| 1 | Preservation solutions used during abdominal transplantation:Current status and outcomes显示文摘Organ preservation remains an important contributing factor to graft and patient outcomes. During donor organ procurement and transportation, cellular injury is mitigated through the use of preservation solutions in conjunction with hypothermia. Various preservation solutions and protocols exist with widespread variability among transplant centers. In this review of abdominal organ preservation solutions, evolution of transplantation and graft preservation are discussed followed by classification of preservation solutions according to the composition of electrolytes, impermeants, buffers, antioxidants, and energy precursors. Lastly, pertinent clinical studies in the setting of hepatic, renal, pancreas, and intestinal transplantation are reviewed for patient and graft survival as well as financial considerations. In liver transplants there may be some benefit with the use of histidine-tryptophan-ketoglutarate(HTK) over University of Wisconsin solution in terms of biliary complications and potential cost savings. Renal grafts may experience increased initial graft dysfunction with the use of Euro-Collins thereby dissuading its use in support of HTK which can lead to substantial cost savings. University of Wisconsin solution and Celsior are favored in pancreas transplants given the concern for pancreatitis and graft thrombosis associated with HTK. No difference was observed with preservation solutions with respect to graft and patient survival in liver, renal, and pancreas transplants. Studies involving intestinal transplants are sparse but University of Wisconsin solution infused intraluminally in combination with an intra-vascular washout is a reasonable option until further evidence can be generated. | 2 | Regulation of macrophage activation in the liver after acute injury: Role of the fibrinolytic system显示文摘The liver functions,in part,to prevent exposure of the body to potentially harmful substances ingested in the diet.While it is highly efficient at accomplishing this,it is frequently prone to liver injury due to the biotransformation of xenobiotics into toxic metabolites.To counter this injury,the liver has evolved a unique capacity to rapidly and efficiently repair itself.Successful resolution of acute liver injury relies on hepatic macrophage populations that orchestrate the reparative response.After injury,Kupffer cells,the resident macrophages of the liver,become activated and secrete proinflammatory cytokines.These cytokines recruit other immune cells,including monocyte-derived macrophages,to the liver where they contribute to the repair process.Monocyte-derived macrophages traffic into the necrotic foci where they rapidly phagocytose dead cell debris.Simultaneous with this process,these cells change phenotype from a proinflammatory macrophage to a prorestorative macrophage that produce pro-mitogenic growth factors and antiinflammatory cytokines.Ultimately this process triggers resolution of inflammation,and along with proliferation of other hepatic cells,restores the liver architecture and function.While the mechanisms regulating specific macrophage functions during repair remain to be elucidated,recent studies indicate a key role for the fibrinolytic system in coordinating macrophage function during repair.In this review,we will highlight the function and role of hepatic macrophages in repair after acute liver injury,and will discuss the role of the fibrinolytic enzyme,plasmin,in regulation of these various processes. | Katherine Roth Jenna Strickland Bryan L Copple | 2020 | World Journal of Gastroenterology2020，26，16： | 3 |

| 4 | Outcomes after arthroscopic repair of rotator cuff tears in the setting of mild to moderate glenohumeral osteoarthritis显示文摘BACKGROUND Rotator cuff pathology is a very common source of shoulder pain.Similarly,osteoarthritis of the glenohumeral joint can cause shoulder pain and produce similar symptoms.Surgical management can be indicated for both pathologies,however,outcomes data is limited when examining rotator cuff repair(RCR) in the setting of glenohumeral arthritis(GHOA).Thus,this study sought to determine outcomes for patients who undergo RCR in the setting of GHOA.AIM To evaluate if a relationship exists between outcomes of RCR in the setting of GHOA.METHODS This was a retrospective analysis of patients who underwent arthroscopic rotator cuff repair with concurrent glenohumeral osteoarthritis between 2010-2017.Patients were stratified based on rotator cuff tear size and glenohumeral osteoarthritis severity.Cohorts were paired 1:1 with patients without glenohumeral osteoarthritis.Patients included had a minimum two year follow-up.Rate of conversion to total shoulder arthroplasty,complication rates following initial surgery,and patient-reported outcome measures were collected.RESULTS A total of 142 patients were included.The number of patients that required total shoulder arthroplasty within two years after index surgery was low.2/71(2.8%) patients with GHOA,and 1/71(1.4%) without GHOA.Following rotator cuff repair,both groups showed favorable patientreported outcomes.CONCLUSION Patients with glenohumeral osteoarthritis who underwent arthroscopic rotator cuff repair showed comparable outcomes to patients without glenohumeral osteoarthritis. | Ian S Hong Allison J Rao Tyler L CarlLee Joshua D Meade Daniel J Hurwit Gregory Scarola David P Trofa Shadley C Schiffern Nady Hamid Patrick M Connor James E Fleischli Bryan Michael Saltzman | 2022 | World Journal of Orthopedics2022，13，7： | 2 |
