1Evolving spectrum of diabetic nephropathy显示文摘Diabetes remains an important health issue as more patients with chronic and uncontrolled diabetes develop diabetic nephropathy(DN), which classically presents with proteinuria followed by a progressive decrease in renal function.However, an increasing proportion of DN patients have a decline in kidney function and vascular complications without proteinuria, known as nonproteinuric DN(NP-DN). Despite the increased incidence of NP-DN, few clinical or experimental studies have thoroughly investigated the pathophysiological mechanisms and targeted treatment for this form of DN. In this review, we will examine the differences between conventional DN and NP-DN and consider potential pathophysiological mechanisms, diagnostic markers, and treatment for both DN and NP-DN. 2Transhiatal versus transthoracic esophagectomy for esophageal cancer显示文摘Esophageal cancer continues to represent a formidable challenge for both patients and clinicians. Relative 5-year survival rates for patients have improved over the past three decades, probably linked to a combination of improved surgical outcomes, progress in systemic chemotherapy and radiotherapy, and the increasing acceptance of multimodality treatment. Surgical treatment remains a fundamental component of the treatment of localized esophageal adenocarcinoma. Multiple approaches have been described for esophagectomy, which can be thematically grouped under two major categories: either transthoracic or transhiatal. The main controversy rests on whether a more extended resection through thoracotomy provides superior oncological outcomes as opposed to resection with relatively limited morbidity and mortality through a transhiatal approach. After numerous trials have addressed these issues, neither approach has consistently proven to be superior to the other one, and both can provide excellent short-term results in the hands of experienced surgeons. Moreover, the available literature suggests that experience of the surgeonand hospital in the surgical management of esophageal cancer is an important factor for operative morbidity and mortality rates, which could supersede the type of approach selected. 3Effects of loadinge-unloading and wettinge-drying cycles on geomechanical behaviors of mudrocks in the Colombian Andes显示文摘The mudrocks in the Colombian Andes,particularly those exhibiting low cementation(bonding),are susceptible to degradation when the environmental conditions change,which are challenging issues for engineering works.In this paper,the changes in physico-mechanical properties of mudrocks were monitored in laboratory,and some influential factors on the mechanical competence of geomaterials were studied.The geotechnical characteristics and experimental designs were developed from physical,chemical,mechanical and compositional points of view.In the tests,the techniques such as vapor equilibrium technique(VET)were employed to apply wettingedrying cycles and to control relative humidity(suctioncontrolled)and loadingeunloading cycles through ultrasonic wave velocities technique.The results show that the main failure mechanisms for the laminated mudrocks start on the microscopic scale by fissures coalescence,exhibiting physico-chemical degradation as well;the global geomechanical behavior presents a state between a ductile,like rock,and a fragile,like soil.The obtained results can provide engineering values according to monitoring laboratory set,when compared with in situ conditions.Mario Camilo Torres-Suarez Adolfo Alarcon-Guzman Rafael Berdugo-De Moya 2014Journal of Rock Mechanics and Geotechnical Engineering2014,6,3:10
4NLRP3 inflammasome blockade reduces adipose tissue inflammation and extracellular matrix remodeling显示文摘The NLRP3-IL-1β pathway plays an important role in adipose tissue(AT)-induced inflammation and the development of obesityassociated comorbidities.We aimed to determine the impact of NLRP3 on obesity and its associated metabolic alterations as well as its role in adipocyte inflammation and extracellular matrix(ECM)remodeling.Samples obtained from 98 subjects were used in a case-control study.The expression of different components of the inflammasome as well as their main effectors and inflammation-and ECM remodeling-related genes were analyzed.The impact of blocking NLRP3 using siRNA in lipopolysaccharide(LPS)-mediated inflammation and ECM remodeling signaling pathways was evaluated.We demonstrated that obesity(P<0.01),obesity-associated T2D(P<0.01)and NAFLD(P<0.05)increased the expression of different com ponents of the inflammasome as well as the expression and release of IL-1β and IL-18 in AT.We also found that obese patients with T2D exhibited increased(P<0.05)hepatic gene expression levels of NLRP3,IL1B and IL18.We showed that NLRP3,but not NLRP1,is regulated by inflammation and hypoxia in visceral adipocytes.We revealed that the inhibition of NLRP3 in human visceral adipocytes significantly blocked(P<0.01)LPS-induced inflammation by downregulating the mRNA levels of CCL2,IL1B,IL6,IL8,S100A8,S100A9,TLR4 and TNF as well as inhibiting(P<0.01)the secretion of IL1-β into the culture medium.Furthermore,blocking NLRP3 attenuated(P<0.01)the LPS-induced expression of important molecules involved in AT fibrosis(COL1A1,COL4A3,COL6A3 and MMP2).These novel findings provide evidence that blocking the expression of NLRP3 reduces AT inflammation with significant fibrosis attenuation.Xabier Unamuno Javier Gomez-Ambrosi Beatriz Ramirez Amaia Rodriguez Sara Becerril Victor Valenti Rafael Moncada Camilo Silva Javier Salvador Gema Fruhbeck Victoria Catalan 2021Cellular & Molecular Immunology2021,18,4:9
5地理信息系统技术在伤寒发热监测中应用显示文摘目的运用地理信息系统(GIS)技术,了解各居民点发热流行率和评价以医疗机构为哨点的伤寒发热监测系统的敏感性。方法收集医学人口普查数据、疾病监测系统信息数据、现场采集居民居住点全球卫星定位数据等资料,建立GIS系统和以人口为基数的发热监测系统;对所有发热就诊病人进行身份和居住地址确认,并制作各居民点年发热就诊率GIS分布图。结果共采集3776个居民居住的全球定位系统(GPS)定位点,人群发热就诊率为1.11‰,发热就诊人次占总门诊人次的16.3%,监测区内的发热就诊病人占所有发热就诊病人的83.5%。在3种类型的医疗机构中,门诊量以个体门诊类最多,但其发热就诊人次占总门诊人次的比率最低(12.6%)。监测区内发热1d或以上的病人地址和身份(ADDRESS-ID)确认率达到95%。根据居民点年发热就诊率GIS分布图,发热就诊率最高的居民点达到17.81‰,最低为0;发热就诊率高的居民点多集中在城市。结论利用GIS技术可直观准确的了解疾病流行率的分布,可迅速直观的寻找出监测系统中的薄弱环节和推断出可能存在的问题。杨进 董柏青 张杰 廖和壮 梁大斌 吴兴华 司国爱 杨宏徽 Jin Kyung Park R Leon Ochiai Camilo J Acosta Moharmmad Ali John D Clemens 2007中国公共卫生2007,23,9:8
65~60岁人群伤寒Vi多糖疫苗和A群脑膜炎球菌多糖疫苗大规模接种结果分析显示文摘为考核国产伤寒Vi多糖疫苗在现场试验的效果 ,以A群脑膜炎球菌多糖疫苗为对照 ,对广西壮族自治区河池市城市和农村的 5~ 6 0岁居民进行了一次大规模接种。结果显示 :应接种对象 1180 71人 ,实际接种 92 4 76人 ,接种率为 78 32 % ,其中伤寒Vi多糖疫苗为 76 87% ,A群脑膜炎球菌多糖疫苗为 79 6 9% ;接种率最高的组群接种率为 92 4 8% ,最低为 6 5 2 0 %。按城乡划分 ,城区的接种率为 77 2 9% ,农村的接种率为 80 5 8% ,组群接种率的高低分布无明显的地理性差异。 5~ 9岁接种率最高为 89 6 5 % ,2 0~ 2 9岁接种率最低为 6 9 0 7% ;男女接种率分别为79 0 7%和 82 11%。有 6 6人被观察到或报告出现不良反应 ,不良反应发生率为 71 37/ 10万。表明两种疫苗的安全性好。在大规模疫苗接种中 ,充分发挥当地干部的宣传组织作用 ,并把接种的有关信息提前通知当地居民 ,对于提高接种率十分重要。补漏接种能使接种率低的组群得到显著的提高。在大规模疫苗接种中必须严格遵守安全注射操作程序 ,避免发生不安全注射事故。两种疫苗的平均接种率、年龄别接种率、性别接种率、组群接种率的地理分布均达到了较好均衡性 ,是可比的 。杨进 司国爱 曾竣 张杰 梁贵臣 梁大斌 廖和壮 李翠云 黄焕新 董柏青 杨宏徽 Rion Leon Ochiai Carolina Danovaro Ali Mohammad Camilo J Acosta 2004中国计划免疫2004,10,4:6
7Cytokine production in patients with cirrhosis and TLR4 polymorphisms显示文摘AIM:To analyze the cytokine production by peripheral blood cells from cirrhotic patients with and without TLR4 D299G and/or T399I polymorphisms.METHODS:The study included nine patients with cirrhosis and TLR4 D299G and/or T399I polymorphisms,and 10 wild-type patients matched for age,sex and degree of liver failure.TLR4 polymorphisms were determined by sequence-based genotyping.Cytokine production by peripheral blood cells was assessed spontaneously and also after lipopolysaccharide(LPS)and lipoteichoic acid(LTA)stimulation.RESULTS:Patients with TLR4 polymorphisms had a higher incidence of previous hepatic encephalopathy than wild-type patients(78%vs 20%,P=0.02).Spontaneous production of interleukin(IL)-6 and IL-10 was lower in patients with TLR4 polymorphisms than in wild-type patients[IL-6:888.7(172.0-2119.3)pg/m L vs 5540.4(1159.2-26053.9)pg/m L,P<0.001;IL-10:28.7(6.5-177.1)pg/m L vs 117.8(6.5-318.1)pg/m L,P=0.02].However,the production of tumor necrosis factor-α,IL-6 and IL-10 after LPS and LTA stimulation was similar in the two groups.CONCLUSION:TLR4 polymorphisms were associated with a distinctive pattern of cytokine production in cirrhotic patients,suggesting that they play a role in the development of cirrhosis complications.Juan Camilo Nieto Elisabet Sánchez Eva Román Silvia Vidal Laia Oliva Carlos Guarner-Argente Maria Poca Xavier Torras Cándido Juárez Carlos Guarner German Soriano 2014World Journal of Gastroenterology2014,20,46:5
8Identification of ta-siRNAs and Cis-nat-siRNAs in Cassava and Their Roles in Response to Cassava Bacterial Blight显示文摘Trans-acting small interfering RNAs (ta-siRNAs) and natural cis-antisense siRNAs (cis-nat-siRNAs) are recently discovered small RNAs (sRNAs) involved in post-transcriptional gene silencing. ta-siRNAs are transcribed from genomic loci and require processing by microRNAs (miRNAs). cis-nat-siRNAs are derived from antisense RNAs produced by the simultaneous transcription of overlapping antisense genes. Their roles in many plant processes, including pathogen response, are mostly unknown. In this work, we employed a bioinformatic approach to identify ta-siRNAs and cis-nat-siRNAs in cassava from two sRNA libraries, one constructed from healthy cassava plants and one from plants inoculated with the bacterium Xanthomonas axonopodis pv.manihotis (Xam). A total of 54 possible ta-siRNA loci were identified in cassava, including a homolog of TAS3, the best studied plant ta-siRNA. Fifteen of these loci were induced, while 39 were repressed in response to Xam infection. In addition, 15 possible cis-natural antisense transcript (cis-NAT) loci producing siRNAs were identified from overlapping antisense regions in the genome,and were found to be differentially expressed upon Xam infection. Roles of sRNAs were predicted by sequence complementarity and our results showed that many sRNAs identified in this work might be directed against various transcription factors. 10以人口为基数的伤寒发热监测结果分析显示文摘目的及时了解、掌握伤寒高发区流行情况。方法在广西河池市的金城江辖区内的金城江镇、东江镇建立以人口为基数的监测系统开展伤寒监测,监测内容包括对发热疑似病人进行病原学、血清学检测;对就诊发热病例进行登记。监测区内的各级医疗机构及大部分的个体诊所均作为监测点。以病原学为确诊依据,肥达试验作为辅助诊断依据。结果监测区内4年的伤寒副伤寒发病数为99例,年均年龄段发病率为20.6/10万。病原学监测9049份标本,分离出伤寒、副伤寒菌99株,其中甲型副伤寒沙门氏菌占70.1%,伤寒沙门氏菌仅占29.9%;伤寒副伤寒症状相似,以发热头痛为主要症状。结论监测区内以散发病例为主要流行趋势,监测的发病率低于以往的常规疫情报告,甲型副伤寒沙门菌是优势菌型;肥达试验在常规监测中的诊断意义较差,确诊伤寒、副伤寒病例的肥达试验诊断符合率低。张杰 董柏青 杨进 王鸣柳 梁贵臣 司国爱 曾竣 谭冬梅 廖和壮 梁大斌 李翠云 吴兴华 RLeon Ochiai Jin-kyung Park Ali Mohammad Camilo J Acosta J.D.Clemens 2006应用预防医学2006,12,6:4

12Iron homeostasis and H63D mutations in alcoholics with and without liver disease显示文摘AIM: To evaluate the prevalence of HFE gene mutation and indices of disturbed iron homeostasis in alcoholics with and without liver disease.METHODS: One hundred and fifty-three heavy drinkers (defined as alcohol consumption > 80 g/d for at least 5 years) were included in the study. These comprised 78 patients with liver disease [liver disease alcoholics (LDA)] in whom the presence of liver disease was confirmed by liver biopsy or clinical evidence of hepatic decompensation, and 75 subjects with no evidence of liver disease, determined by normal liver tests on two occasions [non-liver disease alcoholics (NLDA)], were consecutively enrolled. Serum markers of iron status and HFE C282Y and H63D mutations were determined. HFE genotyping was compared with data obtained in healthy blood donors from the same geographical area.RESULTS: Gender ratio was similar in both study groups. LDA patients were older than NLDA patients (52 ± 10 years vs 48 ± 11 years, P = 0.03). One third and one fi fth of the study population had serum transferrin saturation (TS) greater than 45% and 60% respectively. Serum iron levels were similar in both groups. However, LDA patients had higher TS (51 ± 27 vs 36 ± 13, P < 0.001) and ferritin levels (559 ± 607 ng/mL vs 159 ± 122 ng/mL, P < 0.001), and lower total iron binding capacity (TIBC) (241 ± 88 μg/dL vs 279 ± 40 μg/dL, P = 0.001). The odds ratio for having liver disease with TS greater than 45% was 2.20 (95% confi dence interval (CI): 1.37-3.54). There was no difference in C282Y allelic frequency between the two groups. However, H63D was more frequent in LDA patients (0.25 vs 0.16, P = 0.03). LDA patients had a greater probability of carrying at least one HFE mutation than NLDA patients (49.5% vs 31.6%, P = 0.02). 14Toll-like receptor 4 polymorphisms and bacterial infections in patients with cirrhosis and ascites显示文摘AIM To assess the relationship between the presence of toll-like receptor 4(TLR4) polymorphisms and bacterial infections in cirrhotic patients with ascites. METHODS We prospectively included consecutive patients with cirrhosis and ascites hospitalized during a 6-year period. Patients with human immunodeficiency virus(HIV) infection or any other immunodeficiency, patients with advanced hepatocellular carcinoma(beyond Milan's criteria) or any other condition determining poor short-term prognosis, and patients with a permanent urinary catheter were excluded. The presence of D299 G and/or T399 I TLR4 polymorphisms was determined by sequencing and related to the incidence and probability of bacterial infections, other complications of cirrhosis, hepatocellular carcinoma, and mortality during follow-up. A multivariate analysis to identify predictive variables of mortality in the whole series was performed. RESULTS We included 258 patients: 28(10.8%) were carriers of D299G and/or T399I TLR4 polymorphisms(polymorphism group) and 230 patients were not(wildtype group). The probability of developing any bacterial infection at one-year follow-up was 78% in the polymorphism group and 69% in the wild-type group(P = 0.54). The one-year probability of presenting infections caused by gram-negative bacilli(51% vs 44%, P = 0.68), infections caused by gram-positive cocci(49% vs 40%, P = 0.53), and spontaneous bacterial peritonitis(29% vs 34%, respectively, P = 0.99) did not differ between the two groups. The oneyear probability of transplant-free survival was 55% in the polymorphism group and 66% in the wild-type group(P = 0.15). Multivariate analysis confirmed that age, Child-Pugh score, active alcohol intake, previous hepatic encephalopathy, hepatocellular carcinoma and serum creatinine were associated with a higher risk of death during follow-up. 15口头尸检对河池市5~60岁死亡病例进行死因评估结果分析显示文摘目的分析河池市5~60岁居民死亡主要原因,分析口头尸检(VerbalAutopsy,VA)进行死因调查的效果。方法建立死亡监测系统,对所有报告的监测区内5~60岁死亡个案进行口头尸检调查。结果2002年7月—2003年6月金城江镇、东江镇共死亡624人,全人口总粗死亡率为4.67‰,标化死亡率为5.34‰,男女之比为1.71∶1;5~60岁死亡病例248例,粗死亡率为2.10‰;导致5~60岁居民主要死亡原因为意外死亡、恶性肿瘤、心脑血管疾病、消化系统疾病等,其中意外死亡占死亡构成比的1/3;通过VA进行死因调查,使3.00%的错误死因定义得到更正、5.00%的不明死因得到诊断;无因伤寒导致的死亡。结论交通事故成为当地5~60岁居民死亡的主要因素;随着社会经济发展,伤寒等肠道传染病已不是导致死亡的重要因素;使用VA对于死因的判定比一般的监测系统报告更为准确。杨进 韦士良 刘莉莉 黄兰森 温贤莲 张杰 曾竣 董柏青 吴兴华 RLeon Ochiai Ali Mohammad Camilo J Acosta 2004中国初级卫生保健2004,18,6:3

18Impact of dietary L-arginine supply during early gestation on myofiber development in newborn pigs exposed to intra-uterine crowding显示文摘Background: Intra-uterine crowding(IUC) observed in hyperprolific sows impairs myofiber hyperplasia and overall fetal growth. Arginine supplementation(ARG) in gestation diets has been shown to positively affect litter and muscle development. The study objective was to assess whether the effect of ARG on offspring characteristics, with special emphasis on myofiber hyperplasia, differs under IUC conditions from these responses,because in that situation growth retardation is particularly prevalent due to reduced fetal nutrient supply.Unilateral oviduct ligation(OL) was used as a model for an uncrowded and hyperprolificacy(IN) as a model for a crowded intra-uterine environment.Methods: Five OL and five IN sows were fed a diet supplemented daily with either 43 g L-alanine(Ctrl) or 25 g L-arginine from d 14 to 28 of gestation in a cross-over design(two periods: 5^( th) and 6^( th) parity). At farrowing, two male and two female offspring, respectively, with a low and intermediate birth weight(BtW) were selected. After euthanization, the Semitendinosus muscle(STM) was removed and weighed, and the light and dark portions(STM_d and STM_l) were prepared for myofiber histochemistry using ATPase staining and the entire STM for gene expression analysis of myogenesis-related genes using RT-qPCR. In addition, various organs were removed and weighed. Data were analyzed using the MIXED model in SYSTAT.Results: No effect of either IUC or dietary treatment was found in litter characteristics. Offspring of ARG sows displayed a greater muscle area in STM(P < 0.01) as a result of the greater myofiber hyperplasia(P < 0.01). The increase was more distinct in the STM_l(P < 0.05) than in the STM_d(P = 0.131). Offspring of OL sows were heavier at birth(P < 0.01), had a heavier STM(P < 0.05), liver(P < 0.01) and kidney(P < 0.05), but when expressed relative to birth weight, these differences were absent.
