| 1 | Evaluation and management of patients with refractory ascites显示文摘Some patients with ascites due to liver cirrhosis become no longer responsive to diuretics. Once other causes of ascites such as portal vein thrombosis, malignancy or infection and non-compliance with medications and low sodium diet have been excluded, the diagnosis of refractory ascites can be made based on strict criteria. Patients with refractory ascites have very poor prognosis and therefore referral for consideration for liver transplantation should be initiated. Search for reversible components of the underlying liver pathology should be undertaken and targeted therapy, when available, should be considered. Currently, serial large volume paracentesis (LVP) and transjugular intrahepatic portasystemic stent-shunt (TIPS) are the two mainstay treatment options for refractory ascites. Other treatment options are available but not widely used either because they carry high morbidity and mortality (most surgical options) rates, or are new interventions that have shown promise but still need further evaluation. The decomposition pathway of the parent precipitates was characterized by thermal analysis. The catalysts were calcined at 500 °C for 3 h and characterized by powder X-ray diffraction, Fourier transform infrared, and N2 adsorption-desorption. Nickel cobaltite spinel was formed in the solid state reaction between NiO and Co3O4. The N2O decomposition measurement revealed significant increase in the activity of Co3O4 spinel oxide catalyst with the partial replacement of Co2+ by Ni2+. | 3 | Reversibility of minimal hepatic encephalopathy following liver transplantation in Egyptian cirrhotic patients显示文摘AIM To evaluate the reversibility of minimal hepatic encephalopathy(MHE) following liver transplantation(LT) in Egyptian cirrhotic patients. METHODS This prospective study included twenty patients with biopsy-proven liver cirrhosis listed for LT and twenty ageand sex-matched healthy control subjects. All underwent neuro-psychiatric examination, laboratory investigations, radiological studies and psychometric tests including trail making test A(TMT A), TMT B, digit symbol test and serial dotting test. The psychometric hepatic encephalopathy score(PHES) was calculated for patients to diagnose MHE. Psychometric tests were repeated six months following LT in the cirrhotic patient group. RESULTS Before LT, psychometric tests showed highly significant deficits in cirrhotic patients in comparison to controls(P < 0.001). There was a statistically significant improvement in test values in the patient group after LT; however, their values were still significantly worse than those of the controls(P < 0.001). The PHES detected MHE in 16 patients(80%) before LT with a median value of -7 ± 3.5. The median PHES value was significantly improved following LT, reaching-4.5 ± 5(P < 0.001), and the number of patients with MHE decreased to 11(55%). The pre-transplant model for end-stage liver disease(MELD) score ≥ 15 was significantly related to the presence of post-transplant MHE(P = 0.005). | 4 | 碱促进的钴酸镁催化剂上的氧化亚氮分解(英文)显示文摘The direct decomposition of N2O was investigated over a series of magnesium cobaltite catalysts,MgxCo1-xCo2O4(0.0 ≤ x ≤ 1.0) ,which were prepared by the thermal decomposition of stoichiometric amounts of magnesium hydroxide and cobalt acetate. The thermal genesis of the different catalysts from their precursors was explored using thermogravimetric analysis,differential thermal analysis,and X-ray diffraction. Texture analysis was carried out using N2 adsorption at -196 °C. We found that all the catalysts that were calcined at 500 °C have a spinel structure. N2O decomposition activity was found to increase with an increase in the spinel structure's magnesium content. The influence of alkali cation promoters(Li,Na,K,and Cs) on the activity of the most active catalyst in the MgxCo1-xCo2O4 series,i.e. MgCo2O4,was also investigated. The sequence of the promotional effect was found to be: un-promoted < Li < Na < Cs < K-promoted catalyst. The reason for the increase in activity for the added alkali cations was electronic in nature. Additionally,the dependence of the activity on the K/Co ratio was also determined. The highest activity was obtained for the catalyst with a K/Co ratio of 0.05. A continuous decrease in activity was obtained for higher K/Co ratios. This decrease in activity was attributed to the elimination of mesoporosity in the catalysts with K/Co ratios > 0.05,based on N2 adsorption and scanning electron microscopy results. | Bahaa M. | 5 | Synthesis of Dy_2O_3 nanoparticles via hydroxide precipitation: effect of calcination temperature显示文摘This work described the preparation of dysprosium oxide, Dy2O3, nanoparticles using the homogeneous precipitation method. Dy3+ ions were precipitated using NaOH solution. The obtained product was filtered, dried, and then calcined for 1 h at the temperature range of 300–700 °C in static air. The calcination temperature of the Dy-precursor was chosen based on its decomposition as indicated by the TGA analysis. The crystalline structure and surface morphology of the calcined solids were studied by X-ray diffraction(XRD), field emission scanning electron microscopy(FE-SEM), transmission electron microscopy(TEM) and X-ray photoelectron spectroscopy(XPS). The obtained results revealed that Dy2O3 with crystallites size of 11–21 nm was formed at 500 °C. | 8 | Etiological risk factors for subfertility among Palestinian women in Gaza显示文摘The inability to procreate is frequently considered a personal tragedy and a hardship for couples, impacting on the entire family and even the local community. In Gaza strip, Palestine, there has been no study on etiological risk factors for subfertility. The present study aimed to identify risk factors associated with subfertility among women in Gaza, Palestine. One hundred and sixty-nine women in the study group and 115 women in the control group were included. Cases were selected randomly from those referred to the Al Basma Fertility Center, Gaza, Palestine. Data were collected through close-ended questionnaire, sonography, hormonal analysis and thrombophilia profile that included the methylenetetrahydrofolate reductase (MTHFR 677 C > T), factor V leiden (1691 G > A) and prothrombin (20210 G > A) genes. By using univariate analyses, the effects of different patient-related variables on the presence of subfertility were evaluated. A multiple logistic regression model was constructed, crude and adjusted odds ratios (OR) and 95% confidence intervals (95% CI) were calculated. The findings showed that 73.5 % (169/230) of the women referred to the Al Basma Center sought treatment for subfertility. Different etiological risk factors were associated with subfertility, the most frequent of which in descending order were: thrombophilic disorders, fallopian tube problems, sex hormone abnormalities and polycystic ovary syndrome with an adjusted OR of 21.42, 13.63, 11.69 and 10.29, respectively. In conclusion, several etiological risk factors are responsible for subfertility among women in Gaza. | 12 | Living donor liver transplantation for high model for endstage liver disease score:What have we learned?显示文摘AIM: To assess the impact of model for end-stage liver disease(MELD) score on patient survival and morbidity post living donor liver transplantation(LDLT). METHODS: A retrospective study was performed on 80 adult patients who had LDLT from 2011-2013. Nine patients were excluded and 71 patients were divided into two groups; Group 1 included 38 patients with a MELD score < 20, and Group 2 included 33 patients with a MELD score > 20. Comparison between both groups was done regarding operative time, intra-operative blood requirement, intensive care unit(ICU) and hospital stay, infection, and patient survival.RESULTS: Eleven patients died(15.5%); 3/38(7.9%)patients in Group 1 and 8/33(24.2%) in Group 2 with significant difference(P = 0.02). Mean operative time, duration of hospital stay, and ICU stay were similar in both groups. Mean volume of blood transfusion and cell saver re-transfusion were 8 ± 4 units and 1668 ± 202 m L, respectively, in Group 1 in comparison to 10 ± 6 units and 1910 ± 679 m L, respectively, in Group 2 with no significant difference(P = 0.09 and 0.167, respectively). The rates of infection and systemic complications(renal, respiratory, cardiovascular and neurological complications) were similar in both groups. A construction worker has a probability of 1-in-200 of dying on the job during a 45-year career, mainly due to fires, falls, and being struck by or caught between objects. | 14 | Histopathological changes in the oesophageal mucosa in Egyptian children with corrosive strictures: A single-centre vast experience显示文摘BACKGROUND The caustic ingestion continues to be a major problem worldwide especially in developing countries. The long-term complications include stricture and increased life time risk of oesophageal carcinoma. Patients suffered from corrosive induced oesophageal strictures have more than a 1000-fold risk of developing carcinoma of the oesophagus.AIM To determine the possibility of oesophageal mucosal dysplasia after prolonged dilatation in post corrosive stricture.METHODS This observational study was conducted at the Paediatric Endoscopy Unit in Cairo University Children's Hospital. It included children of both sexes older than 2 years of age who had an established diagnosis of post-corrosive oesophageal stricture and repeated endoscopic dilatation sessions for more than 6 mo. All patients were biopsied at the stricture site after 6 mo of endoscopic dilatation. A histopathological examination of an oesophageal mucosal biopsy was performed for the detection of chronic oesophagitis, inflammatory cellular infiltration and dysplasia.RESULTS The mean age of the enrolled children was 5.9 ± 2.6 years; 90% of the patients had ingested an alkaline corrosive substance(potash). The total number of endoscopic dilatation sessions were ranging from 16 to 100 with mean number of sessionswas 37.2 ± 14.9. Histopathological examination of the specimens showed that 85%of patients had evidence of chronic oesophagitis(group A) in the form of basal cell hyperplasia, hyperkeratosis and subepithelial fibrosis. Thirteen percent of the patients had evidence of reactive atypia(group B) in the form of severe neutrophilic intraepithelial inflammatory cellular infiltration, and 2 patients(2%)had mild squamous dysplasia(group C); we rebiopsied these two patients 6 mo after the initial pathological assessment, guided by chromoendoscopy by Lugol's iodine.CONCLUSION The histopathology of oesophageal mucosal biopsies in post-corrosive patients demonstrates evidence of chronic oesophagitis, intraepithelial inflammatory cellular infiltration and dysplasia. | 15 | Spot urinary sodium for assessing dietary sodium restriction in cirrhotic ascites显示文摘AIM: To evaluate the accuracy of spot urinary Na/K and Na/creatinine (Cr) ratios as an alternative to 24-h urinary sodium in monitoring dietary compliance in patients with liver cirrhosis and ascites treated with diuretics. METHODS: The study was carried on 40 patients with liver cirrhosis and ascites treated with diuretic therapy. Patients were divided into two groups according to 24-h urinary sodium. We measured spot urine Na/K ratio, Na/ Cr ratio and 24-h urinary sodium. Student's t test was used to compare the interval variables and χ2 test to compare the nominal variables between the two groups. Receiver operator characteristic curve was used to identify the best cutoff point for Na/K and Na/Cr ratio. RESULTS: The best cutoff point for Na/K ratio was 2.5 (P < 0.001) and area under the curve (AUC) was 0.9, and for Na/Cr ratio, the best cutoff point was 35 (P < 0.001) and AUC was 0.885. Na/K ratio showed higher sensitivity and accuracy compared to Na/Cr ratio (87.5% and 87% for Na/K ratio; 81% and 85% for Na/Cr ratio, respectively).
