1Analysis of multiple factors of postsurgical gastroparesis syndrome after pancreaticoduodenectomy and cryotherapy for pancreatic cancer显示文摘AIM: To explore the etiology, pathogenesis, diagnosis, and treatment of postsurgical gastroparesis syndrome (PGS) after pancreatic cancer cryotherapy (PCC) or pancreaticoduodenectomy (PD), and to analyze the correlation between the multiple factors and PGS caused by the operations.METHODS: Clinical data of 210 patients undergoing PD and 46 undergoing PCC were analyzed retrospectively.RESULTS: There were 32 (67%, 32/46) patients suffering PGS in PCC group, including 29 with pancreatic head and uncinate tumors and 2 with pancreatic body and tail tumors.Ten patients (4.8%, 20/220) developed PGS in PD group,which had a significantly lower incidence of PGS than PCC group (x = 245, P<0.001). In PCC group, 9 patients with PGS were managed with non-operative treatment (drugs,diet, nasogastric suction, etc.), and one received reoperation at the 16th day, but the symptoms were not relieved. In PD group, all the patients with PGS were managed with non-operative treatment. The PGS in patients undergoing PCC had close association with PCC,tumor location, but not with age, gender, obstructive jaundice, hypoproteinemia, preoperative gastric outlet obstruction and the type and number of gastric biliary tract operations. The mechanisms of PGS caused by PD were similar to those of PGS following gastrectomy. The damage to interstitial cells of Cajal might play a role in the pathogenesis of PGS after PCC, for which multiple factors were possibly responsible, including ischemic and neural injury to the antropyloric muscle and the duodenum after freezing of the pancreatico-duodenal regions or reduced circulating levels of motilin.CONCLUSION: PGS after PCC or PD is induced by multiple factors and the exact mechanisms, which might differ betweent hese two operations, remain unknown. Radiography of the upper gastrointestinal tract and gastroscopy are main diagnostic modalities for PGS. 3Western medication plus Traditional Chinese Medicine preparations in patients with chronic heart failure:a prospective,single-blind,randomized,controlled,and multicenter clinical trial显示文摘OBJECTIVE: To assess the efficacy and safety in patients with chronic heart failure(CHF) of Western medication plus Traditional Chinese Medicine(TCM) preparations.METHODS: This prospective, single-blind, randomized, controlled, and multicenter clinical trial began on September 17, 2008, and was completed on June 25, 2011. A total of 340 inpatients, aged 40-79 years, with exacerbating CHF from 10 hospitals were enrolled and randomly allocated within 24 h of admission. The trial included three intervention periods. During hospitalization, the control group received western medication for CHF and the treatment group received Danhong injection with Shenfu injection or Shenmai injection. After discharge,all patients were treated with Qiliqiangxin capsules and Buyiqiangxin tablets or a placebo for 6 months. After the 6-month intervention, both groups received only continuous western medication. The primary endpoint was all-cause mortality.The efficacy assessments were as follows: B-type natriuretic peptide(BNP), Lee's HF score, the 6-minute walking test(6 MWT), left ventricular ejection fraction(LVEF), and the Minnesota Living with Heart Failure Questionnaire(MLHFQ). The safety assessments were as follows: blood and urine routine examination, hepatic and renal function, electrolytes in blood and adverse events.RESULTS: Compared with the control group, the treatment group showed a 30.99% reduction in all-cause mortality and an improved survival rate.The treatment group showed greater improvement in 6 MWT(P = 0.02) than the control group on discharge, after 12-month follow-up, there was a time-group interaction for MLHFQ(P = 0.03). 4Myocardial infarction secondary prevention study (MISPS)显示文摘Background Traditional Chinese medicine (TCM), especially herbal medicine, has been widely used in China and now is also being increasingly used in other countries for the treatment of cardiovascular diseases. Although many studies have demonstrated that certain Chinese herbal products are effective and safe for the treatment of cardiovascular diseases, most of these lack sufficient quality. Therefore, large randomized clinical trials and further scientific research to determine its safety, effectiveness are necessary.QiShen YiQi Dripping Pills (QSYQDP) is a herbal preparation clinically used in the treatment and prevention of coronary artery disease. Preliminary observations have shown its safety and effectiveness. Methods/Design This randomized, controlled trial will recruit 3600 patients with a history of myocardial infarction. Patients will be randomized into two groups by a Centr-Randomized System. One group receives QSYQDP, the other group receive aspirin. This trial protocol will describe eligibility criteria, detailed information on the treatment definition, blinding, endpoints, statistical methods, sample size determination, data management, legal aspects, and the current status of the trial. Discussion This trial is one of the first randomized, controlled clinical trial to evaluate the efficacy and safety of traditional Chinese herbal medicine in the treatment and secondary prevention of coronary artery disease. 6Improving CLM4.5 Simulations of Land–Atmosphere Exchange during Freeze–Thaw Processes on the Tibetan Plateau显示文摘Soil is heterogeneous and has different thermal and hydraulic properties, causing varied behavior in heat and moisture transport. Therefore, soil has an important effect on land–atmosphere interactions. In this study, an improved soil parameterization scheme that considers gravel and organic matter in the soil was introduced into CLM4.5(Community Land Model). By using data from the Zoige and Madoi sites on the Tibetan Plateau, the ability of the model to simultaneously simulate the duration of freeze–thaw periods, soil temperature, soil moisture, and surface energy during freeze–thaw processes, was validated. The results indicated that:(1) the new parameterization performed better in simulating the duration of the frozen, thawing, unfrozen, and freezing periods;(2) with the new scheme, the soil thermal conductivity values were decreased;(3) the new parameterization improved soil temperature simulation and effectively decreased cold biases;(4) the new parameterization scheme effectively decreased the dry biases of soil liquid water content during the freezing, completely frozen, and thawing periods, but increased the wet biases during the completely thawed period; and(5) the net radiation, latent heat flux, and soil surface heat flux of the Zoige and Madoi sites were much improved by the new organic matter and thermal conductivity parameterization.Siqiong LUO Xuewei FANG Shihua LYU Yu ZHANG Boli CHEN 2017Journal of Meteorological Research2017,31,5:13
7Effects of Citral on Aspergillus flavus Spores by Quasi-elastic Light Scattering and Multiplex Microanalysis Techniques显示文摘Citral refined from Litsea cubeba oil has been found to have a strong influence on fungi, especially Aspergillus flavus. Multiplex microanalysis and quasi-elastic light scattering techniques were applied to study the effects of citral on Aspergillus flavus spores from the levels of membrane, organelle and intracellular macromolecule. It was found that citral injured the wall and the membrane of A. flavus spore, resulting in decrease of its elasticity. After entering the cell, citral not only influenced the genetic expression of mitochondrion reduplication and its morphology, but also changed the aggregation of protein-like macromolecules. As a result, cells, organelles and macromolecules lost their normal structures and functions, eventually leading to the loss of germination ability of A. flavus spores. 8Incidence and treatment of hepatic artery complications after orthotopic liver transplantation显示文摘AIM: To investigate the incidence and treatment of hepatic artery complications after orthotopic liver transplantation.METHODS: From February 1999 to May 2002, orthotopic liver transplantations (OLT) were performed in 72 patients with end-stage liver diseases with an average age of 40.2±13.6years (ranged from 11 to 68 years), 56 were males and 16 females. The preoperative evaluation for the 72 patients was performed using duplexsonography, abdominal CT scan,and angiography of the hepatic artery. All donor grafts were perfused and preserved in University of Wisconsin solution at 4 ℃. OLT was performed with standard techniques with or without a veno-venous bypass. Reconstructions of hepatic artery were performed between the branch patches of gastroduodenal/hepatic or splenic/common hepatic artery confluence of the donors and recipients, and an end-to-end anastomosis between other arterial vessels of the donors and recipients was done. Arterial anastomosis was performed with interrupted 7-0/8-0 monofilament polypropylene suture under 3.5 x loupe magnification. Diagnosis of the complications of hepatic artery after OLT was based on the clinical presentations, ultrasound findings and arterial angiography. All patients were followed up regularly for duplex ultrasound scan after discharge.RESULTS: The overall incidence of arterial complications in 72 patients after OLTs was 1.4 % (1/72). One 3 cm pseudoaneurysm at the side of anastomotic site of hepatic artery was found by urgent arteriogram due to hemoperitoneum secondary to bile leakage after OLT.Subsequently the pseudoaneurysm was successfully embolized and the blood flow toward the donor liver in hepatic artery remained. The overall postoperative 30-day mortality rate was 8.33 %. The one-year survival rate was 83.72 % in 50 patients with benign diseases and was 71.43 % in 22 patients with malignant diseases following OLT. 11Chemical comparison of dried rehmannia root and prepared rehmannia root by UPLC-TOF MS and HPLC-ELSD with multivariate statistical analysis显示文摘To identify the chemical differences which lead to the different therapeutic effects of dried rehmannia root(DRR)and prepared rehmannia root(PRR),we compared the chemical composition of decoctions of randomly purchased DRR and PRR using ultra performance liquid chromatography(UPLC)coupled with time-of-fight mass spectrometry and high performance liquid chromatography(HPLC)coupled with evaporative light scattering detection(ELSD)with the aid of multivariate statistical analysis.Both approaches clearly revealed compositional and quantitative differences between DRR and PRR.UPLC-MS data indicated stachyose,rehmaiono-side A(or rehmaionoside B),acteoside(or forsythiaside,or isoacteoside),6-O-coumaroylajugol(or 6-O-E-feruloylajugol,or 6-O-Z-feruloylajugol)as important discriminators between DRR and PRR decoctions.HPLC-ELSD analysis showed that the content of fructose in the decoctions of PRR was about four times greater than that of DRR(P<10^(-5)),while sucrose content in the decoctions of PRR was only about one seventh of that in DRR(P<0.01).Our results suggest that some compounds,such as fructose,stachyose and rehmaionoside,may be responsible for the differing therapeutic effects of DRR and PRR.Furthermore,improvements in quality control for PRR,which is currently lacking in the Chinese Pharmacopoeia,are recommended.Qiande Liang Jing Ma Zengchun Ma Yuguang Wang Hongling Tan Chengrong Xiao Ming Liu Beibei Lu Boli Zhang Yue Gao 2013Acta Pharmaceutica Sinica B2013,3,1:8
12The Quest for the Modernization and Internationalization of Traditional Chinese Medicine显示文摘Traditional Chinese medicine (TCM) is deeply rooted in ancient Chinese culture and has been practiced by Chinese people for thousands of years in order to maintain their health and fight against disease. This ancient Chinese wisdom has accumulated from the long struggle to cope with various diseases through hundreds or even thousands of trial-and-error practices. However, due to its empirical character, TCM has long been criticized as being deficient in scientific evidence, and is still not widely accepted by the mainstream conventional medical system. The complexity of the chemical components of TCM and the clarification of its mechanisms remain an enormous challenge in the conversion of TCM into an evidence-based medicine. 13Multimodal Identification by Transcriptomics and Multiscale Bioassays of Active Components in Xuanfeibaidu Formula to Suppress Macrophage-Mediated Immune Response显示文摘Xuanfeibaidu Formula (XFBD) is a Chinese medicine used in the clinical treatment of coronavirus disease 2019 (COVID-19) patients. Although XFBD has exhibited significant therapeutic efficacy in clinical practice, its underlying pharmacological mechanism remains unclear. Here, we combine a comprehensive research approach that includes network pharmacology, transcriptomics, and bioassays in multiple model systems to investigate the pharmacological mechanism of XFBD and its bioactive substances. High-resolution mass spectrometry was combined with molecular networking to profile the major active substances in XFBD. A total of 104 compounds were identified or tentatively characterized, including flavonoids, terpenes, carboxylic acids, and other types of constituents. Based on the chemical composition of XFBD, a network pharmacology-based analysis identified inflammation-related pathways as primary targets. Thus, we examined the anti-inflammation activity of XFBD in a lipopolysaccharide-induced acute inflammation mice model. XFBD significantly alleviated pulmonary inflammation and decreased the level of serum proinflammatory cytokines. Transcriptomic profiling suggested that genes related to macrophage function were differently expressed after XFBD treatment. Consequently, the effects of XFBD on macrophage activation and mobilization were investigated in a macrophage cell line and a zebrafish wounding model. XFBD exerts strong inhibitory effects on both macrophage activation and migration. Moreover, through multimodal screening, we further identified the major components and compounds from the different herbs of XFBD that mediate its anti-inflammation function. Active components from XFBD, including Polygoni cuspidati Rhizoma, Phragmitis Rhizoma, and Citri grandis Exocarpium rubrum, were then found to strongly downregulate macrophage activation, and polydatin, isoliquiritin, and acteoside were identified as active compounds. Components of Artemisiae annuae Herba and Ephedrae Herba were found to substantially inhibit endogenous macrophage migration, while the presence of ephedrine, atractylenolide I, and kaempferol was attributed to these effects. 14Simulated effect of soil freeze-thaw process on surface hydrologic and thermal fluxes in frozen ground region of the Northern Hemisphere显示文摘Soil freeze-thaw process is closely related to surface energy budget,hydrological activity,and terrestrial ecosystems.In this study,two numerical experiments(including and excluding soil freeze-thaw process)were designed to examine the effect of soil freeze-thaw process on surface hydrologic and thermal fluxes in frozen ground region in the Northern Hemisphere based on the state-of-the-art Community Earth System Model version 1.0.5.Results show that in response to soil freeze-thaw process,the area averaged soil temperature in the shallow layer(0.0175−0.0451 m)decreases by 0.35℃in the TP(Tibetan Plateau),0.69℃in CES(Central and Eastern Siberia),and 0.6℃in NA(North America)during summer,and increases by 1.93℃in the TP,2.28℃in CES and 1.61℃in NA during winter,respectively.Meanwhile,in response to soil freeze-thaw process,the area averaged soil liquid water content increases in summer and decrease in winter.For surface heat flux components,the ground heat flux is most significantly affected by the freeze-thaw process in both summer and winter,followed by sensible heat flux and latent heat flux in summer.In the TP area,the ground heat flux increases by 2.82 W/m2(28.5%)in summer and decreases by 3.63 W/m2(40%)in winter.Meanwhile,in CES,the ground heat flux increases by 1.89 W/m2(11.3%)in summer and decreases by 1.41 W/m2(18.6%)in winter.The heat fluxes in the Tibetan Plateau are more susceptible to the freeze-thaw process compared with the high-latitude frozen soil regions.Soil freeze-thaw process can induce significant warming in the Tibetan Plateau in winter.Also,this process induces significant cooling in high-latitude regions in summer.The frozen ground can prevent soil liquid water from infiltrating to deep soil layers at the beginning of thawing;however,as the frozen ground thaws continuously,the infiltration of the liquid water increases and the deep soil can store water like a sponge,accompanied by decreasing surface runoff.The influence of the soil freeze-thaw process on surface hydrologic and thermal fluxes varies seasonally and spatially.Di Ma SiQiong Luo DongLin Guo ShiHua Lyu XianHong Meng BoLi Chen LiHui Luo 2021Research in Cold and Arid Regions2021,13,1:4
15Effects of Jiawei Wendan decoction on hippocampal p-CREB protein expression in a rat model of depression显示文摘BACKGROUND:Jiawei Wendan decoction can elevate hippocampal brain-derived neurotrophic factor(BDNF) protein expression in rats with depression.It has been hypothesized that Jiawei Wendan decoction can exhibit antidepressant effects through the hippocampal signal transduction pathway of cyclic adenosine monophosphate response element binding protein(CREB)-BDNF.OBJECTIVE:Using phosphorylated-CREB(p-CREB) as an entry point,the present study was designed to observe intervention effects of Jiawei Wendan decoction compared with fluoxetine.DESIGN,TIME AND SETTING:A randomized,controlled,cellular biology experiment was performed at the Central Laboratory of Guangxi University of Traditional Chinese Medicine.MATERIALS:A total of 40 healthy,male,Sprague-Dawley rats were included in the present study.Rhizoma Acori Talarinowii(Shichangpu),Flos Albiziae(Hehuanhua),Rhizoma Pinelliae(Banxia),Caulis Bambusae in Taeniam(Zhuru),Fructus Aurantii Immaturus(Zhishi),Poria(Fuling),and Radix Bupleuri(Chaihu),the primary ingredients of Jiawei Wendan decoction,were purchased from First Affiliated Hospital of Guangxi University of Traditional Chinese Medicine.The raw drug was decocted at a concentration of 1.5 g/mL.Fluoxetine capsules were purchased from Shanghai Zhongxi Pharmaceutical Co.,Ltd.,China.METHODS:Following behavioral testing,36 rats were selected from the initial 40 rats according to similar behavioral scores,and were randomly divided into 4 groups:model(n = 8),Jiawei Wendan decoction-treated(n = 10),fluoxetine-treated(n = 10),and normal control(n = 8).All rats,except for those in the normal control group,were separately raised in a chronic and unpredictable,mild-stimulation environment for 21 days to establish a depression model.The Jiawei Wendan decoction-treated and fluoxetine-treated groups were intragastrically administered Jiawei Wendan decoction(12 g/kg/d) and fluoxetine(1.8 mg/kg/d),respectively.The model and normal control groups received double-distilled water(2 mL/kg/d) once a day for a total of 21 days.MAIN OUTCOME MEASURES:Prior to depression induction and subsequent to administration,memory and degree of depression were measured by open field and tail suspension tests,respectively.At 24 hours after administration,p-CREB expression in the hippocampal CA3 region was analyzed by the immunohistochemical SABC method.RESULTS:Two rats from the Jiawei Wendan decoction-treated group,and one from the fluoxetine-treated group,died of unknown causes.The remaining rats in each group,except for one rat that was randomly selected from the fluoxetine-treated group,were included in the final analysis.Following administration,the model group exhibited significantly decreased numbers of hippocampal p-CREB-positive cells,horizontal and vertical movements,and significantly prolonged duration of immobility in tail-suspension test,compared with the normal control group(P < 0.01).The numbers of p-CREB-positive cells,as well as horizontal and vertical movements,were significantly greater,and the duration of immobility was significantly shorter,in the Jiawei Wendan decoction and fluoxetine groups,compared with the model group(P < 0.05-0.01).There was no significant difference in the above-mentioned three indices between the Jiawei Wendan decoction and fluoxetine groups(P > 0.05).CONCLUSION:Jiawei Wendan decoction increased hippocampal p-CREB expression by influencing the rat hippocampal signal transduction pathway of CREB-BDNF.Jiawei Wendan decoction exhibited antidepressant effects equivalent to fluoxetine.Liping Zhang Meng Xia Li Wu Boli Zhang 2008Neural Regeneration Research2008,3,9:4
16Systems pharmacology for traditional Chinese medicine with application to cardio-cerebrovascular diseases显示文摘Identified as a treasure of natural herbal products,traditional Chinese medicine(TCM)has attracted extensive attention for their moderate treatment effect and lower side effect.Cardio-cerebrovascular diseases(CCVD)are a leading cause of death.TCM is used in China to prevent and treat CCVD.However,the complexity of TCM poses challenges in understanding the mechanisms of herbs at a systems-level,thus hampering the modernization and globalization of TCM.A novel model,termed traditional Chinese medicine systems pharmacology(TCMSP)analysis platform,which relies on the theory of systems pharmacology and integrates absorption,distribution,metabolism,excretion and toxicity(ADME/T)evaluation,target prediction and network/pathway analysis,was proposed to address these problems.Here,we review the development of systems pharmacology,the TCMSP approach and its applications in the investigations of CCVD and compare it with other methods.TCMSP assists in uncovering the mechanisms of action of herbal formulas used in treating CCVD.It can also be applied in ascertaining the different syndrome patterns of coronary artery disease,decoding the multi-scale mechanisms of herbs,and in understanding the mechanisms of herbal synergism.Yingxue Fu Yonghua Wang Boli Zhang 2014Journal of Traditional Chinese Medical Sciences2014,1,2:4
