| 5 | Chemokine expression in hepatocellular carcinoma versus colorectal liver metastases显示文摘AIM: To evaluate and compare the expression profiles of CXCL12 (SDF-1), CCL19 (MIP-3β), CCL20 (MIP-3α) and CCL21 (6Ckine, Exodus2) and their receptors on RNA and protein levels in hepatocellular carcinoma (HCC) versus colorectal liver metastases (CRLM) and to elucidate their impact on the carcinogenesis and progression of malignant liver diseases. METHODS: Chemokine expression was analyzed by RT-PCR and ELISA in 11 cases of HCC specimens and in 23 cases of CRLM and corresponding adjacent non- tumorous liver tissues, respectively. Expressions of their receptors CXCR4, CCR6 and CCR7 were analyzed by RT- PCR and Western blot analysis in the same cases of HCC and CRLM. RESULTS: Significant up-regulation for CCL20/CCR6 was detected in both cancer types. Moreover, CCL20 demonstrated significant overexpression in CRLM in relation to the HCC tissues. Being significantly up- regulated only in CRLM, CXCR4 displayed an aberrant expression pattern with respect to the HCC tissues. CONCLUSION: Correlation of CXCR4 expression with CRLM suggests CXCR4 as a potential predictive factor for CRLM. | 8 | Liver surgery in cirrhosis and portal hypertension显示文摘The prevalence of hepatic cirrhosis in Europe and the United States, currently 250 patients per 100000 inhabitants, is steadily increasing. Thus, we observe a significant increase in patients with cirrhosis and portal hypertension needing liver resections for primary ormetastatic lesions. However, extended liver resections in patients with underlying hepatic cirrhosis and portal hypertension still represent a medical challenge in regard to perioperative morbidity, surgical management and postoperative outcome. The Barcelona Clinic Liver Cancer classification recommends to restrict curative liver resections for hepatocellular carcinoma in cirrhotic patients to early tumor stages in patients with Child A cirrhosis not showing portal hypertension. However, during the last two decades, relevant improvements in preoperative diagnostic, perioperative hepatologic and intensive care management as well as in surgical techniques during hepatic resections have rendered even extended liver resections in higher-degree cirrhotic patients with portal hypertension possible. However, there are few standard indications for hepatic resections in cirrhotic patients and risk stratifications have to be performed in an interdisciplinary setting for each individual patient. We here review the indications, the preoperative risk-stratifications, the morbidity and the mortality of extended resections for primary and metastatic lesions in cirrhotic livers. | 10 | Split liver transplantation: Current developments显示文摘In 1988, Rudolf Pichlmayr pioneered split liver transplantation(SLT), enabling the transplantation of one donor liver into two recipients-one pediatric and one adult patient. In the same year, Henri Bismuth and colleagues performed the first full right/full left split procedure with two adult recipients. Both splitting techniques were rapidly adopted within the transplant community. However, a SLT is technically demanding, may cause increased perioperative complications, and may potentially transform an excellent deceased donor organ into two marginal quality grafts. Thus, crucial evaluation of donor organs suitable for splitting and careful screening of potential SLT recipients is warranted. Furthermore, the logistic background of the splitting procedure as well as the organ allocation policy must be adapted to further increase the number and the safety of SLT. Under defined circumstances, in selected patients and at experienced transplant centers, SLT outcomes can be similar to those obtained in full organ LT. Thus, SLT is an important tool to reduce the donor organ shortage and waitlist mortality, especially for pediatric patients and small adults. In addition, patients with these disorders often demonstrate a heightened sensitivity to experimental visceral stimulation, termed visceral pain hypersensitivity that is likely to be important in their pathophysiology. Knowledge of how the brain processes sensory information from visceral structures is still in its infancy. However, our understanding has been propelled by technological imaging advances such as functional Magnetic Resonance Imaging, Positron Emission To-mography, Magnetoencephalography, and Electroen-cephalography (EEG). Numerous human studies have non-invasively demonstrated the complexity involved in functional pain processing, and highlighted a number of subcortical and cortical regions involved. This review will focus on the neurophysiological pathways (primary afferents, spinal and supraspinal transmission), brain- imaging techniques and the influence of endogenous and psychological processes in healthy controls and patients suffering from functional gastrointestinal dis- orders. Special attention will be paid to the newer EEG source analysis techniques. Understanding the pheno- typic differences that determine an individual's response to injurious stimuli could be the key to understanding why some patients develop pain and hyperalgesia in response to inflammation/injury while others do not. | 14 | Volatile and C-H-O isotopic compositions of giant Fe-Ti-V oxide deposits in the Panxi region and their implications for the sources of volatiles and the origin of Fe-Ti oxide ores显示文摘The Panzhihua,Hongge,and Baima Fe-Ti-V oxide deposits in the Panzhihua-Xichang(Panxi) region are hosted in large layered mafic-ultramafic intrusions.The layered intrusions intrude either the Neoproterozoic Dengying Formation,composed mainly of limestone,or the Paleoproterozoic Hekou Formation,composed of meta-sedimentary-volcanic rocks.It remains unclear if the wall rocks have been involved during the fractionation of magmas and have affected the sequence of crystallization of Fe-Ti oxide.Volatiles and their C-H-O isotopic compositions of magnetite,apatite,clinopyroxene,and plagioclase of different types of ores from the three intrusions are analyzed using a technique of stepwise heating mass spectrometer to evaluate the role of wall rocks in the formation of Fe-Ti oxide ores.Volatiles released from magnetite are composed mainly of H 2 O and CO 2,whereas the other minerals are composed mainly of H 2 O,CO 2 and H 2.At 800-1200°C temperature interval,the average 13 C values of CO 2 of all the minerals from the three intrusions range from 7.7‰ to 13.5‰ and the average 18 O CO 2 values from 19.1‰ to 19.5‰,which are scattered in a mixed field with basalt and the two types of wall rocks as end-members,indicating that CO 2 from the wall rocks may have been involved in the magmas from which the three intrusions formed.At 400-800 C temperature interval,both 13 C values(13.7‰ to 17.9‰ on the average) and 18 O values(16.2‰ to 19.2‰ on the average) of CO 2 of all the minerals are lower than those for 800-1200 C temperature interval,and much closer to the values of the wall rocks.Abundant H 2 O released at the 400-800 C temperature interval has relatively low D values ranging from 90‰ to 115‰,also indicating the involvement of fluids from the wall rocks.The average bulk contents of volatiles released from magnetite of the Hongge,Baima,and Panzhihua intrusions are 4891,2996,and 1568 mm 3 STP/g,respectively,much higher than those released from other minerals in total,which are 382,600,and 379 mm 3 STP/g,respectively,indicating that magnetite crystallized from magmas with much more volatiles than other minerals.This can be interpreted as that crystallization of clinopyroxene and plagioclase in the early fractionation of magmas resulted in volatiles such as H 2 O that were eventually enriched in the residual magmas and,at the same time,fluids from the wall rocks may have been involved in the magmas and were trapped in magnetite,which crystallized later than clinopyroxene and plagioclase. | WANG Christina Yan | 2012 | Science China Earth Sciences2012,55,11: | 15 |

| 16 | Inflammatory bowel diseases and spondyloarthropathies: From pathogenesis to treatment显示文摘Spondyloarthropathies(SpA) include many different forms of inflammatory arthritis and can affect the spine(axial SpA) and/or peripheral joints(peripheral SpA) with Ankylosing spondylitis(AS) being the prototype of the former. Extraarticular manifestations, like uveitis, psoriasis and inflammatory bowel disease(IBD) are frequently observed in the setting of SpA and are, in fact, part of the SpA classification criteria. Bowel involvement seems to be the most common of these manifestations. Clinically evident IBD is observed in 6%-14% of AS patients, which is significantly more frequent compared to the general population. Besides, it seems that silent microscopic gut inflammation, is evident in around 60% in AS patients. Interestingly, occurrence of IBD has been associated with AS disease activity. For peripheral SpA, two different forms have been proposed with diverse characteristics. Of note, SpA(axial or peripheral) is more commonly observed in Crohn's disease than in ulcerative colitis. The common pathogenetic mechanisms that explain the link between IBD and SpA are still ill-defined. The role of dysregulated microbiome along with migration of T lymphocytes and other cells from gut to the joint('gut-joint' axis) has been recognized, in the context of a genetic background including associations with alleles inside or outside the human leukocyte antigen system. Various therapeutic modalities are available with monoclonal antibodies against tumour necrosis factor, interleukin-23 and interleukin-17, being the most effective. | 18 | Current developments in pediatric liver transplantation显示文摘In 1953, the pioneer of human orthotopic liver transplantation(LT), Thomas E Starzl, was the first to attempt an orthotopic liver transplant into a 3 years old patient suffering from biliary atresia. Thus, the first LT in humans was attempted in a disease, which, up until today, remains the main indication for pediatric LT(p LT). During the last sixty years, refinements in diagnostics and surgical technique, the introduction of new immunosuppressive medications and improvements in perioperative pediatric care have established LT as routine procedure for childhood acute and chronic liver failure as well as inherited liver diseases. In contrast to adult recipients, p LT differs greatly in indications for LT, allocation practice, surgical technique, immunosuppression and postoperative life-long aftercare. Many aspects are focus of ongoing preclinical and clinical research. | 19 | 负面事件对旅游目的地的影响及应对策略--基于动态视角的实验研究显示文摘近年来,负面事件层出不穷,在网络媒体的推波助澜下,旅游目的地遭遇严重的形象危机。辨识负面事件类型,洞悉负面事件影响机制及动态变化,采取适配性应对策略,是化解旅游目的地负面事件危机的关键。文章基于信任理论和情境危机沟通理论,通过3个情境实验,从动态视角考察负面事件对旅游目的地的影响及应对策略。研究结果表明:旅游目的地负面事件包括能力型负面事件和道德型负面事件。其中,能力型负面事件通过能力信任降低旅游目的地评价,道德型负面事件通过善意信任降低旅游目的地评价。并且,相对于能力型负面事件,道德型负面事件的负面影响更大。能力型负面事件发生后,旅游目的地评价随着时间的推移缓慢回升,而道德型负面事件发生后,旅游目的地评价持续处于低位。根据负面事件的影响趋势,对于能力型负面事件,旅游目的地商家在事件发生第2天正面应对效果最佳;对于道德型负面事件,旅游目的地政府在事件发生第2天正面应对效果最佳。文章引入时间维度,为负面事件的影响及应对策略研究提供了新的视角,为旅游目的地危机处理提供了切实有效的措施。 | 胡家镜 万秋 刘悦 Christina G.CHI | 2020 | 旅游学刊2020,35,5: | 12 |
