1Portal vein thrombosis in cirrhosis: Controversies and latest developments显示文摘Portal vein thrombosis(PVT) is encountered in livercirrhosis, particularly in advanced disease. It has been a feared complication of cirrhosis, attributed to significant worsening of liver disease, poorer clinical outcomes and potential inoperability at liver transplantation; also catastrophic events such as acute intestinal ischaemia. Optimal management of PVT has not yet been addressed in any consensus publication.We review current literature on PVT in cirrhosis; its prevalence, pathophysiology, diagnosis, impact on the natural history of cirrhosis and liver transplantation,and management. Studies were identified by a search strategy using MEDLINE and Google Scholar. The incidence of PVT increases with increasing severity of liver disease: less than 1% in well-compensated cirrhosis, 7.4%-16% in advanced cirrhosis. Prevalence in patients undergoing liver transplantation is 5%-16%.PVT frequently regresses instead of uniform thrombus progression. PVT is not associated with increased risk of mortality. Optimal management has not been addressed in any consensus publication. 2MicroRNAs, development of Barrett’s esophagus, and progression to esophageal adenocarcinoma显示文摘Barrett's esophagus is a premalignant condition caused by gastroesophageal reflux. Once developed, it can progress through varying grades of dysplasia to esoph-ageal adenocarcinoma. Whilst it is well accepted that Barrett's esophagus is caused by gastroesophageal reflux, the molecular mechanisms of its pathogenesis and progression to cancer remain unclear. MicroRNAs (miRNAs) are short segments of RNA that have been shown to control the expression of many human genes. They have been implicated in most cellular processes, and the role of miRNAs in disease development is be-coming increasingly evident. 3In-vehicle VOCs composition of unconditioned,newly produced cars显示文摘The in-vehicle volatile organic compounds(VOCs) concentrations gains the attention of both car producers and users. In the present study, an attempt was made to determine if analysis of air samples collected from an unconditioned car cabin can be used as a quality control measure. The VOCs composition of in-vehicle air was analyzed by means of active sampling on Carbograph 1TD and Tenax TA sorbents, followed by thermal desorption and simultaneous analysis on flame ionization and mass detector(TD-GC/FID-MS). Nine newly produced cars of the same brand and model were chosen for this study. Within these, four of the vehicles were equipped with identical interior materials and five others differed in terms of upholstery and the presence of a sunroof; one car was convertible. The sampling event took place outside of the car assembly plant and the cars tested left the assembly line no later than 24 hr before the sampling took place. More than 250 compounds were present in the samples collected; the identification of more than 160 was confirmed by comparative mass spectra analysis and 80 were confirmed by both comparison with single/multiple compounds standards and mass spectra analysis. In general, aliphatic hydrocarbons represented more than 60% of the total VOCs(TVOC) determined. Depending on the vehicle, the concentration of aromatic hydrocarbons varied from 12% to 27% of total VOCs. The very short period between car production and sampling of the in-vehicle air permits the assumption that the entire TVOC originates from off-gassing of interior materials. 4miR-200 family expression is downregulated upon neoplastic progression of Barrett's esophagus显示文摘AIM: To investigate miR-200 family expression in Barrett's epithelium, gastric and duodenal epithelia, and esophageal adenocarcinoma. METHODS: Real-time reverse transcriptase-polymerase chain reaction was used to measure miR-200, ZEB1 and ZEB2 expression. Ingenuity Pathway Analysis of miR-200 targets was used to predict biological outcomes. RESULTS: Barrett's epithelium expressed lower levels of miR-141 and miR-200c than did gastric and duodenal epithelia (P < 0.001). In silico analysis indicated roles for the miR-200 family in molecular pathways that distinguish Barrett's epithelium from gastric and duodenalepithelia, and which control apoptosis and proliferation. All miR-200 members were downregulated in adenocarcinoma (P < 0.02), and miR-200c expression was also downregulated in non-invasive epithelium adjacent to adenocarcinoma (P < 0.02). The expression of all miR-200 members was lower in Barrett's epithelium derived high-grade dysplastic cell lines than in a cell line derived from benign Barrett's epithelium. We observed signif icant inverse correlations between miR-200 family expression and ZEB1 and ZEB2 expression in Barrett's epithelium and esophageal adenocarcinoma (P < 0.05). 5MicroRNA signatures in chemotherapy resistant esophageal cancer cell lines显示文摘AIM:To investigate expression of microRNA(miRNA)and potential targets in chemotherapy resistant esoph-ageal cancer cell lines.METHODS:An in-vitro model of acquired chemotherapy resistance in esophageal adeno-(EAC)and squamous cell carcinoma(ESCC)cells was used,and microRNA expression profiles for cisplatin or 5-fluorouracil(5-FU)resistant variants vs chemotherapy sensitive controls were compared using microarray and quantitative real-time polymerase chain reaction(PCR).The expression of chemotherapy-relevant genes potentially targeted by the dysregulated microRNAs in the chemotherapy resistant variants was also evaluated.RESULTS:Chemotherapy resistant sublines were found to have specific miRNA signatures,and these miRNA signatures were different for the cisplatin vs 5-FU resistant cells from the same tumor cell line,and also for EAC vs ESCC cells with resistance to the same specific chemotherapy agent.Amongst others,miR-27b-3p,miR-193b-3p,miR-192-5p,miR-378 a-3p,miR-125a-5p and miR-18a-3p were dysregulated,consistent with negative posttranscriptional control of KRAS,TYMS,ABCC3,CBL-B and ERBB2 expression via these miRNAs.CONCLUSION:The current study supports the hypothesis that microRNA expression has an impact on chemotherapy resistance in esophageal cancer.Richard Hummel Corina Sie David I Watson Tingting Wang Alfiya Ansar Michael Z Michael Mark Van der Hoek Joerg Haier Damian J Hussey 2014World Journal of Gastroenterology2014,20,40:8

7Recurrent anal fistulae:Limited surgery supported by stem cells显示文摘AIM:To study the results of stem-cell therapy under a Compassionate-use Program for patients with recurrent anal fistulae.METHODS:Under controlled circumstances,and approved by European and Spanish laws,a Compassionate-use Program allows the use of stem-cell therapy for patients with very complex anal fistulae.Candidates had previously undergone multiple surgical interventions that had failed to resolve the fistulae,and presented symptomatic recurrence.The intervention consisted of limited surgery(with closure of the internal opening),followed by local implant of stem cells in the fistula-tract wall.Autologous expanded adipose-derived stem cells were the main cell type selected for implant.The first evaluation was performed on the 8th postoperative week;outcome was classified as response or partial response.Evaluation one year after the intervention confirmed if complete healing of the fistula was achieved.RESULTS:Ten patients(8 male)with highly recurrent and complex fistulae were treated(mean age:49years,range:28-76 years).Seven cases were nonCrohn’s fistulae,and three were Crohn’s-associated fistulae.Previous surgical attempts ranged from 3to 12.Two patients presented with preoperative incontinence(Wexner scores of 12 and 13 points).After the intervention,six patients showed clinical response on the 8th postoperative week,with a complete cessation of suppuration from the fistula.Three patients presented a partial response,with an evident decrease in suppuration.A year later,six patients(60%)remained healed,with complete reepithelization of the external opening.Postoperative Wexner Scores were 0 in six cases.The two patients with previous incontinence improved their scores from12 to 8 points and from 13 to 5 points.No adverse reactions or complications related to stem-cell therapy were reported during the study period.CONCLUSION:Stem cells are safe and useful for treating anal fistulae.Healing can be achieved in severe cases,sparing fecal incontinence risk,and improving previous scoring.Damian Garcia-Olmo Hector Guadalajara Ines Rubio-Perez Maria Dolores Herreros Paloma de-la-Quintana Mariano Garcia-Arranz 2015World Journal of Gastroenterology2015,21,11:7
8Estrogen,male dominance and esophageal adenocarcinoma:Is there a link?显示文摘Esophageal adenocarcinoma is a cancer with poor prognosis,and its incidence has risen sharply over recent decades.Obesity is a major risk factor for developing this cancer and there is a clear male gender bias in the incidence that cannot be fully explained by known risk factors.It is possible that a difference in the expression of estrogen,or its signaling axes,may contribute to this gender bias.We undertook a comprehensive literature search and analyzed the available data regarding estrogen and estrogen receptor expression,and the possible sex-specific links with esophageal adenocarcinoma development.Potentially relevant associations between visceral vs subcutaneous fat deposition and estrogen expression,and the effect of crosstalk between estrogen and leptin signaling were identified.We also found limited studies suggesting a role for estrogen receptor β expression in esophageal adenocarcinoma development.The current literature supports speculation on an etiological role for estrogen in the male gender bias in esophageal adenocarcinoma,but further studies are required.Huiqi Yang Olga A Sukocheva Damian J Hussey David I Watson 2012World Journal of Gastroenterology2012,18,5:7
9Benzene, toluene and xylenes levels in new and used vehicles of the same model显示文摘this work was to determine the level of benzene, toluene, o-xylene and m, p-xylene(BTX) in air samples collected from the cabins of new and used vehicles of the same model. Ten new vehicles were examined in order to check interior emission from materials used to equip the passenger compartment. In order to compare and define the impact of exhaust gases, air samples were also collected from two used cars, at diferent mileages(up to 20,000 km). All vehicles tested were of the same type. Samples were collected onto Carbograph 1TD sorbent, thermally desorbed and examined with the use of gas chromatography with flame ionisation and mass spectrometry detectors. All results obtained were referred to Polish and German requirements for indoor air quality(both in public buildings and in workspace environments). Average benzene, toluene, o-xylene and m, p-xylene concentrations in new cars were determined at the level of 11.8 μg/m3, 82.7 μg/m3, 21.2 μg/m3and 89.5 μg/m3, respectively. In the used cars, BTX concentration increased with increasing vehicle mileage. 11A signal-substrate match in the substrate-borne component of a multimodal courtship display显示文摘The environment can impose strong limitations on the efficacy of signal transmission. In particular, for vibratorycommunication, the signaling environment is often extremely heterogeneous at small scales. Nevertheless, natural selection isexpected to select for signals well-suited for effective transmission. Here, we test for substrate-dependent signal efficacy in thewolf spider Schizocosa stridulans Stratton 1991. We first explore the transmission characteristics of this important signaling modalityby playing recorded substrate-borne signals through three different substrates (leaf litter, pine litter, and red clay) andmeasuring the propagated signal. We found that the substrate-borne signal of S. stridulans attenuates the least on leaf litter, thesubstrate upon which the species is naturally found. Next, by assessing mating success with artificially muted and non-mutedmales across different signaling substrates (leaf litter, pine litter, and sand), we explored the relationship between substrate-bornesignaling and signaling substrate for mating success. We found that muted males were unsuccessful in obtaining copulations regardlessof substrate, while mating success was dependent on the signaling substrate for non-muted males. For non-muted males,more males copulated on leaf litter than any other substrate. 15HASTY modulates miRNA biogenesis by linking pri-miRNA transcription and processing显示文摘Post-transcriptional gene silencing mediated by microRNAs(miRNAs)modulates numerous developmental and stress response pathways.For the last two decades,HASTY(HST),the ortholog of human EXPORTIN 5,was considered to be a candidate protein that exports plant miRNAs from the nucleus to the cytoplasm.Here,we report that HST functions in the miRNA pathway independent of its cargo-exporting activity in Arabidopsis.We found that Arabidopsis mutants with impaired HST shuttling exhibit normal subcellular distribution of miRNAs.Interestingly,protein-protein interaction and microscopy assays showed that HST directly interacts with the microprocessor core component DCL1 through its N-terminal domain.Moreover,mass spectrometry analysis revealed that HST also interacts independently of its N-terminal domain with the mediator complex subunit MED37.Further experiments revealed that HST could act as a scaffold to facilitate the recruitment of DCL1 to genomic MIRNA loci by stabilizing the DCL1-MED37 complex,which in turn promotes the transcription and proper processing of primary miRNA transcripts(primiRNAs).Taken together,these results suggest that HST is likely associated with the formation of the miRNA biogenesis complex at MIRNA genes,promoting the transcription and processing of primiRNAs rather than the direct export of processed miRNAs from the nucleus.Damian A.Cambiagno Axel J.Giudicatti Agustin L.Arce Delfina Gagliardi Lei Li Wei Yuan Derek S.Lundberg Detlef Weigel Pablo A.Manavella 2021Molecular Plant2021,14,3:5

17Seasonal variation for the antidiabetic activity of Loranthus micranthus methanol extract显示文摘Objective:To determine the season in which the Eastern Nigeria mistletoe,Loranthus micranthus,parasitic on Persea americana possesses optimum antidiabetic activity and to determine the seasonal variation in the constituents.Methods:The antidiabetic activities of the aqueous methanol extracts of the leaves of Eastern Nigeria mistletoe,Loranthus micranthus, harvested in two seasons of the year,the onset of rainy season(April) and the peak of rainy season(July) were compared.The tests were carried out on six(6) groups(A-F) of alloxaninduced diabetic rats.Groups A and B received 200 mg/kg and 400 mg/kg of the April sample extracts respectively while groups C and D received same doses of the July sample extracts. Group E and F which were the positive and negative controls received 10 mg/kg of glibenclamide and 2 ml/kg of 3%tween 20 respectively.The blood glucose levels of the animals were monitored hourly with a glucometer for six hours.The phytochemical analysis of the plant extracts were also carried out by standard procedures.Results:The results showed that group A and B exhibited significant(P<0.05) percentage reduction in the fasting blood sugar(FBS) level of the animals(38.9%and 39.2%respectively) with maximum reduction observed at the 5th and 6th hour respectively compared to glibenclamide(71.3%).Group C showed no significant (P>0.05) FBS reduction(15.9%) while group D exhibited highly significant(P<0.01) reduction (47.5%) with the maximum reduction occurring after 6 hours.The phytochemical analysis of the crude methanol extracts revealed the presence of carbohydrates,glycosides,saponins, tannins,flavonoids,steroids,terpenoids,acidic compounds,resins and oils.These were present in different proportions in both seasons.Conclusions:This study shows that there is a seasonal, dose-dependent variation in the chemical composition viz-a-viz the antidiabetic activity of the plant under study.This activity is highest at the peak of the rainy season.Patience Ogoamaka Osadebe Edwin Ogechukwu Omeje Philip Felix Uzor Ernest Kenechukwu David Damian Chiedozie Obiorah 2010Asian Pacific Journal of Tropical Medicine2010,3,3:4
18Random checkerboard based homogenization for estimating effective thermal conductivity of fully saturated soils显示文摘This paper proposes homogenization scheme for estimating the effective thermal conductivity of fully saturated soils. This approach is based on the random checkerboard-like microstructure. Two modeling scales and two modeling approaches are distinguished and used, i.e. microscale and mesoscale and 1-step and 2-step homogenizations, respectively. The 2-step homogenization involves sequential averaging procedure, i.e. first, at microscale, a mineralogical composition of soil skeleton is considered and averaging process results in estimation of the skeleton effective thermal conductivity, and then, at mesoscale, a random spatial packing of solid skeleton and pores via random checkerboard microstructure is modeled and leads to evaluation of the soil overall thermal conductivity. The 1-step homogenization starts directly at the mesoscale and homogenization procedure yields evaluation of the overall soil thermal conductivity. At the mesoscale, the distinct nature of soil skeleton, as composed of soil separates,is considered and random variability of soil is modeled via enriched random checkerboard-like structure.Both approaches, i.e. 1-step and 2-step homogenizations, interrelate mineralogical composition with the soil texture characterized by the volume fractions of soil separates, i.e. sand, silt and clay. The probability density functions(PDFs) of thermal conductivity are assumed for each of the separates. The soil texture PDF of thermal conductivity is derived taking into consideration the aforementioned functions. Whenever the random checkerboard-like structure is used in averaging process, the Monte Carlo procedure is applied for estimation of homogenized thermal conductivity. 19Possible role of human cytomegalovirus in pouchitis after proctocolectomy with ileal pouch-anal anastomosis in patients with ulcerative colitis显示文摘AIM: To detect the presence of human cytomegalovirus (HCMV) proteins and genes on the ileal pouch of patients with ulcerative colitis who have undergone proctocolectomy with ileal pouch-anal anastomosis (iPAA). METHODS: immunohistochemistry, polymerase chain reaction (PCR) and PCR sequencing methods were utilized to test the presence of HCMV in pouch specimens taken from 34 patients in 86 endoscopies. RESULTS: HCMV genes and proteins were detected in samples from 12 (35.2%) patients. The rate of detection was significant in the endoscopies from patients diagnosed with pouchitis (5 of 12, 41.6%), according to the Japanese classification of pouchitis, in comparison to patients with normal pouch (7 of 62, 11.2%; P = 0.021). in all patients with pouchitis in which the HCMV was detected, it was the first episode of pouchitis. The virus was not detected in previous biopsies taken in normal endoscopies of these patients. During the follow- up, HCMV was detected in one patient with recurrent pouchitis and in 3 patients whose pouchitis episodes improved but whose positive endoscopic findings persisted.
