| 1 | Heart failure after myocardial infarction in the era of primary percutaneous coronary intervention:Mechanisms,incidence and identification of patients at risk显示文摘Myocardial infarction(MI) remains the most common cause of heart failure(HF) worldwide. For almost 50 years HF has been recognised as a determinant ofadverse prognosis after MI, but efforts to promote myocardial repair have failed to translate into clinical therapies. Primary percutaneous coronary intervention(PPCI) has driven improved early survival after MI, but its impact on the incidence of downstream HF is debated. The effects of PPCI are confounded by the changing epidemiology of MI and HF, with an ageing patient demographic, an increasing proportion of non-STelevation myocardial infarction, and the recognition of HF with preserved ejection fraction. Herein we review the mechanisms of HF after MI and discuss contemporary data on its incidence and outcomes. However, the advent of targeted, specific anti-cytokine agents as directed therapy for inflammatory and neoplastic conditions raises the prospect of a new era for anti-adhesion strategies. | 5 | Randomized study comparing incidence of radial artery occlusion post-percutaneous coronary intervention between two conventional compression devices using a novel airinflation technique显示文摘AIM To compare post-percutaneous coronary intervention(PCI) radial artery occlusion(RAO) incidence between two conventional radial artery compression devices using a novel air-inflation technique.METHODS One hundred consecutive patients post-PCI were randomized 1:1 to Safeguard or TR band compression devices. Post-radial sheath removal, each compression device was inflated with additional 2 m L of air above index bleeding point during air-filled device application and gradually down-titrated accordingly. RAO was defined as absence of Doppler flow signal performed at 24 h and at 6 wk post-PCI. Patients with missing data were excluded. Statistical significance was defined as P < 0.05.RESULTS All patients had 6 F radial sheath inserted. No significant differences were observed between Safeguard Radial(n = 42) vs TR band(n = 42) in terms of age(63 ± 11 years vs 67 ± 11 years), clinical presentation(electives, n = 18 vs n = 16; acute coronary syndrome, n = 24 vs n = 26) and total procedural heparin(7778 ± 2704 IU vs 7825 ± 2450 IU). RAO incidence was not significantly different between groups at 24 h(2% vs 0%, P = 0.32) and 6 wk(0%, both).CONCLUSION Safeguard Radial and TR band did not demonstrate significant between-group differences in short-term RAO incidence. Lack of evidence of RAO in all postPCI patients at 6 wk follow-up, regardless of radial compression device indicate advantage of using the novel and pragmatic air-inflation technique. | 8 | NOTES:The question for minimal resection and sentinel node in early gastric cancer显示文摘Surgical therapy for gastric cancer involves both removal of the cancer lesion and complete lymph node dissection.Natural orifice transluminal endoscopic surgery(NOTES) is considered to represent the next revolution in surgery.Many surgeons and endoscopists believe that NOTES may be a superior alternative for early gastric cancer treatment.Sentinel node(SN) navigation surgery for gastric cancer:Single institution results of SN mapping for early gastric cancer are increasingly being considered acceptable.Furthermore,a major large-scale clinical trial of SN mapping for gastric cancer has recently been completed by The Japan Society of SN Navigation Surgery study group.They reported false negative rate of 7.0 while the sensitivity of metastasis detection based on SN status was 93.Combination of SN biopsy and NOTES:This concept was first described by Cahill et al who proved the feasibility of lymphatic mapping and SN biopsy by NOTES.Lymphatic channel filling was immediately observable via the intraperitoneal optics.Partial resection of the stomach by hybrid NOTES:Several centers have already reported gastrectomy assisted by NOTES using the transvaginal route.However,the main problem of full-thickness resection of gastric wall remains endoscopic gastric closure.Establishing an endoscopic suturing method would be an important step toward expanding potential indications.NOTES is met with both enthusiasm and skepticism but will gain its own place as human creativity eventually provides solutions to its technical limitations.In the near future,NOTES can evolve the capacity to complement the existing armamentarium for gastric cancer surgery. | Mitsuhiro Asakuma Ronan A Cahill Sang-Woong Lee Eiji Nomura Nobuhiko Tanigawa | 2010 | World Journal of Gastrointestinal Surgery2010，2，6： | 3 |

| 12 | Left ventricular pseudoaneurysm: A case report and review of the literature显示文摘Left ventricular(LV) pseudoaneurysm is a rare complication that is reported in less than 0.1% of all patients with myocardial infarction. It is the result of cardiac rupture contained by the pericardium and is characterized by the absence of myocardial tissue in its wall unlike true aneurysm which involves full thickness of the cardiac wall. The clinical presentation of these patients is nonspecific, making the diagnosis challenging. Transthoracic echocardiogram and cardiac magnetic resonance imaging are the noninvasive modalities whereas coronary arteriography and left ventriculography are invasive modalities used for the diagnosis.
