1UGT1A1 gene polymorphism: Impact on toxicity and efficacy of irinotecan-based regimens in metastatic colorectal cancer显示文摘AIM: To investigate the correlation between uridine diphosphate glucuronosyl transferase 1A1 (UGT1A1) gene polymorphisms and irinotecan-associated side effects and parameters of drug efficacy in patients with metastatic colorectal cancer (mCRC) receiving a low-dose weekly irinotecan chemotherapeutic regimen. METHODS: Genotypes were retrospectively evaluated by gene scan analysis on the ABI 310 sequencer of the TATAA box in the promoter region of the UGT1A1 gene in blood samples from 105 patients who had received 1st line irinotecan-based chemotherapy for mCRC. RESULTS: The distribution of the genotypes was as follows: wild type genotype (WT) (6/6 ) 39.0%, heterozygous genotype (6/7) 49.5%, and homozygous genotype (7/7) 9.5%. The overall response rate (OR) was similar between patients carrying the (6/7, 7/7) or the WT genotype (6/6) (44.3% vs 43.2%, P = 0.75). Neither time to progression [(TTP) 8.1 vs 8.2 mo, P = 0.97] nor overall survival [(OS) 21.2 vs 18.9 mo, P = 0.73] differed significantly in patients who carried the(6/6) when compared to the (6/7, 7/7) genotype. 2温度与有机负荷对猪粪厌氧发酵过程的影响显示文摘为提高畜禽养殖场沼气工程能源利用效率,降低工程运行成本。该文通过全混式半连续试验,以猪粪为发酵原料,研究不同温度(20、28、38℃)和有机负荷(0.3、1.3、2.3、3.3、4.3kg/(m3·d))条件下,甲烷产率、挥发性脂肪酸(volatile fattyacid,VFA)及总无机碳(total inorganic carbon,TIC)的变化特征。结果表明:20℃条件下,反应器启动缓慢,甲烷产率低。当有机负荷增加至1.3kg/(m3·d)以上时,反应器运行不稳定,需外加NaHCO3调节pH值;在28℃与38℃条件下,各有机负荷甲烷产率无明显差异,但是在较高有机负荷3.3和4.3kg/(m3·d)下运行时,28℃条件下产生挥发性脂肪酸积累的现象,系统缓冲能力缺乏,VFA/TIC值分别达到0.4和0.6。当实际沼气工程低负荷率运行时,可选择28℃作为发酵温度,以达到节省能源、提高沼气工程经济效益的目的。该研究可为实际沼气工程运行管理提供数据参考。郭建斌 董仁杰 程辉彩 Joachim Clemens 庞昌乐 2011农业工程学报2011,27,12:29

5Alcoholic pancreatitis:New insights into the pathogenesisand treatment显示文摘Acute pancreatitis is a necro-inflammatory disease of the exocrine pancreas that is characterized by inappropriate activation of zymogens, infiltration of the pancreas by inflammatory cells, and destruction of the pancreatic exocrine cells. Acute pancreatitis can progress to a severe life-threatening disease. Currently there is no pharmacotherapy to prevent or treat acute pancreatitis. One of the more common factors associated with acute pancreatitis is alcohol abuse. Although commonly associated with pancreatitis alcohol alone is unable to cause pancreatitis. Instead, it appears that alcohol and its metabolic by-products predispose the pancreas to damage from agents that normally do not cause pancreatitis, or to more severe disease from agents that normally cause mild pancreatic damage. Over the last 10 to 20 years, a tremendous amount of work has defined a number of alcohol-mediated biochemical changes in pancreatic cells. Among these changes are: Sustained levels of intracellular calcium, activation of the mitochondrial permeability transition pore, endoplasmic reticulum stress, impairment in autophagy, alteration in the activity of transcriptional activators, and colocalization of lysosomal and pancreatic digestive enzymes. Elucidation of these changes has led to a deeper understanding of the mechanisms by which ethanol predisposes acinar cells to damage. This greater understanding has revealed a number of promising targets for therapeutic intervention. 6In vivo radiometric analysis of glucose uptake and distribution in mouse bone显示文摘Bone formation and remodeling occurs throughout life and requires the sustained activity of osteoblasts and osteoclasts,particularly during periods of rapid bone growth.Despite increasing evidence linking bone cell activity to global energy homeostasis,little is known about the relative energy requirements or substrate utilization of bone cells.In these studies,we measured the uptake and distribution of glucose in the skeleton in vivo using positron-emitting^(18)F-fluorodeoxyglucose([^(18)F]-FDG) and non-invasive,high-resolution positron emission tomography/computed tomography(PET/CT) imaging and ex vivo autoradiography.Assessment of [^(18)F]-FDG uptake demonstrated that relative to other tissues bone accumulated a significant fraction of the total dose of the glucose analog.Skeletal accumulation was greatest in young mice undergoing the rapid bone formation that characterizes early development.PET/CT imaging revealed that [^(18)F]-FDG uptake was greatest in the epiphyseal and metaphyseal regions of long bones,which accords with the increased osteoblast numbers and activity at this skeletal site.Insulin administration significantly increased skeletal accumulation of [^(18)F]-FDG,while uptake was reduced in mice lacking the insulin receptor specifically in osteoblasts or fed a high-fat diet.Our results indicated that the skeleton is a site of significant glucose uptake and that its consumption by bone cells is subject to regulation by insulin and disturbances in whole-body metabolism.Meredith L Zoch Diane S Abou Thomas L Clemens Daniel LJ Thorek Ryan C Riddle 2016Bone Research2016,4,1:9
7地理信息系统技术在伤寒发热监测中应用显示文摘目的运用地理信息系统(GIS)技术,了解各居民点发热流行率和评价以医疗机构为哨点的伤寒发热监测系统的敏感性。方法收集医学人口普查数据、疾病监测系统信息数据、现场采集居民居住点全球卫星定位数据等资料,建立GIS系统和以人口为基数的发热监测系统;对所有发热就诊病人进行身份和居住地址确认,并制作各居民点年发热就诊率GIS分布图。结果共采集3776个居民居住的全球定位系统(GPS)定位点,人群发热就诊率为1.11‰,发热就诊人次占总门诊人次的16.3%,监测区内的发热就诊病人占所有发热就诊病人的83.5%。在3种类型的医疗机构中,门诊量以个体门诊类最多,但其发热就诊人次占总门诊人次的比率最低(12.6%)。监测区内发热1d或以上的病人地址和身份(ADDRESS-ID)确认率达到95%。根据居民点年发热就诊率GIS分布图,发热就诊率最高的居民点达到17.81‰,最低为0;发热就诊率高的居民点多集中在城市。结论利用GIS技术可直观准确的了解疾病流行率的分布,可迅速直观的寻找出监测系统中的薄弱环节和推断出可能存在的问题。杨进 董柏青 张杰 廖和壮 梁大斌 吴兴华 司国爱 杨宏徽 Jin Kyung Park R Leon Ochiai Camilo J Acosta Moharmmad Ali John D Clemens 2007中国公共卫生2007,23,9:8
8低氧诱导因子-1α在绝经后骨质疏松中调控作用的实验研究显示文摘目的探讨在绝经后骨质疏松的发生、发展过程中,低氧诱导因子-lα(HIF-1α)对于成骨细胞功能的调控作用。方法 2004年10月至2006年5月,应用 Cre-Loxp 重组酶技术,建立成骨细胞条件性敲除 HIF-1α小鼠,取3个月龄雌性野生型和敲除型小鼠各24只行卵巢切除术,术后0、4、8周取材行 HE 染色、四环素荧光双标记、Micro-CT、RT-PCR、Western-blotting 检测。结果与野生型小鼠相比,敲除型小鼠骨小梁的数目、体积、厚度,骨密度,骨矿沉积速度,血管内皮生长因子(VEGF)、RunX2、ALP、OC 基因在 mRNA 水平的表达,VEGF、RunX2在蛋白水平的表达均明显降低,尤其以术后8周最为明显。结论在绝经后骨质疏松的发生、发展过程中,成骨细胞条件性敲除 HIF-1α后成骨功能降低,HIF-1α能够调控成骨细胞的成骨功能。刘晓东 邓廉夫 王君 齐进 周琦 王晋申 魏立 朱雅萍 Clemens TL 2007中华外科杂志2007,45,18:8
9A+C群脑膜炎球菌多糖疫苗安全性评估显示文摘目的 评估A+C群脑膜炎球菌多糖疫苗的安全性。方法 以A+C群脑膜炎球菌多糖疫苗为观察组,伤寒Vi多糖疫苗为对照组,按整群随机分层配对的原则,将研究现场分为108个组群,观察组和对照组各分配54个组。对两组同时建立接种反应监测系统,按统一表格和方法对两组的接种反应进行监测、记录,并进行流行病学调查。结果 两组共接种34543人,其中A+C群脑膜炎球菌多糖疫苗接种18167人,伤寒Vi疫苗接种16376人。A+C群脑膜炎球菌多糖疫苗速发接种反应率为0.44‰,一般接种反应率为0.38%o;伤寒Vi疫苗速发接种反应率为0.79%,一般接种反应率为0.73%o;跟踪随访接种反应对象共1239人,其中接种A+C群脑膜炎球菌多糖疫苗的771人,接种伤寒Vi疫苗的468人。接种后第1天,A+C群脑膜炎球菌多糖疫苗组的局部反应发生率要明显高于伤寒Vi疫苗(X^2。=13.98,P=0.0002);接种后第2—3天局部反应和全身反应的发生率,两组差异无统计学意义;两组各自接种人群和未接种人群发热发生率的差异无统计学意义,两组已接种人群的发热发生率差异也无统计学意义;两组均未发现严重反应。结论 A+C群脑膜炎球菌多糖疫苗与伤寒Vi疫苗均可在不同年龄组人群开展大规模接种,局部及全身接种反应轻微,接种反应率低,有良好的安全性。董柏青 叶强 崔萱林 杨进 龚健 杨明 廖和壮 韦士良 张杰 吴兴华 司国爱 杨宏徽 Ataru Tsuzuki JinKyung Park Mohammad Ali R. 10The hypoxia-inducible factor-1α activates ectopic production of fibroblast growth factor 23 in tumor-induced osteomalacia显示文摘Tumor-induced osteomalacia(TIO) is a rare paraneoplastic syndrome in which ectopic production of fibroblast growth factor 23(FGF23) by non-malignant mesenchymal tumors causes phosphate wasting and bone fractures. Recent studies have implicated the hypoxia-inducible factor-1α(HIF-1α) in other phosphate wasting disorders caused by elevated FGF23, including X-linked hypophosphatemic rickets and autosomal dominant hypophosphatemia. Here we provide evidence that HIF-1α mediates aberrant FGF23 in TIO by transcriptionally activating its promoter. Immunohistochemical studies in phosphaturic mesenchymal tumors resected from patients with documented TIO showed that HIF-1α and FGF23 were co-localized in spindleshaped cells adjacent to blood vessels. Cultured tumor tissue produced high levels of intact FGF23 and demonstrated increased expression of HIF-1α protein. Transfection of MC3T3-E1 and Saos-2 cells with a HIF-1α expression construct induced the activity of a FGF23 reporter construct. Prior treatment of tumor organ cultures with HIF-1α inhibitors decreased HIF-1α and FGF23 protein accumulation and inhibited HIF-1α-induced luciferase reporter activity in transfected cells. Chromatin immunoprecipitation assays confirmed binding to a HIF-1α consensus sequence within the proximal FGF23 promoter, which was eliminated by treatment with a HIF-1α inhibitor. 11言语噪声下汉语普通话声调测试系统的编制显示文摘目的 基于汉语普通话的语音学特点,编写叠加有言语背景噪声并符合成人心理物理学特征的声调识别测试材料,以期实现模拟真实语言环境下对声调识别能力的客观评价方案.方法 确立符合言语测听和语音学要求,兼顾简短、常用、有意义的编写原则.参考现代汉语常用字表,科学选取日常生活中常用的80个音节且四个声调均有对应实际含义的320个实词(80个音节×四声),构成声调识别测试材料词库.由两位专业播音员在本底噪声<30 dB(A)的专业录音工作室中,使用Electro Voice RE-20麦克风连接Huilian RMC-200型记录系统分别录制男声版和女声版的音频素材.采用Cool Edit Pro 2.0声音处理软件而对实词的音频素材进行强度处理、降低背景噪声并加入2 ms的静音间隔形成基础音频材料,并进一步行数字化处理.以听力正常人为受试者,根据其主观感受对基础音频材料的响度进行调整.结果 以16名听力正常人为受试者对320个实词进行主观响度调整,去除变异度过大的32个测试项,最终确立288个测试实词(即72个音节×四声),录制男、女声普通话声调识别测试材料各一套.以录制本组测试材料播音员的长时平均语谱为目标,采用FIR滤波器对高斯白噪声进行滤波,调整竞争噪声的均方根幅度,提取具有言语特异性的掩蔽噪声.在言语识别测试平台的控制下,可根据测试要求对噪声强度进行调整,实现固定信噪比条件下的汉语普通话声调识别测试.结论 成功编制具有言语特异性掩蔽噪声的汉语普通话声调识别测试材料,可为声调识别能力的研究提供评价工具.刘博 亓贝尔 Andreas Krenmayr 陈雪清 王硕 Reinhold Schatzer Clemens Zierhofer 韩德民 2014中华耳鼻咽喉头颈外科杂志2014,49,9:6
12A global assessment of recent trends in gastrointestinal cancer and lifestyle-associated risk factors显示文摘Background:Gastrointestinal(GI)cancers were responsible for 26.3%of cancer cases and 35.4%of deaths worldwide in 2018.This study aimed to analyze the global incidence,mortality,prevalence,and contributing risk factors of the 6 major GI cancer entities[esophageal cancer(EC),gastric cancer(GC),liver cancer(LC),pancreatic cancer(PC),colon cancer,and rectal cancer].Methods:Using the Global Cancer Observatory and the Global Health Observatory databases,we reviewed the current GI cancer incidence,prevalence,and mortality,analyzed the association of GI cancer prevalence with national human development indices(HDIs),identified the contributing risk factors,and estimated developing age-and sex-specific trends in incidence and mortality.Results:In 2020,the trend in age-standardized rate of incidence of GI cancers closely mirrored that of mortality,with the highest rates of LC,EC,and GC in Asia and of colorectal cancer(CRC)and PC mainly in Europe.Incidence and mortality were positively,but the mortality-to-incidence ratio(MIR)was inversely correlated with the national HDI levels.High MIRs in developing countries likely reflected the lack of preventive strategies and effective treatments.GI cancer prevalence was highest in Europe and was also positively correlated with HDIs and lifestyle-associated risk factors,such as alcohol consumption,smoking,obesity,insufficient physical activity,and high blood cholesterol level,but negatively correlated with hypertension and diabetes.Incidences of EC were consistently and those of GC mostly decreasing,whereas incidences of CRC were increasing in most countries/regions,especially in the younger populations.Incidences of LC and PC were also increasing in all age-gender populations except for younger males.Mortalities were decreasing for EC,GC,and CRC in most countries/regions, and age-specific trends were observed in PC and LC with adecrease in the younger but an increase in the older population.Conclusions: On the global scale, higher GI cancer burden was accompanied,for the most part, by factors associated with the so-called Western lifestylereflected by high and very high national HDI levels. In countries/regionswith very high HDI levels, patients survived longer, and increasing GI cancercases were observed with increasing national HDI levels. 13Alcoholic pancreatitis:Lessons from the liver显示文摘The association between alcohol consumption and pancreatitis has been recognized for over 100 years. Despite the fact that this association is well recognized, the mechanisms by which alcohol abuse leads to pancreatic tissue damage are not entirely clear. Alcohol abuse is the major factor associated with pancreatitis in the Western world. Interestingly, although most cases of chronic pancreatitis and many cases of acute pancreatitis are associated with alcohol abuse, only a small percentage of individuals who abuse alcohol develop this disease. This situation is reminiscent of the association between alcohol abuse and the incidence of alcoholic liver disease. The liver and the pancreas are developmentally very closely related. Even though these two organs are quite different, they exhibit a number of general structural and functional similarities. Furthermore, the diseases mediated by alcohol abuse in these organs exhibit some striking similarities. The diseases in both organs are characterized by parenchymal cell damage, activation of stellate cells, aberrant wound healing, and fibrosis. 16Converting lateral scanning into axial focusing to speed up three-dimensional microscopy显示文摘In optical microscopy,the slow axial scanning rate of the objective or the sample has traditionally limited the speed of volumetric imaging.Recently,by conjugating either a movable mirror to the image plane in a remote-focusing geometry or an electrically tuneable lens(ETL)to the back focal plane,rapid axial scanning has been achieved.However,mechanical actuation of a mirror limits the axial scanning rate(usually only 10–100 Hz for piezoelectric or voice coil-based actuators),while ETLs introduce spherical and higher-order aberrations that prevent high-resolution imaging.In an effort to overcome these limitations,we introduce a novel optical design that transforms a lateral-scan motion into a spherical aberration-free axial scan that can be used for high-resolution imaging.Using a galvanometric mirror,we scan a laser beam laterally in a remote-focusing arm,which is then back-reflected from different heights of a mirror in the image space.We characterize the optical performance of this remote-focusing technique and use it to accelerate axially swept light-sheet microscopy by an order of magnitude,allowing the quantification of rapid vesicular dynamics in three dimensions.We also demonstrate resonant remote focusing at 12 kHz with a two-photon raster-scanning microscope,which allows rapid imaging of brain tissues and zebrafish cardiac dynamics with diffraction-limited resolution.Tonmoy Chakraborty Bingying Chen Stephan Daetwyler Bo-Jui Chang Oliver Vanderpoorten Etai Sapoznik Clemens F.Kaminski Tuomas P.J.Knowles Kevin M.Dean Reto Fiolka 2020Light(Science & Applications)2020,9,1:4
