| 1 | Human amniotic membrane transplantation: Different modalities of its use in ophthalmology显示文摘The amniotic membrane(AM) is the inner layer of the fetal membranes and consist of 3 different layers: the epithelium, basement membrane and stroma which further consists of three contiguous but distinct layers: the inner compact layer, middle fibroblast layer and the outermost spongy layer. The AM has been shown to have anti-inflammatory, anti-fibrotic, anti-angiogenic as well as anti-microbial properties. Also because of its transparent structure, lack of immunogenicity and the ability to provide an excellent substrate for growth, migration and adhesion of epithelial corneal and conjunctival cells, it is being used increasingly for ocular surface reconstruction in a variety of ocular pathologies including corneal disorders associated with limbal stem cell deficiency, surgeries for conjunctival reconstruction, as a carrier for ex vivo expansion of limbal epithelial cells, glaucoma surgeries and sceral melts and perforations. However indiscriminate use of human AM needs to be discouraged as complications though infrequent can occur. These include risk of transmission of bacterial, viral or fungal infections to the recipient if the donors are not adequately screened for communicable diseases, if the membrane is not processed under sterile condi-tions or if storage is improper. Optimal outcomes can be achieved only with meticulous case selection. | 2 | The roles of microglia in viral encephalitis:from sensome to therapeutic targeting显示文摘Viral encephalitis is a devastating disease with high mortality,and survivors often suffer from severe neurological complications.Microglia are innate immune cells of the central nervous system(CNS)parenchyma whose turnover is reliant on local proliferation.Microglia express a diverse range of proteins,which allows them to continuously sense the environment and quickly react to changes.Under inflammatory conditions such as CNS viral infection,microglia promote innate and adaptive immune responses to protect the host.However,during viral infection,a dysregulated microglia-T-cell interplay may result in altered phagocytosis of neuronal synapses by microglia that causes neurocognitive impairment.In this review,we summarize the current knowledge on the role of microglia in viral encephalitis,propose questions to be answered in the future and suggest possible therapeutic targets. | Chintan Chhatbar Marco Prinz | 2021 | Cellular & Molecular Immunology2021,18,2: | 9 |
| 3 | 高效液相色谱-荧光检测法测定人血浆中的匹伐他汀(英文)显示文摘Pitavastatin belongs to the class of coenzyme A reductase inhibitors.Very few methods of assaying pitavastatin from human plasma are available in literature.An analytical method is presented for the determination of the drug from human plasma making use of the fluorescent property of the drug.The drug is extracted from plasma using ethyl acetate under neutral condition and then analyzed by reversed-phase high performance liquid chromatography(HPLC) with fluorescence detection(λEx 245 nm;λEm 420 nm).Analysis of pitavastatin was carried out on a C18 HPLC column using a gradient flow of mobile phase(0.01 mol/L monobasic potassium phosphate(pH 3.20)-acetonitrile,63:37,v/v).Fluorescein isothiocyanate was used as internal standard.The dynamic range of assay was 3 to 50 ng/mL.The intraday precision was less than 10% and accuracy ranged from 95.2% to 112.6%.The same for interday check was less than 12% and 92.8% to 105.1%,respectively.The drug was found to be stable under the assay conditions.The developed method is simple,precise,accurate,and stable.This indicates that it can be applied to routine analysis of this drug in human subjects where there are large numbers of samples without the need of specialized instruments like column switching. | OJHA Ashwini GUTTIKAR Swati VAYEDA Chintan PADH Harish | 2007 | 色谱2007,25,5: | 8 |
| 4 | An attending physician float shift for the improvement of physician productivity in a crowded emergency department显示文摘BACKGROUND:Patients backlogged in the emergency department(ED) waiting for an inpatient bed(boarders) continue to require the attention of ED physicians,exacerbating crowding in the ED.To address this problem,we added a 'float shift' to our winter schedule solely to care for boarders.We sought to quantify the effect of this float shift,hypothesizing greater physician productivity.METHODS:We performed a retrospective observational study in our community hospital ED,measuring the number of new patients seen in each 10-hour shift in the presence or absence of a float shift physician.We calculated the number of new patients seen per shift for each of the 7 daily shifts,during February(float shift scheduled) and May(float shift unscheduled) of 2008.We then compared the mean number of patients seen per shift in February with May.RESULTS:Total monthly patient volume was 6 656 for February and 6 775 for May,with the mean daily census being 230 and 219 patients,respectively.The number of new patients seen during each shift was greater in February than in May,with a mean increase of 1.1 patients per shift(with the float shift).Surveying participants about intervention effectiveness showed 92%of residents,but only 65%of attending physicians,in favor of maintaining the float shift.CONCLUSION:The presence of a 'float shift' physician caring only for boarding patients allows other physicians to maintain and even increase their productivity in our ED,despite the presence of longer throughput times and increased time on diversion. | Muhammad Umer Nasim Chintan Mistry Robert Harwood Erik Kulstad Laura Tommaso | 2013 | World Journal of Emergency Medicine2013,4,1: | 6 |
| 5 | Features of primary pancreatic lymphoma:A bi-institutional review with an emphasis on typical and atypical imaging features显示文摘BACKGROUND Primary pancreatic lymphoma(PPL)is a rare neoplasm.Being able to distinguish it from other pancreatic malignancies such as pancreatic ductal adenocarcinoma(PDAC)is important for appropriate management.Unlike PDAC,PPL is highly sensitive to chemotherapy and usually does not require surgery.Therefore,being able to identify PPL preoperatively will not only direct physicians towards the correct avenue of treatment,it will also avoid unnecessary surgical intervention.AIM To evaluate the typical and atypical multi-phasic computed tomography(CT)imaging features of PPL.METHODS A retrospective review was conducted of the clinical,radiological,and pathological records of all subjects with pathologically proven PPL who presented to our institutions between January 2000 and December 2020.Institutional review board approval was obtained for this investigation.The collected data were analyzed for subject demographics,clinical presentation,laboratory values,CT imaging features,and the treatment received.Presence of all CT imaging findings including size,site,morphology and imaging characteristics of PPL such as the presence or absence of nodal,vascular and ductal involvement in these subjects were recorded.Only those subjects who had a pre-treatment multiphasic CT of the abdomen were included in the study.RESULTS Twenty-nine cases of PPL were diagnosed between January 2000 and December 2020(mean age 66 years;13 males/16 females).All twenty-nine subjects were symptomatic but only 4 of the 29 subjects(14%)had B symptoms.Obstructive jaundice occurred in 24%of subjects.Elevated lactate dehydrogenase was seen in 81%of cases,whereas elevated cancer antigen 19-9 levels were present in only 10%of cases for which levels were recorded.The vast majority(90%)of tumors involved the pancreatic head and uncinate process.Mean tumor size was 7.8 cm(range,4.0-13.8 cm).PPL presented homogenous hypoenhancement on CT in 72%of cases.Small volume peripancreatic lymphadenopathy was seen in 28%of subjects.Tumors demonstrated encasement of superior mesenteric vessels in 69%of cases but vascular stenosis or occlusion only manifested in 5 out of the twentynine individuals(17%).Mild pancreatic duct dilatation was also infrequent and seen in only 17%of cases,whereas common bile duct(CBD)dilation was seen in 41%of subjects.Necrosis occurred in 10%of cases.Size did not impact the prevalence of pancreatic and CBD dilation,necrosis,or mesenteric root infiltration(P=0.525,P=0.294,P=0.543,and P=0.097,respectively).Pancreatic atrophy was not present in any of the subjects.CONCLUSION PPL is an uncommon diagnosis best made preoperatively to avoid unnecessary surgery and ensure adequate treatment.In addition to the typical CT findings of PPL,such as homogeneous hypoenhancement,absence of vascular stenosis and occlusion despite encasement,and peripancreatic lymphadenopathy,this study highlighted many less typical findings,including small volume necrosis and pancreatic and bile duct dilation. | Nicole Segaran Kumaresan Sandrasegaran Catherine Devine Mindy X Wang Chintan Shah Dhakshinamoorthy Ganeshan | 2021 | World Journal of Clinical Oncology2021,12,9: | 2 |

| 11 | Web Resources for Metagenomics Studies显示文摘The development of next-generation sequencing(NGS) platforms spawned an enormous volume of data. This explosion in data has unearthed new scalability challenges for existing bioinformatics tools. The analysis of metagenomic sequences using bioinformatics pipelines is complicated by the substantial complexity of these data. In this article, we review several commonly-used online tools for metagenomics data analysis with respect to their quality and detail of analysis using simulated metagenomics data. There are at least a dozen such software tools presently available in the public domain. Among them, MGRAST, IMG/M, and METAVIR are the most well-known tools according to the number of citations by peer-reviewed scientific media up to mid-2015. Here, we describe 12 online tools with respect to their web link, annotation pipelines, clustering methods, online user support, and availability of data storage. We have also done the rating for each tool to screen more potential and preferential tools and evaluated five best tools using synthetic metagenome.
