extratropical ENSO Precursor and Leading Modes of Atmospheric Variability in the Southern Hemisphere显示文摘Previous studies suggest that the atmospheric precursor of El Ni ?no–Southern Oscillation(ENSO) in the extratropical Southern Hemisphere(SH) might trigger a quadrapole sea surface temperature anomaly(SSTA) in the South Pacific and subsequently influence the following ENSO. Such a quadrapole SSTA is referred to as the South Pacific quadrapole(SPQ).The present study investigated the relationships between the atmospheric precursor signal of ENSO and leading modes of atmospheric variability in the extratropical SH [including the SH annular mode(SAM), the first Pacific–South America(PSA1) mode, and the second Pacific–South America(PSA2) mode]. The results showed that the atmospheric precursor signal in the extratropical SH basically exhibits a barotropic wavenumber-3 structure over the South Pacific and is significantly correlated with the SAM and the PSA2 mode during austral summer. Nevertheless, only the PSA2 mode was found to be a precursor for the following ENSO. It leads the SPQ-like SSTA by around one month, while the SAM and the PSA1 mode do not show any obvious linkage with either ENSO or the SPQ. 20Surgical treatment for uncinate process carcinoma of the pancreas显示文摘Objectives: To analyze the clinical features of unci-nate process carcinoma of the pancreas and to im-prove the resection rate.Methods: From January 1990 to June 1999, 10 pa-tients with pancreas uncinate process carcinoma re-ceived Whipple’s operation. Portal vein (PV) resec-tion and reanastomosis were performed in 5 patients,and the resected length varied from 2.0 to 4.2 cm.Two patients underwent PV lateral wall partial resec-tion.Results: Among the 7 patients undergoing PV resec-tion, 1 died of hepatic failure 3 days after operation.One patient suffered from postoperative chylous asci-tes. These 6 patients survived 13 to 29 months post-operatively. Among the 3 patients without PV resec-tion, 2 survived 13 months and 14 months respective-ly.
